Prior Authorization is a not a popular topic. In fact, we have not found many organizations willing to talk about it. Simply put, it is a complex challenge. We were pleasantly surprised when Frank Harvey, the CEO of Surescripts was willing to come on-camera to share the work they are doing in this area. Surecripts is collaborating with Cleveland Clinic and OptumRx to streamline the prior authorization process. What used to take days or hours can now be completed in seconds – without changing physician workflows. This is possible through careful coordination of data between the provider, payer, and Surescripts. This advancement not only ensures patients can access critical therapies sooner, but also reduces provider burnout. For now, this process is only for a limited number of therapeutic areas, but Surescripts plans to expand to more in the coming year. I'm gonna encore this episode with David Scheinker, PhD, for several reasons; but here's a big one: Why are we as an industry not doing what David Scheinker suggests in this episode? Why are we not doing, I don't know, kinds of logical things to reduce admin burden in this country when everyone agrees admin burden is a problem? But let me back up for a moment for context. Two things happened since this show originally aired. One is that I was invited to a fireside chat by the Advisory Board to talk with Abby Burns, one of the amazing hosts over at Radio Advisory; and we talked about value in the healthcare industry. And if you define value as benefit divided by costs, and you can cut costs—like cut admin burden costs in half—then you have created some really nice communal value, which we talked about at length during that aforementioned fireside chat. Here's the other thing that happened since this show originally aired. I read the book by Mike Leavitt, mainly because Steve Schutzer, MD, kept talking about it. The title of the book is Finding Allies, Building Alliances. Maybe I will do a book report about this at some point, but let me share a couple of key quotes just to get the party started here. Mike Leavitt wrote, “A diverse alliance, well led and well managed, can bring resources to bear on a problem that no organization can match—even the largest of organizations. The synergy of resources—from financial to intellectual—can deal effectively with a wide range of issues confounding organizations today.” I found that very interesting. Here's the second quote, which deals with what the top reason is that such diverse alliances may wish to hook up. “[It's] a common pain: A shared problem that motivates people and groups to work together in ways that could otherwise seem counterintuitive.” Hmm … so, back to administrative burden. Let's review the facts that David Scheinker, PhD, shares in the interview that follows. He says any given transaction will cost provider organizations 14% of the total transaction costs to manage to get paid. Yes, it costs 14% of a transaction merely to get paid for the transaction. This is a big reason why both Peter Hayes, in the episode with him (EP424), and also Marshall Allen (EP425) talk about for why cash prices can be a whole lot less than going through insurance prices because you can skip a lot of insurance burden. Now, on the payer side, add to that 14% an additional 5% to 15% to pay said transaction. That 30% of healthcare is waste stat that keeps getting tossed around. Listen to the show with Will Shrank, MD (EP413) for more on that. But, yeah … here's 20% to 30% of every transaction that is waste. And we haven't even gotten into redundant care or inappropriate back surgery yet. Our industry spends up to 30% of our money just trying to get paid and pay. Here's a case study for you. You know who has already solved for this whole “it's really hard to get paid and pay” dilemma? Derivative traders. It used to cost derivative traders $100,000 to do a contract, any given contract. And they worked together and got this down to $5000 by doing some of the stuff that David Scheinker talks about in the show. And, I don't know, I feel like the healthcare industry could also do this, too, if they wanted to. But there are a whole bunch of reasons why our industry cannot seem to get together and be as ruthlessly practical as derivative traders—or banks, who have figured out how to work together to process credit cards to reduce their own common pain. Here are but a few of the reasons, potentially, why the healthcare industry doesn't get together to reduce administrative burden in some of the ways that Dr. Scheinker talks about. 1. Some organizations actually make a lot of money off of that transactional waste. As but one example—and not to just pick on one, but we don't have all day—how about some RCM (revenue cycle management) companies who may or may not be owned by the same vertically integrated stacks as the payers themselves? As I have said any number of times, one person's—or potentially an entire country's, as the case may be—one party's waste, is somebody else's honeypot; and I am not sure if this is any exception. 