Joe's epic journey from being a survivor and rescue worker as a fire fighter on 9/11 at Ground Zero, losing 26 friends and co-workers, then dealing with PTSD, addiction, cluster headaches (often termed, "suicide headaches") and eventually finding a trifecta of healing in physical pain, psychological trauma, and emotional turmoil through therapeutic use of psilocybin mushrooms  Joe is a retired FDNY firefighter who spends his time advocating for causes close to his heart, he also sits on the board of Clusterbusters, a non profit that supports research for better treatments and a cure for cluster headache, Joe has found Psychedelics are an effective treatment for his Cluster attacks as well as a treatment for PTSD and Addiction. Joe is a patient advocate for the Headache on the Hill event where he helps lobby Congress for increased recognition of, and more equitable policies toward Americans with disabling headache disorders. Joe also advocated along side fellow first responders and Jon Stewart in 2015 and 2019 to get 9/11 health and compensation bills passed, and again with Burn Pits 360 to help pass the PACT Act in 2022. He is also volunteer Board member for the Ray Pfeiffer foundation, helping 9/11 first responders with medical needs.

I first heard about burn pits from veterans when I was a pulmonary fellow at the Portland, VA. Our active duty military are often exposed to hazards when deployed, and the damage can be lifelong. Today I am joined by Captain Le Roy Torres and his wife Rosie Torres, the founders of of Burn Pits 360. They share their experience and journey through its ups and downs, from Balad, Iraq to the emergency room to the operating room to camping on the steps of the Capitol with Jon Stewart.  What Can You Do? Look up your Senator and Representative and tell them that you are someone who wants to ensure that our soldiers and veterans receive the healthcare they need both during and after their service. Ask them what they are doing about clean air for all. November 11th is Veteran's Day. Make sure you thank a veteran for their service. The Ghost Budget: How U.S. war spending went rogue, wasted billions, and how to fix it

HKS Senior Lecturer Linda Bilmes, an expert on public finance who has studied post-9/11 war costs for the past 20 years, says their staggering $5 trillion cost was enabled by what she calls “The Ghost Budget.” Using an unprecedented combination of borrowing, accounting tricks, and outsourcing, presidential administrations, Congress, and the Pentagon were able to circumvent traditional military budget processes in a way that kept war costs out of the public debate and resulted in trillions being spent with minimal oversight. The result: corporations and wealthy investors raking in huge profits, massive waste and fraud, and—combined with the Bush and Trump tax cuts—a shifting of the burden of the costs of war away from the wealthy and onto middle- and lower-income people and future generations. Of course by any metric, the United States-led wars in Afghanistan and Iraq were costly. Human life? At least 430,000 Iraqis, Afghans, and Pakistani civilians dead, along with more than 7,000 U.S. military personnel and thousands of civilian contractors. Democratic progress? Afghanistan is once again an authoritarian theocracy under the Taliban, and instead of transforming Iraq and the region, the U.S. invasion and occupation undermined popular sentiment toward democracy, unleashed sectarian violence, and strengthened autocratic regimes. But the budgetary problems are something we can address now, Bilmes says, with congressional reforms and planning prudently for the long-term costs of the wars, including caring for veterans. “The Ghost Budget” is also the title of Bilmes' next book, which will be published next year.Linda Bilmes' Policy RecommendationsCreate a veterans trust fund with an oversight board to pay for the long-term costs of caring for military personnel who served in Afghanistan and Iraq, costs which will not peak for as much as 50 years.Amend existing laws to automatically cover Iraq and Afghanistan veterans for toxic exposure to burn pits.Pass legislation requiring a set aside of a certain amount of funding long-term veterans care for every dollar appropriated for war spending.Restrict the ability of the White House and Congress to use the emergency and OCO (Overseas Contingency Operations) funding mechanisms to spend money on conflicts and to move war spending back into the main defense budget process.Address budgetary dysfunction in Congress by strengthening and empowering the House and Senate budget committees and streamlining their complicated and confusing budget subcommittee structures. Episode Contributors:Linda J. Her research focuses on budgeting and public administration in the public, private and non-profit sectors. She is a full-time Harvard faculty member, teaching budgeting, cost accounting and public finance, and teaching workshops for newly-elected Mayors and Members of Congress. Since 2005, she has led the Greater Boston Applied Field Lab, an advanced academic program in which teams of student volunteers assist local communities in public finance and operations. She also leads field projects for the Bloomberg Cities program. She served as the Assistant Secretary and CFO of the U.S. Department of Commerce under President Bill Clinton.  