Using the FORWARD databank, our next guest Dr. Kaleb Michaud, PhD, identified a cohort and gathered data on rehabilitation dose and their functional outcomes. His study's objective (titled: “Examining Rehabilitation Dose in Adults With Rheumatoid Arthritis: Association With Baseline Factors and Change in Clinical Outcomes“) determine if there was a meaningful change in physical function, pain and fatigue over a six-month period when evaluating baseline factors and rehabilitation dose. Britt Ringstrom is such a fascinating person. A hugely knowledgable and experienced strength and fitness coach with a deep knowledge on wellbeing Starting as an athlete throughout high school and college, she has achieved very high level achievements. Many of which have been achieved AFTER being diagnosed with the very serious illness Rheumatoid Arthritis (RA). For most people this is a life changing diagnosis that is difficult to see past. Britt however felt that she wanted ownership over this chronic illness. She wanted to define the fight and achieve results that defy the usual path. Her story is fascinating, informative and inspiring.
Kirstan O'Neil is a 24-year-old small business owner from Northwest Florida. She runs an RV resort with her family and helps manage the day-to-day operations of the business. Prior to contracting Lyme disease, Kirstan had a very active and enjoyable life. She regularly went to the gym, enjoyed cooking and gardening with her grandma, and started working with her family at a young age. At the age of 12, Ms. O'Neil contracted Lyme disease and developed a “strep-like cold,” nosebleeds, swollen glands on her neck, joint pain, chest pain, and bad urinary tract infections (UTIs). Doctors dismissed her symptoms and misdiagnosed her with things like Rheumatoid Arthritis (RA), “hormonal issues,” and acid reflux. Unfortunately, as time went on, Ms. O'Neil developed even more severe symptoms like debilitating migraines. She had to “fight back the tears” to just leave the house with her family because of excruciating pain. Ms. O'Neil was finally diagnosed with Lyme disease at the age of 23 after seeing many different doctors. She treated Lyme disease using a variety of herbal formulas from Byron White, but first detoxed her body from extremely high levels of heavy metals and candida using a successful regimen we've never discussed on the Tick Boot Camp Podcast. This week, Tiffany and Patrice discuss the importance of biomarkers. Do biomarkers hold the key to understanding disease severity and predicting disease onset? In this episode, we talk about biomarkers, or a measurable indicator present in the blood or tissues, and how their presence in our bodies can predict disease severity and guide treatment development. Furthermore, evidence that identifying biomarkers early - BEFORE DISEASE ONSET - could actually prevent a person from developing the disease. Now, there is still the environmental plus genetic factors to consider, but the findings are promising. While the research focuses on Rheumatoid Arthritis (RA), showing how those who are sero-positive - in addition to having other genetic biomarkers - have a higher likelihood of developing severe disease and bone erosion, Tiffany points out this is similar to Axial Spondyloarthritis, where many of those with non-radiographic disease do not have the biologic HLA-B27 gene that is present in most with radiographic evidence. The bottom line is that learning about biomarkers can help patients navigate their own healthcare journey, but also could possibly prevent their loved ones from developing these diseases in the future. Abstract: Background/Purpose: There is an urgent need to identify effective biomarkers for early diagnosis of Rheumatoid Arthritis (RA) and accurate monitoring of disease activity. Here we define a RA meta-profile using publicly available cross-tissue gene expression data and apply machine learning to identify putative biomarkers, which we further validate on independent datasets. Methods: We carried out a comprehensive search for publicly available microarray gene expression data in the NCBI Gene Expression Omnibus database for whole blood and synovial tissues from RA patients and healthy controls. The raw data from 13 synovium datasets with 284 samples and 14 blood datasets with 1,885 samples were downloaded and processed. The datasets for each tissue were merged and batch corrected and split into training and test sets. We then developed and applied a robust feature selection pipeline to identify genes dysregulated in both tissues and highly associated with RA. From the training data we