In this episode, Sachin interviews Dr. Peter Osborne, his first mentor. Sachin met Dr. Osborne in California at a conference Dr. Osborne was hosting, called Market Functional Medicine. Sachin went to the event after downloading a lead magnet e-booklet from Dr. Osborne on how to build your practice. Sachin learned about the conference from an email. That conference changed Sachin's life. It was wonderful for Sachin to hear someone who was a few years ahead of where he wanted to be, so open and willing to share his knowledge, holding nothing back. Sachin got some wonderful advice that he immediately applied to make an instant difference in his practice. [4:23] How has Dr. Osborne found passion to keep tapping into and how has he continued doing it all these years? Dr. Osborne says the passion found him. It came from the lack of knowledge, empathy, and discernment that he saw in medicine in his exposure to medical environments while in chiropractic school. [6:36] Functional medicine practitioners have to be great clinicians as well as great at business. Hospitals and medical doctors aren't going to refer patients, so you have to understand business. Less than one percent of people will see a functional medicine practitioner. Let's change that so we can help more people. Dr. Osborne is passionate about helping people. [7:37] In Dr. Osborne's view, if you're not passionate about helping people you should leave the practice. The way to be successful is to love people and have great empathy, great concern; that you care, and that you're going to do what it takes. You're going to spend more than a 20-minute appointment with a person if they start crying and you need that time to help them. [8:14] One of Dr. Osborne's mentors taught him to help the people and the money will follow. If you go for the money, you'll never be happy. [10:01] It can be challenging to keep up with the literature. There is so much you can learn on the internet from others who have different expertise than you. Your patients have the same access to those interviews as you do. Dedicate time for study. Learn how to learn. Humbly understand that we don't know what we don't know. Continue to learn more. You'll get better. [11:52] Sachin talks about things he's learned over the years, like breathwork. Dr. Osborne advises you to get really good at something first before you try to get really great at everything. Dr. Osborne is the master of gluten. That's what people know about him. The biggest change in his clinical life was understanding that topic better than anyone else. [13:21] After mastering gluten, Dr. Osborne broadened his horizons. He organizes his week into blocks. Monday through Wednesday is his clinical schedule. On those days, he is not studying extra. Thursdays and Fridays are filled with blocks dedicated to research, writing, interviews, running his business, thinking about his business, and his family, meditation, and exercise. [14:02] It boils down to just being organized and having intent. Thinking about what it is that you want. So many people want to be successful but they don't think about what needs to happen for that success to happen. Organization has to be a big part of your thought process. If it's not, there are too many distractions and shiny objects that will pull you in. [14:28] A mentor of Dr. Osborne, Craig Ballantyne, said “Stop it! You're going in too many directions. Just stop. Think about the direction you need to be going. What is the one thing that you can do today that serves your clientele that will increase your revenue? Focus on that. Everything else is just a distraction.” Identify first what will grow your business and then do it. [15:39] Once you identify the needle movers, structure your day around those priorities. Prioritizing what you do is as important as taking action. This good advice made Sachin think of things he is avoiding and the things that are distracting him! We all fall into traps if we don't have systems to guide us. View your business as an entity that has needs, like a plant. [17:25] Dr. Osborne sees certifications and additional degrees as shiny objects. You don't need them for credibility. If you're getting people better consistently, that is your credibility. Dr. Osborne has a DC license, but he practices under his pastoral license and nobody questions it. People are seeking a new model because medical doctors failed them. Focus on outcomes. [21:27] How does Dr. Osborne stay so committed to the topic of gluten? Dr. Osborne attributes it to the outcome. Dr. Osborne states there's no diet that helps people more than the gluten-free diet, done properly. Dr. Osborne can stay passionate about great outcomes. If you're not helping people it shows up in your practice. [23:19] To optimize time in his practice, Dr. Osborne automated the educational process about gluten. He switched from one-on-one meetings to a webinar with 20 people in a room. It's a 90-minute webinar and for the last five minutes, Dr. Osborne answers questions live. That shaves 20 hours off his week for five minutes of questions. [23:12] Learn. Do. Teach. So