President Donald Trump on Friday signed out a new artificial intelligence national security memorandum that the White House says establishes “a new framework to put the most advanced, secure, and reliable AI systems into the hands of America's warfighters and intelligence professionals while ensuring their responsible use.” The memo rescinds the Biden administration's National Security Memorandum-25 from October 2024 that similarly set governance for the use and safety of Artificial Intelligence (AI) in national security and intelligence systems. The new guidance sets policies driving four key actions around AI in the national security space. The Department of the Air Force has tapped Ashley Devoto — a veteran and cybersecurity expert — as its new chief information officer, the department announced Thursday. Devoto enters the role after the department has been without a permanent CIO for over a year following the departure of Venice Goodwine in March 2025. With a decades-long career in cybersecurity fields, Devoto will now oversee the Air and Space Forces' modernization and sustainment efforts for information technology and more. As CIO, she will lead the department's enterprise information technology, data and artificial intelligence, cybersecurity, and defense business systems portfolios and steward approximately $17 billion in technology investments that enable the missions of more than 800,000 Airmen, Guardians, civilians, and contractors worldwide. Episode 220: Approach of Insomnia in Adults     ARREAZA: Today we are going to talk about how to approach sleep issues in adults — from the initial assessment all the way through treatment. And I think what is going to surprise a lot of our listeners is the changes in management in recent years, especially what is recommended as first-line therapy. So, let's jump right in. MOIRA: Sleep is one of those topics that touches every specialty, but Primary Care in particular, so I'm excited to dig into this. ARREAZA: Let's start with the big picture–statistics. How common are sleep problems in adults? MOIRA: Incredibly common. Chronic insomnia affects roughly 10% of the general population, and that number has only grown in recent years . During the COVID-19 pandemic, for instance, prevalence rates of insomnia symptoms were reported globally at 20 to 45% (wow). And, importantly, those sleep problems did not simply resolve once infection rates dropped, insomnia symptoms and fatigue have continued even as mood improves in people recovering from COVID-19 infection.  ARREAZA: Incredible that we are in 2026 and still talking about COVID-19. And we clinicians need to understand that insomnia isn't just an annoyance. It has long-term consequences. Also, financially, insomnia causes direct and indirect costs of up to $100 billion each year. MOIRA: Exactly. Insomnia is both a risk factor for, and a symptom of, several psychiatric disorders, and it is a predictor of death by suicide, making it an important target for intervention. It's highly comorbid with medical and psychiatric disorders and is associated with significantly increased healthcare utilization and costs. People with insomnia also perform more poorly on complex cognitive tasks. So, we're talking about a condition that affects cognition, mental health, physical health, and quality of life. ARREAZA: And yet, it still gets overlooked in many clinical encounters. Let's be honest, dealing with insomnia is not easy on patients… and doctors! MOIRA: That's the paradox. Primary care practitioners are often poorly informed about sleep disorders, which remain underdiagnosed and sub-optimally managed. In one Italian epidemiological survey, insomnia was reported by 64% of over 3,000 patients interviewed under general practitioners, with 20% reporting both nighttime and daytime symptoms. So, the patients are there, we're just not always asking the right questions or knowing what to do when they tell us about their sleep. ARREAZA: Great. Let's talk about assessment. In my experience, we need a full encounter to address sleeping issues. Patients tend to mention insomnia as you start walking out of the room. Let's say a patient tells us, "Doctor, I can't sleep," how de we approach this? MOIRA: The first step is a comprehensive sleep and health history. Clinical assessment should describe the sleep disturbance and elicit etiological and exacerbating factors. You want to understand the nature of the complaint; is it difficulty to fall asleep, difficulty staying asleep, early morning awakening, or some combination? How long has it been going on? What's the impact on daytime functioning? ARREAZA: That's why I think it should be addressed in a full encounter, if possible, because understanding the full extent of the problem requires time. We need to think about contributing factors too. MOIRA: Absolutely. Factors such as medications, medical disorders, and psychiatric disorders can all increase the risk for insomnia. You need to screen for comorbid conditions, depression, anxiety, PTSD, and chronic pain. Insomnia is actually both a risk factor for and a symptom of several psychiatric disorders. You also want to rule out other primary sleep disorders. Comorbid insomnia and sleep apnea, for example, is highly prevalent and debilitating. If someone has both insomnia and obstructive sleep apnea, treating only one without addressing the other may lead to suboptimal outcomes. ARREAZA: Now that you mention comorbid conditions, let's mention nocturia. I feel like it's very common with my older patients. MOIRA: Great point. Nocturia (waking from sleep at night to void) and chronic insomnia frequently co-exist in older adults, contributing synergistically to sleep disturbance. Treatments typically target either nocturia or insomnia rather than simultaneously addressing the shared mechanisms for these disorders. There's emerging work on integrated cognitive-behavioral treatment programs that address both conditions simultaneously, which is a promising direction. Con él hablamos debido a que Eduardo Vázquez, condenado a prisión perpetua por el femicidio de Wanda Taddei en 2010 (quien agonizó 11 días tras ser rociada con alcohol y prendida fuego frente a sus hijos), podría acceder a salidas transitorias. El miércoles pasado, el magistrado lo incorporó al "período de prueba" basándose en informes que destacan su "buena conducta", participación en programas y ausencia de riesgo mental. Sin embargo, la decisión enfrenta fuerte oposición por parte de la fiscalía, el Servicio Penitenciario Federal y el Consejo Correccional de Devoto que la rechazaron por unanimidad, argumentando que no cumple con los requisitos de reinserción. “Cuándo Vazquez asesinó a Wanda (2010) moría una mujer cada 30 horas. Ahora está muriendo una mujer cada 28 horas por violencia de género” De esto hablamos con el papá de Wanda, quien apelará el fallo, calificándolo de "espantoso" y un "premio al asesino", ya que Vázquez no ha cumplido ni la mitad de la pena efectiva cuyo mínimo es de 35 años.

