My partner and I have been learning a lot from you and others about how to understand what's going on with him and to support him better, but I feel like other people in our life just don't get it. For example, my in-laws refuse to acknowledge that our child has challenges. They don't believe in the diagnosis or any of the treatment or supports we have been pursuing. They almost seem ashamed of it. They blame our parenting and continue doing and saying things that aren't helpful, even though those things are not working well. Do you have any advice on how we can address this? I want to have them in our kids' lives, but it's just so stressful for all of us.” In this episode, I will address when extended family members, especially your own parents, seem unwilling to accept your child's challenges, differences, or needs. These different opinions in parenting among extended family can be extremely stressful, but let's first understand the potential underlying reasons for their lack of acknowledgment or support. 2022 is the year to regain your energy. With everything that's been going on the last two years, it's about time we prioritize our own energy levels. In this episode, we're talking about some of the simple and not-so-simple ways to help increase your energy this year. The Five Aspects of Physical Energy Sleep: getting enough sleep Light: getting enough daylight Nutrition: eating well often Exercise: small or big movements every day Water: drinking enough water Increasing Your Physical Energy We start out by talking about our physical energy levels because when we feel physically energized, it's easier to tackle the mental energy. There are five aspects of physical energy we can start to improve using some pretty simple methods. Getting enough quality sleep every night will almost immediately improve your energy. Sleep is essential for our health. Aim to get at least seven hours of uninterrupted sleep; I like to use Melatonin and sleep in a cool, darkroom. If you notice your physical energy dropping, take notice of how much sugar you're consuming and try to reduce this. It's also important to drink enough water, get a moderate amount of exercise, and get some natural daylight every day. Tips for Mental Energy Preservation The more difficult piece is the mental energy. Racing thoughts struggle with boundaries, and feeling overwhelmed is common and tricky to overcome. But there are ways to identify which thoughts go along with the overwhelm you feel. When I'm feeling overwhelmed or have a particularly long to-do list, I start applying the 3, 2, 1 method. By reducing the overwhelmingly long list in this way, and knowing I only have to get three things done makes the tasks feel less daunting and way more manageable. The 3, 2, 1 method also helps to reduce feelings of decision fatigue. It's exhausting having to make decisions all day, every day, so anything we can do to reduce the overwhelm helps our energy levels. I also talk about how boundaries improve our mental energy. The Philadelphia Eagles hosted and defeated the New York Giants Sunday afternoon by a score of 34-10. The Philadelphia Sixers are on the road in the Nation's Capital taking on the Washington Wizards Sunday Night. Jalen Hurts and the Eagles offense struggled mightily in the first half as the score at halftime was 3-3. However the offense exploded in the second half by scoring 31 straight points starting with a Boston Scott touchdown run early in the 3rd quarter. DeVonta Smith and Lane Johnson added touchdowns for the offense and Alex Singleton returned an interception for a touchdown for the defense. The Sixers are coming off a disappointing loss Thursday night to the hands of the Atlanta Hawks. Joel Embiid and Tobias Harris look to carry the Sixers to a victory against the Wizards. Believe me, filling in for the uncontested master of podcasts, Stacey Richter, is just a tad unnerving! My name is Dr. Steve Schutzer. I'm an orthopedic surgeon specializing in joint replacement surgery, and I think it's fair to say that I'm more comfortable, in my own lane, doing complex surgery than doing this introduction to our encore podcast 294 entitled “Building a Center of Excellence: A Playbook for Physician Entrepreneurs,” which aired originally in October 2020. But when Stacey graciously offered me the honor of doing so, I said to myself (sic: Steve, suck it up) what an opportunity to share with the devoted listeners of this show my humble perspectives on the prominent position COEs (also known as Centers of Excellence) are playing in this rapidly accelerating, evolving, and exciting healthcare landscape. So, there's an ancient Chinese proverb that goes like this: “When the wind of change blows, some build walls, and others build windmills”—or in this case, Centers of Excellence! And the winds of change in healthcare are blowing, maybe even reaching gale force. In the year since episode 294 aired, there's been unambiguous upsurge of activity, in part fueled by the pandemic, that has collectively and finally moved the healthcare value agenda across the chasm, over the inflection point—and there's no turning back. Unaccountable fee for service as the predominant payment model for healthcare services is, well, shall we say, on its last legs—being replaced by reimbursement models that are aligned with the clinical and financial outcome of the services actually delivered to our patients. For COEs, that's characteristically in the form of predictable bundled payments and fully warrantied episodes of care. Question: Where do COEs fit in this new landscape? Answer: COEs are the common pathway for all healthcare purchasers (whether they're self-funded employers, advanced primary care groups, Medicare Advantage—all of them) to steer agnostically to high-quality specialists focused on a defined set of healthcare services and who are willing to assume total cost of care for their product. And the favorable impact of COEs on the ROI for purchasers has now moved beyond the realm of theory to indisputable. Take, for example, the recent report by the RAND Corporation published earlier this year in Health Affairs: A study of over 2300 patients who had either total joint, spine, or bariatric surgery done under the Carrum Health program at one of their COEs. Carrum Health is a value-based national COE platform that connects self-insured employers with top providers under standardized bundled payment arrangements. And now in full disclosure, I serve as medical advisor for the company; and our program, the Connecticut Joint Replacement Institute in Hartford, Connecticut, is actually a Carrum COE. But in this independent RAND analysis of two years of medical claims data, the savings per procedure when the surgery was done at a Carrum COE was over $16,000 per procedure. Readmission rates were reduced 80% on average. Out-of-pocket cost to the patient? Zero. And an astonishing 30% of patients who were in the queue awaiting surgery ultimately were treated nonoperatively! Peter Hayes is president and CEO of the Healthcare Purchaser Alliance of Maine and a frequent guest on this podcast. His organization has been under contract with Carrum for approximately two years and recently reported an ROI of 58% and plan savings approaching $1 million. And these data also closely reflect that reported in the Harvard Business Review two years ago by Ruth Coleman and colleagues from their experience with Walmart COEs. Finally, you know, I heard Stacey say of COEs in one of her podcasts, “This is not something you can do on a Tuesday.” Agree. Prescient advice. As you will hear once again in just a moment, this takes work. But physician leaders and entrepreneurs, take heed. Although you won't be able to stand this up on a Tuesday, there's no reason why you can't begin next Monday. Based on your earlier experiences of Christmas you are likely to have a number of associations with Christmas - some positive, some negative. Many couples experience Christmas as stressful for a number of reasons: giving and receiving gifts that are appropriate, concerns about spending a lot of money, spending time with your partner's family and managing the various chores on the day(s). If you are single and spending Christmas without family, the holiday period can be experienced as intensely lonely.