identified a set of overlapping differentially expressed genes following the condition of co-directionality. The classification performance of each gene in the resulting set was evaluated on the testing sets using AUROC. Five independent datasets were used to validate and threshold the feature selected (FS) genes. Finally, we define the RAScore, composed of a geometric mean of the selected RAScore Panel genes and demonstrate its clinical utility. Results: The result of the feature selection pipeline was a set of 25 upregulated and 28 downregulated genes. To assess the robustness of these feature selected genes, we trained a Random Forest machine learning model with this set of 53 genes and then with the set of 32 common differentially expressed genes and tested on the validation cohorts. The model with FS genes outperformed the model with common DE genes with AUC 0.89 +/- 0.04 vs 0.86 +/- 0.05. The FS genes were further thresholded on the 5 independent datasets resulting in 10 upregulated genes, TNFAIP6, S100A8, TNFSF10, DRAM1, LY96, QPCT, KYNU, ENTPD1, CLIC1, ATP6V0E1, that are involved in innate immune system pathways, including neutrophil degranulation and apoptosis and expressed in granulocytes, dendritic cells, and macrophages; and 3 downregulated genes, HSP90AB1, NCL, CIRBP, involved in metabolic processes and T-cell receptor regulation of apoptosis and expressed in lymphoblasts. To investigate the clinical utility of the 13 validated genes, the RA Score was developed and found to be highly correlated with DAS28 (r = 0.33 +/- 0.03, p = 7e-9) and able to distinguish OA and RA samples (OR 0.57, 95% CI [0.34, 0.80], p = 8e-10). Moreover, the RA Scores were not significantly different for RF-positive and RF-negative RA sub-phenotypes (p = 0.9) suggesting the generalizability of this score in clinical applications. The RA Score was also able to monitor the treatment effect among RA patients (t-test of treated vs untreated, p = 2e-4) and distinguish polyJIA from healthy individuals in 10 independent pediatric cohorts (OR 1.15, 95% CI [1.01, 1.3], p = 2e-4). Conclusion: The RAScore, consisting of 13 putative biomarkers, identified through a robust feature selection procedure on public data and validated using multiple independent data sets may be useful in the diagnosis and treatment monitoring of RA. Angela's triathlon ambitions came to a halt four years after she was diagnosed with RA after her 24th birthday. Multiple surgeries and much physical therapy later, she was determined to find a way to reinvent herself in a way that truly represented her core being. "I used my pain and I turned it into power," she says in this heartfelt conversation. "You too can reinvent yourself in a way that represents you, that aligns with your core values." Many of us have experienced the 'Who am I Now?' phenomenon, as these diseases challenge our capabilities to continue doing some of the things we once loved - things that we identify with as personal identifiers. The invisibility of these diseases adds to the frustration. But we can shift gears, we can find our new normals, all without having to change who we are as a person. About AngelaAngela Durazo made history in 2018 as the first female to enter the F4 US Championships. Not only did Durazo compete in the 37-car field of the USF4 series, she forged her F4 talent with a 1st place win in the Formula IMR Challenge series. Durazo also secured two 2ndplace podiums of the 6 race F4 Winter Championships series and she took home a notable 5th place in the Fp4 F4 West Coast Championships. She is also a 2x award winning Actress as well as a former professional Triathlete (Sprint and Olympic distance). John Brumbaugh, owner of “Spokane Nutrishop & Cryotherapy” joins the show to discuss the potential health benefits of Whole Body Cryotherapy (WBC). Listen to John and Dr. Luke discuss the history of WBC, how it works, and the people who experience improved health with its use. Show Notes The history of Whole Body Cryotherapy (WBC): the use of cold therapy dates back as far as 2500 B.C., when the Egyptians used cold water to treat various injuries and inflammatory conditions. Modern-day WBC, which uses nitrogen gas instead of water, started in the 1970’s in Japan by Dr. Toshima Yamauchi, whose primary focus was helping people with Rheumatoid Arthritis (RA). How does cryotherapy work? WBC features freezing cold nitrogen gas, which is pumped into an upright chamber. The extreme cold (as low as -300 degrees F) is sensed by your skin and essentially “tricks” your body into thinking you’re freezing. This triggers rapid vasoconstriction in the