we learn something that excites in functional medicine, then apply it in our practices, and do it. And then, as we're doing it, we're teaching it to our patients. And that's where mastery comes. Once you're past the teaching aspect, anytime you can automate the process but still deliver the value of the education, use the technology to do it. [24:45] Dr. Osborne explains how automation has made him so successful. He wanted his clients to get him, not a coach or nutritionist he hired to mimic him. [25:26] Sachin cites a book written about Naval Ravikant. Naval talks about four levers you can pull in business: human capital, money capital, media, and code — taking your process and codifying it so it can be replicated in other clinics. The third and fourth levers are the most powerful ones. Sachin observes that Dr. Osborne is doing well with those two levers. [27:22] Sachin suggests that if a practitioner has a six-month waiting list, the practitioner may be successful, but people are suffering unnecessarily for six months to be seen. If you have a waiting list, pull the levers of media and code to serve your clients sooner. [27:57] Technology provides even more than a one-on-one session, Dr. Osborne points out. The clients get access to a recorded question and answer session, with five minutes of live questions at the end, and also have access to the replay of the session. The spouse can also watch the replay. It also saves your staff from being asked a lot of questions. [29:05] In functional medicine, you're delivering educational value to people so that they can spread their wings and fly. You're trying to fire your patients. No other medical model has that in mind, but we do because we want them to get better. If we educate them, they can fly. Part of that education is staff time. Now, staff can refer patients to your recordings to answer questions. [31:39] Besides the degrees and diplomas, functional medicine practitioners might be distracted by things that they shouldn't be focused on. If you're new, think of every way that you can leverage yourself as being your first priority and not an afterthought. If you don't have staff, things like maintenance that shouldn't take your attention distract you from your clients. [32:46] If Dr. Osborne could do one thing over, earlier in his practice, it would be to leverage himself better. Have time in your schedule when you can think about your business, how you can improve it, and how you can leverage yourself. That's what's going to save your resources of time, money, and relationships. The website is the leverage piece. It houses your content. [33:55] The more you spend that time leveraging yourself to serve your communities, the easier it gets. At first, it's a lot of work. Dr. Osborne has over 1,500 videos and over 1,000 blog posts online. Those took time to write and produce. With these leveraged, your business seasonally gets better and richer as a result of higher service. [34:51] Dr. Osborne warns against relying on AI to give you informative, reliable articles. He has tested it a lot. It made up false references, after very specific instructions about the article. Every footnote, every time, was fake. Remember, your brain is your number one asset. Outsourcing your brain to an artificial source to emulate what you could put together will “dummify” you. [36:56] A false source can certainly damage your reputation. Business is about reputation management. Don't rely on AI for your business. You could destroy your reputation. Do your own work. Jim Rohn says you can't have someone do your pushups for you. Dr. Osborne will use AI for the framework of an article, then turn it over to a writer to research it and fill it in. [40:07] Dr. Osborne recommends you navigate and find your strengths. He is good with video and ad-lib, so he would rather make videos than spend hours researching and writing blogs. [40:55] Dr. Osborne reveals his current work on an affiliate program for healthcare providers to use his DNA testing services through Gluten Free Society, his hub. Your clients and patients can be DNA tested for both gluten sensitivity and celiac disease. He offers DNA interpretation. [49:02] The third thing Dr. Osborne would do, starting over, would be to leverage himself a lot sooner. He would have hired a lot sooner. He and his wife, practicing together, went too long without bringing in help. Leverage yourself first with technology, then with people. Dr Osborne. How are you? Peter 01:42 I’m doing fantastic. Thanks for asking. How about yourself? Stu 01:45 Yeah, really, really good at. It’s 8:00 on a Friday morning, so I should be a little bit dusty, but I’m feeling okay. I’ve had a good breakfast and I’m ready to go. First off for our audience that might not be familiar with your work, I would love it if you could just tell our listeners a little bit about what you do and perhaps why you do it as well. Peter 02:07 Yeah, so I started out about 18 years ago clinical practice. I left the VA Hospital here in Houston, Texas after a very disgruntled stint in the rheumatology department, and I was very disgruntled