Dr. Carl Dieffenbach ( https://www.niaid.nih.gov/about/carl-w-dieffenbach-phd ) is the Director of the Division of AIDS at the National Institute of Allergy and Infectious Diseases, part of the U.S. National Institutes of Health. Dr. Dieffenbach has oversight of a global HIV/AIDS research portfolio of over one billion dollars and a staff of over 150 federal employees and is responsible for planning, implementing, managing, and evaluating programs in 1) fundamental basic laboratory research, 2) discovery and development of therapies and treatment strategies for HIV infection and its co-infections and/or complications through basic research and clinical trials, and 3) discovery and development of vaccines, topical microbicides, and other prevention strategies through basic research and clinical trials. Over the years Dr. Dieffenbach played a key role in restructuring the DAIDS-supported clinical trials research networks and has actively fostered collaboration and partnerships with other federal agencies, international research organizations, professional societies, foundations, community advocacy groups, and industry. Dr. Dieffenbach received his bachelor's degree in biochemistry from the University of Maryland and his Ph.D. in biophysics from The Johns Hopkins University and completed his postdoctoral fellowship with the Uniformed Services University of the Health Sciences (USUHS) Department of Pathology. Mental health research in the past several years is further exposing and emphasizing the critical link between a child's growing up experience and their eventual mental wellbeing (or lack thereof!) in later life. Studies show that how a little one is raised, for good or bad, shapes their overall perspective and their coping mechanisms that they will use to combat stress, suffering, and relational problems as they age. Many people who later seek counseling or therapy acknowledge that their issues stem from negative childhood patterns and experiences. The first part of today's episode focuses on my guest Justine's fertility journey which culminated with her pregnancy at age 43 with twins. The second part focuses on Justine's professional work in the area of gluten sensitivity, Celiac, and fertility. Justine Bold has worked as an academic in the UK at the University of Worcester since 2008. She is now also Program Director for CPD in the Medical School at Cardiff University. She has written articles on infertility, appeared in the media and coedited a book entitled ‘Integrated approaches to Infertility, IVF and Recurrent Miscarriage' that was published in 2016. She has published widely in academic journals on gluten and celiac disease and also on the integrative management of female reproductive health conditions. She is working towards PhD by publication having also co-authored a book on Mental Health in 2019 and recently contributed chapters to an upcoming text book ‘Gluten related disorders' to be published by Elsevier. Justine had a long personal journey to motherhood starting with a pregnancy loss in her early thirties finally becoming a mum in her forties. She is passionate about raising awareness about the infertility experience and in helping healthcare professionals to understand patients and clients to improve patient care and she is currently working on research projects in this area. She also appeared in the UK at Fertility Fest in 2018 and at the Fertility Show in Manchester in 2019. Mark is the founder and CEO of Organic Pastures Dairy Company, a leading California and national brand of truly raw dairy products including milk, butter, cream, kefir, truly raw cheddar cheeses. Founder and chairman of Raw Milk Institute and is an internationally recognized speaker and expert with an emphasis on Raw Milk production standards, gut biome, milk genomics, nutritional benefits, food safety and the medical benefits of raw unprocessed milk and products. Mark is deeply involved with raw milk food safety research and is associated with the International Milk Genomics Consortium at UC Davis. Denise and Andrew collaborated on a way to combat loneliness in college students. Show Summary Denise and Andrew had very different entry points into design. Denise’s journey began with a love for people and cultures. She started her undergrad as an anthropology student, but she wanted to not just study culture, but to shape it. That led her into design. She studied product design at the Illinois Institute of Technology, and landed an internship at IDEO, where she ended up staying for eight years while also teaching design at the California College of Arts. Denise opened her own design practice and started doing design research into younger generations — not just designing products for them, but also working to understand their way of seeing and experiencing the world. Now, she works at Hope Lab, where the focus is on creating digital technologies that help young people live happier, healthier lives. Andrew’s interests were influenced at an early age by his father, a graphic designer, and his mother, a civic leader focused on social impact. He studied business and English literature at the University of Colorado, but also minored in technology, arts and media, where he studied software development and honed his self-taught graphic design skills. An internship at a Denver agency allowed him to continue developing that skill set, but also gave him the opportunity