Dizia São Bernardo, um servo de Maria nunca perecerá, um servo de Maria nunca perecerá, entre todas as devoções à Nossa Senhora, existe uma que se destaca, que não é como as outras, é a devoção do Santo Rosário, o terço.Essa devoção, esse terço une a eficácia infalível da oração, a poderosa intercessão de Deus, Deus não negará a salvação àquele que pedir humildemente, verdadeiramente, pedi e recebereis, é uma promessa, não é uma condição, é pedi e recebereis, pedir verdadeiramente a salvação, vocês vão receber a salvação, pedir a salvação, receberemos a salvação,pedir, por isso mais uma  vez, rezar, pedir é sinal de salvação, se você não pede a vida eterna meu caro, preocupe-se, de verdade, se você não pede a sua salvação, preocupe-se, mude a sua oração,mude a sua maneira de falar com Deus daqui para frente, se tem um bloqueio no seu coração que possa pedir a salvação, preocupe-se. E essa é a beleza do terço, ele une a nossa oração que é infalível, nesse momento, a interseção poderosa da mãe de Deus, pela oração os nossos nomes podem de fato serem escritos no Livro da Vida, não por nossos méritos, mas porque tudo que pedimos com fé o pai nos concede, mas é verdade também que nós rezamos pouco, nós rezamos mal e muitas vezes nós não podemos pedir, mas o Rosário, rezado fielmente e somado às súplicas de Nossa Senhora, nos alcança graças que sozinhos jamais nós alcançaríamos. Por isso, de maneira muito concreta nessa noite, nessa festa do Imaculado Coração de Maria, a oração piedosa e diária do Rosário é um dos maiores sinais de predestinação. Todos os teólogos estão de acordo com isso, a oração diária do Rosário, do terço de Nossa Senhora, e a oração, ou seja, constante, não só de vez em quando, mas a oração diária do terço, e o terço piedoso, ou seja, vivido na graça, em estado de graça, seguindo os mandamentos, vivendo o mínimo da nossa religião cristã, esse terço diário e piedoso é sinal de salvação. E eu vou dizer mais uma vez, é impossível que a Virgem Maria abandone nos últimos instantes da vida aquele que invocou todos os dias com confiança e piedade, pedindo e rogando a confiança, rogai por nós, Santa Mãe de Deus, rogai por nós pecadores, agora e na hora da nossa morte.É impossível, moralmente falando, que a Virgem Maria nos abandone desse momento. Por isso, o terço é sinal de predestinação, é sinal de salvação. Nós não hesitamos em afirmar, como é a teologia católica, quem reza o terço todos os dias com devoção, esforçando-se para evitar o pecado e viver cristamente, pode estar seguro de que vai obter pela mediação de Maria a graça daperseverança final. Quem reza o terço todos os dias, piedosamente, ou seja, piedoso aqui quer dizer, esforçando-se para viver na graça, esforçando-se para viver a vida cristã, vai obter com segurança, de maneira segura, por mediação de Maria, o céu. Esse não vai para o inferno, esse não vai ser condenado. E aqui se inclui, além do terço, meus caros, que é a mais bela devoção Nossa Senhora, por isso eu digo isso mil vezes, se tiver que escolher entre uma devoção a ser feita durante o dia, rezem o terço.Se tiver que escolher entre leitura espiritual e meditação e terço, rezem o terço. In this episode, Richard Boon joins us to talk about his life in music, taking in the birth of Buzzcocks, the Smiths on Rough Trade and his time as "the world's coolest librarian" in Stoke Newington. The punk instigator takes us back to 1976, when he went to see the Sex Pistols live in High Wycombe together with a certain Howard Trafford and Peter McNeish. This shock to the system led to Richard inviting them to play his art studio in Reading, plus the near-mythological Lesser Free Trade Hall show that everyone in Manchester later claimed to have been at. Once Howard and Pete re-christened themselves Devoto and Shelley and formed Buzzcocks, Richard fell into managing the group and put out their seminal Spiral Scratch EP on his New Hormones label. We then ask Richard about his move to Rough Trade around the same time as he brought them the Smiths. Clips from Martin Aston's 1986 audio interview with Morrissey form the backdrop for a discussion of why the band were special, their reluctance to film music videos and their relationship with their fans.  Following discussion of featured writer Cath Carroll, whose early interview with Morrissey and Marr plus pieces on Linder and New Order round out the Manchester theme, Mark and Jasper finish up on library highlights including Sandy Bull, Yazz, the Spice Girls, and Lewis Capaldi.
