In a recent episode of Oncology on the Go, several oncologists discussed the impact of the COVID-19 pandemic on oncology care, 5 years later. Each doctor discussed a different aspect of multidisciplinary care, including medical oncology, radiation oncology, and epidemiology. Fakih, MD, professor in the Department of Medical Oncology & Therapeutics Research, associate director for Clinical Sciences, medical director of the Briskin Center for Clinical Research, division chief of GI Medical Oncology, and co-director of the Gastrointestinal Cancer Program at City of Hope Comprehensive Cancer Center; ·      Elizabeth Zhang-Velten, MD, a radiation oncologist at Keck Medicine of University of Southern California;  ·      Frances Elain Chow, MD, neuro-oncologist at the University of Southern California (USC) Norris Comprehensive Cancer Center ·      James Yu, MD, MHS, FASTRO, assistant professor adjunct, Department of Radiation Oncology, Smilow Cancer Hospital at Saint Francis Hospital, and ONCOLOGY® editorial advisory board member.  The COVID-19 pandemic disrupted routine cancer care in a number of ways. Many patients were unable to receive timely screening, diagnosis, and treatment, Fakih noted. Additionally, Bardia stated that the pandemic led to a decrease in the number of patients participating in clinical trials. One of the most significant changes in oncology care, according to Salani, has been the increased use of telehealth. Telehealth has allowed patients to receive care from the comfort of their own homes, which has been especially beneficial for patients who live in rural areas or who have difficulty traveling. Telehealth has also made it easier for patients to connect with their doctors and to receive support from other members of their care team.  For Gomez, the COVID-19 pandemic also highlighted the importance of addressing the structural and social drivers of health. These are the conditions in which people are born, grow, live, work, and age that can affect their health. For example, people who live in poverty or who lack access to healthy food are more likely to develop cancer. The pandemic has led to a renewed focus on addressing these disparities. Overall, the COVID-19 pandemic has had a profound impact on oncology care. However, it has also led to a number of positive changes, such as the increased use of telehealth and the focus on addressing the structural and social drivers of health. In the years to come, it will be important to continue to build on these changes in order to improve the lives of patients with cancer.

In a conversation with CancerNetwork®, James B. Yu, MD, MHS, FASTRO, and Julian C. Hong, MD, MS, spoke about their study titled AI Use in Prostate Cancer: Potential Improvements in Treatments and Patient Care, which was published in the May issue of the journal ONCOLOGY®. Yu is a radiation oncologist in the Department of Radiation Oncology at Smilow Cancer Center Hospital at Saint Francis Hospital and an adjunct assistant professor of Medical Oncology at Yale School of Medicine. Hong is an assistant professor in the Department of Radiation Oncology of Bakar Computational Health Sciences Institute at the University of California, San Francisco (UCSF). Yu and Hong focused on the growing overlap between the advancement of artificial intelligence (AI)–based tools and the prostate cancer treatment field. In their study, they detailed AI-based developments related to diagnostic image analysis, the ability to “predict” prostate cancer outcomes, evaluating prostate cancer histopathology, and defining tumors and normal tissue to help plan radiation oncology treatment strategies. Additionally, they reviewed how these tools make use of machine learning algorithms, a subset of AI in which computers can assess data and interact with users without explicit instructions.  “We're trying to incorporate AI and machine learning into more trials to make these types of predictions because, at the end of the day, we're trying to deliver better care and improve outcomes for patients…. It takes trials to figure those things out, so it's a little bit of a work in progress.” Hong said, regarding potential next steps for improving the utility of these tools in the prostate cancer field. According to the authors, pushing the boundaries of AI would need to involve building upon prior retrospective data with additional sources of information to affirm that such tools can impact patient care. Additionally, the discussion highlighted potential future implications related to data ownership or privacy and how patients and physicians interact with these programs as AI becomes more prevalent in medical practice.  “We're not going to be replaced [by AI]. There will always be the need for the human connection any time there's disease or cancer,” Yu said. “It's a super exciting area, and the more people understand the limitations of AI rather than thinking of it as a panacea, the more the field will move forward.”