She currently serves as the sole United States member of the United Nations Committee of Experts on Public Administration (CEPA), and as Vice-chair of Economists for Peace and Security. She serves on the Board of Directors of the Institute for Veterans and Military Families at Syracuse University. She was a member of the National Parks Second Century Commission and served on the U.S. National Parks Service Advisory Board for eight years. She has testified to Congress on numerous occasions and has authored or co-authored numerous books, including the New York Times bestseller “The Three Trillion Dollar War: The True Cost of the Iraq Conflict” (with Joseph E. Stiglitz)  and “The People Factor: Strengthening America by Investing in Public Service” (with W. Scott Gould). She was also featured in the Academy-award nominated documentary "No End in Sight," and was the recipient of the 2008 Speaking Truth to Power Award from the American Friends Service Committee. Last week was Veterans' Day, and for some individuals in the armed service, that means reflecting back on their service, as well as the potential cause of their cancer. For both print and web-first features, CURE® spoke with veterans about their cancer journeys, including one Iraq veteran whose cancer was likely caused by exposure during his deployment overseas.  Regarding new drug indications last week, the FDA seemed to be working in the gastrointestinal space: we saw a new drug approved for certain patients with colorectal cancer, and other indication in the gastric cancer space slightly changed from the way it was originally approved a few years ago.  Finally, it is the open enrollment period for people who are eligible for Medicare. We heard from an expert at City of Hope about why patients with cancer should do their research before choosing a plan.  Veteran, Colon Cancer Survivor Explains Importance of PACT Act  Last year, President Joe Biden signed the bipartisan Sergeant First Class Heath Robinson Honoring our Promise to Address Comprehensive Toxics (PACT) Act into law last year, with the White House calling it “the most significant expansion of benefits and services for toxic exposed veterans in more than 30 years.” More than 4.1 million veterans received free toxic exposure screenings through the VA, with the government processing nearly half a million PACT Act claims and delivering more than $1.85 billion in PACT Act-related benefits to veterans and survivors, including $215 million in benefits to veterans with cancer, during the first year of the PACT Act, the White House stated. Under the PACT Act, for example, all gastrointestinal cancers are considered toxic exposure presumptive conditions — meaning veterans do not have to prove that the cancer started during or worsened because of their military service — for veterans from the Gulf War and post-9/11 eras such as Dan Nevins, a U.S. Army Reserve veteran and stage 3 colon cancer survivor who served in Iraq. Read more: Veterans On The Front Lines of Lung Cancer Dan is a bilateral, below-knee amputee living with a traumatic brain injury as a result of his service, and he received his cancer diagnosis in late 2021. Spoke with us about his experiences being exposed to burn pits in Iraq. FDA Approves Fruzaqla for Previously Treated Metastatic Colorectal Cancer Last week came with another Food and Drug Administration approval in the oncology space. Fruzaqla for patients with metastatic colorectal cancer that has been previously treated with certain chemotherapy drugs, anti-VEGF therapies, and — if the patient has RAS wildtype disease and its medically appropriate — an anti-EGFR therapy.  The approval, which was announced on Wednesday night, was based on findings from two clinical trials: FRESCO and FRESCO-2, which both showed that Fruzaqla improved overall survival (time from treatment until death of any cause) compared to placebo in this patient population.  Of note, Fruzaqla is an oral drug that is not a chemotherapy agent, which may help patients avoid an abundance of clinics for treatment, as well as some of the side effects associated with chemotherapy, though we should note that Fruzaqla, like all drugs, does have its own set of potential side effects.  FDA Amends Keytruda Approval for Gastric, GEJ Cancer Also on the FDA front, the agency announced on Thursday that it was revising the approval indication of Keytruda plus Enhertu and chemotherapy for the frontline treatment of locally advanced unresectable or metastatic HER2-positive gastric or gastroesophageal junction adenocarcinoma.  The Keytruda-containing regimen was originally approved in May 2021 and was for patients regardless of their cancer's PD-L1 status. PD-L1 is a protein found on the surface of tumor cells and acts as a kind of cloak of invisibility from the immune system. Checkpoint inhibitor drugs like Keytruda inhibit the function of PD-L1, thereby allowing the immune system to find and fight the cancer. With the amended approval, the FDA is now stating that this Keytruda regimen should only be used in patients whose cancers have that PD-L1 protein, as determined by a companion diagnostic test.  This change came after follow-up data from the KEYNOTE-811 trial were presented at the European Society for Medical Oncology Congress a couple of weeks ago. The findings showed that while the Keytruda plus Enhertu and chemotherapy improved outcomes over placebo plus Enhertu and chemotherapy, the benefit was particularly better for those whose disease was PD-L1 positive.  When It Comes to Cancer Care, Not All Medicare Plans Are the Same The Medicare open enrollment period has officially begun and will run until Dec. 7.  In a CURE® exclusive article penned by Dr. Harlan Levine of the City of Hope, a popular type of Medicare — Medicare Advantage — is outlined, along with their potential pros and cons when facing a cancer diagnosis. For example, did you know what while Medicare Advantage tends to be a more affordable option, it may not cover research medical centers that provide patients with cancer access to advanced treatments and clinical trials?  Dr. Levine and others are advocating for Medicare Advantage to cover treatment at comprehensive cancer centers, but until then, he warns individuals to be weary of the plans that they choose and always read the fine print.   For more news on cancer updates, research and education, don't forget to subscribe to CURE®'s newsletters here.

