Today on the podcast is one of the best female runners in the NCAA, Ceili McCabe! Ceili runs for West Virginia, where she has cemented herself as one of the greatest runners in program history. Ceili is an 8XAll-American, was 3rd at the 2021 NCAA XC Championships, is a 2xWorld Championship qualifier, a 2024 Paris Olympian, the record holder in the steeplechase for Canada, and a Canadian XC Champion. She's run 8:50 in the 3k, 9:20 in the Steeple, and 4:29 in the Mile. With these accolades, she's one of the most decorated athletes in the NCAA at the moment. I hadn't heard her story publicized on a larger level, so I wanted to sit down with her and hear about her journey that has led her to this point. THE EVAN JAGER SPECIAL Evan Jager is one of the most decorated and accomplished runners in US Distance history. Evan is the American Record Holder in the Steeplechase, along with the Rio Olympic Silver Medalist in the Steeple and London World Championships Silver Medalist in the Steeple. On top of these feats, Evan has won a US title at the Steeplechase SEVEN times. He is one of the original members of Nike's Bowerman Track Club having dropped out of college (not to sound dramatic, he was just really fast and went pro) to follow Coach Jerry Schumacher. Evan holds PRs of 3:32 (1500m), 8:00.45 (Steeplechase), 7:35 (3000m), and 13:02 (5000m). Evan is a 2xUS Olympian. AC: Very few details have leaked, however it's likely that Turkey has secretly been in talks to join BRICS for many months, since the new additions of the UAE, Saudia Arabia, Iran, and others earlier this spring.This morning, local Turkish media (and users on social media) published videos of what have been claimed to be US service members from the USS WASP ARG being assaulted/abused in the streets of Izmir during a routine port call. AC: The USS Wasp did arrive in Izmir yesterday, as confirmed by social media posts by U.S. Fleet Forces Command. However, it's not clear as to if the people being assaulted in the viral videos that have circulated social media are indeed US service members. No statements by defense officials have emerged to confirm or deny this incident as of this report.Middle East: US CENTCOM released a press statement over the weekend highlighting the wounding of 7 US service members during an operation in western Iraq. The report claims that 15 ISIS militants were killed during the operation, which was conducted in coordination with local Iraqi support.Israel: Protests voicing opposition to Netanyahu ignited again last night. Domestic demonstrations against the Israeli government have been regular over the past few months, but after the discovery of 6 dead hostages in Gaza a few days ago, demonstrations have become more widespread.France: Early this morning another major church fire broke out in Pas-de-Calais. This was conducted via a covert operation in the Dominican Republic, as American operatives allegedly acting via the DoJ, DHS, and DoS stole the plane as it was parked on the ground.Far East: Tensions between the Philippines and China again came to a head near the Sabina Shoal over the weekend, as another collision occurred between PLAN and Philippine Coast Guard vessels. The attack was carried out by the Youth Union of Turkey (TGB), a well-known revolutionary organization (comprised largely of a conglomeration of Socialists and Turkish Nationalists). Of note, this attack is not the first carried out by this group; nearly-identical attacks were carried out on US service members in 2011, 2013, and 2014. TGB is particularly fond of more-kinetic actions that take the form of restraining US service members and placing a bag over their head, echoing the historical tones of the organization's Socialist origins. Quincy Hall claimed the 400m gold medal while Kenneth Rooks shocked the world with a silver medal finish in the 3000m steeplechase. As the events roll on, it's clear that the American athletes are making a big splash and setting the stage for an exciting finish to the Games. In tonight's edition of the Torch Talk, the team breaks down… – Quincy Hall won the Olympic gold in the 400m at Paris 2024 in 43.40, making him the 4th-fastest man ever. Brit Matthew Hudson-Smith took bronze and Muzala Samukonga of Zambia took bronze. This was the fastest 400m race in history, as five men dipped under 44 seconds. – Soufiane El Bakkali became the first man in nearly 100 years to successfully defend his Olympic steeplechase gold, winning with a time of 8:06.05. Kenneth Rooks won silver with a big PB, and Abraham Kibiwot of Kenya won bronze. – Nina Kennedy won the women's pole vault at the Paris 2024 Olympics, clearing a height of 4.90m. N Engl J Med 2014;371:1016-1027Background: Prior trials have demonstrated that combining P2Y12 inhibitors with aspirin in patients with acute coronary syndrome reduces cardiovascular events. Prasugrel, in the TRITON-TIMI 38 trial, and ticagrelor, in the PLATO trial, were administered in the hospital.Cardiology Trial's Substack is a reader-supported publication. To receive new posts and support my work, consider becoming a free or paid subscriber.Older trials had suggested that early administration of glycoprotein IIb/IIIa inhibitors improves outcomes in patients with ST elevation myocardial infarction (STEMI).The ATLANTIC trial sought to test the hypothesis that pre-hospital compared to in-hospital administration of the P2Y12 inhibitor, ticagrelor, improves outcomes in patients with STEMI.Patients: Patients were enrolled if they had STEMI and had experienced symptoms