Cancer is one of the most terrifying illnesses of our age - a sudden change that happens silently inside our bodies and slowly eats away at us until there's nothing left. It's also one of the biggest failures of the pharmaceutical age - somewhere between 90 and 97% of all cancer drugs fail at clinical trials. Which is why we're unbelievably excited to bring you our conversation with Dr. Thomas Seyfried, Professor of Biology at Boston College who appears to be hot on the trail of a cure cancer that has nothing to do with the oncology-industrial-complex. According to the dozens of papers he and his colleagues have published over the last few decades, a strict ketogenic diet coupled to a glutamine inhibitor is sufficient to weaken cancer cells to the point that the immune system can actually clear the tumor. A recent paper from his group published in February of 2025 shows the stunning effects of his protocol in a small group of eighteen glioblastoma patients. Median survival for these patients for the last hundred years has been 12-15 months, but with Seyfried's intervention appears to have been extended up to 84 months. They saw dramatic psychiatric and metabolic improvements that warrant further research investigations and offer hope to patients looking for new treatment options. Dr. Bret Scher interviews Shebani Sethi, MD, ABOM of Stanford University School of Medicine and Founding Director of the Stanford Metabolic Psychiatry Clinic. She discusses the details of this study which represents a milestone in the field of metabolic psychiatry: It is the first to investigate ketogenic therapy in serious mental illness since 1965. Standard of care treatments for bipolar disorder and schizophrenia typically exacerbate metabolic dysfunction, often leading to increased cardiometabolic risk and comorbidities that shorten lifespan. Today we have our good friend and colleague Dr. Dominic D'Agostino returning for his third appearance on STEM-Talk. Dom, as most of our longtime listeners know, is well-known for his research into the ketogenic diet and the physiological benefits of nutritional ketosis. Since our last conversation with Dom in 2019, a tremendous body of research has been added to the literature about the therapeutic potential of ketosis. The high-fat, low-carb ketogenic diet has been linked to advances in the treatment of Alzheimer's, cancer, migraines, type-2 diabetes, psoriasis, sleep apnea, psychiatric disorders, traumatic brain injuries as well as a host of other diseases and disorders, which we cover in today's interview. In episode 14 of STEM-Talk, we talked to Dom about his development and testing of metabolic therapies involving the ketogenic diet for a wide range of diseases and conditions. In episode 87, Dom returned to reflect on his 10 years of research focused on the high-fat/low-carbohydrate diet. In today's interview, we talk to Dom about this latest work as well as his extensive research on hyperbaric oxygen. Dom is a tenured Associate Professor in the Department of Molecular Pharmacology and Physiology at the University of South Florida Morsani. He specializes in neuroscience, molecular pharmacology, nutrition, and physiology. Dom also is our colleague and a research scientist here at the IHMC. Show notes  [00:02:50] Dawn opens the interview mentioning Dom's recent IHMC Evening Lecture, in which he mentions the film “First Do No Harm” starring Meryl Streep. The film is based on the true story of a four-year-old boy diagnosed with severe epilepsy, whose extreme seizures continued despite extensive medical treatments. The boy's mother reached to Dr. John Freeman, a physician who had successfully treated patients with a ketogenic diet. Dawn asks Dom to give some context about this fictional film based on a true story. [00:05:05] Dawn asks Dom to discuss the many evidence-based applications of the ketogenic diet that he highlighted in his IHMC evening lecture. [00:07:11] Ken asks Dom about another story involving Russell Winwood, a man with severe chronic obstructive pulmonary disease, also known as COPD. Russell reached out to Dom with respect to treating his COPD with a ketogenic diet. [00:11:21] Ken asks if Russell only engaged in the ketogenic diet or if also used exogenous ketones. [00:12:10] Ken mentions that the ketogenic diet has the broad potential to be an anti-inflammatory diet. Ken goes on to mention that COPD is an inflammatory disease. As Dom's case report suggested, Ken wonders if the ketogenic diet has the potential to have strong therapeutic effects for other inflammatory conditions as well. Ken asks what other conditions Dom thinks might benefit from therapeutic ketosis. [00:14:02] Dawn mentions that Dom has been busy since his last appearance on STEM-Talk, having authored or collaborated on more than 40 papers, one of which garnered a lot of attention and was published in Frontiers in