Dr. Brandy Lipscomb was a combat medic with the Washington National Guard. She was deployed to Iraq during Operation Iraqi Freedom from 2004-2005. During her time in Iraq, Dr. Lipscomb was stationed at one of the largest burn pits in the country. She came home thinking that life would return to normal, but it never did. In Iraq, some scars of the US-led invasion remain hidden, even as the 20th anniversary is marked this week. The US military for years disposed of waste in burn pits: wide swathes of land, constantly smoking. Some were the size of a football field. In them were plastics, electronics, and military and medical waste. The environmental impacts of the invasion – including those burn pits – continue to plague Iraqis to this day. After years of lobbying, the US government approved legislation to address the health impacts on US soldiers, but there has been no discussion of compensation for the Iraqi civilians. In August 2022, U.S. President Joe Biden signed an act expanding benefits and healthcare to U.S. veterans exposed to toxins in burn pits. But what about Iraqi exposure to burn pits? Kali Rubaii's research addresses their longterm diffuse exposure at all stages of their life, with effects that are varied and widespread. Rubaii has worked closely with Iraqi families since 2009, leading a team of doctors, epidemiologists and activists to conduct a case control study among families experiencing birth defects that may be linked to burn pits and bombings. In this interview, she speaks with Malihe Razazan about her work. Courtesy of Voices of the Middle East & North Africa (VOMENA). ---- Kali Rubaii earned her PhD in Anthropology from University of California, Santa Cruz and her BA in International Relations from University of California, Davis. Specialization: displacement, ecologies of war, spatial politics, forensic ethnography, health justice, Middle East. Kali Rubaii is an assistant professor in the Department of Anthropology at Purdue University, interested in sharpening resistance strategies that target the vulnerable nexus between coercive power and the physical world. Her research explores the environmental impacts of less-than-lethal militarism, and how military projects (re)arrange political ecologies in the name of “letting live.” Her book project, Counter-resurgency, examines how farmers in Anbar, Iraq struggle to survive and recover from transnational counterinsurgency projects. She is currently conducting fieldwork for two ethnographic projects: Taking toxicity as an analytic for material politics, she is working with a team of doctors, epidemiologists, and environmental activists to document the links between the epidemic of birth defects in Fallujah and military environmental damage. She is also researching the corporate-military enterprise of concrete production in post-invasion Iraq and how it enforces global regimes of class and citizenship.

In this special episode, Captain Torres explains how landfill-like burn pits in Iraq gave him a chronic lung injury and a toxic brain injury.  Captain Le Roy Torres served in the U.S. Army Reserve for 24 years, and spent a year on duty in Balad, Iraq. While there, Torres lived and worked in close proximity to a burn pit. These burn pits act as landfills, where everything from trash, to jet fuel, to medical waste was burned. They were extremely large, and the one near CPT Torres was approximately 10 acres in diameter. It burned 24/7, pumping toxic chemicals and smoke into the air, and into the lungs of soldiers. As a result of breathing in these fumes, CPT Torres developed chronic lung and brain injuries, which forced him to visit the hospital over 400 times in 10 years. Burn Pits 360 was founded by Torres and his wife in an attempt to improve post-deployment health outcomes for veterans, especially those caused by the burn pits. In other news of the day, former President Donald Trump weighs in on the potential prisoner trade currently on the table to bring WNBA star Brittney Griner home from Russia, Trump embraces a "Let's Go Brandon" chant at the LIV Golf event he hosted this past weekend, Speaker of the House Nancy Pelosi sets off for Asia while the Chinese threaten to literally shoot her plane down if she flies over Taiwan, President Joe Biden tests positive for COVID for a second time in as many weeks, TikTok is giving China all the data they need to crumble us, fellow 77 WABC Radio hosts Greg Kelly and Rudy Giuliani contrast one another when it comes to who exactly was responsible for bringing down the "squeegee guys," Jimmy Fallon hops on the bandwagon and pokes fun at President Biden's incompetence, and Ukrainian diplomat Ihor Zhovka tells Americans they need to "sacrifice their cozy life to help Ukraine."
