In this episode learn about how the Generational Health Program at Sharp Memorial Hospital in San Diego was developed and expanded to create an extensive care program that provides specialised care for older adults. What started as a pilot program in 2019 is now a multidisciplinary age-friendly collaboration that begins right at admission. Host Christina Shenvi is joined by 3 experts from Sharp Memorial Hospital who provide insight into how this program operates to provide the highest level of care for older patients. Diane Wintz, MD, a trauma surgeon who was heavily involved in creating the Generational Health Program. Her goal was to change how traditional health care models approach elderly patients presenting to the ED. Kelly Wright, RN, trained as an oncology nurse and now serves as manager of the program, and Stacy Nilsen, PhD RN, who's focus is on the feasibility, implementation, education and data assessment of the program. Together they discuss the creation and expansion of the program and highlight the most important aspects of what makes it a success.

More and more, the country is devolving into health inequity—between the haves and have-nots. There's also a widening gap in nutrition, with obesity and attendant medical conditions disproportionally afflicting marginalized communities. Today's guest, Tambra Rae Stevenson, is addressing these disparities. She's a healthy food advocate and policy influencer, and a self-professed nutrition warrior. Among her initiatives is WANDA—Women and Girls Advancing Nutrition Dietetics and Agriculture, an organization building a movement of women and girls of African descent to become food “sheroes” in their communities through education, advocacy, and innovation. We discuss barriers to optimal nutrition, food “deserts”, and special needs of underserved populations. There's a paucity of trained nutrition educators of color, and Stevenson is spear-heading initiatives to incentivize development of a cadre of community-based advocates for healthy diets. We discuss the importance of involving kids early, through programs like school and community gardens. Stevenson shares her personal journey exploring her African roots, discovering the benefits of centuries-old traditions of ancestral foods; she's leveraging these insights to promote culturally-appropriate healthier food choices. On this week's episode of The Coaches Roundtable, we're bringing you another Q&A Podcast! In this episode, we answered the following questions: (Cassandra) Relationship with depression/hormones and eating are there any studies on this I have anxiety/depression and I notice when they get bad, the depression specifically, I tend to want to eat more “junk”. Even if i have food prepped and available I'll go out of my way for that junk food for comfort. I keep trying to break up with food but food doesn't get the message. How can I end this bad relationship. My food issue is and has always been starchy carbs. I crave starches like bread & potatoes. I have never been able to find an alternative that satisfies that craving. Right now I'm on Mounjaro for my blood sugar so it's not a problem because I have no appetite and and eating mostly proteins but at some point I need to break the starch curse. I'm down 14 lbs since 9-1 but I think a lot of it is because after I eat my protein and a vegetable I am not hungry due to the Mounjaro so I do not eat any starches. That is not a long term solution. Why do we self sabotage???Two seconds "feeling good" eating something we shouldn't followed by the guilt for eating it. What are we trying to fill with food? Such a vicious cycle. The genome project eventually led to the lesser-known but also very important Human Epigenome Project — health impacts beyond our genetic code. How are we impacted by environment and behavior, in addition to (or despite) the effects of our genes? Is Sweating More Better or Not? [8:41] BJ says: Robb/Nicki, Long time listener and sharer of all things Robb Wolf. Your books and podcasts are the easiest to read and listen to. I appreciate that, being a guy whose biggest hobby is learning/understanding health, but who barely survived Biology 101. Also, thank you for being open to new ideas and evolving over your career. It's reassuring to know that you and Nicki are willing to change your stance on an issue regarding a particular food, diet, politics, whatever. When I read Wired To Eat and was introduced to the term "hyper palatable", a lot of pieces of the puzzle fell into place for me. I'm a 47 year old male, 5'8", 165lbs. I've eaten all the diets over the years...that include meat. Never considered vegetarian or vegan. Been as heavy as 210+. Pretty good health markers today. Admittedly, I still pray that one day you'll promote an all pizza and french fry way of eating. Okay, so I've always liked getting in the sauna (not steam room) at the gym. I believe it provides similar benefits to cold therapy, but cold therapy is...so cold!! I've envied other who guys who would walk in and start sweating immediately (like within a few minutes). It has always taken me about 10-15 minutes to bead up and start sweating. I figured it had a little to do with how often they get in, versus me doing it a couple times per week at most. I recently had a sauna built in my home. I've been getting in it regularly for just a month, and have noticed a significant increase in how quickly I begin to sweat. Yes, I think I've got my room a bit hotter than at the gym, but not much. I'm really enjoying it and it feels great but it's got me thinking. Is sweating "easier" really a positive effect? Or, am I just convincing myself of this like how people try to believe that eating 6 meals a day is how we're supposed to eat to rev up our metabolism? That idea (so many daily meals) has never made sense to me...why not leave my car running overnight or while I'm at work so it's good and warmed up when I'm ready to drive again...just doesn't make sense. It's not causing me to sweat all day in normal circumstances. I don't feel warmer or uncomfortable through the day. In the mornings, I don't want to