Republicans have been all over the map lately in their Supreme Court messaging. Sen. Roger Wicker (R-Miss.) called President Joe Biden's vow to only consider Black women for the vacancy “affirmative action.” Sen. Susan Collins (R-Maine) said Biden doing so during the campaign was “clumsy” and risked further politicizing perceptions of the high court. And Sen. Lindsey Graham (R-S.C.) welcomed Biden's bid to diversify the Supreme Court, saying “it's about time” it looked more like America. So what gives? Subscribe to POLITICO Playbook. Journalist and filmmaker Eleanor Goldfield returns to the program to address several topics in media and world events, including the ongoing scandal of Julian Assange's imprisonment, the folly of the US and NATO threatening Russia, and the shallowness of commercial media. Then media scholar Nolan Higdon revisits the program to explain how tech giants are infiltrating the classroom by sponsoring carefully-tailored "media-literacy" course materials. The discussion delves into the differences between critical, critical, and corporate media literacy. Both guests weigh in on the latest flap between Joe Rogan, Neil Young, and Spotify with a focus on the problem of big tech deplatforming, censorship, and free speech issues. Eleanor Goldfield is a creative radical, journalist, artist, and organizer. She is co-host of the Common Censored podcast with Lee Camp. She also produced a documentary, Hard Road of Hope, about West Virginia communities confronting pollution from fracking. The Craig Murray article she mentions (about Julian Assange) can be found here. Nolan Higdon is a lecturer in education at the University of California Santa Cruz campus, he teaches media studies and history. He's also the author of the book "The Anatomy of Fake News;” co-author of "Podcaster's Dilemma” with Nicholas Baham III; and co-author with Mickey Huff of "Let's Agree to Disagree,” a critical thinking textbook out from Routledge in early 2022. His recent article (co-written with media scholar Allison Butler) is titled "Time to Put Your Marketing Cap On." It appears in the Review of Education, Pedagogy and Cultural Studies journal. We could go on. And because it's not uncommon for us to hear, “But agreements can lead to inadvertent bureaucracy,” we did go on…in this episode. This week's question is from Janetta, “I have two kids, one has been diagnosed with ADHD and anxiety. He struggles with regulating his behavior and can be quite difficult to manage at times. My partner and I have been learning a lot from you and others about how to understand what's going on with him and to support him better, but I feel like other people in our life just don't get it. For example, my in-laws refuse to acknowledge that our child has challenges. They don't believe in the diagnosis or any of the treatment or supports we have been pursuing. They almost seem ashamed of it. They blame our parenting and continue doing and saying things that aren't helpful, even though those things are not working well. Do you have any advice on how we can address this? I want to have them in our kids' lives, but it's just so stressful for all of us.”                                           In this episode, I will address when extended family members, especially your own parents, seem unwilling to accept your child's challenges, differences, or needs. These different opinions in parenting among extended family can be extremely stressful, but let's first understand the potential underlying reasons for their lack of acknowledgment or support.  That understanding then gives you a framework for considering how to communicate and navigate through the differences of opinion and any uncomfortable family dynamics.  You can submit a question by emailing us at support@drbeurkens.com with the subject line "Podcast Question." 2022 is the year to regain your energy. With everything that's been going on the last two years, it's about time we prioritize our own energy levels. In this episode, we're talking about some of the simple and not-so-simple ways to help increase your energy this year.   The Five Aspects of Physical Energy Sleep: getting enough sleep Light: getting enough daylight Nutrition: eating well often Exercise: small or big movements every day Water: drinking enough water   Increasing Your Physical Energy We start out by talking about our physical energy levels because when we feel physically energized, it's easier to tackle the mental energy.  There are five aspects of physical energy we can start to improve using some pretty simple methods. Getting enough quality sleep every night will almost immediately improve your energy. Sleep is essential for our health. Aim to get at least seven hours of uninterrupted sleep; I like to use Melatonin and sleep in a cool, darkroom. If you notice your physical energy dropping, take notice of how much sugar you're consuming and try to reduce this. It's also important to drink enough water, get a moderate amount of exercise, and get some natural daylight every day.   