Most digital media is designed to be addictive, and young brains are especially susceptible. Successful digital media content is stimulating, engaging, and targets the brain's natural reward centers. This creates an unfair advantage over young brains, which have not yet fully developed their executive functioning. In today's episode, Dr. Amanda Giordano introduces us to the “Four C's” model of addiction, healthy vs unhealthy engagement with digital media, and interventions for youth and their families. She co-coordinates a national network of school counseling faculty, was president of the Association for Child and Adolescent Counseling (2020-2021), and represented Virginia at several White House School Counseling Convenings under First Lady Michelle Obama.   She recently was elected to the American Counseling Association governing council (2022-2025). She is on the editorial review board for multiple journals related to school counseling, including ASCA's Professional School Counseling journal. Finally, she is proud to be a recipient of the 2020 American Counseling Association Research Award, and the 2018 Insiders Business: “Top 40 Under 40” recognition.     She lives in Virginia with her spouse and three spunky young children. Therapy is about as much of the American experience these days as baseball, pickup trucks, and apple pie. Professional counseling is now seen as more than just a last resort for psychological distress, but as a healthy, essential path for resolving personal issues. In 2019, nearly 20% of Americans received some form of mental health treatment ranging from medication to therapy. Over 40% of Americans have seen a counselor at some point in their lives.   Recently in the New York Times, journalist Susan Dominus asked an important question, especially given that the U.S. is in the grip of an ever-worsening mental health crisis: “Does therapy really work?”   On one hand, dozens of studies confirm the value of talk-based therapy. A landmark 1977 study, for example, found that those with significant psychological distress “fared better than 75 percent of those with similar diagnoses who went untreated.” University of Wisconsin-Madison researcher Bruce Wampold put it, “the fact that you can just go talk to another human being … and get effect sizes that are measurable” is kind of miraculous.    Other research, Dominus explains, is less clear. A 2021 study found that more than half of depression patients saw little or no benefit from talk therapy, and only one third found their depression receding long term. Another study found that only 50% of patients responded to cognitive behavioral therapy regarding anxiety disorders. The uncertainty has led some to push for alternative treatments, including more prescriptions of drugs like psilocybin, the active ingredient in so-called magic mushrooms. One prominent researcher mused, “Maybe we have reached the limit of what you can do by talking to somebody.”  Of course, the results of therapy depend on a number of factors. While counseling is a powerful tool, it can only aim at the question, “What's going on inside of me?” Often missed, which is especially consequential for a culture in a crisis of mental health, are the fixed reference points outside of ourselves by which we can be known and orient who we are.   Psychology is one of the many areas of modern life that has taken what sociologists call “an inward turn,” characterized by radical individualism and reliance on self-definition. Rather than pursue healing or the restoration of relationships, counseling can devolve into endless rounds of affirmation, a sort of perpetual re-baptism in the church of self-expression. This is just one way that therapy has replaced religion for many seekers. Self-discovery is the new salvation, and therapists the new priests. The key feature of psychology as religion, however, is the self as the new deity.   This has only enabled, as Lisa Selin Davis observed recently at The Free Press, so many of the West's top schools and institutions to embrace and employ Critical Race Theory rhetoric and LBGTQ politics. The American Counseling Association now divides counselors and clients into either “privileged” and “marginalized” groups with a dedicated script for each and little mercy for those who dissent. More states have passed so-called “anti-conversion-therapy” laws, which threaten professionals who do anything other than only affirm a client's proposed gender identity.   As a result, deeper mental health issues are never addressed, and anyone who speaks up can find themselves out of a job. One therapist in training put it, “My concern is that we're not helping people heal and transcend. We're just helping people live in their victim mentality.” In a tragic irony, the inward turn has made it harder, not easier, for the struggle to know themselves.  