Evidence from the local phase, which included 2,000 pages of investigation, will now be sent to the Dicastery for the Causes of Saints in Rome for review. During a Mass on Sunday in celebration of the next phase toward Dutton's potential canonization, Honolulu Bishop Larry Silva said that Dutton “exiled himself” to the remote part of Molokai island where lepers were forcibly segregated “so that he could do penance for his own wayward life.” Dutton, a Civil War veteran for the Union from Wisconsin, was married after the war but pursued a divorce after his wife was unfaithful and left him after one year. After he stopped drinking, he began studying Catholicism and officially became Catholic in Memphis, Tennessee, in 1883 when he turned 40. Dutton went to the remote part of the island with those suffering from leprosy, now known as Hansen's disease, “so that he could change course completely from thinking about himself and his own needs to unselfish service of others in extreme need.” He became an expert in caring for the sick and continued his work after Damien died in 1889 from leprosy. Dutton himself inherited responsibility of an orphanage for boys and young men in 1895 and served there for the following 35 years. Doctor Sean Levchuck was born in the Bronx and raised in Commack, New York. He graduated from Commack High School South and continued his education at the State University of New York at Stony Brook where he played four years of college ice hockey. After graduating from college, he attended St. Georges University School of Medicine in Grenada, West Indies. He graduated in 1989 and did his pediatric residency at Winthrop University Hospital in Mineola. He served as the pediatric chief resident from 199-92. Upon completion of his residency, he did a fellowship in pediatric cardiology at Saint Christopher's Hospital for Children in Philadelphia where he met his wife, Leslie. He joined Pediatric Cardiology of Long Island and Saint Francis Hospital in 1995. He became involved in the Gift of Life in 1996 following the retirement of doctors Zoltan Mesko and Fillipo Balboni. He organized several fund-raising events for the Gift of Life including the Skate for Life in 1997, 1999, 2001, and 2003. These events were highly successful and involved retired NHL greats and celebrity ice hockey players playing a charity hockey game. In all total, the events raised over $700,000 for the Gift of Life. He has been honored by the Gift of Life as an Angel award winner on several occasions. He is currently the chairman of Pediatrics and Pediatric Cardiology at Saint Francis Hospital and a senior partner at Pediatric Cardiology of Long Island. He serves on the board of directors of Companions in Courage, a charity founded by NHL Hall of Famer Pat Lafontaine.Additionally, he is a part-time clinical instructor of anatomy at Saint Anthony's High School in Huntington NY.  He is board certified in Pediatrics and Pediatric Cardiology. "If we can't ban assault weapons then we should raise the age to purchase them from 18 to 21," Biden said. He also called for a ban on high-capacity magazines, background checks, red flag laws and a repeal of the immunity that protects gun manufacturers from legal liability if their weapons are used in violence. The remarks came the day after the 233rd mass shooting in the U.S. this year took place in Tulsa, Okla., that resulted in five people dead including the shooter at Saint Francis Hospital. As I watched the mass shooting unfold at the Robb Elementary School in Uvalde, TX,  a panic set in.  I could see that no one was  going inside and that meant that there was no enforcement entry to the classroom, and that meant that with each minute, another child's life was in danger.  One thing I can't tell you is why the Uvalde police Chief waited.  Everyone in law enforcement is trained to go toward a shooter, not to stand around.  The possibility of getting shot is part of being an oath sworn officer of the peace. I speak from experience as I have done dozens of these high-risk entries in my career.  Watching the press briefing for the mass shooting at Saint Francis Hospital and the clear and concise timeline given by the Tulsa Police Chief drew such a stark comparison between the two mass shootings.  Not just because in Tulsa, the response to go into the hospital and get the gunman was without hesitation, but because of the amount of information given to the public less than 24 hours after the episode.  It's been 9 days since Uvalde and we still don't have all of the details.  Last week, FBI director Christopher Wray, during a Congressional hearing, warned that so called "lone wolf shooters" carrying out an attack for whatever reason are perhaps the most, active imminent threat the nation faces currently.  If the past 2 weeks is any indication, that warning is beyond urgent

The RaDonda Vaught Case: A Fatal Medication ErrorMs. Terri Stewart, MSN, RNOn March 25th, a jury in Nashville, Tennessee convicted Registered Nurse, RaDonda Vaught of criminally negligent homicide and impaired adult abuse. By the time this episode airs, Ms. Vaught will be sentenced in this conviction. This case has riled up the nursing community (both for and against the conviction). The American Nurses Association has said put out a statement along with the Tennessee Nurses Association that in part says, “Like many nurses who have been monitoring this case closely, we were hopeful for a different outcome. It is a sad day for all of those who are involved, and the families impacted by this tragedy….this ruling will have a long-lasting negative impact on the profession”. Ms. Stewart and I discuss details of the case, medication errors in general terms and reporting of those errors, the impact this case