(Target audience: Dileep George himself, and anyone coming from a similar place.) Dileep George is a researcher working at the intersection of AI and neuroscience. He started his career by co-founding Numenta in 2005 with Jeff Hawkins (while a Stanford PhD student), then he left to co-found Vicarious in 2010 with D. Scott Phoenix, and moved to DeepMind in 2022 when DeepMind acquired Vicarious. Dileep was recently interviewed by Daniel Faggella on his "The Trajectory" podcast: YouTube, Apple podcasts, X/Twitter. It's a fun interview that touched on many topics, most of which I'll ignore, in favor of one very important action-relevant disagreement between Dileep and myself. …And this is the point where everyone these days seems to assume that there are only two possible reasons that anyone would ever bring up the topic of Artificial General Intelligence (AGI) safety in conversation: The person is advocating for government regulation of large ML training runs …or the person is advocating against government regulation of large ML training runs. But, no! That's not my disagreement! That's not why I'm writing this post!! Quite the contrary, I join Dileep in being basically unenthusiastic about governmental regulation of large ML training runs right now. Instead, this post is advocating for Differential Intellectual Progress within technical AI research of the type that Dileep is doing - and more specifically, I'm advocating in favor of figuring out a technical approach to sculpting AGI motivations in docile and/or prosocial directions (a.k.a. "solving the technical alignment problem") before figuring out the exact data structures and parameter-updating rules that would constitute an AGI's ability to build and query a powerful world-model. The first half of this post (§1-2) will try to explain what I'm talking about, what it would entail, and why I think it's critically important. The second half of this post (§3) is more specifically my pessimistic response to Dileep's suggestion that, as AGI is gradually developed in the future, people will be able to react and adapt to problems as they arise. I really think Dileep is a brilliant guy with the best of intentions (e.g. he's a signatory on the Asilomar AI Principles). I just think there are some issues that he hasn't spent much time thinking through. I hope that this post will help. Post outline: Section 1 lists some areas of agreement and disagreement between Dileep and me. In particular, we have a giant area of agreement in terms of how we expect future AGI algorithms to work. Our massive common ground here is really why I'm bothering to write this post at all - it makes me hopeful that Dileep & I can have a productive exchange, and not just talk past each other. Section 2 argues that, for the kind of AGI that Dileep is trying to build, there's an unsolved technical alignment problem: How do we set up this kind of AGI with the motivation to behave in a docile and/or prosocial way? Section 3 is my pessimistic push-back on Dileep's optimistic hope that, if AGI is developed gradually, then we can regulate or adapt to problems as they arise: Section 3.1 lists some big obvious societal problems that have been around for a long time, but nevertheless remain unsolved, along with generic discussions of some underlying challenges that have prevented them from being solved, and why those challenges may apply to AGI too. Section 3.2 dives more specifically into the question of whether we can "keep strong AI as a tool, not a successor", as Dileep hopes. I think it sounds nice but will be impossible to pull off. The release of the Malayalam film Voice of Sathyanathan, which was supposed to be the on-screen return of controversial actor Dileep, has been postponed by a week. The film was slated to release on July 14 this weekend, however, the filmmakers have now decided to postpone the release due to torrential rain in Kerala. The comedy-drama features a star-studded ensemble of Joju George, Jude Anthany Joseph, Siddique, Vijayaraghavan, Veena Nandakumar, along with Jagapathi Babu and Bollywood veteran Anupam Kher. The film will now be released on July 21st.

Adit joins the show to talk about his career, love for cryptocurrency and more! ***********Unconventional in reel & real life, Adit Dileep is an actor, writer, producer and former investment banker.Born in Bangalore, India, his parents named him Aditya, which means The Sun, a name he aspires and grows towards. His family is made up of his mother and father and his older sister, Divya.At age two, his family moved to Singapore for his father's IT profession, and his mother gave up her position as a Journalist interviewing Indian Celebrities for India Today. His family moved throughout their eight years in Singapore, and moved again to the United States before Y2K.After settling in Bridgewater, New Jersey, Aditya, now 10 years old and fresh off the boat, had to start over. Upon learning that nobody on the school bus could pronounce this new boy's name, some student called him "Adit," and it stuck.Adit wanted to be an actor, but nobody really knew what he meant or why he had such crazy ideas. When it came time for him to go to college, his parents banned him from applying to art school and gave him three options: medicine, law or engineering. Never one to give in to authority, Adit chose business school, thinking he could learn how to be self-sufficient and run his own brand and company.Adit went on to be accepted in the Scholar's Program at NYU Stern Business School. Fun fact, Aziz Ansari followed the exact same path. While Aziz went straight into comedy after graduating, Adit took jobs in various different sectors within Marketing, Startups, Publishing and of course, Accounting and Finance. He graduated with a