blood vessels in the extremities. As these blood vessels constrict/tighten, the blood is sent deeper into your core to conserve your body temperature. As the blood flows into your core, it passes through your body’s natural filtration system, where lactic acid, amino acid waste and inflammation are naturally metabolized. How long does it take? The whole process takes about 3 minutes. Once you step out of the chamber, your skin senses that you’re no longer freezing, and your filtered blood quickly returns to your extremities and skin as your blood vessels open up. Your core temperature doesn’t drop much, as the nitrogen gas is completely “dry” and isn’t able to steel much heat from your body (vs. water, which will steel your heat very rapidly). What does cryotherapy feel like? A WBC session is much LESS intense than an ice bath or cold tub. Depending on the setting you choose (there’s a beginner setting if you’re new to cryo), it will feel very similar to walking onto your back porch in the winter with your shirt off and the wind blowing. In other words, your skin will feel extremely cold. However, the effect on your core temperature is fairly negligible, so you experience the same benefits of an ice bath without the extreme sensation of the water and without the rapid drop in your core temperature. Who can benefit from cryotherapy? While the “evidence” is still largely anecdotal, several groups of people are reporting benefits from WBC. This includes athletes, people with metabolic issues, and people with autoimmune/inflammatory issues. Many people also report improved energy and improved sleep. Weight loss may also be a secondary benefit, although this is not the main focus of cryotherapy. Who should AVOID cryotherapy? People with heart conditions, especially related to high or low blood pressure, should exercise caution before starting WBC. As stated above, your blood vessels will rapidly constrict during the session, followed by rapid dilation when you exit the chamber. This can have a mild effect on your blood pressure. In his first solo full-length episode, Luke shares some key insights into how he helps his clients reduce their levels of pain and inflammation naturally. During this episode, he shares the first 4 strategies, and the next 4 will be covered in Part 2 (8 strategies total). Show Notes One of the biggest frustrations in medicine these days is that people feel like they’re being “farmed out” to multiple specialists for their various conditions, and no one is in charge of finding the underlying, root cause of the problem. For example, you might see a GI specialist for your stomach issues, an Endocrinologist for your Diabetes, and a Rheumatologist for your joint pain. But what if the underlying issue with these conditions was simply too much systemic inflammation and an overactive immune system? Wouldn’t it make perfect sense to reduce your level of information, with the hope that it would have a positive impact on every aspect of your health? Is that too much to hope for? Luke starts off this episode by sharing his background with autoimmune disease, which started when he was diagnosed with severe Rheumatoid Arthritis (RA) at the age of 21. When Luke and his wife wanted to start a family, the side effects of his medications became more of a consideration, as they could affect the developing child. After that point, Luke started actively looking for ways to improve his health naturally, with the ultimate goal of reducing his dependence on heavy-duty medications. Over the last 6-7 years, he’s acquired an array of information related to natural medicine, and he shares 4 strategies in this episode (Luke will share 4 more strategies on Part 2 of this topic) Strategy #1: Eat fewer carbs and sugars. When it comes to the types of food you eat, there are only 3 basic categories: carbohydrates, protein and fat. These 3 are referred to as “macro-nutrients”. One of the best ways to reduce your level of inflammation is to decrease the amount of sugars and simple carbs you ingest, while favoring high quality proteins and fats instead. Note: this does NOT mean you’re cutting calories. Rather, you’re replacing low-quality carbs for higher-quality options. Strategy #2: Eliminate food sensitivities and triggers. Although few people have food allergies, most people will have a handful of foods that they’re sensitive to. Figuring out which foods you’re sensitive to can be tricky, as testing isn’t always that helpful or accurate. However, if you pay attention to what you eat, you’ll likely be able to pinpoint a couple foods that don’t agree with your system (i.e. they