Neuroscience. This paper investigated whether therapeutic ketosis via ketone esters could represent a viable way to treat epilepsy and other seizure disorders. Dawn asks Dom to elaborate on this paper's findings and their significance. Ken asks Dom to give a quick primer on ketone esters and why so many researchers in the field are excited about their potential. The doctor ordered walking only for exercise. I specifically asked about upper body workouts and was told no go, which makes sense because upper body almost always involves core muscles. I'm a 71 yo female who normally spends over an hour every other day lifting heavy things in the gym. Tips please on how to avoid muscle loss, particularly advice on protein consumption. Advice on collagen intake desired as well. Thanks.   2. Thyroid & Iodine [22:41] Marilee says: Hi Robb & Nicki, I recently listened to a podcast with Dr. Alan Christianson and read his book The Thyroid Reset Diet. His premise as I understand it is that most auto-immune and hypo-thyroid conditions are due to an excess of iodine in the body and can resolve once that iodine is depleted/used up by going on a low iodine diet. This includes primarily removing salt (unless clearly non-iodized or kosher), egg yolks and dairy although there are many other sources listed including certain skin care products. My labs have indicated the need for treatment for hypo for many years as well as having autoimmune thyroid disease. However any prescription thyroid medicine as well as thyroid support supplements leave me feeling much worse so I have been untreated for several years. While I don't feel nearly as bad as my labs would indicate I also don't feel great. I would like to know what you think of this idea and whether you think his eating plan would help. SummaryWe have been taught to not skip meals, make carbohydrates the most abundant macronutrient we consume, and to snack throughout the day. Sadly, this all appears to be wrong. Fasting exists in many forms and growing evidence suggests it is essential to health. We are all spending way too much time fed and not enough time fasting. Want to stimulate autophagy? Then eat less, exercise more, and feel better! Morbidity and MortalityObesity rates have more than tripled worldwide since 1975. Over 50% of people in the United States are now obese and 30% or more have metabolic syndrome. Houston, we have a problem! StoryJesus fed for 40 days in the desert during what we now call Lent. Islamic peoples celebrate Ramadan, a remembrance of Mohamad's 1st revelation. During this holy month, Islamic people do not eat during the daylight hours. Buddhist monks fast during religious retreats and the Buddha did not eat food after lunch. For a time fasting fell out of favor, but it is coming back, and just in time. Key Points1. Autophagy is biological process that allows cells to recycle nutrients and get rid of harmful waste. It is essential for health and best stimulated through fasting.2. By fasting and/or limiting carbohydrate intake, mammals enter ketosis. Ketosis is marked by the mobilization of stored fat for nutrition, the synthesis of glucose from glycerol and amino acids, as well as the usage of ketones for nutrition. Maintaining the body in a state of ketosis is beneficial for the treatment of metabolic syndrome.3. What about if you are doing a longer water fast and need a little brain boost?  Ketone Esters may help you prolong the fast. Let's dive into the "Sports Protocol":  SUGGESTED SERVING SIZE Each "serving" is technically 60ml of KE4 (30g of the Ketone Ester), but do not start with a full bottle. More is NOT always better. More may lower blood glucose too much, lower top speed, and reduce performance substantially. It is based on weight, so if you weight 2x the average person, you may need more. And if you are lightweight, you will need much less. Numbers below are based on 70 kilos. Start in practice, NOT in a race. (Updated March 2021) Micro dosing KE4:  (Micro dosing is a fancy word for taking smaller amounts of KE4) If using KE1 (compare) and adjust. 1 bottle of KE1 is = to about 2 capfuls of KE4) Pre-Workout:  Start here. (All Athletes, regardless of diet) Regardless of your diet.A) Full Keto & fat adapted B) New to keto or intermittent fasting, or C) Full carb/western diet. Step 1: Empty stomach (at least 2-3 hours, if not overnight). This really is key. Without this, you may have to take more or do the dual fuel protocol with carbs and it still not be better. Why? (see this video) and more details on Fasted Workouts here Step 2: MOST COMMON: 1 to 3 capfuls of KE4 (same as 1/2 to 1.5 bottles of KE1) about 15 min before warmup or workout. * 1-2 capful for females or much lighter males, * 2-3 capfuls for NON keto males. * 1-4 capfuls for keto males, (Keto athletes find benefit at lower amounts, BUT some are able to take more without the blood sugar dropping