I had the privilege of speaking with Katrece Nolan, a breast cancer survivor from Northern Virginia. We had such a lovely and long conversation that I ended up producing this episode in two parts. Today we dive into Katrece's compelling story with inflammatory breast cancer (IBC). Diagnosed in her late 30s, Katrece's story began with a simple observation of asymmetry in her breasts, which led her to seek medical attention. Despite initial challenges in getting timely appointments, her persistence paid off when she was diagnosed with IBC, a rare and aggressive form of breast cancer.Katrece detailed the rigorous treatment process, which included chemotherapy, surgery, and radiation, emphasizing the importance of having knowledgeable doctors and advocating for oneself. She also highlighted the emotional and logistical challenges of managing a cancer diagnosis while raising three young children, and the critical role of community and family support.Katrece's story is a powerful reminder of the importance of self-advocacy, the need for specialized knowledge in treating rare cancers. In part II (EPS 423), we celebrate Katrece's survivorship of 11+ years and what survivorship and surveillance look like for those diagnosed with breast cancer. SURVIVINGBREASTCANCER.ORGAttend a free virtual SurvivingBreastCancer.org event:https://www.survivingbreastcancer.org/eventsFollow us on InstagramSurvivingBreastCancer.org: https://www.survivingbreastcancer.org/Breast Cancer Conversations: https://www.instagram.com/breastcancerconversations/About SurvivingBreastCancer.org: SurvivingBreastCancer.org, Inc. “Inflammatory breast cancer is very aggressive and rare and a lot of times the diagnosis can be missed,” said Margaret Gatti-Mays, MD, MPH, a medical oncologist who specializes in breast cancer and is one of the leaders of the Stefanie Spielman Comprehensive Breast Center's Inflammatory Breast Cancer Program. In this episode, she explains that this rare form of breast cancer “accounts for less than 2 percent of all breast cancers, adding that “one-third of all inflammatory breast cancers will be metastatic [when first diagnosed] while only 5 percent of the more typical types of breast cancer we see are metastatic [when first diagnosed].” One of the reasons inflammatory breast cancer is initially misdiagnosed is that the symptoms “are non-specific,” Gatti-Mays said. “The onset is rapid and includes redness in the breast, or it can be pink, red or purple, and appear over one-third of the breast, as well as a rapid swelling of the breast; it feels full and swollen and the skin of the breast looks like an orange peel, with dimpling.” These symptoms can be misdiagnosed as mastitis or duct ectasia, which are more common and non-cancerous. Mastitis and duct ectasia are initially treated with anti-biotics. “If the symptoms don't improve, I encourage women to come to the Stefanie Spielman Comprehensive Breast Center,” Gatti-Mays said. “We have a diagnostic clinic and patients can self-refer and get imaging. Because of how aggressive inflammatory breast cancer is, there's a sense of urgency to get a biopsy, make a diagnosis and evaluate if it has spread.” The team that comprises the Spielman's Inflammatory Breast Cancer Program are experts in diagnosing and treating this type of cancer. “Breast cancer care has become more complicated and having a team at a comprehensive cancer center is important to identify patients who need to start treatment immediately.” Treatment options include surgery, radiation, chemotherapy, and immunotherapy. Soon after she arrived at the Spielman, Gatti-Mays treated a pregnant woman with inflammatory breast cancer. “That drove me to want to learn more and figure out how to better treat these patients,” she said. “I realized a lot of my Spielman colleagues shared this interest” and this led to the formation of the Inflammatory Breast Cancer Program. “As a physician-scientist, I can say with absolute certainty that the patients I see in my clinic gives me the fuel we need to push forward and do better and better.”