make you feel fatigued, increase your joint pain, or upset your stomach). Identifying and eliminating these foods/triggers is an important way you can reduce the inflammation in your body. Common food sensitivities include: gluten, other grains, dairy, eggs, nuts and alcohol. Strategy #3: Decrease your toxic load. Now more than ever, we’re exposed to a higher level of toxic chemicals on a daily basis, which has a negative effect on our immune systems. Reducing your exposure to these toxic chemicals is an integral part of reducing the stress on your immune system, which will help lower your inflammation. One of the best ways to reduce your toxic exposure is to avoid eating foods with high levels of pesticides, or replacing these “conventional” items with “organic” ones. The “Dirty Dozen” is a list of the top 12 foods with the highest levels of residual pesticides. This is a great place to start! Other good examples of chemicals to reduce/eliminate include: lawn/garden chemicals (such as Roundup) and various chemicals in other household items and beauty supplies (cleaning solutions, shampoo, other hair care products and makeup). Strategy #4: More probiotics! If you ever work with a physician who specializes in natural health, such as a Naturopath, many of them will tell you that your health starts with “healing your gut”. And while we don’t understand all the intricacies of the bacteria that live in our gut, most natural physicians will tell you that ingesting probiotics is essential to your gut health. This includes eating fermented foods, such as sauerkraut, yogurt (dairy-free if possible!), kimchi and other fermented veggies. There are also probiotic drinks, such as kombucha, as well as probiotic supplements. There are moments in our life that stimulate us to make changes to our current situation. It might be an illness, the birth of a new baby, a new career, etc. Our guest today, Alfredo Allende decided to quit smoking and start Triathlon-ing with the birth of his first daughter. It all started with running - to help aid his depression & anxiety from nicotine withdrawal. But he was always getting injured as his bones couldn’t handle the workload. Something was off. So, he started cross-training with swimming and biking. After dealing with pain, fatigue, etc. for a number of months, Alfredo was recently diagnosed with Rheumatoid Arthritis (RA), an autoimmune disease with no cure - where your body pretty much attacks itself, destroying your joints & organs. But that hasn't stopped him from pursuing this sport and everything it means to him. María Bio Over 45 years ago, María, a person living with rheumatoid arthritis (RA), arrived in the United States with the dream of building a better life for herself and her three young daughters. To do this, María launched a successful cheese factory. When she sold her business in 2007, it was the second largest manufacturer of Hispanic cheeses in the United States. When she was 35 years old, María began experiencing extreme fatigue, which she at first blamed on her grueling entrepreneurial schedule. After her doctor ordered a blood test, she received the devastating diagnosis of RA. Undaunted, she continued growing her business and raising her girls, oftentimes having to resort to supervising manufacturing from a wheelchair because the pain in her legs and feet had become unbearable. Among her many blessings, she counts her trusted rheumatologist who has journeyed the RA path with her for the last 30 years. Today, María is a passionate advocate for helping others cope with this chronic autoimmune disease. Live Right Now - Episode 007 – Acidity and Inflamation After presenting my Eat Right Now program to a vibrant senior living community, I walked outside and observed two sweet ladies walking together, each carrying large bottles of Bragg’s Apple Cider. “May I help?” They politely declined, stating if not for the amber liquid, they’d be wracked with arthritis, unable to tend their garden. Both were 90 and claimed drinking this particular brand of ‘living’ vinegar was miraculous. My dearest achy-breaky, pain-pill-popping pals, you deserve delicious relief. Inflamed Americans limp and hobble under the erroneous belief that arthritis is a natural part of aging. Again, we’ve been hornswoggled. Millions agonize unnecessarily due to some really, really bogus, not-so-erudite medical advice, and the slime of low grade foods oozing from the Western diet. The word arthritis comes from the Greek arthron meaning "joint" and the Latin itis meaning "inflammation". It can result from youthful sports injuries, aggressive autoimmune