Tips for Mental Energy Preservation The more difficult piece is the mental energy. Racing thoughts struggle with boundaries, and feeling overwhelmed is common and tricky to overcome. But there are ways to identify which thoughts go along with the overwhelm you feel. When I'm feeling overwhelmed or have a particularly long to-do list, I start applying the 3, 2, 1 method. By reducing the overwhelmingly long list in this way, and knowing I only have to get three things done makes the tasks feel less daunting and way more manageable.  The 3, 2, 1 method also helps to reduce feelings of decision fatigue. It's exhausting having to make decisions all day, every day, so anything we can do to reduce the overwhelm helps our energy levels. I also talk about how boundaries improve our mental energy. What is your word for 2022? My word is energy. How can you start to take some of your energy back this year? Let me know in the comments on the episode page!   In This Episode  How to create the most optimal sleep environment [5:00] How sugar impacts your energy levels [9:00] Simple ways to increase your physical energy every day [13:30] How to identify the thoughts that go with overwhelm [17:30] How to apply the 3, 2, 1, strategy to slow racing thoughts [20:00] How decision fatigue drains your energy [22:30] The role of boundaries in energy protection [25:00]   Quotes “We don't exercise to change how our body looks, we exercise for our health and how we feel.” [11:46] “Agree to agree that you can't control everything. If you focus on what you can control, you've gained back over 50% of your energy. Most of the things that you stress over are things that you can't control.” [21:22] “Boundaries are basically saying out loud what you want and that's where you start.” [26:04] “If you can learn to set boundaries, you will take back a lot of emotional energy. A ton. And you're worth it.” [26:52]   Resources Mentioned Join G.O.A.L.S. The Philadelphia Eagles hosted and defeated the New York Giants Sunday afternoon by a score of 34-10. The Philadelphia Sixers are on the road in the Nation's Capital taking on the Washington Wizards Sunday Night. Jalen Hurts and the Eagles offense struggled mightily in the first half as the score at halftime was 3-3. However the offense exploded in the second half by scoring 31 straight points starting with a Boston Scott touchdown run early in the 3rd quarter. DeVonta Smith and Lane Johnson added touchdowns for the offense and Alex Singleton returned an interception for a touchdown for the defense. The Sixers are coming off a disappointing loss Thursday night to the hands of the Atlanta Hawks. Joel Embiid and Tobias Harris look to carry the Sixers to a victory against the Wizards. Jalen Hurts - 17/29, 199 yards, 2 TD, 0 INTMiles Sanders - 7 carries, 45 yardsBoston Scott - 12 carries, 41 yards, TDJordan Howard - 9 carries, 37 yardsDeVonta Smith - 5 receptions, 80 yards, TDQuez Watkins - 3 receptions, 43 yardsThe first half of football Sunday afternoon was a snooze fest as the Eagles came out flat and allowed the Giants to hang out. If the Giants actually had a NFL caliber quarterback this game could have turned out differently. To his credit Jalen Hurts and the offense turned things around and totally dominated in the second half. The Eagles defense played well through the whole game and didn't allow the Giants to get out to any type of lead while the offense struggled. The Eagles will run the table to make the playoffs, do you Agree or Disagree with that. Relentless Health Value
Encore! EP294: Building a Center of Excellence: A Playbook for Physician Entrepreneurs, With Steve Schutzer, MD

Relentless Health Value

Play Episode Listen Later Dec 23, 2021 33:52

Believe me, filling in for the uncontested master of podcasts, Stacey Richter, is just a tad unnerving! My name is Dr. Steve Schutzer. I'm an orthopedic surgeon specializing in joint replacement surgery, and I think it's fair to say that I'm more comfortable, in my own lane, doing complex surgery than doing this introduction to our encore podcast 294 entitled “Building a Center of Excellence: A Playbook for Physician Entrepreneurs,” which aired originally in October 2020. But when Stacey graciously offered me the honor of doing so, I said to myself (sic: Steve, suck it up) what an opportunity to share with the devoted listeners of this show my humble perspectives on the prominent position COEs (also known as Centers of Excellence) are playing in this rapidly accelerating, evolving, and exciting healthcare landscape. So, there's an ancient Chinese proverb that goes like this: “When the wind of change blows, some build walls, and others build windmills”—or in this case, Centers of Excellence! And the winds of change in healthcare are blowing, maybe even reaching gale force. In the year since episode 294 aired, there's been unambiguous upsurge of activity, in part fueled by the pandemic, that has collectively and finally moved the healthcare value agenda across the chasm, over the inflection