with the way that people were being treated. They were being treated with high levels of immune suppressants and steroid medications and surgical joint procedure replacements. And to me it was not a solution, it wasn’t an answer, nobody got better. It just seemed like a system. And I’m a veteran myself. So having these veterans come through the hospital and not being treated properly, it really didn’t sit well with me. So when I started my own private practice, one of the things that I made it a point of was to implement diet, lifestyle and nutrition into people with chronic autoimmune problems, because that’s where I was. 02:57 At VA Hospital, rheumatology, it’s rheumatoid arthritis, lupus, Scleroderma, dermatomyositis. Diseases like psoriatic arthritis and spondyloarthritis. So joint pains, muscle pains, chronic autoimmune in nature. And there just wasn’t a real viable medical option outside of immune suppression, which to me was not a solution. You can also look at what the immune system uses, Vitamin A and D and Zinc are key for regulatory actions The immune system's proteins need methylation, so B6, B9, B12 are key for that as are Vitamin C and E as antioxidants Iron, Copper, Magnesium (maybe more minerals) All these have key role in the immune system, so it's unsurprising that these deficiencies are linked with diseases Question 2: If my children (an older daughter and a baby son) never try gluten and dairy, how will I know if they have an intolerance or Celiac Disease? Stacy: we decided to just assume that our kids have it, given our family history, and to not test If it's important for you to test Celiac, you'll have to expose your kids Sarah: chances are very good that your daughter (who was never exposed) will rebel and eat some gluten eventually, and it's ok to wait for that challenge Stacy: reminder that some people can have issues with gluten and no physical symptoms. Stacy and Cole just get emotional and depressed when they're reacting They can't tell you to fix anything if you DO get a positive result—you just have to avoid it anyway. Sarah: research shows there's a lower risk of Celiac if gluten is introduced while breastfeeding, and the longer you breastfeed the lower the risk, so introducing to a baby is more backed by research In the same situation, Sarah would probably include a little wheat a couple of times toward the end of breastfeeding (around 2 years) so they can communicate whether something is wrong In peanut allergy studies, babies were fed a very small amount to help reduce allergies, so it doesn't have to be a huge serving Question 3: Jessica's son did the cheek swab DNA test, with her second child she's just said "we have Celiac." Is the DNA test helpful? Sarah: studies of people with Celiac susceptibility genes, HLADQ2 and HLADQ8, have shown that people without Celiac who also have digestive symptoms still have a zonulin response to gluten. They're still getting a leaky gut in response to gluten, even though it's not Celiac 60% of the population has one or both of these genes, which explains non-Celiac gluten sensitivity Having one of those risk genes is a compelling argument not to mess with Gluten, because of the risk of a leaky gut reaction Question 4: The Dr. from the Gluten Free Society's website claims there are two other genes associated with Gluten sensitivity, HLADQ1 and HLADQ3, which no standard Celiac test looks for. Any other research on this? Sarah hasn't seen anything on that, but she's never specifically looked through the research for that 3% of Celiacs don't have HLADQ2 or HLADQ8, that 3% could have that other selection of genes Already more than a dozen HLA variants linked with autoimmune diseases Question 5: Can Sarah share her Paleo road trip snacks? Matt: tap water is one of the greatest inventions of mankind. The fact that we have potable water delivered to our house for cheap, any time we want, is great. It goes through so much processing to get to potability in our houses that he really feels like honoring tap water by drinking it Sarah She agrees, but a lot of the members of her family have chloramine sensitivity (her brother missed a year of school due to this sensitivity) so Sarah has always used at least a Brita filter Her municipal water is Dasani so that's basically what they're drinking if they use a charcoal filter Sarah recently won a reverse-osmosis filter, but she feels like she has to add back in so many minerals to make it a helpful source of minerals Sarah thinks we should remineralize our tap water - she uses Trace Minerals and EM Drops Mostly adding Magnesium, Calcium, Potassium and some trace minerals. Most people (yes even Paleo peeps) are mineral deficient She's adding probably up to 20% of her family's RDA to their water Reminder: Sarah comes from socialist Canada and sort of automatically trusts the government, but recognizes that doesn't necessarily reflect the American experience Pay attention to stuff, there's plenty of contaminated water out there, so you might want to make sure your municipal water is safe BUT she thinks tap water is fine Matt When he was