effect that non keto people get.) Warning, if you take it too early like 30-60 min and don't use the fuel, you may get tired (i.e. blood sugar crash). Some sprinters have taken it 45 min before without issue. Just test in practice the EXACT protocols you plan to use. Note that glucose is not necessary (and could be worse) with this small amount since blood glucose does not drop too much with micro dosing. Glucose also causes inflammation, it is a dirty fuel with lactic acid as a byproduct (ketones create zero lactic acid). The theory here is micro dosing may trigger endogenous ketone production (where your body makes ketones), whereas more may hault that production or it can also drop blood sugar too much. Some may stick with this protocol and not move up in quantity. Play around with different amounts Then try 15ml up to 25ml of KE4 (again, until you reach 30ml, you may not need carbs.) If you do notice a difference with 10-15ml, some may drop to 5ml to see how that works. I wouldn't start with 5ml because it may be too subtle, but you can if you feel the energy is still available. Tips: Don't go out faster expecting a better final result. Go out as normal as possible and then later in the practice you can increase your speed if you want. Trey Suntrup, PhD is a product engineer who earned his doctorate in physics and electrical engineering from the University of California, Santa Barbara in 2015. He is currently the Head of Product at Readout Health, the St. Louis startup that recently launched the Biosense breath ketone meter. Following a successful clinical trial in the autumn of 2019, Biosense has entered the consumer, clinical, and research market as a tool to help those wanting to lose weight or manage blood glucose with a ketogenic diet. My chat today with Rachelle Martin and Olaf Sorensen is anything but dull! These two are the ultimate self-experimenters, particularly when it comes to diet (and, in Olaf’s case, finding the perfect exercise regime but that will be the subject of another podcast).Both Rachelle and Olaf started experimenting with a low carb diet several years ago, Rachelle, to correct some health issues and Olaf, to find the optimal way to manage his energy for long distance running e.g. ultra marathons. We heard about Rachelle's ketogenic diet in Episode #7 of Amanda’s Wellbeing Podcast (link below). Olaf, as you will hear, found the low carb way of eating agreed with his physiology but not content to stop there, he took it further and experimented with the carnivore diet and says, he felt so good, he never went back. Today, Olaf eats meat, organ meat, fat and eggs. We chat about the importance of the source of protein consumed and Olaf’s business with his parents, Grassland Nutrition, where they produced freeze dried organ meats in both powdered and capsule form. James McCarter, MD, PhD. is a researcher and author of over 60 scientific publications and patents. He recently led research and clinical operations for San Francisco-based Virta Health, a nationwide medical provider that delivers the first clinically-proven treatment to safely and sustainably reverse type 2 diabetes without medications or surgery. This week we bring back regular guest, Dr. Jerome Craig, to teach us all about living a gluten free lifestyle. As you may know, Dr. Craig is the voice, vision and wisdom behind many of our Fitlandia webinars. Today we’re talking about being gluten free and asking if it is just another diet fad or truly beneficial for our health. If you’ve listened to previous Fitlandia podcasts you know we’re passionate about bringing credible, science-backed information to our listeners - no more fads, no more diets, no more quick fixes. So, this gluten free thing is a hot topic right now, and was initially thought to be healthy. But now there’s a backlash about the research, or lack thereof about the health benefits of eating gluten free. So, we posed this question to Dr. Craig, “Do you believe being gluten free is a diet fad?” He tells us that it’s become a very popular way of eating. The statistics show that 20 million Americans are eating gluten free and a third of those people are trying to avoid gluten. It’s very much front and center in today’s culture. So, is it a fad? Yes, no and maybe. It is a necessity for everyone? Yes, no and maybe. Dr. Craig states, it’s important to look at the science. It’s hard to say, one way or another, exactly what being gluten free or not will do for your body but a diet concentrated mostly on any grain isn’t going to be the most nutritious. The best thing about grains is how much fiber they provide. Fiber is good for us, helping to maintain blood glucose levels but also creates digestive distress for many people and that’s when the gluten argument gets a little foggy. So, what is gluten? Typically, we know of gluten as the protein found in wheat, barley and rye. Gluten