potentially has on the nursing profession and lessons learned moving forward. More About Ms. Stewart:Terri Stewart, a Memphis native, joined the College of Nursing faculty in October of 2017. She received her diploma in nursing from Baptist Hospital School of Nursing in Memphis TN and her Bachelor of Science in Nursing from Union University in Memphis, TN. She received her Master of Science in Nursing with a concentrated study in Nursing Administration from the University of Tennessee Health Science Center College of Nursing. Believe me, filling in for the uncontested master of podcasts, Stacey Richter, is just a tad unnerving! My name is Dr. Steve Schutzer. I'm an orthopedic surgeon specializing in joint replacement surgery, and I think it's fair to say that I'm more comfortable, in my own lane, doing complex surgery than doing this introduction to our encore podcast 294 entitled “Building a Center of Excellence: A Playbook for Physician Entrepreneurs,” which aired originally in October 2020. But when Stacey graciously offered me the honor of doing so, I said to myself (sic: Steve, suck it up) what an opportunity to share with the devoted listeners of this show my humble perspectives on the prominent position COEs (also known as Centers of Excellence) are playing in this rapidly accelerating, evolving, and exciting healthcare landscape. So, there's an ancient Chinese proverb that goes like this: “When the wind of change blows, some build walls, and others build windmills”—or in this case, Centers of Excellence! And the winds of change in healthcare are blowing, maybe even reaching gale force. In the year since episode 294 aired, there's been unambiguous upsurge of activity, in part fueled by the pandemic, that has collectively and finally moved the healthcare value agenda across the chasm, over the inflection point—and there's no turning back. Unaccountable fee for service as the predominant payment model for healthcare services is, well, shall we say, on its last legs—being replaced by reimbursement models that are aligned with the clinical and financial outcome of the services actually delivered to our patients. For COEs, that's characteristically in the form of predictable bundled payments and fully warrantied episodes of care. Question: Where do COEs fit in this new landscape? Answer: COEs are the common pathway for all healthcare purchasers (whether they're self-funded employers, advanced primary care groups, Medicare Advantage—all of them) to steer agnostically to high-quality specialists focused on a defined set of healthcare services and who are willing to assume total cost of care for their product. And the favorable impact of COEs on the ROI for purchasers has now moved beyond the realm of theory to indisputable. Take, for example, the recent report by the RAND Corporation published earlier this year in Health Affairs: A study of over 2300 patients who had either total joint, spine, or bariatric surgery done under the Carrum Health program at one of their COEs. Carrum Health is a value-based national COE platform that connects self-insured employers with top providers under standardized bundled payment arrangements. And now in full disclosure, I serve as medical advisor for the company; and our program, the Connecticut Joint Replacement Institute in Hartford, Connecticut, is actually a Carrum COE. But in this independent RAND analysis of two years of medical claims data, the savings per procedure when the surgery was done at a Carrum COE was over $16,000 per procedure. Readmission rates were reduced 80% on average. Out-of-pocket cost to the patient? Zero. And an astonishing 30% of patients who were in the queue awaiting surgery ultimately were treated nonoperatively! Peter Hayes is president and CEO of the Healthcare Purchaser Alliance of Maine and a frequent guest on this podcast. His organization has been under contract with Carrum for approximately two years and recently reported an ROI of 58% and plan savings approaching $1 million. And these data also closely reflect that reported in the Harvard Business Review two years ago by Ruth Coleman and colleagues from their experience with Walmart COEs. Finally, you know, I heard Stacey say of COEs in one of her podcasts, “This is not something you can do on a Tuesday.” Agree. Prescient advice. As you will hear once again in just a moment, this takes work. But physician leaders and entrepreneurs, take heed. Although you won't be able to stand this up on a Tuesday, there's no reason why you can't begin next Monday. Schutzer, MD, graduated with honors from Union College and the University of Virginia School of Medicine. Following a surgical internship at the University of Rochester, he served as lieutenant in the Medical Corps of the United States Navy. In 2014, Dr. Schutzer and two colleagues, Ms. Steph Kelly and Ms. Maureen Geary, launched a consulting company, Novel Healthcare Solutions, whose mission is to establish effective and trusting business relationships between physicians and hospital partners—and then create orthopedic Centers of Excellence. In the final installment of this season, Paul speaks with Walter Harrison '68 H'18. Walt Harrison's career embodies service in many forms—education, military, and a dedication to the Hartford community. After graduating from Trinity, Walt served as a captain in the Air Force before earning his doctorate and pursuing a career as an educator. He was president of the University of Hartford for nearly 20 years, and he has had many positions on local boards, including Saint Francis Hospital and Medical Center, Hartford Promise, and Capitol Squash (located on Trinity's campus). In 2018, Trinity awarded him an honorary degree. Walt is currently a member of Trinity's board of trustees.