growing up in Mass., well water is pretty hit-or-miss, so his dentist prescribed fluoride to everyone He and his brother (who did not have well water) had stained teeth from too much fluoride So, it's just a matter of knowing what's in your water Sarah There's not a lot of evidence that fluoride is causing health problems, but also little evidence it's helping dental health Fluoridated toothpaste, on the other hand, has some really compelling science backing its efficacy There's some science to show that fluoride might sequester in the pineal gland and over 60, 70, 80 years decrease the amount of melatonin secreted by that gland, linked with sleep disturbances in the elderly, but that's sort of a big leap at this point But you can take that out with a normal carbon filter if you are concerned Rate and Review us! This week we bring back regular guest, Dr. Jerome Craig, to teach us all about living a gluten free lifestyle. As you may know, Dr. Craig is the voice, vision and wisdom behind many of our Fitlandia webinars. Today we’re talking about being gluten free and asking if it is just another diet fad or truly beneficial for our health. If you’ve listened to previous Fitlandia podcasts you know we’re passionate about bringing credible, science-backed information to our listeners - no more fads, no more diets, no more quick fixes. So, this gluten free thing is a hot topic right now, and was initially thought to be healthy. But now there’s a backlash about the research, or lack thereof about the health benefits of eating gluten free. So, we posed this question to Dr. Craig, “Do you believe being gluten free is a diet fad?” He tells us that it’s become a very popular way of eating. The statistics show that 20 million Americans are eating gluten free and a third of those people are trying to avoid gluten. It’s very much front and center in today’s culture. So, is it a fad? Yes, no and maybe. It is a necessity for everyone? Yes, no and maybe. Dr. Craig states, it’s important to look at the science. It’s hard to say, one way or another, exactly what being gluten free or not will do for your body but a diet concentrated mostly on any grain isn’t going to be the most nutritious. The best thing about grains is how much fiber they provide. Fiber is good for us, helping to maintain blood glucose levels but also creates digestive distress for many people and that’s when the gluten argument gets a little foggy. So, what is gluten? Typically, we know of gluten as the protein found in wheat, barley and rye. Gluten is made up of two molecules that are added together; a larger, polymeric glutenin and a smaller protein, gliadin. When combined, they create a lot of stickiness. As wheat is ground and kneaded into dough, it becomes very elastic which is popular in bread making. The air molecules within the dough are trapped, making the bread strong and fluffy. This is where the science and the argument come in - the gluten free market place is a huge industry now. Dr. Craig tells us, “The gluten free market has more than doubled in sales between 2011 and 2016 and today is a $15 billion market.” Although thriving, these products are not necessarily better for you, just because they are gluten free. Most of these baked goods are corn or rice based and don’t provide us more nutrition. Gluten Free Society posts a variety of glutens. Their argument is with 400 glutens occurring in nature; only 40 of those exist in the human food supply. Most seeds are made with components that are meant to be protected and not digested. These seeds are grown so animals eat and spread them to reassure regrowth of the plants. For optimal health and digestion, it is recommended to not only watch wheat glutens, but all grains because every grain has gluten in it. That’s right; gluten is not just in wheat, rye and barley but also rice, corn and millet which are used in gluten free foods. These all have a group of plant storage proteins, called prolamins, which for most of us are not easily digestible. If we look back at traditional ways of making breads, they used to ferment it. These grains required time to absorb water and the fermentation process gave the bread the gluten-like texture we know and love. They Don't Make 'Em Like they Used To Today, however, commercial bakeries use a gluten concentrate in creating doughs. This modern way of milling flour strips the nutrients, leaving the starch and then the concentrate is added to make the bread fluffy again. This is why eating “gluten” causes intestinal distress for so many people because our immune systems and guts are reacting to it. Additives such as coloring, sweeteners, shortening and bleaching agents are added to commercial bread products and are not usually labeled because they’re not technically food ingredients. No wonder our bodies are inflamed! One of our big issues with gluten today is with studies of Celiac; a serious genetic autoimmune disorder. Celiac provokes problems with the immune system if gluten is consumed. Most celiacs have to follow a strict gluten free diet. Even non-celiac people may have sensitivity to wheat/ gluten