2. Legacy technology and data systems and all the sunk costs therein 3. As Kaye Davis and Katrina Hubbard reminded me about the other day, there are some serious regulations in healthcare due to everybody being a vendor of CMS that adds a layer of regulatory complication to many collaborations. Also, state laws sometimes have an unintended side effect of making it tough to collaborate. Now, are there any precedents for this type of collaboration in the healthcare industry? Yeah, actually Surescripts, which, don't forget, was created by an alliance of PBMs (pharmacy benefit managers) who worked together because they all wanted to enable e-prescribing and needed a joint platform to do it. Look, I could say a lot about this one, but nonetheless, so much of what gets talked about in the show today with Dr. David Scheinker is very, very actionable. Just want to note that since David Scheinker was on the show, he and his team have done some major research over the past few years into ways that contracts can be standardized. If enough of you reach out and say that you're interested, we, for sure, can have David come back on the show and discuss. David Scheinker, PhD, is a clinical professor of pediatrics. He's the executive director of systems design and collaborative research at Stanford Children's Health. He also founded and directs SURF Stanford Medicine. And with that, here is your original episode. Administrative costs in the United States have a bad rap. You don't have to look too far to find an article about how there's now, like, 10 administrators for every 1 physician in this country. Or 3 to 4 billing people for every physician. Or consider what Dan O'Neill was talking about in episode 359. He was talking about IPAs (independent physician associations) and other managed care entities. As Dan mentions, contracting with some of these IPAs is like an “I love 1990” flashback. The contracting process transpires via mail. Not email, mind you. Mail. Like, stick-a-stamp-on-the-envelope mail. So, in sum, there's a lot of pretty well-founded complaining about administrative costs in this country. A lot of this administrative stuff is truly inefficient and a fantastical waste of time. So, here we are freaking out about staffing shortages, overlooking that doctors at the heights of their careers are spending some percentage of their time not counseling, treating, or diagnosing patients but twiddling their thumbs on hold with one insurance company or another slowly burning out by the inefficiency of it all. Or doing pajama time, and we all know that too much pajama time means also burnout on a silver platter. So then, let's get granular here. If we're trying to quantify admin costs, how you do that is to quantify how much each transaction costs. How much does it cost to send a bill and get paid for it? How much does it cost to file an appeal and a denial of a prior auth? Add all those transactions together and you get the full cost of the administrative burden. In this healthcare podcast, we're digging into a paper about admin costs written by David Scheinker, PhD (my guest today); Barak Richman, PhD, JD; Arnold Milstein, MD, MPH; and Kevin Schulman, MD, MBA. I have the pleasure of speaking with David Scheinker, PhD (as I mentioned), who is the lead author on this paper. Just to underline a major takeaway from this conversation with Dr. David Scheinker, he reiterates a recommendation to eliminate a big proportion of administrative costs. I guess I should say spoiler alert here, but the major takeaway/recommendation is this: Standardize healthcare contracts between payers and providers. Every payer and every provider finds one contract template and uses it. I don't mean one template per payer or per provider, although that probably would be a revelation in and of itself. But I mean that all payers use one basic provider contract. A couple of specifics here: The template that I'm referring to (and that Dr. David Scheinker is referring to) consists of parameters. What do I mean when I say parameters? Consider what Airbnb does when you're looking for a place to stay, as an example. How many bedrooms (that's a parameter)? How many bathrooms (that's a parameter)? How many amenities (that's a parameter)? After everybody picks their standard set of parameters, at that point, all parties can negotiate and come up with whatever they want for what is the price of an extra bedroom or whatever value you're gonna assign to that parameter. Go nuts there, but from a data collection and analytic perspective and a getting paid perspective, it is way easier to do it that way—meaning it's way easier to execute and report when all of the contracts use the same parameters. Also, you can build tech to do a lot of that because you don't have to write algorithms with exponential variables. Pooja Babbrah then opens the episode explaining that she and co-host Jocelyn Keegan share their key takeaways and insights from the biggest health technology conference in the industry. Jocelyn started the conversation off by sharing her observation that the work over the last 15 years to forge scalable connections to improve data fluidity has been fruitful and reached a tipping point. Importantly, the conversations we heard at HIMSS24 showed an acknowledgment as important as data fluidity is the content of what we are exchanging, and that data quality is just as important. Jocelyn then shared her second observation which is that we need to build a skillset and pool of resources who can do the next phase of work that is needed. She shared that when she worked in the financial industry which was at the time transitioning to interoperability and automation, they had to hire and develop the critical thinkers needed to think through how to best leverage technology, approach change management and do the abstractions needed to be successful. Health IT is at a similar place; we probably need more clinical informaticists because how we use technology in clinical workflows is extremely important, but we also need operational experts to help pull through how shifts in one workflow should get pulled through in others. She went on to say that we need people understand data mapping and why clean data is so important. Pooja thanked Jocelyn for her insights before sharing her own. She mentioned that she heard talk about whether ViVE and HIMSS could both survive because they occur so close together, and are big investments for companies to make to attend or exhibit. She