response, and infuriating innocent cartilage; the glistening white surface of bone joints. When this gliding surface is no longer intact, agonizing discomfort, swelling and stiffness result. Arthritis affects 46 million and is not inevitably a natural part of aging; rather, it’s years of physical self-abuse or a nutritional deficiency resulting from feed-bagging the gory-awful Standard American Diet of dead food. The reality is the pain of arthritis can be prevented, subdued, and aided thru whole foods nutritional and vitamin supplementation. Anything triggering an immune response also triggers inflammation. Alien foods your immune system recognizes as not belonging in your temple can ignite allergic reactions resulting in joint irritation. Oh, your doctor never told you? Now, add red meats, egg yolks, dairy, wheat gluten, corn, corn syrup, sugar, flour, processed foods, and alcoholic beverages to the list. Especially for individuals with Rheumatoid Arthritis (RA) the gluten in wheat, oats, barley, and rye are troublemakers. Doctors report a higher than average number of folks with autoimmune disorders who are allergic to gluten. A new study also discovered folks with rheumatoid arthritis have a 50 percent increased risk of diabetes compared to people without arthritis. Get tested for gluten allergies known as Celiac disease. If you dig milk, butter, cheese, yogurt, or casein and whey in other food fare such as bread or milk chocolate, then you can potentially trigger the symptoms of your food allergy, in this case arthritis. Allergy symptoms may show up hours or even days later, well after a food is absorbed into your delicate ecosystem. Bacteria, viruses, and parasites also trigger an immune response so take a probiotic supplement. Since we’re primates created for a mostly plant-based diet, many people have limited capacity to digest animal proteins efficiently as they age, so when they eat excessive animal protein the Temple’s ability to cleave all the amino acid bonds is limited. Hence the proteins may not totally break down into amino acid building blocks which lead to the creation of antibodies against themselves; a.k.a. auto immune disease Over 100 types of arthritis are auto immune related diseases the body's immune system becomes lead astray, attacking what it was designed to protect. For those with autoimmune disease everything you place into their temple is either making them healthier or sicker. There’s simply too much research disproving the disease is caused by aging. So, get over yourself and ditch the, “If it dies, it fry’s,” mentality and foods from the 50’s. Scientific reports reviewed thirty-one studies concluded that fasting, followed by a vegetarian diet, is useful in the treatment of rheumatoid arthritis. Non-prescription anti-inflammatories include vitamin C, turmeric, ginger, cayenne, glucosamine. Be mindful it’s never too late to self-manage and douse the conflagration of inflammation with informed dietary modifications and the no-nonsense use of nutritional supplements. Recognizing the relationship between diet and your symptoms can bring blissful relief. Go against societies flow. Take the road less traveled by choosing the dinner fork in road to joyous relief and you’ll enrich your quality of life, shrink medical costs, and liberate your beautiful temple. You can do it. Last week we touched on osteoarthritis as it relates to sciatica but that's just the tip of the iceberg. [00:01:07][10.6] Jean: [00:01:08] Indeed. First off we should tell our listeners that there are more than 100 different forms of arthritis and related diseases. The most common are osteoarthritis rheumatoid arthritis and (psoriatic) arthritis. [00:01:22][13.6] Lita: [00:01:23] I've heard about osteoarthritis and rheumatoid arthritis but psoriatic arthritis is a new one to me. [00:01:29][6.2] Jean: [00:01:30] Well according to the Arthritis Foundation psoriatic arthritis is an autoimmune inflammatory disease in which the immune system attacks the body causing inflammation and pain. Psoriatic arthritis affects the joints causing arthritis as well as the skin causing psoriasis. And from 30 to 40 percent of the people with psoriasis, the percentage also develops an inflammatory arthritic condition. The symptoms may include joint pain, stiffness, skin rashes, Nail changes, and fatigue, eye problems, swelling and tenderness in the hands and feet as well. [00:02:06][35.9] Ron: [00:02:07] Wow. If there are seven and a half million people in the United States with psoriasis in 30 to 40 percent of them developed this form of arthritis that means between two and a half to three