B.S. in Finance & Marketing and, still at a loss for how to make his dreams come true, took the highest paying job he could find in investment banking.Two years later, he saved up enough to quit and educate himself on the industry. Bright-eyed and idealistic, Adit printed out Yale Drama School's syllabus and crafted his own curriculum with reputable studios and teachers in New York City. Abstract: Genetic findings have highlighted key roles for microglia in the pathology of neurodegenerative conditions such as Alzheimer's disease (AD). Distinct mutations in the microglial protein TREM2 (triggering receptor expressed on myeloid cells 2) are associated with different forms of neurodegeneration in humans; R47H/+ mutations increase AD risk, while loss-of-function mutations such as TREM2 T66M result in more severe forms of neurodegeneration. We employed gene editing and stem cell models to gain insight into the effects of these mutations on human iPSC-derived microglia. We found divergent effects of TREM2 R47H/+ and T66M mutations on gene expression, with R47H/+ cells exhibiting a pro-inflammatory gene expression signature. Both the TREM2 R47H/+ and T66M mutations caused similar impairments in microglial movement and the uptake of multiple substrates, while R47H/+ microglia were hyper-responsive to inflammatory stimuli, consistent with their gene expression signature. We developed an in vitro laser-induced injury model in neuron-microglia co-cultures, finding an impaired injury response by TREM2 R47H/+ microglia. Furthermore, in xenotransplantation experiments, mouse brains transplanted with TREM2 R47H/+ microglia exhibited reduced synaptic density. Consistently, we observed upregulation of multiple complement cascade components in TREM2 R47H/+ microglia, suggesting that inappropriate synaptic pruning may underlie the effect. Thus, these findings identify shared and distinct effects of these two TREM2 mutations on microglial gene expression and function. While the TREM2 T66M mutation impairs microglial movement and uptake processes, the TREM2 R47H/+ mutation additionally confers multiple potentially detrimental effects on human microglia, likely to underlie its association with AD. The 2017 assault against a popular Malayalam actor, who was allegedly abducted in a moving car in Kerala's Kochi, continues to rock the state with new evidence surfacing, which may give it a fresh direction. The new allegations levelled by filmmaker Balachandra Kumar come at a time when the trial case, which involves top Malayalam actor Dileep as a prime accused, is nearing completion. The case also threw a shadow over the Malayalam film industry, which in the start supported the survivor but quickly moved to the Dileep camp. Even the powerful Association of Malayalam Movie Artists, which revoked Dileep's membership initially, took him back. The case has dragged on in the Kerala HC for over two years, with several key witnesses turning hostile and resignation of two public prosecutors. These twists and turns in the case were also highlighted in the survivor's statement, who, on 10 January, revealed her identity for the first time. In a social media post, she said that “this has not been an easy journey. The journey from being a victim to becoming a survivor.” So what are these new startling allegations? Why are they important? This initiative, driven by Dr. Dileep Yavagal, aims to help more stroke survivors, communities, and economies in the developed and developing world by reduces the level of disability cause by stroke. We talk about that initiative this week, but first we get an update on Stem Cells. Dr. Yavagal was a guest on Strokecast back in episode 42. We talked his work in stem cell research to treat acute stroke patients. The work was promising but still experimental. It still is. Dileep gives us an update on the progress and research protocols involving the treatment. The bottom line is there is NO approved stem cell therapy in the US today. Research has not sufficiently demonstrated safety and effectiveness. But they're working on it. The only stem cell therapy available for stroke is experimental in research studies. If you choose to participate in those studies, great! Research studies provide the treatment for free. If someone tries to charge you or collect a fee for stem cell therapy, run, hobble, or wheel away as fast as you can. It is not a legitimate therapy at this time. Bio Dr. Dileep R. Yavagal, MD, FAHA, FAAN, FSVIN is the Director of Interventional Neurology and Co-Director of Neuroendovascular Surgery at the University of Miami & Jackson Memorial Hospitals and Clinical Professor of Neurology and Neurosurgery at the University of Miami School of Medicine. He has recently been appointed to lead the Neurological Cell Therapy Platform at the Interdisciplinary Stem Cell Institute at the University. Dr. Yavagal is an international thought leader in endovascular therapy for ischemic and hemorrhagic stroke as well as a pioneer in the translation of intra-arterial delivery of cell therapy for stroke. He was the national Co-PI of the first US multicenter clinical trial of Intra-arterial delivery of autologous bone marrow stem cells for ischemic stroke: RECOVER Stroke. He was on the on the steering committee of the SWIFT-Prime and MR RESCUE, both landmark randomized clinical trials of endovascular stroke therapy. He co-authored the landmark 2015 AHA Endovascular Stroke Therapy Guidelines as well as the recent groundbreaking DAWN stroke trial in the New England Journal of Medicine. He is the founder and Past-President of the Society for Vascular and Interventional Neurology (SVIN). He has also co-authored the AHA Policy statement on Stroke Systems of Care. Dr. Yavagal has received several state and federal