is made up of two molecules that are added together; a larger, polymeric glutenin and a smaller protein, gliadin. When combined, they create a lot of stickiness. As wheat is ground and kneaded into dough, it becomes very elastic which is popular in bread making. The air molecules within the dough are trapped, making the bread strong and fluffy. This is where the science and the argument come in - the gluten free market place is a huge industry now. Dr. Craig tells us, “The gluten free market has more than doubled in sales between 2011 and 2016 and today is a $15 billion market.” Although thriving, these products are not necessarily better for you, just because they are gluten free. Most of these baked goods are corn or rice based and don’t provide us more nutrition. Gluten Free Society posts a variety of glutens. Their argument is with 400 glutens occurring in nature; only 40 of those exist in the human food supply. Most seeds are made with components that are meant to be protected and not digested. These seeds are grown so animals eat and spread them to reassure regrowth of the plants. For optimal health and digestion, it is recommended to not only watch wheat glutens, but all grains because every grain has gluten in it. That’s right; gluten is not just in wheat, rye and barley but also rice, corn and millet which are used in gluten free foods. These all have a group of plant storage proteins, called prolamins, which for most of us are not easily digestible. If we look back at traditional ways of making breads, they used to ferment it. These grains required time to absorb water and the fermentation process gave the bread the gluten-like texture we know and love. They Don't Make 'Em Like they Used To Today, however, commercial bakeries use a gluten concentrate in creating doughs. This modern way of milling flour strips the nutrients, leaving the starch and then the concentrate is added to make the bread fluffy again. This is why eating “gluten” causes intestinal distress for so many people because our immune systems and guts are reacting to it. Additives such as coloring, sweeteners, shortening and bleaching agents are added to commercial bread products and are not usually labeled because they’re not technically food ingredients. No wonder our bodies are inflamed! One of our big issues with gluten today is with studies of Celiac; a serious genetic autoimmune disorder. Celiac provokes problems with the immune system if gluten is consumed. Most celiacs have to follow a strict gluten free diet. Even non-celiac people may have sensitivity to wheat/ gluten because many ingredients in wheat can be problematic. The best way to feel better is to avoid eating these types of foods for a period of time, and there are certain probiotics that may help people digest gluten. Dr. Craig points out, “If you eliminate all grains from your diet, you’re going to change your body’s microbiology. This colony changes when you change what you eat, and when a certain food is eliminated and then brought back into the diet later – there is likely to be difficulties with digestion.” Studies do show people who suffer with gut inflammation, leaky gut or IBS greatly benefit from a grain free diet. Eliminating grains not only helps us cut out wheat glutens but we naturally incorporate more fruits and vegetables into our diet instead of processed foods. This gives us greater health benefits by increasing our nutrients and helping to manage blood sugar levels. It’s not about replacing the gluten with other non-gluten baked goods but transitioning into more of a “paleo style” way of eating by removing all flours and adding in more root vegetables and nutrient dense foods. We need to listen to our bodies. When we slow down and pay attention to how our body feels, we can use that as a guide for dietary choices. The easiest way to see what might be causing you inflammation is to take a break from it, see how great your body feels, and introduce it back slowly later if you choose. Tips on Reintroducing Foods Back Into Your Diet Grains for example: • Look up fermentable ways to make your own bread so you know exactly what you’re adding in to it • Start with consuming small amounts and see if you feel any gastrointestinal distress • Probiotics which help with digestion – some in the bacillus or lactobacillus family or any lactate fermenting will help break down these foods. With so many people suffering from gut inflammation and borderline immune dysfunction, its no wonder that being gluten free has become so popular and is seen as a trending fad. People who experience food disorders or GI distress tend to benefit from a low FODMAP diet. A low FODMAP diet, or FODMAP elimination diet, refers to a temporary eating pattern that has a very low amount of food compounds called FODMAPs. The acronym, as described by co-creator Sue Shepard, stands for: • Fermentable – meaning they are broken down (fermented) by bacteria in the large bowel • Oligosaccharides – “oligo” means “few” and “saccharide” means sugar. These molecules are made up of individual sugars joined together in a chain • Disaccharides – “di” means two. This is a double sugar molecule • Monosaccharides – “mono” means single. This is a single sugar molecule • And Polyols – these are sugar alcohols (however, they don’t lead to intoxication!) This way of eating is primarily beneficial for people with GI problems because any fructans, the fructose molecule found in wheat are removed as well as plant fibers which lead to bacterial overgrowth in the gut. Leaky gut can also be caused by a gluten dense diet resulting in a buildup of antibodies to the hormone, Zonulin. This hormone, which acts like a border control in the gut, opens up gap junctions allowing large particles to pass through. However, gluten stimulates the release of Zonulin which can promote leaky gut. How do glutens affect autoimmune disease? Protein molecules that define a grain may be mistaken by our immune system to look like the digestive tract called molecular mimicry. Hyperpermeability, or the increase of permeability (leaky gut), is at the heart of immune dysfunction. Protein leaks out of the digestive tract directly into the blood stream which creates a negative response in our bodies. This can be caused by food particles or toxins that make it across the bacteria that lines the gut. Our bodies become hyper-vigilant and when the immune system dials in with an unrecognizable substance in the system, our bodies can start attacking its own cells. To help with this, we want to broaden the spectrum of probiotic foods in our diet, since we tend to miss out on nutrients if we eliminate food groups for long periods of time. For people who are trying to reintroduce foods back to their diets, Dr. Craig suggests starting with fermented foods which are easier to digest. A small amount of toxins help to keep our immune system alert and makes it easier to adjust after detoxification. As you start reintroducing foods look for: • Family history (especially celiac) to try and avoid gluten and grains • Gut inflammation (heart burn, IBS) correlations with food allergies • Intestinal permeability (many food sensitives) you will start to react to more foods • Skin issues (eczema, psoriasis, discoloring, bruising) • Digestion (gas, bloating, diarrhea, constipation, heartburn) • Brain function (brain fog, hard time concentrating, fatigue after meals which are all associated with immune function and a difficulty regulating blood sugar and insulin levels. • Anxiety / Depression – inflammatory process which grains tend to feed into this response For more on how grains affect our neurological health, check out Grain Brain by David Perlmutter. Dominic D’Agostino looks like a bodybuilder. But that doesn’t mean that he eats a diet typical for that sport; on the contrary, the research scientist—and amateur athlete—can go an entire day without eating and says his performance—both in the lab and in the gym—improves because of it. D’Agostino is perhaps rare in the world of science in that he practices what he preaches. As associate professor in the department of molecular pharmacology and physiology at the University of South Florida, and a visiting research scientist at IHMC, D’Agostino develops and tests metabolic therapies for a range of diseases and conditions for which the ketogenic diet is the cornerstone. The low-carb, moderate-protein, high-fat ketogenic diet is what he also follows for health and greater mental clarity. The ketogenic diet for decades has been used, albeit perhaps sparingly in the clinic, to treat epileptic seizures. D’Agostino is working on the development of exogenous ketones in the form of ketone esters for cancer and neurological disorders as well. He specifically looked at how the brain responded to hypoxia. 7:12: He did a post-doc with Jay Dean and also became a recreational diver. “Dean was the only person studying cellular and molecular mechanisms of extreme environments.” 8:36: Of Dean, he said, “The tools he created are filling gaps in the understanding of dive physiology.” 10:19: Nutritional ketosis is important for the metabolic management of diseases, especially seizures. 10:45: Nutritional ketosis works similarly to fasting: you liberate free fatty acids from the adipose tissue and break down stored glycogen levels in the liver. Once the glycogen levels reach a certain level, you start accelerating the oxidation of fatty acids in liver. 11:11: Dom explains how ketosis works: the heart (and muscles) prefers fatty acids over glucose, but they don’t readily cross the blood-brain barrier. So brain energy metabolism will transition from glucose to a fuel source called ketone bodies, which is a by-product of accelerated fat oxidation in the liver.