million people have it? That's not rare at all. [00:02:22][14.9] Lita: [00:02:22] No I'm kind of surprised I never heard of it. And rheumatoid arthritis affects one and a half million while osteoarthritis, or the degenerative bone disease affects 27 million. That's the it's the killer, no not the killer, it's is the winner that has got, the most people that have it. [00:02:39][17.3] Jean: [00:02:40] In the arthritis race. [00:02:40][0.7] Lita: [00:02:41] Yes and in that arthritis race the osteoarthritis wins with 27 million. Oh yeah. [00:02:46][4.7] Ron: [00:02:46] I Wouldn't want to win. [00:02:47][0.3] Lita: [00:02:47] No no you're right. Besides being that the most common type, osteoarthritis is acquired from either wear and tear on a joint or from a previous injury to their joint. Rheumatoid or psoriatic arthritis. Oh and psoriatic arthritis they're both diseases that involved the immune system attacking the tendons ligaments and joints. [00:03:09][22.0] Jean: [00:03:10] Sounds painful. Your own immune system attacking from within seems rather sinister. [00:03:15][4.6] Lita: [00:03:16] mmhmm. [00:03:16][0.0] Jean: [00:03:16] The makings of a crime novel but in real life the people suffering from rheumatoid or psoriatic arthritis probably wish it was just a novel instead of something they have to live with day to day. [00:03:26][9.5] Lita: [00:03:26] Yeah [00:03:26][0.0] Ron: [00:03:28] While researching for today's topic I read the rheumatoid arthritis can also affect the skin, eyes, lungs, the heart, kidneys, the nerve tissue and also the blood vessels. It can increase your risk of hardened or blocked arteries, inflammation of the sac surrounding your heart. It can scar lung tissue or it can increase your risk of lymphoma. Cancers that develop in the lymph system. [00:03:53][25.1] Lita: [00:03:54] With the increased risk to these major systems. I imagine that it affects a person's lifespan. [00:03:58][4.4] Ron: [00:04:00] It can buy as much as 10 to 15 years. It's not as simple achy joint disease. [00:04:05][5.3] Lita: [00:04:06] No. Right. That's true Ron. And even if it doesn't get to the critical organs the joint deterioration is progressive over time. Jean, can you tell our listeners what treatments are available for rheumatoid arthritis. [00:04:19][13.3] Jean: [00:04:20] Sure. Again I turned to the Arthritis Foundation for answers. The goals of, for treatment are to stop inflammation, relieve symptoms, prevent organ and joint damage, improve function, and overall well-being. And lastly to reduce long term complications to stop inflammation aggressive treatment as early as possible is recommended. This would include but is not limited to arthritis medication using natural treatments to ease pain and at times surgical intervention is required. Joint surgery may include the total replacement of the joint or the resurfacing or repairing of the damaged areas. [00:04:58][38.2] Ron: [00:04:59] That right. The natural treatment your doctor may prescribe include Supplements Herbs vitamins and minerals. Our Web site will include more information and videos about rheumatoid arthritis. Let's talk a little bit more about osteoarthritis. This is the most prevalent form of arthritis. [00:05:28][10.4] Jean: [00:05:29] Well I can start us off with some basics. Osteo arthritis is a type of joint disease that results from the breakdown of joint cartilage as well as the bones the cartilage is attached to the most common symptoms are joint pain and stiffness. Early on you may only notice these symptoms following exertion but with the disease as it progresses to the point where you know you might feel pain and stiffness all the time. [00:05:53][24.2] Ron: [00:05:54] Right. In the most common joints affected are the fingers the neck, lower back knees and the hips. Osteo arthritis may be caused by previous knee injury but can also be due to abnormal joint or limb development, or even inherited factors. It turns into a vicious cycle of pain because as you feel the pain, so you stop using that body part in order to not feel the pain. But then the muscles in that area get weak causing more mechanical stress than the joint in joint inflammation and deterioration. [00:06:25][30.9] Lita: [00:06:26] Vicious cycle is right even though there's no cure, There are things that you can do to help alleviate the pain and stop the cycle of deterioration. The Arthritis Foundation recommends that you start with hot and cold compresses which Jean was talking about earlier which, they may be beneficial. Heat may help with the stiffness and cold may help reduce the joint pain. Epsom salt baths