shared that her view is that they are different enough in programmatic and attendance mix that she believes they both have value and will survive. Adding to Jocelyn's point about data quality, Pooja added that data governance and sending the right data to the right people at the right time versus just sending a data tsunami. Pooja continued by adding that it's also important to meet people where they are. This was clear at the post-acute care listening session. Some care facilities are not yet FHIR-enabled, some can send data via FHIR but perhaps are not yet API-enabled, how can we meet those facilities where they are even if they aren't using cutting-edge technology? Most stakeholders are going to be somewhere on a spectrum of tech adoption. We need to be flexible in how we do things. Pooja went on to say that another observation is that the importance of consent management will continue to grow, and that real work is starting to be done to figure out what patient-centered consent management might look like. We need to be able to exchange the right data at the right time with the right people, but we need to ensure we have the right consent to go along with it. Pooja mentioned some industry initiatives on consent including the HL7 FHIR at Scale Taskforce Accelerator, which has launched a new consent project. Public meetings for the FAST Consent project have launched and will be held regularly. A consent learning lab was held at HIMSS24 for the second year in a row. Pooja expressed that she attended both last year's and this year's events and she could feel a shift including the presence of regulators who were in the room. She explained that the conversation focused on actual work and progress being made in California and Florida as well as a huge focus on the role of consent in connecting health data and human services data.Next Pooja and Jocelyn listened and reacted to stakeholders who stopped by the POCP Kiosk in the Interoperability showcase to give their perspectives on the final CMS prior authorization & interoperability rule, API adoption, and pharmacy interoperability. Interviewees at the kiosk included:Colin Banas, Chief Medical Officer for DrFirst, shared insights on medication management and prior authorization.Polina Vaserman, Vice President of product Management with MHK, discussed the complexities and proposed improvements in prior authorization final rule.Lathe Bigler, Vice President of Clinical Network Services at First Databank provided perspectives on leveraging FHIR APIs for improving price transparency and patient care.Lenel James, Business Lead - Health Information Exchange & Innovation, Blue Cross Blue Shield Association, highlighted the use of FHIR APIs to enhance data accessibility for all stakeholders, including patients, payers, and providers, underscoring the importance of making healthcare data available across the care continuum to improve service delivery and care coordination.Melanie Marcus, Chief Marketing and Customer Experience Officer at Surescripts highlighted the role of pharmacies in expanding care access and the necessity for interoperability and policy support.Kathryn Ayers Wickenhauser, Sr. Director of Community Strategy at DirectTrust discusses the critical role of pharmacists in delivering care and the role of Direct Messaging can play in supporting this work. Themes from the interviews and host response include: CMS Interoperability and Prior Authorization: There has been broad acknowledgment of the positive progress this final rule will make towards enhancing interoperability and refining the prior authorization processes is widespread, with some hoping for additional policies to go even further.Healthcare Ecosystem and CMS Policies: The industry acknowledges the complexities of medications requiring prior authorization. There's an expectation for CMS to broaden its research and policy scope to include specialty medications, recognizing their critical importance.Impact of Technology and Innovation: The importance of adopting standards like FHIR for improving healthcare interoperability is emphasized. Leveraging technology and standard adherence is highlighted as crucial for enabling effective communication among healthcare stakeholders, reducing administrative burdens, and improving patient care quality.Pharmacy's Essential Role: Pharmacies are spotlighted as key to healthcare delivery, especially in underserved areas. There's strong advocacy for improved pharmacy interoperability and integration into the care team, stressing the need for policies, payment reforms, and access to health information to support their expanded role.Using Existing Technologies: The discussions promote the practical use of existing technologies and workflows to address healthcare delivery challenges effectively. This pragmatic approach centers on solving immediate patient care and business problems with available tools and sharing successful industry practices to foster broader adoption.Future Directions and Collective Engagement: A desire for ongoing engagement with health IT innovations, with a focus on interoperability, is clear in future industry events. The importance of collaborative efforts within the community and across the industry is underscored as critical for driving advancements in health IT and interoperability.After Pooja and Jocelyn listened and reacted to the interviews, Pooja shifted to close out the episode. She took a moment to reflect on the rich dialogue and insights shared by professionals across the healthcare ecosystem. She acknowledged the collective and collaborative effort that is fundamental to advancing health IT, underscoring the critical role of technology, the growing importance of pharmacies in the healthcare delivery ecosystem, and the collective anticipation for the evolution of policies concerning specialty medications and the work toward a better way to manage consent. Show Notes: John Driscoll is the President of Walgreens Boots Alliance and the co-host of the second largest healthcare podcast on YouTube, CareTalk. In this episode, host Will Bachman welcomed John to the show. John has extensive experience in the healthcare industry, having played a leading role in creating successful companies like MedCo, Surescripts, Oxford Health Plans, and Care Centrix. John discusses topics such as monkeypox, mask mandates, baby formula shortages, Silicon Valley Bank's failure, and healthcare related topics. He also shares his  opinions on a variety of healthcare issues.  