because many ingredients in wheat can be problematic. The best way to feel better is to avoid eating these types of foods for a period of time, and there are certain probiotics that may help people digest gluten. Dr. Craig points out, “If you eliminate all grains from your diet, you’re going to change your body’s microbiology. This colony changes when you change what you eat, and when a certain food is eliminated and then brought back into the diet later – there is likely to be difficulties with digestion.” Studies do show people who suffer with gut inflammation, leaky gut or IBS greatly benefit from a grain free diet. Eliminating grains not only helps us cut out wheat glutens but we naturally incorporate more fruits and vegetables into our diet instead of processed foods. This gives us greater health benefits by increasing our nutrients and helping to manage blood sugar levels. It’s not about replacing the gluten with other non-gluten baked goods but transitioning into more of a “paleo style” way of eating by removing all flours and adding in more root vegetables and nutrient dense foods. We need to listen to our bodies. When we slow down and pay attention to how our body feels, we can use that as a guide for dietary choices. The easiest way to see what might be causing you inflammation is to take a break from it, see how great your body feels, and introduce it back slowly later if you choose. Tips on Reintroducing Foods Back Into Your Diet Grains for example: • Look up fermentable ways to make your own bread so you know exactly what you’re adding in to it • Start with consuming small amounts and see if you feel any gastrointestinal distress • Probiotics which help with digestion – some in the bacillus or lactobacillus family or any lactate fermenting will help break down these foods. With so many people suffering from gut inflammation and borderline immune dysfunction, its no wonder that being gluten free has become so popular and is seen as a trending fad. People who experience food disorders or GI distress tend to benefit from a low FODMAP diet. A low FODMAP diet, or FODMAP elimination diet, refers to a temporary eating pattern that has a very low amount of food compounds called FODMAPs. The acronym, as described by co-creator Sue Shepard, stands for: • Fermentable – meaning they are broken down (fermented) by bacteria in the large bowel • Oligosaccharides – “oligo” means “few” and “saccharide” means sugar. These molecules are made up of individual sugars joined together in a chain • Disaccharides – “di” means two. This is a double sugar molecule • Monosaccharides – “mono” means single. This is a single sugar molecule • And Polyols – these are sugar alcohols (however, they don’t lead to intoxication!) This way of eating is primarily beneficial for people with GI problems because any fructans, the fructose molecule found in wheat are removed as well as plant fibers which lead to bacterial overgrowth in the gut. Leaky gut can also be caused by a gluten dense diet resulting in a buildup of antibodies to the hormone, Zonulin. This hormone, which acts like a border control in the gut, opens up gap junctions allowing large particles to pass through. However, gluten stimulates the release of Zonulin which can promote leaky gut. How do glutens affect autoimmune disease? Protein molecules that define a grain may be mistaken by our immune system to look like the digestive tract called molecular mimicry. Hyperpermeability, or the increase of permeability (leaky gut), is at the heart of immune dysfunction. Protein leaks out of the digestive tract directly into the blood stream which creates a negative response in our bodies. This can be caused by food particles or toxins that make it across the bacteria that lines the gut. Our bodies become hyper-vigilant and when the immune system dials in with an unrecognizable substance in the system, our bodies can start attacking its own cells. To help with this, we want to broaden the spectrum of probiotic foods in our diet, since we tend to miss out on nutrients if we eliminate food groups for long periods of time. For people who are trying to reintroduce foods back to their diets, Dr. Craig suggests starting with fermented foods which are easier to digest. A small amount of toxins help to keep our immune system alert and makes it easier to adjust after detoxification. As you start reintroducing foods look for: • Family history (especially celiac) to try and avoid gluten and grains • Gut inflammation (heart burn, IBS) correlations with food allergies • Intestinal permeability (many food sensitives) you will start to react to more foods • Skin issues (eczema, psoriasis, discoloring, bruising) • Digestion (gas, bloating, diarrhea, constipation, heartburn) • Brain function (brain fog, hard time concentrating, fatigue after meals which are all associated with immune function and a difficulty regulating blood sugar and insulin levels. • Anxiety / Depression – inflammatory process which grains tend to feed into this response For more on how grains affect our neurological health, check out Grain Brain by David Perlmutter.