To date, $20 million has been raised. The additional funds will allow Saint Francis Hospital to increase the current footprint of the outpatient Cancer Center to provide improved access to vital oncology services. Construction is set to begin by the end of this calendar year.

Lately, several of the Relentless Health Value episodes have focused on digital health companies and their disruptive potential on referral flows of traditional provider organizations. We also talked about other goings-on with the potential to encroach on hospital systems and independent docs alike. For example, we’ve got Walmart getting, in a big way, into the health clinic business. We’ve got VillageMD and Walgreens teaming up. We’ve got mergers in the on-site clinic space. There’s just a lot of action. But let’s talk about what Dan O’Neill called “physician entrepreneurship” in EP287. Dan said that now is a fantastic time for entrepreneurial physicians to reinvent the practice model. This is true because many, including Dr. Matt Anderson in EP292, have said that it’s not an entirely safe bet if you’re a doc right now to hope that all the practice changes initiated by COVID (like telehealth, etc) go away like a bad hangover the second this pandemic gets stuffed back into Pandora’s box.   So, there’s risk mitigation strategies at play here, but there’s also a great opportunity for those who figure out how to legitimately improve patient outcomes in a way that consumers and patients love and that employers can easily contract for. Here’s the bottom line: Some, not all, of these new-fangled deliverers of health care have great marketing and maybe a great Net Promoter Score; but effectiveness is less than well validated. Don’t get me wrong. There’s a whole lot of providers who aren’t sure what kind of results they deliver and who aren’t exactly delivering amazing and sticky customer experiences. So, we certainly can’t forget that, as Bob Matthews has said, in the land of the blind, the one-eyed man is king. But what about a physician practice known in a local community that works together to create a Center of Excellence? Now, that’s interesting in this land of the blind. You get all the history and the advantage of being the “default care provider.” But you also are well poised for a post-COVID future, even in the face of all this disruptive activity. In this health care podcast, I speak with Steve Schutzer, MD. Dr. Schutzer is a physician executive for the orthopedic service line at Trinity Health of New England and medical director of Connecticut Joint Replacement. Dr. Schutzer knows a lot about setting up a COE, otherwise known as a Center of Excellence. Schutzer, MD, graduated with honors from Union College and the University of Virginia School of Medicine. Following a surgical internship at the University of Rochester, he served as lieutenant in the Medical Corps of the United States Navy. After his tour of duty, Dr. Schutzer did his general surgical training at the University of Rochester and then completed his orthopedic residency at the University of Connecticut. He was then a fellow in adult hip and reconstructive surgery at the Massachusetts General Hospital, after which he entered practice with Orthopedic Associates of Hartford. He is currently on the staff of Saint Francis Hospital in Hartford, Connecticut. Dr. Schutzer is a founding member and medical director of the Connecticut Joint Replacement Institute (CJRI), a Center of Excellence at Saint Francis. He is also president of the management company overseeing the Institute, the Connecticut Joint Replacement Surgeons, LLC, as well as physician executive for the orthopedic service line at Trinity Health of New England. John Rodis, MD, MBA, is an OB/GYN specializing in high-risk pregnancies. He’s been a board examiner and a department chairman. He’s been a COO and a CEO of a 600-bed Level 1 trauma center. He’s also an author working on a book to help consumers make better choices. Dr. Rodis has said he feels an inflection point is coming in the transition to value. First, we have the pressure of large employers. Second, we’ve got doctors themselves who are being crushed by the current environment and who may also have realized that FFS (fee for service) is kinda risky in the middle of a pandemic. The third force toward the inflection point is the new breed of fee-only transparent brokers. And then fourth, we’ve got the government, particularly state governments who are struggling fiscally coming out of this pandemic and also realizing that the current health care system is pretty rigged to profit on the backs of taxpayers and firefighters and teachers. In the face of this transition, health systems who aren’t keeping up with the times are at increasing risk. Given that Dr. Rodis has held pretty much every job in health care at this point, he’s probably the perfect person to ask how to quantify that risk, number one, and then what health system leaders should be doing in the face of it. Rodis, MD, MBA, is a high-risk obstetrician by training and has had a long and illustrious