research grants to study endovascular stem cell therapies for ischemic stroke using small and large animal models of stroke in his research laboratory. He is considered a pioneering researcher the field of intra-arterial delivery of stem cells in stroke therapy. Dr. Yavagal is Chair of the Society of Vascular and Interventional Neurology’s (SVIN) global campaign called Mission Thrombectomy 2020+ (MT2020+) and is leading a no-holds-barred charge to accelerate access and remove geographic disparities to mechanical thrombectomy (MT), an interventional treatment for a common type of disabling stroke. MT2020+ is calling on public health policymakers to increase physical, financial and diagnostic access to MT for patients with disabling strokes. Stem Cell Updates Dr. Yavagal's research is about the use of stem cell therapy to treat stroke patients in the first few days of their stroke -- the acute phase. The process his team is exploring involves placing the donor stem cell at the point of injury by using a catheter through the groin or wrist. This method is similar to his other key projects around mechanical thrombectomy. Both involving similar equipment -- a catheter the surgeon navigates through the patient's arteries to remove or deliver a payload. And it takes place in an angio suite, which is an operating room with special imaging equipment. In a study with the patients' own cells, his team has demonstrated safety, and they have seen results indicating effectiveness, too, even though the study wasn't designed for it. Recent large animal studies have demonstrated that the process is safe. The next step is to do studies to demonstrate the safety of donor stem cells with humans. Then they can explore effectiveness. At this point (January 2021) there are no FDA approved stem cell therapies for stroke patients. But we're getting there. Mission Thrombectomy 2020+ Mechanical Thrombectomy is the gold standard for the treatment of large vessel ischemic stroke, especially when combined with tPA. In mechanical thrombectomy, a surgeon runs a catheter from the femoral artery in the groin or radial artery in the wrist. They navigate to a clot in the brain and drag it out to restore blood flow. tPA is a clot busting medicine that works to break up clots in the body to restore blood flow. When both treatments are used, patients experience much better outcomes. The challenge is they have to be used quickly. Patients have only 90-minutes to 24 hours to get treatment. The actual window varies widely based on the details of the patient's stroke, MRI, general health, and the specifics f the hospital they get to. Most of the time, the window is 3 hours That window has been growing though. When I had my stroke in June of 2017, the window was much smaller. I woke up with symptoms so I had my stroke sometime between 1:00 AM and 7:00 AM. That put me outside the window for the interventions at the time. Six months later the window expanded, and it continues to get better. Would my right MCA at the basal ganglia thrombus been eligible for removal a few months or a year later if my stroke had just held off a little longer? I'm not sure I want to know. Mission Thrombectomy 2020+ is an initiative led by Dr. Yavagal to double the number of thrombectomies performed around the world in both developed and developing countries. A large part of the work involves creating national and regional committees to work with local health ministries. The committees educate politicians, government officials, administrators, and health care influencers on the benefits of mechanical thrombectomy for patients and society. Disability and shortened life is expensive for a community. It's expensive from the loss of the inherent value of human life and quality of life for many survivors. But it's also expensive in sheer economic impact. Long term treatment costs money. Loss of worker productivity costs money. Loss of productivity from caregivers costs money. The opportunity cost from untreated stroke is enormous. In many cases. Prompt mechanical thrombectomy can save lives, reverse some stroke damage, and dramatically reduce the number and severity of disabilities a survivor will live with. When you can make a case for reducing both human and economic costs, you've got a pretty compelling case. That's the case Mission Thrombectomy 2020+ makes around the world. The Jet Plane Comparison Thrombectomy isn't cheap to start up. You need Angio Suites -- specialized emergency room with specialized imaging equipment. It requires super tiny catheters to go through the blood vessels to retrieve the clots. And it requires expert training for neurosurgeons, nurses, and all the other folks who make hospitals work. How can developing nations or less prosperous communities in developed nations afford all that? If they see the value, they'll find a way. Dr. Yavagal compares it to jet travel. Smaller communities or developing nations still often have air service. That's expensive, too. Airport runways aren't just blacktop highways. Airport infrastructure is much more complex and expensive than it looks. Airplanes are expensive. Maintenance on aircraft is expensive. Staff to fly and repair planes require specialized skill. How can so many places afford it? Because the see the value air travel brings. It's critical infrastructure to develop and prosper. And it brings tremendous benefits in both human and economic capital. They find the money because they see the value. And that's the goal of Mission Thrombectomy 2020+. He still does. Taking care of the ducks is a great way to help out after stroke -- it's a way to feel needed and to put value back into the world. Dileep and I discuss his theoretical account of how the thalamus and cortex work together to implement