point—and there's no turning back. Unaccountable fee for service as the predominant payment model for healthcare services is, well, shall we say, on its last legs—being replaced by reimbursement models that are aligned with the clinical and financial outcome of the services actually delivered to our patients. For COEs, that's characteristically in the form of predictable bundled payments and fully warrantied episodes of care. Question: Where do COEs fit in this new landscape? Answer: COEs are the common pathway for all healthcare purchasers (whether they're self-funded employers, advanced primary care groups, Medicare Advantage—all of them) to steer agnostically to high-quality specialists focused on a defined set of healthcare services and who are willing to assume total cost of care for their product. And the favorable impact of COEs on the ROI for purchasers has now moved beyond the realm of theory to indisputable. Take, for example, the recent report by the RAND Corporation published earlier this year in Health Affairs: A study of over 2300 patients who had either total joint, spine, or bariatric surgery done under the Carrum Health program at one of their COEs. Carrum Health is a value-based national COE platform that connects self-insured employers with top providers under standardized bundled payment arrangements. And now in full disclosure, I serve as medical advisor for the company; and our program, the Connecticut Joint Replacement Institute in Hartford, Connecticut, is actually a Carrum COE. But in this independent RAND analysis of two years of medical claims data, the savings per procedure when the surgery was done at a Carrum COE was over $16,000 per procedure. Readmission rates were reduced 80% on average. Out-of-pocket cost to the patient? Zero. And an astonishing 30% of patients who were in the queue awaiting surgery ultimately were treated nonoperatively! Peter Hayes is president and CEO of the Healthcare Purchaser Alliance of Maine and a frequent guest on this podcast. His organization has been under contract with Carrum for approximately two years and recently reported an ROI of 58% and plan savings approaching $1 million. And these data also closely reflect that reported in the Harvard Business Review two years ago by Ruth Coleman and colleagues from their experience with Walmart COEs. Finally, you know, I heard Stacey say of COEs in one of her podcasts, “This is not something you can do on a Tuesday.” Agree. Prescient advice. As you will hear once again in just a moment, this takes work. But physician leaders and entrepreneurs, take heed. Although you won't be able to stand this up on a Tuesday, there's no reason why you can't begin next Monday. You can contact Dr. Schutzer at steve.schutzer@gmail.com and learn more at the Novel Healthcare Solutions website.   Steven F. Schutzer, MD, graduated with honors from Union College and the University of Virginia School of Medicine. Following a surgical internship at the University of Rochester, he served as lieutenant in the Medical Corps of the United States Navy. After his tour of duty, Dr. Schutzer did his general surgical training at the University of Rochester and then completed his orthopedic residency at the University of Connecticut. He was then a fellow in adult hip and reconstructive surgery at the Massachusetts General Hospital, after which he entered practice in Hartford, Connecticut. Dr. Schutzer is a founding member and medical director of the Connecticut Joint Replacement Institute (CJRI), a Center of Excellence at Saint Francis Hospital in Hartford, where he served as medical director between 2007 and 2021. He is currently the physician executive for the orthopedic service line at Trinity Health of New England. He is on the staff of Saint Francis Hospital and a member of Advanced Orthopedics New England. In 2014, Dr. Schutzer and two colleagues, Ms. Steph Kelly and Ms. Maureen Geary, launched a consulting company, Novel Healthcare Solutions, whose mission is to establish effective and trusting business relationships between physicians and hospital partners—and then create orthopedic Centers of Excellence. Dr. Schutzer is also vice president and co-founder of Upswing Health, a health technology start-up whose charge is to help 10 million lives alleviate suffering from musculoskeletal pain by the end of 2023. 04:52 Why would competitive physician groups gang together? 09:02 “Even if you never … bundle, going through the implementation process … will yield incredible unrecognized value.” 10:19 “It demands an end-to-end care redesign process.” 11:40 “The value of a COE is really unquestionable.” 11:48 “For every dollar saved [in a COE], two-thirds was in the quality side, and one-third was in the price point.” 14:38 Slide deck discussing the definition of a COE and its seven building blocks.15:06 “I'm talking about business relationships between the physicians … these are the most fundamental [relationships].” 16:24 “It is all about trust.” 16:49 What is the most central issue as to why a COE does well or fails? 