ERβ as a Mediator of Estrogen Signaling in Inflammatory Breast Cancer

A new editorial paper was published in Oncotarget's Volume 14 on June 12, 2023, entitled, “ERβ as a mediator of estrogen signaling in inflammatory breast cancer.” In this new editorial, researchers Harika Nagandla and Christoforos Thomas from Houston Methodist Neal Cancer Center discuss inflammatory breast cancer (IBC)—a rare and aggressive form of breast cancer which accounts for 2–4% of all new breast cancer cases detected in the United States. Even with the application of standard multi-modality treatment approach that incorporates neoadjuvant chemotherapy, radiation and surgery, the 5-year survival rate for IBC is only about 40–50%. Breast cancer can be typically stratified into different types based on the presence of molecular drivers such estrogen receptor (ERα), progesterone receptor (PR) or human epidermal growth factor receptor 2 (HER2), which inform the treatment choice. For IBC, there is a substantially higher incidence of ERα negativity compared with other forms of breast cancer that can reach up to 60%. A specific targetable driver signaling pathway has not been identified so far. About one in three patients already have distant metastasis at the time of diagnosis, contributing to the aggressiveness and poor outcomes associated with IBC. Despite the absence of ERα from the majority of IBC tumors, estrogen signaling has been implicated in progression of the disease through ERα-independent pathways. ERβ is a ligand activated transcription factor that mediates effects of estrogen, along with ERα in different tissues during growth and development by regulating transcription of target genes. Tumor suppressive effects of ERβ have been documented in diverse cancer types such as thyroid, kidney, prostate, glioblastoma, ovarian and breast cancer. “The work from our group [6] establishes ERβ as a tumor suppressor in IBC by demonstrating its strong antimetastatic activity in preclinical models of the disease and delineating the mechanism of action.” DOI - https://doi.org/10.18632/oncotarget.28425 Correspondence to - Christoforos Thomas - cthomas3@houstonmethodist.org Sign up for free Altmetric alerts about this article - https://oncotarget.altmetric.com/details/email_updates?id=10.18632%2Foncotarget.28425 Subscribe for free publication alerts from Oncotarget - https://www.oncotarget.com/subscribe/ Keywords - inflammatory breast cancer, IBC, ERβ, metastasis, actin based cell migration About Oncotarget Oncotarget is a primarily oncology-focused, peer-reviewed, open access journal. About 1-5 percent of breast cancers in the U.S are Inflammatory Breast Cancer. This is an aggressive form of breast cancer, with signs that tend to arise quickly, often within weeks or months. The main symptoms of IBC are swelling and redness in the breast, and IBC can be hard to see on a mammogram because it may only show up as a sign of inflammation. Because of this and the frequent lack of a breast lump, IBC may first be mistaken for an infection or mastitis. Today's guest today is Amy Capello. Amy was diagnosed with Stage 3 IBC at the age of 38 and is here to help educate us on this rare disease in hopes that it can help other women to recognize it more quickly, and to share her refreshing perspective on life since her diagnosis. Inflammatory breast cancer (IBC) is rare and accounts for only 1% to 5% of all breast cancers, but it is considered an aggressive cancer because it grows quickly and is more likely to come back after treatment than other types of breast cancer. It causes symptoms of breast inflammation like swelling and redness, which is caused by cancer cells blocking lymph vessels in the skin causing the breast to look "inflamed." Gayathri R. Devi, PhD, is a two-time American Cancer Society grantee who recently received a Mission Boost Grant to “boost” her inflammatory breast cancer research and move it closer to patients. Dr. Devi joined the podcast to talk about risk factors for IBC, how it's different from other breast cancer types, and recent advances in her lab with promising clinical implications. Dr. Devi is Program Director for the Duke Consortium for Inflammatory Breast Cancer, Associate Professor of Surgery and Pathology at Duke School of Medicine, and the Director of the Duke North Carolina Central University bridge office as part of the Duke School of Medicine Clinical and Translational Sciences Institute. 3:18 – The symptoms and signs of inflammatory breast cancer (IBC) 6:34 – How inflammatory breast cancer differs from other, more common breast cancers 9:36 – Risk factors for inflammatory breast cancer 14:22 –Instead of a single tumor mass, IBC patients have small groups of tumor cells called emboli found in the breast, skin and lymph nodes around the breast tissue. What are emboli? How do they form? 16:00 – Why do emboli form in this way? 18:51 – What makes these emboli resistant to treatment and able to spread? 21:43 – On her ACS-funded research, which focuses on the environment in which the IBC emboli form, in the breast. Why is the breast environment so important? 28:35 – Adaptive stress response 31:15 – “I'll give you an example here and talk a little about our research findings that are clinically relevant.” 38:25 – How do we target inflammatory breast cancer therapeutically? 40:47 – The 3M approach: Models, Mechanisms, and Measures 45:30 – If she could wave a magic anti-IBC wand, where would we be in 5 years? 48:27 – The impact that ACS funding has had on this area of research 49:54 – “Another very important distinction about ACS…” 51:47 – How inflammatory breast cancer recently affected her family