for at least 30 minutes but no longer than 6 hours, and were expected to have EKG to balloon inflation of less than 120 minutes. Patients were excluded if they had prior intracranial bleeding, moderate to severe liver disease, gastrointestinal bleeding within 6 months, planned fibrinolytic therapy or required dialysis.Baseline characteristics: The average age of patients was 61 years with 80% being men. The average weight was 80 kg. About 14% had diabetes, 9% had prior myocardial infarction, 4% had chronic obstructive pulmonary disease and 2% had chronic renal failure. TIMI risk score was 0-2 in 61% of the patients. About 90% had Killip class I. Coronary angiography was performed in 98% of the patients and percutaneous coronary intervention (PCI) with stent placement was performed in 82%. The use of glycoprotein IIb/IIIa inhibitors was high in the study and was administered before percutaneous coronary intervention in 29% of the patients.Procedures: Patients were randomized 1:1 to receive ticagrelor en route to the hospital/ catheterization lab (group 1) or at the catheterization lab (group 2). In group 1, patients received ticagrelor 180mg en route to the hospital and placebo in the catheterization lab. In group 2, patients received placebo en route to the hospital and ticagrelor 180mg in the catheterization lab. Following that, all patients received ticagrelor 90mg twice daily for at least 30 days and the treatment was recommended to continue for 12 months. Clinical endpoints were adjudicated up to 30-days post randomization.Endpoints: There were two coprimary endpoints – proportion of patients who did not have 70% or greater resolution in their ST-segment elevation before PCI and proportion of patients without TIMI grade III flow in the infarcted artery before PCI. Review of EKG and angiographic data was blinded.A secondary prespecified endpoint included the composite of all-cause death, myocardial infarction, stent thrombosis, stroke or urgent revascularization at 30 days.Analysis was performed based on the modified intention-to-treat principle, defined as patients who received at least one loading dose of the study drug. Patients with missing EKG or angiographic data were excluded from the primary endpoint analysis.The sample size estimate was based on an anticipated event rate of 15% in the control group for the EKG endpoint. They estimated that 779 patients would be needed in each group to show a 6% absolute difference with 80% power and an alpha of 2.5%.Results: The trial randomized 1,862 patients, 909 patients to the prehospital group and 953 to the in-hospital group. The median time from randomization to angiography was 48 minutes and the median time between the two loading doses was 31 minutes.There was no significant difference in the proportion of patients who did not have 70% or more ST segment resolution before PCI (86.8% for the pre-hospital group vs 87.6% for the in-hospital group, OR: 0.93, 95% CI: 0.69 – 1.25; p= 0.63) or the proportion of patients who did not have TIMI III flow in the infarcted artery before PCI (82.6% for the pre-hospital group vs 83.1% for the in-hospital group, OR: 0.97, 95% CI: 0.75 – 1.25; p= 0.82).There was also no significant difference for the secondary composite endpoint (4.5% vs 4.4%, OR: 1.03, 95% CI: 0.66 – 1.60; p= 0.91). Stent thrombosis at 30-days was lower in the pre-hospital group (0.2% vs 1.2%, OR: 0.19, 95% CI: 0.04 – 0.86; p= 0.02). Myocardial infarction was not significantly different between both groups (0.8% vs 1.1%; p= 0.53). All-cause death was numerically higher in the pre-hospital group (3.3% vs 2.0%, OR: 1.68, 95% CI: 0.94 – 3.01; p= 0.08).Major bleeding not related to CABG was not significantly different between both treatment groups (1.3% in both groups using the TIMI criteria and 2.9% in the pre-hospital group vs 2.5% in the in-hospital group, using the STEEPLE criteria).Conclusion: In patients with STEMI, pre-hospital administration of ticagrelor did not improve outcomes compared to in-hospital administration. Although pre-hospital administration of ticagrelor reduced stent thrombosis at 30-days, this did not reduce all-cause mortality. In fact, all-cause mortality was numerically higher in the pre-hospital group.A notable finding is that within the in-hospital group, definite stent thrombosis occurred in 1.2% of patients while 1.1% were adjudicated to have myocardial infarction. Stent thrombosis is a serious condition that leads to myocardial infarction. The trial protocol used many definitions for myocardial infarction. This underscores the complexity of counting and adjudicating events in clinical trials and highlights the importance of relying on outcomes less susceptible to bias, such as mortality.Cardiology Trial's Substack is a reader-supported publication. Last time we saw Matt Wilkinson, he was preparing for the D3 National Meet. Now he's in his first Pro season with Under Armour's Dark Sky team in Flagstaff, Arizona. Before he got to Flagstaff, he had a few seasons at Minnesota where he swept the Steeple and 5k at the Big 10 conference meet. He talks about how he's handled each transition from D3 to D1 to Pro and how he's overcoming his imposter syndrome with each race. With each step in the journey he's embraced his new teammates that he trains with and gains confidence knowing he can hang with them in workouts. As he prepares for the Olympic Trials, he's excited to see what a full year of training with his UA team can have on his performance. Matt is running for the long haul.
