One of the biggest things we can do to create resentments is to have expectations. The most common individuals that we place expectations on are our loved ones and family members. We often idealize, hope for, or expect our family members to act in a way in which they are incapable of doing. Even if we have seen that they are unable to meet our expectations, we still have them. These types of false expectations only cause us harm. Once we release our expectations and see the relationship with clarity, we can begin to achieve emotional freedom from the attachment. In this episode of Omar Pinto Coaching, Omar helps Julia and Jo Anna process their relationships with their parents. Both of them maintain false expectations of their parents, which has only ended up hurting themselves. We needed to find a way to train pastors who knew how to apply the theology they were learning to real church situations with real people who have real problems. This was the challenge that led to Immerse. Immerse is a competency-based MDiv program where students earn their Masters degree while continuing to work in their ministry context. Rory Staunton was a healthy 12-year old boy, known for his smile and his work standing up for others. A simple fall during basketball practice caused an abrasion on his arm, which is the suspected beginning of a cascade of events that led to his death from sepsis. Rory was seen by both his pediatrician and a local emergency department, and was sent home with a diagnosis of a viral illness. He returned the next day in septic shock and died shortly thereafter. A review of the medical records revealed that there were errors that occurred during his emergency department visit. These errors were the focus of a controversial article in the New York Times, that included both details of the case, as well as the name of the physician that provided care. A backlash from the medical community occurred leading to multiple physician-written op-ed pieces, as well as over 1600 comments on the online version of the article. This talk will attempt to move away from the controversy of the actual article and instead focus on how these common errors could have occurred during any busy shift and what we can do to prevent them in the future. My intention in giving this talk is to continue to use this case to raise awareness of both pediatric sepsis and common medical error and hopefully lead to fewer outcomes like Rory’s.
STRANDED ON A WHAT?! Cruising for the first time without our two sons as crew, Jo Anna and I head north from Virginia to Nova Scotia. After dealing with fog for four days, we discover a netherworld of outlandish people and animals all living in a shack and a castle, where it seems a spell is cast. Realizing that the cruising experience doesn't get any more fulfilling than this, we stay for a month, visiting nowhere else in Nova Scotia. This is Part One of a two-part Caper, the preamble to Part Two, a bizarre survival thriller.
Is there a specific time during our shift when we are too fatigued to safety practice? That was the question that led to a research project comparing the clinical performance of providers during the first hour of a day shift and the final hour of a string of night shifts. These providers were pulled out of their real-time clinical duties and video-taped while performing simulated critical care cases. The hypothesis was that the day shift providers would out-perform the night shift, but the opposite proved true. Blinded reviewers assigned the day shift providers lower performance scores and noticed some surprising medical errors committed during these simulated cases. So are we “awake” when we come to work? Should some type of case-based warm up exercise be encouraged just prior to a shift? Also, upon reviewing the data, it was found that the majority of the providers studied had been off the day prior to their morning shift. Jan Paderewski, a famous pianist said, “If I miss one day of practice, I notice it. If I miss two days, the critics notice it. If I miss three days, the audience notices it.” Perhaps clinicians, similar to others who are elite in their field, truly need daily practice or some type of deliberate exercise prior to a shift to perform at the highest levels of care. How can we determine when we are not at our maximum level of mental sharpness during a shift? Can anything be done to improve our abilities in real time? This lecture will review the available literature surrounding mental fatigue and critical care based shift work and focus on techniques both before and during shifts to recognize and potentially mitigate any clinical sluggishness and improve patient care.
SHOOTING VENUS In this continuation of Scrimshaw's passage around Cape Thank God, I attempt to explain the dominant presence of navigating in a family crew before GPS. Jo Anna and I found it necessary to continually check each other's work, and we often found mistakes! This challenge was somewhat amplified when we became dependent on celestial navigation, and meeting that challenge brought us closer than ever as mom and dad, man and wife, captain and mate, and our "deck apes." Also in this Caper is the answer from another cruising couple, Fran and Mort Van Howe, as to what their sailing has meant to them in their lives.
THE WAY TO CAPE THANK GOD Intending to describe family cruising with one's wife as literally First Mate, I get hung up in the details of our cruise through the San Blas Islands and to Cartegena, Colombia (our favorite port). From there, we beat up through the Southwest Caribbean to the islands of San Andres and Providencia where, seeking local knowledge of the route ahead, I benefit from meeting Captain John Bull. This is all to set the scene for the next Capercast, which tells of our greatest navigational challenge, wherein Jo Anna and I -- while dealing with the urgency of finding our way -- become close to being one, an entity together with our sons.
Storm Coming... Sailing past the tip of Baja in the dark, with no lighthouse operating, we find ourselves crossing to the Mexican Mainland, a passage of about 300 miles, with a chubasco obviously approaching. There being no protection from such storms at Cabo, we head back there anyway, hoping mainly to get Jo Anna ashore safely. We are met again by a troupe of gracious Mexicans who solve all our problems. However, this leg of the voyage ends here, and we return to California overland for the arrival of our firstborn son Steven. A chronological jump occurs here. I mention a bit about what we’re jumping over, to which we shall return if listener feedback so indicates, but we shall proceed in Caper # 7 with more recent stories.
Arthur Piver and the Modern Trimaran About ten years after Woody Brown launched the first truly modern catamaran, a San Francisco man named Arthur Piver succeeds in developing a three-hulled vessel that delivers the all-round performance and maneuverability that the catamarans of the day do not. I just happen to be there at the right time – late 1950s – and am so consumed by this revelation that I build two of Piver’s early boats and, together with my bride Jo Anna, take one of them to sea.