take a super hot shower like I always have, but that doesn't seem like a big deal. So, to sweat more or not to sweat more? Whatcha think? Celiac or not: is it worth testing to know for sure? [14:03] Becky says: I've been 100% gluten-free for the last year - ever since my DNA consult with Dr. Anthony Jay where I believe his exact words were "avoid gluten like the plague". Prior to that, I was occasionally having gluten when out (never at home) but have been eating a paleo-style diet for the last 5+ years. I've done a little research on the test for celiac which requires you to eat gluten in order to measure the antibodies in the blood. It sounds terrible to make myself sick eating gluten (even as much as I miss some good French bread) in order to confirm or deny whether or not I have celiac disease. However, I am wondering if it is worth it to be tested to confirm if I have it. Besides avoiding gluten, is there anything else I should be aware of or alter in my lifestyle if I confirmed that I have celiac disease? Thank you! 3. Sugar Effects Occurring Generations Later Theory [22:39] John says: Robb, I thought of something yesterday. If I'm reading the theory correctly, the obesity effects of sugar could take a couple of generations to manifest (i.e. each subsequent generation is affected worse as seen in increasing obesity rates). If that's so, why did the nutritional transition (~10,000 years ago) manifest instantly in height, bones, teeth, disease, etc and then the human race slowly adapted (hemochromatosis, lactose tolerance, amylase increase, etc)? It seems that the sugar generational effects theory is going in reverse of the human adaptation seen in the great nutritional transition. Cheers! She began her career as a student youth pastor and later worked as a community-based mental health therapist. Prior to joining The Summit, she worked in private practice in Brookhaven, GA. She has had the privilege of working alongside individuals, couples, and families dealing with relationship issues, grief and loss, life transitions, addiction, and various personal concerns such as anxiety, depression, OCD, and other behavioral challenges. As part of her work, she is committed to working with individuals who identify as LGBTQ+ or anyone questioning/exploring their gender identity. Kelly utilizes an integrative approach with her clients to explore the negative effects of stressful or traumatic life experiences that get in the way of achieving their goals. She has advanced training in trauma-informed care and evidence-based practices. Through her work with children and adolescents, she has found that understanding attachment and the caregiver relationship is vital to helping families function more effectively. Caregivers are incorporated throughout the course of therapy. A lifelong learner, she attends many continuing education workshops each year and reads books and professional journals related to mental health, therapy, nutrition, and wellness. She has also facilitated grief support groups and co-presented trauma-informed workshops to other mental health professionals. Sharmen May Gowens, YWCA of Greater Atlanta’s mission-driven CEO, is committed to “eliminating racism and empowering women and promoting peace, justice, freedom, and dignity for all.” After serving as interim CEO for one year from 2012-to 2013, she accepted the call as CEO in 2015. She is committed to supporting and enriching our community. She currently serves on the Board of the Atlanta Metropolitan College Foundation; previous board affiliations include YWCA of Greater Atlanta, the Atlanta Business League, and the Better Business Bureau. She was named one of Atlanta’s Top 100 Women of Influence for 10 years starting in 2006 by The Atlanta Business League and is now a Tenured Woman of Influence. Dr. Amelia Burke-Garcia is a seasoned health communication professional with nearly 20 years of experience in health communication program planning, implementation, and evaluation, with specific expertise in developing and evaluating digital and social media communication and research. At NORC (National Opinion Research Center), she leads the organization's Digital Strategy and Outreach Program Area, where she designs and implements strategies that leverage the power of digital media to influence behavior. In this role, Burke-Garcia develops new digital and mobile data collection methodologies and communication solutions. Most recently, she acted as director for the award-winning How Right Now/Que Hacer Ahora campaign, which is aimed at increasing people’s ability to cope and be resilient amidst the COVID-19 pandemic. Amelia currently leads two grants focused on exploring vaccine hesitancy amongst communities of color which build on her earlier work exploring messages and motivations of vaccine-hesitant or refusing social media influencers (findings from which were published in Vaccine in 2020). Over the course of her career, Dr. Burke-Garcia has spearheaded some of the most innovative communication programs and studies on a variety of health topics including designing a targeted social media intervention with mommy bloggers to help social media users lower their risk for breast cancer and leveraging MeetUp groups and the Waze mobile application to move people to action around flu vaccination and HIV testing, respectively. Amelia is the author of the book entitled, Influencing Health: A Comprehensive Guide to Working with Online Influencers and has been named to VeryWellHealth.com’s list of 10 Modern Female Innovators Shaking Up Health Care. She holds a Ph.D. in Communication from George Mason University, a Master’s degree in Communication, Culture, and Technology from Georgetown University, and a joint honors Bachelor’s degree in International Development Studies and Humanistic Studies from McGill University. Our youngest consumers, Gen Z, are much more aware of the health risks of fast food, stress, sugary drinks, various medications, and social media than previous generations. This awareness has been leading them further away from choices that negatively impact their overall health.