may also help with inflammation and pain. You can add two cups of epsom salt to a tub of warm water. Exercise is another great way to keep that motion in the affected joint. This is what they do in physical therapy. If you can use a warm therapy pool you'll get the opportunity to exercise and the water's buoyancy helps take your body weight off of the joint while surrounded by warm water. It's a win win win. [00:07:18][51.3] Jean: [00:07:20] Yay! [00:07:20][0.0] Lita: [00:07:20] Your doctor may prescribe pain medications or over-the-counter medications such as ibuprofen. [00:07:24][4.5] Jean: [00:07:26] In addition there are many supportive devices available that can add support to the affected joints. These include braces, canes, grabbing tools, gloves, knee taping and shoe inserts. You can actually find a list of several of these items on our website. As far as natural remedies green tea has been shown to increase cartilage production and help reduce inflammation. But even though it is considered a natural product anything taken in large amounts is not good for you and great amounts of green tea can affect the liver. So it's best to take it a moderation and under the guidance of your doctor and the same warning should come into play with tumoric and ginger both have anti inflammatory properties but are not safe in large doses like we always say check with your doctor first. [00:08:11][45.5] Ron: [00:08:13] Indeed. Let me add, that from a recreational therapy standpoint, the best type of exercise for osteoarthritis, use light resistance, it improves flexibility and they offer an aerobic element, they're considered to be low impact. Some of these activities include bike riding, swimming, Taichi, walking and yoga. Your doctor would probably suggest starting slowly and building up over time. And if you wear proper footwear your joints will appreciate that. We will be interviewing a sports podiatrist in the coming weeks and we will be sure to ask him specifically what shoes are best for different activities in which shoes you should avoid. I then felt trapped by my medical system when I was presented with the standard immune-modulating and anti-inflammatory drugs as my only options. I was told that diet and stress levels had nothing to do with my disease.I examined my body, mind, heart, and spirit for clues to my disease. In the process of my research and inquiry into the “WHY” of my disease, I discovered Ayurveda. Ayurvedic medicine is an ancient science that encompasses all of these aspects of health: body, mind, heart, spirit, and story.Within one year of changing lifestyle habits from each of the four areas of my WHOLE being, I was free of autoimmunity. Tests could find was no sign of RA in my blood. According to Western medicine, this was impossible. I discovered I had only been trapped by my lifestyle choices and how they impacted my genetics.Shortly thereafter, I discovered Functional Medicine and realized this was a model that took the precepts of Ayurveda and translated them into Western language for the modern person. Objective: The “Comprehensive ICF Core Set for Rheumatoid Arthritis (RA)” is an application of the International Classification of Functioning, Disability and Health (ICF) and represents the typical spectrum of problems in functioning of patients with RA. The objective of this study was to validate this ICF Core Set from the perspective of physicians. Methods: Physicians experienced in RA treatment were asked about the patients’ problems, patients’ resources and aspects of environment from their scope of practice. This survey was conducted via e-mail in three rounds using the Delphi technique. The responses were linked to the ICF by two trained individuals according to empirically tested linking rules. Consensus of the results was calculated statistically by the statistical Kappa coefficient. Results: 79 physicians in 41 countries named 512 patients’ problems spanning all ICF components. 227 ICF categories were linked to the physicians’ responses. 16 ICF categories were not represented in the Comprehensive ICF Core Set for RA although at least 75% of the participants have rated them as important. 19 issues were not covered by the ICF classification and 26 answers were linked to the not yet developed ICF component Personal factors. Conclusion: The validity of the ICF components Body Structures, Activities and Participation and Environmental Factors was well supported by the physicians whereas there was less support for the validity of the component Body Functions. Several issues arose that are not covered and need to be investigated further.
