In this session we will examine one of the common greatest human factors challenges within pre-hospital care, that of acute fatigue within clinical practice. The ambulance services are trying out different ways of working to help staff feel less tired at work and safer on scene, but these actions are often localised and have no empirical underpinning, and also we don't know whether they are making working environments safer. Kristy is investigating whether patient and staff outcomes can be improved through development and implementation of a fatigue risk management system (FRMS), as is done in other safety-critical industries like aviation and transport. The evidence suggests individual components of a FRMS which may be effective, Kristy is currently investigating the optimal packaging of these interventions. Kristy believes that FRMS adoption in the NHS needs local tailoring and understanding of barriers and facilitators. Kristy is looking at a way to integrate a comprehensive fatigue risk management system for the UK NHS ambulance sector that is acceptable, feasible, and likely to improve patient outcomes and staff wellbeing and experience. Within the episode we will examine: 1. Kristy's research approach to an agreed set of evidence-based and emerging components of a FRMS for the UK ambulance sector that are considered feasible and acceptable. 2. The wider CATNAPS study and its wide ranging primary and secondary outcomes. 2. The components of a comprehensive FRMS that are in use and why. 3. The ways in which front-line staff and patients experience current fatigue actions and potential to improve safety culture and reporting. 4. Development and usability testing of the FRMS and its implementation guide that allows tailoring to organisational and local context and is underpinned by a new theory of change and logic model. 5. The 12 hour day and and night shift, both pros and cons of this pattern of working. 6. Fatigue mitigation and how the FRMS may serve to support this. 7. Second and third order effects of fatigue. Rarely does it have one simple cause but more often it requires a multifocal approach. Dr. Wada shares her thinking about fatigue as both physician and Sjogren's patient. "Fatigue" does not equal "tired." Fatigue is something else, and it can be exponentially worse. It isn't solved with "just get more sleep," and day after day of it can really wear you down. Fatigue — real fatigue — is a common symptom of menopause. It makes work hard, exercise impossible, even thinking straight can feel like it takes all your resources. The kind of fatigue we're talking about can be a result of less estrogen to "feed" your brain. If you have some brain fog, forgetfulness, are easily distracted and occasionally confused, it's all part of the same process: your brain is trying to adapt to less estrogen. While this part of perimenopause and menopause is temporary, it can last a year or two or longer. And if you don't know what's happening, it can be frightening. We hear from women all the time that they were terrified they were developing early onset dementia or that they'd been to a round of doctors to deal with "chronic fatigue." Finding out it's likely a (temporary) result of menopause is a huge relief. Even worse than the symptom itself is that often, no one even mentions the culprit could be diminishing hormones, leading women to get expensive tests, waste time on incorrect diagnoses, and live in fear over a totally normal process. There are things you can do to fight the fatigue: the right nutrition and exercise are powerful tools, and there are medications that can help while your brain and body adjust to the new normal.  In this podcast, we hear from Gennev Director of Health Coaching, Registered Dietitian Nutritionist and Exercise Physiologist Stasi Kasianchuk and Naturopathic Physician Dr. Wendy Ellis on how to handle menopause fatigue.

It can be difficult to figure out how to maintain a healthy balance and feel "normal" again, specifically from an energy perspective.   In this series, I want to share with you some of the lessons learned about to fight fatigue.   Join me as I discuss some easy ways to start to change your diet:   -Start by writing down what you're eating. -Evaluate that list of foods and determine your next steps. -If you need help, visit a dietician and nutritionist as appropriate. -Start small by changing your food one item at a time. -Examine how you feel as you start to remove foods that don't help you and write down how you feel when you start to eat better. -Evaluate your energy levels after changing your diet.

Should you toughen up and push through your training, or should you be smart and back off? Matt explores the concept of fatigue in today's podcast. Fatigue is not a negative word. In fact, it's a part of growth and you should lean into it. It is earned from hard work and the adaptions can be positive, but you must make smart decisions in managing your fatigue.  Context is everything when you're talking about this vital topic. There's a big difference between expected and anticipated fatigue (in a hard session or in a heavy block close to a race) and unanticipated fatigue (not responding to several light days, poor performance over several training weeks, persistent injury, and illness, etc.). In today's episode, Matt takes a deep dive on both objective and subjective markers of fatigue. How do you know if what you're feeling is normal or not? A lot of it depends on where you are at in your season; what lens you are looking through. He describes how to ease back into a regular training schedule after a few days of rest and even goes through multiple case studies to illustrate when it's appropriate to back off or push through a tough session. Your training can’t be just checking the box of training sessions.