to dig into user experience and into understanding human behavior and using those insights to guide designing product solutions. He moved into a dual role with Cactus and Grit Digital Health, leading both companies’ creative technology practices before moving into a full-time position at Grit Digital Health, where the focus is on creating digital health solutions for college students designed to help them improve their mental health and wellness. Habits for Health Creating habits for health is probably the very best thing you can do in life! Wow, that sounds extreme, but it is 100% true. “Once begun, half done,” as the old saying goes, is true of anything. And the truth is that, if you don’t eat right, it you don’t exercise, if you don’t sleep a solid 8 hours per night, and so on- you are not going to be healthy. And, by not being healthy, you will be destined, over time, to be unhappy as well. That’s just how life works, grasshopper… Even if you are very successful, say, in your career, and make lots of money- if you are unhealthy, and don’t have a fulfilling family life and social life, along with a well balanced spiritual life- you lose! You lose the happiness prize! The good news is that all of these pillars of a healthy, fit, happy life are all derived simply from developing good habits. And developing good habits is a pretty straightforward, and not really difficult endeavor. Step 1 is simply to figure out what good habits are essential for health: 1. Eat a healthy, paleo type of diet. Most people find this the hardest to do, so start here. Start with a big breakfast, so get up plenty early to make it, along with packing a good lunch. Do not have any grains with your breakfast, such as toast. Also, no juice, which is just sugar water. A green smoothie is ideal, but you can just start with eggs and bacon and/or sausage, and coffee. If you have no problems with dairy, load that coffee with cream! And remember- don’t limit yourself as to how many eggs or how much meat you can have- eat as much as you desire- it’ll keep you full. Do this every day, for a solid week. Then, go for another week! Each night have meat with vegetables for supper, and sometimes have a sweet potato or white potato, loaded with sour cream and butter. Or perhaps some white rice with your pork chop. Just no grains at all, and don’t drink milk, especially not skimmed. Cottage cheese, full fatted, is also great for meals or snacks. Now, do it for a third week. You should never let yourself be hungry, ever! Stay satiated, and happy with protein and especially lots of good fat. Never touch margarine or other industrial seed/vegetable oils- eat the real stuff! Congratulations- that was the hardest part of the whole habit making and changing strategy. You now follow a paleo type of diet, and after 3 weeks you should feel and look good enough that you will never desire to give up this healthiest of all habit. 2. Keep up with your diet habit, but now add in the sleep habit- go to sleep each night early enough that you get a solid 8-9 hours of sleep per night. You could do this first, making it easy then to get up early and make your food for the day, but it is your choice. The food changes make the most obvious improvements, so I’ve started there, but it’s your choice. Keep this up for a couple of weeks, and you will be ready for the next step. 3. Exercise! This is also a tough one for folks to whom it is new, but it is as important as your nutrition and sleep. You may want to go to bed even a little earlier, just to make sure you have the time to exercise in the morning. I don’t want you to do a lot- in fact, it is key that you do not! I want you to start with 10 minutes of exercise, all done in your living room. Turn on the TV, Netflix or Amazon Prime is ideal since they pick up playing right where you left off last viewing. As the show plays, I always start with virtual resistance exercise, which is where you flex each muscle group through its whole range of motion for reps. For instance, you can do the “high reach”, where you flex one arm at a time as high as you can reach, flexing through the whole range of motion for, say, 8 reps. Next, do flies, where you cross your arms as if doing dumbbell flies in front of your chest, flexing with internally generated resistance. Then, pulldowns as if chinning yourself, flexing all

5/5. The NHS is under huge strain as it struggles to cope with an ageing population and illnesses caused by unhealthy lifestyles on top of increasingly expensive drugs, procedures and treatments. It's an unsustainable situation, says former NHS chief executive Nigel Crisp, and requires a complete rethink of the way we think about and manage health. In the final programme of the "Healthy Visions" series Lord Crisp develops the ideas covered in the previous four programmes to argue that all of society must take responsibility for health. Citizens must become partners in health promotion and health care. Planners and designers must put health at the centre of new projects. And the NHS must change from a hospital-centred and illness-based system to one where patients and communities are in the driving seat. Lord Crisp visits the St Paul's Way Transformation Project in Tower Hamlets to see how housing, health care facilities, education and leisure can be planned to promote good health and prevent disease, with a particular focus on preventing diabetes.