17:26 “It's not just data. It has to be actionable data because physicians naturally don't trust data.” 22:55 “Employers are definitely taking note to patient-reported outcomes.” 23:38 What is the seventh element that is necessary for a COE, and what is fundamental to that element? 24:28 Where will fee-for-service doctors be in 2 to 3 years? 25:46 “The only way that we can accrue the value that we deserve is through these types of relationships.” 26:12 “The supreme motivator is opportunity.” 28:03 How do physicians and providers begin a transformation of the marketplace they're in? 28:38 “What they need from us is product. They need products to disrupt the status quo.” 31:27 “The problem is that there are vendors who are working at the margin.” You can contact Dr. Schutzer at steve.schutzer@gmail.com and learn more at the Novel Healthcare Solutions website.   @SSchutzer of @THOfNewEngland discusses #centersofexcellence on this week's #healthcarepodcast. #healthcare #podcast #digitalhealth #healthtech Why would competitive physician groups gang together? @SSchutzer of @THOfNewEngland discusses #centersofexcellence on this week's #healthcarepodcast. #healthcare #podcast #digitalhealth #healthtech “It demands an end-to-end care redesign process.” @SSchutzer of @THOfNewEngland discusses #centersofexcellence on this week's #healthcarepodcast. #healthcare #podcast #digitalhealth #healthtech “The value of a COE is really unquestionable.” @SSchutzer of @THOfNewEngland discusses #centersofexcellence on this week's #healthcarepodcast. #healthcare #podcast #digitalhealth #healthtech “It's not just data. The Relationship Maze
Having less stress at Christmas - synching your expectations around money, spending time with wider family and chores

The Relationship Maze

Play Episode Listen Later Dec 20, 2021 25:08

If you celebrate Christmas you may have all sorts of expectations of the festive days. Based on your earlier experiences of Christmas you are likely to have a number of associations with Christmas - some positive, some negative. Many couples experience Christmas as stressful for a number of reasons: giving and receiving gifts that are appropriate, concerns about spending a lot of money,  spending time with your partner's family and managing the various chores on the day(s). If you are single and spending Christmas without family, the holiday period can be experienced as intensely lonely. In this episode we discuss these potential stressors and consider ways of preparing for Christmas as best as possible.ExpectationsYou are likely to have a particular script about the way you would ideally like to spend Christmas; this may either be a wish for a corrective experience, now wanting Christmas to be the way you always wanted it to be but never quite got or a wish for the Christmas that you always had as a child and that you want to replicate as best as possible. Check with your partner what they want to avoid possible disappointments. Agree ahead of the Christmas days what you both wish for.Gifts You may have a very clear idea in your head about the kind of present you want to get for your partner. Your partner may have the same thoughts. You may find that your ideas about Christmas presents vary wildly. One of you may be used to getting very extravagant presents; the other may buy a 'small' present. There can easily be disappointments about the presents that are received. It helps to clarify from the outset what your expectations (and means) are and to agree whether you want to surprise each other or indicate what you would like to receive.MoneyMoney has a transactional value that goes beyond monetary value. Money has symbolic significance; it can stand in for love, power, success, autonomy, dependence or sexiness. Each of us has grown up with a specific concept of money depending on how it (or the lack of it) was experienced when you are growing up. Partners in relationships need to have conversations about money and how it gets managed in the relationship, in particular when living together. At Christmas disparities and different ideas about money often come to the fore. Be clear what and how you want to spend your money over the festive period if you often experience challenges in this area. The wider family - your partner's parentsYou may enjoy your partner's family/parents in which case there is no stress around spending extended time together. You may, however, find your partner's family challenging. There will need to be acceptance up to a point that your partner would like to spend Christmas with their family, however, you can also make prior agreements to have time out if and when needed. Domestic choresOften stress arises as one partner perceives to do more work in preparing for the day or hosting the event. Develop a plan with your partner agreeing who does what: who does the shopping, cooking, wrapping of presents or looking after the guests on the day.  Make a list of the various tasks ahead of time and agree who manages which task.