The Future of Pharmacies  John started his career in health care after being influenced by his mother, a nurse. After college, he developed housing programs for homeless and mentally ill people. He then became a management consultant, before moving to Asheville Health Plan. He has worked in multiple parts of the health industry spectrum, gaining experience in Medicare and Medicaid.  He has found his career to be full of big and challenging problems, but is motivated to improve the world and create successful businesses. His main initiatives involve investing in Village MD, Summit, and Espadin Shield Specialty Pharmacy to show how an integrated pharmacy and clinical model can improve outcomes at lower cost. Walgreens is also working with health plans and has 9000 stores and 90,000 clinicians to provide access and convenience, trust, and traffic. Their goal is to become essential in healthcare by leveraging their expertise in retail and pharmacy services. He discusses how pharmacies can become better partners to health plans by taking advantage of their advantages in brand marketing, trust, convenience, and patient specificity. He shares  a specific example of how a large regional health plan achieved a return rate of three times greater on colorectal screenings than before they partnered with the pharmacy. The pharmacy was also able to increase the rate of colorectal screenings up to 51% in underserved areas due to the support of non-clinical personnel in their stores. Additionally, the pharmacy is partnering with health plans to reduce admissions to nursing homes by 20% through nurse visits, follow-ups, and care navigation. The system leads to better patient and contact information.  Overall, the pharmacy is providing an invaluable service in helping health plans lower costs while improving outcomes. He explains how his company invests in bespoke automation machines to allow pharmacists to spend more face-to-face time with patients, and how pharmacists could be more involved and take on more responsibility in the treatment of patients and how this would expand access to more care in an overburdened health system. Pharmaceutical Healthcare,  the Pandemic, and Value-based Care John reflects on  health regulations and restrictions and also the risk taking and flexibility that came into place under the COVID pandemic. He confirms how important it is to prepare for pandemics in terms of vaccine supplies, communication, and pre-deploying and integrating national and state employees in operations. He also notes the critical role of pharmacies in crisis situations. He tackles the issue of employee burnout and the current pharmacist shortage and the steps Walgreens is taking to solve the problem across states. On value-based care, he explains how it is the future, especially in the U.S. where healthcare is more expensive than any country in the world, how this is unacceptable, and how this should be approached to address health issues earlier to avoid hospitalization.  The CareTalk Podcast  John talks about the inspiration and intention of his podcast, what the content is and what has surprised him throughout the conversations. Guests that have been on his podcast include Andy Slavitt, Zeke Emanuel, Tony Cosgrove, and Amy Abernathy. When asked what he learned from his guests,  John shares that he was surprised to find out that even though healthcare is very complicated and people are in silos, there is still room for novel and provocative opinions that can be explained. He also found out that everybody knows that drug costs are too high, but there are practical ways to bring them down without destroying the biotechnology industry that is creating new drugs to cure aging and extend lifespans.  The podcast explores various issues surrounding health care and how to best address them. Toby Cosgrove from the Cleveland Clinic brought up the importance of creating heart-centered care in a large inner city hospital. It was noted that there are a lot of people working on solutions to healthcare problems, such as telehealth and AI to solve labor shortages. He also touches on the potential for AI and Chat GPT to be used in health care. While it is still early days, he mentions that there have been some companies experimenting with using these tools. Health Care and AI John talks about the impact of artificial intelligence on healthcare. He believes that AI has the potential to reduce administrative costs and waste in healthcare by simplifying billing and reconciliation processes. He also believes that AI can help in drug discovery, clinical pathways, and personalized care.  He is looking forward to having guests on CareTalk with whom he can discuss the economy, healthcare costs, and the pandemic's effect on inflation. He wants to discuss  the current chaotic economic landscape that is impacting healthcare, and the labor industry in particular. He mentions the growth of traveling nurses who are paid by private equity firms and how hospitals have had the worst six months financially in US history.