There are some, many of them Christians, striving to rethink psychology and counsel others by looking outward as well as inward, to know themselves by first knowing what is true and good. We can only know ourselves by first knowing reality, ultimately God and the world He made. This week we are talking about trauma-based relationships and how to develop healthy connections with others. Join Hannah Jenarine, Education and Prevention Coordinator as she discusses trauma-bonds, red flags, and self-awareness with Leslie Pender. Leslie Pender is a Licensed Mental Health Counselor (LMHC) in the state of Florida and the private practice owner of Seeds of Transition Counseling Services, LLC which is in the Metrowest area of Orlando, FL. Ms. Pender earned her Bachelor of Science degree in Social Science with a concentration in Sociology from the University of Maryland University College and her Master of Science degree from Capella University in Mental Health Counseling. It was during her residency for licensure that Ms. Pender began to focus her research on social justice advocacy and cultural competency within the mental health counseling field. Ms. Pender's work as a LMHC has consisted of collaborative work with the Florida Department of Children and Families and other agencies in which she has advocated for practices and policies that provide mental health services for children and families within the foster care system from a trauma-informed care and culturally competent perspective. Ms. Pender is a member of Chi Sigma Iota (International Honor Society for Counseling Students, Counseling Educators and Professional Counselors), the American Counseling Association, the Florida Counseling Association and Black Therapists Rock in which she engages with other counseling professionals locally, nationally and internationally to help eradicate the stigma as it relates to mental health and mental illness, especially within communities of color. As the wife of an Army veteran, Ms. Pender is an advocate for increasing awareness of the impact of post-traumatic stress on the marital relationship and family dynamic of the veteran. In addition to providing therapy services, Ms. Pender provides and has conducted workshops on self-care, stress management, women's empowerment and guided vision planning. Welcome back to Therapy Chat! Continuing the theme of tending to our own needs, today's topic is one that's important to therapy professionals and to first responders, too. To anyone who works with people who are suffering, Vicarious Trauma is important.   Host Laura Reagan, LCSW-C revisits a past episode in which she speaks about Secondary Traumatic Stress and Vicarious Trauma after attending a workshop on this topic by Laura Van Dernoot Lipsky, the author of Trauma Stewardship. This week is Part 1 and next week, in part 2, we will hear from Laura van Dernoot Lipsky herself! Here's what you'll hear in this episode: The terms Vicarious Trauma and Secondary Traumatic Stress are interchangeable. Therapists bear witness to the traumatic stories of clients and are affected by them. The nature of therapy work requires empathy; it's honorable, brave, and important work intended to make the world a better place. There are small ways to lessen the impact of trauma, by mindfully checking in with yourself and using positive coping methods. As a therapist, how much are you “numbing?” We discuss examples. Laura recommends spending 12-60 minutes each day, for six days a week, working out to the degree of breaking a sweat. We owe it to the people we help to take care of our Vicarious Trauma, and regular exercise is one way to do that. Isolation is common in trauma work, because we feel like “nobody understands.” The American Counseling Association lists several signs of Vicarious Trauma, including: Having difficulty talking about feelings Feeling diminished joy Feeling trapped by work Limited range of emotions Exaggerated startle reflex Hopelessness Trouble sleeping Exhaustion Conflict with other staff Trouble with intimacy Feeling withdrawn and isolated Impatience, apathy A change in worldview What can you do to make a difference? — Eye Movement Desensitization and Reprocessing is an extensively researched psychotherapy method that can be performed via telehealth or in person. EMDR is used to accelerate the resolution of trauma in fewer sessions than traditional psychotherapy. EMDR does not require talking about details but rather, mental processing of events. This can be especially helpful for issues that bring up guilt, shame or general feelings of discomfort we cannot pinpoint. Valeria interviews Mary Moore — she is a licensed professional counselor associate and the Clinical Director of Lake Austin Psychotherapy based in Austin, Texas. Dr. Anna and her husband suffered from dual factor infertility and when she gave birth through c-section to her twins, endometriosis was discovered. Dr. Anna is an international award-winning author and educator who has worked in the mental health profession for more than 15 years. She holds a doctorate in counseling and is a licensed professional counselor in multiple states, including Puerto Rico. She currently owns Charlandra Counseling Services, a virtual infertility counseling center; and is an assistant professor at Nyack College. Dr. Anna is an active leader in the American Counseling Association, and author of "Body Betrayal: Living and Understanding Infertility" and "Introduction to 21st Century Counseling: A Multicultural and Social Justice Perspective".