career as a renowned health care leader. Most recently, he was the president of Saint Francis Hospital and Medical Center in Hartford, Connecticut. Dr. Rodis had two stints at Saint Francis, starting there early in his career as a maternal-fetal medicine fellow in 1985. In between, he was head of obstetrics and gynecology at Stamford Hospital, served as its chief medical officer, was a faculty member at the UConn School of Medicine, and became chair of OB/GYN at Saint Francis in 2011. Dr. Rodis took the helm at Saint Francis as a recent state provider tax was quickly expanding into a huge fiscal burden on Connecticut’s acute care hospitals. In December 2015, he was named its eighth president. Dr. Rodis was the first physician to be appointed president in Saint Francis’s 118-year-history. He guided the hospital through an era of consolidation and rising state taxes. His last challenge arrived in spring 2020 as he oversaw an expansion of temporary facilities as the hospital faced the crush of the coronavirus pandemic. Also, during his tenure as president, Dr. Rodis ushered the Saint Francis team through significant change and transformation, much of which has been recognized at the national and regional level. Those transformations led them to achieve an “A” Safety Grade from Leapfrog seven of the last eight grading cycles. Saint Francis was also named Best Regional Hospital in 2019 by US News & World Report and was the only hospital in Connecticut named one of the nation’s 100 Top Hospitals by IBM Watson Health. Dr. Rodis’s passion is patient safety and, in particular, providing consumers (patients) with reliable data to make informed health care decisions. He departed his post as president of Saint Francis in May 2020. Aimee specializes in space planning and interior design development for corporate and healthcare clients. Her leadership role with Page builds on her commitment and collaborative approach to her work. “I like the idea of freedom within commitment. Being truly committed to my work allows me to take design risks, and the projects I work on are better for it,” Aimee says, adding, “Collaboration is key. Everyone has something to contribute to the project process.” My favorite part of our conversation was the story Aimee shared about how in order to win a project, the developer asked Aimee and her team to design a hospital out of legos in two hours (without prior knowledge and in the developer’s office) to assess how well the team could collaborate. That and how Aimee came into healthcare design kicking and screaming and now she loves it! Aimee realized at an early age that she had the talent and ambition to work as a creative professional. “Being an interior designer is all I’ve ever wanted to do,” Aimee explains. With that goal set, she prepared for her career by graduating from Texas Christian University with a Bachelor of Science in Interior Design, a program accredited by the Foundation for Interior Design Education Research (FIDER). With her work, she takes great pride in applying lessons learned from her previous projects to add value to the next. “Each new project brings the opportunity to build upon both my technical knowledge and creativity,” she says. “A new project also brings the chance to work with new team members I may not have worked with before. I like this relationship-building because it allows me to expand my ‘go-to’ resources within Page. She completed her pharmacy degree at University of California-San Francisco School of Pharmacy. After graduation, Dr. Yu completed a Pharmacy Practice Residency at University of Kentucky Medical Center followed by a HIV Care/Infectious Diseases Residency at University of Illinois at Chicago. She achieved board certification as a Pharmacotherapy Specialist with added qualifications in Infectious Diseases in 2015. She currently participates routinely in the Antimicrobial Stewardship Program at Children’s Mercy Hospital and is a preceptor for both student pharmacists and pharmacy residents. Her current research interests include the role of antimicrobial stewardship in decreasing antibiotic resistance and clinical outcomes, non-occupational HIV post-exposure prophylaxis, and the role of rapid diagnostics and antimicrobial stewardship        About the Host of PPAG Conversations Bob John, Pharm.D., BCPS, BCPPS, a member of the PPAG Board of Directors, is currently a NICU Clinical Pharmacy Specialist at The Children’s Hospital at Saint Francis and the Residency Program Director for his institution’s PGY1. William Anthony Colón nació en el Saint-Francis Hospital del barrio de Mott Haven al suroeste del Bronx en 1950. Su abuela Antonia Román dijo desde que lo vio que iba a ser una estrella de la música, pero hasta que cumplió once años nada indicó que así fuera a ser. El niño se crió en un ambiente de enorme pobreza y violencia, donde era más fácil ser “una bala perdida” que un músico destacado. A Colón le tocó vivir la época de los incendios provocados en las casas de protección oficial del Bronx y de la discriminación simbolizada por el blockbusting.