visual inference. We talked previously about his Recursive Cortical Network (RCN) approach to visual inference, which is a probabilistic graph model that can solve hard problems like CAPTCHAs, and more recently we talked about using his RCNs with cloned units to account for cognitive maps related to the hippocampus. On this episode, we walk through how RCNs can map onto thalamo-cortical circuits so a given cortical column can signal whether it believes some concept or feature is present in the world, based on bottom-up incoming sensory evidence, top-down attention, and lateral related features. In Episode 035 back in October, I talk with Dr. Nirav Shah about the general state of stem cell therapy for stroke survivors. Afterwards, Nirav connected me with the lead researcher on the University of Miami's stem cell research team, Dr. Dileep Yavagal so I could talk to him about his research. That brings us to today's episode. Dr. Yavagal specializes in vascular neurosurgery. That includes procedures like thrombectomy, where a doctor inserts a catheter into a blood vessel in the groin to go up into the brain a pull out the clot. Mechanical thrombectomy can be done up to 24 hours after stroke symptoms begin, and it can have a tremendous impact on minimizing the damage from an ischemic stroke. What it doesn't do today is help patients recover from stroke. It means fewer brain cells die, but the dead and damaged ones don't get any help. When we say time is tissue or time is brain, this is why. Every minute that a clot blocks a vessel, more brain dies. To recover functionality today, we rely on neuroplasticity. PT, OT, Speech therapy, home exercises, eStim, SSRIs, acupuncture, and more therapies are all about encouraging the brain to rewire in such a way that we can work around that dead spot in our heads. But what if there was another way? That's the question stem cell therapy tries to answer. Stem cells, as you may recall from my chat with Nirav, are cells that can become other cells. Embryonic stem cell are the ones we here about in the news, but not the ones used in the trials today. These are critical in embryos because those stem cells turn into all the other cells in our bodies -- nerves, muscle, brain, heart, left pinky, etc. Adults have stem cells, too. The most common source is our bone marrow -- the soft tissue inside our bones where the body actually creates blood. Research is now looking at how we can use the stem cells to drive the growth of fresh neurons in the brain. There are two major approaches in the research, today. The Stanford study demonstrated the safety of its procedure in a small study, but more work is being done to test the effectiveness. That process focused on chronic stroke survivors -- those several years post stroke. In that study, researches a hole in the skull to inject the patient's own stem cells into the damaged area. Results are preliminary, but promising. Dr. Yavagal's work at the University of Miami is different in several ways. First, his work is focused on using stem cell therapy within the first day or two of the onset of symptoms. Secondly, his work relies on donor stem cells since the patient's own stem cells are not available in quantity right after the stroke. Third, his procedure involves delivering the cells to the brain through a catheter, similar to that used during thrombectomy or used to repair an aneurysm. His results are also quite prom, and he's preparing the next phase of study to move the science along and develop safe, effective treatments. The key question we always ask is, "When will this therapy be available?" The answer is we are not there today, despite the clinics popping up claiming to offer the therapy. However, it appears we are 2-3 years out if things go well, or potentially 4-5 years out if they don't. In addition to his work with stem cells, Dr. Yavagal is also one of the leaders of the Thrombectomy 2020 program -- an international initiative to reach 202,000 thrombectomies a year by 2020 and to continue doubling after that. There are communities in the US and around the world where this therapy is simple not available, and yet it can be such an extremely powerful way of reducing the disability caused by stroke, saving hundreds of thousands of people from having to deal with the deficits of stroke and saving billions of dollars in healthcare and disability expenses. We'll talk more about Thrombectomy 2020 in a future episode. Who is Dr. Dileep Yavagal? Dr. Dileep R. Yavagal, MD, FAHA, FAAN, FSVIN is the Director of Interventional Neurology and Co-Director of Neuroendovascular Surgery at the University of Miami & Jackson Memorial Hospitals and Clinical Professor of Neurology and Neurosurgery at the University of Miami School of Medicine. He has recently been appointed to lead the Neurological Cell Therapy Platform at the Interdisciplinary Stem Cell Institute at the University. Dr. Yavagal is an international thought leader in endovascular therapy for ischemic and hemorrhagic stroke as well as a pioneer in the translation of intra-arterial delivery of cell therapy for stroke. He was the national Co-PI of the first US multicenter clinical trial of Intra-arterial delivery of autologous bone marrow stem cells for ischemic stroke: RECOVER Stroke. He was on the on the steering committee of the SWIFT-Prime and MR RESCUE, both landmark randomized clinical trials of endovascular stroke therapy. He co-authored the landmark 2015 AHA Endovascular Stroke Therapy Guidelines as well as the recent groundbreaking DAWN stroke trial in the New England Journal of Medicine. He is the founder and Past-President of the Society for Vascular and Interventional Neurology (SVIN). He has also co-authored the AHA Policy statement on Stroke Systems of Care. Dr. Yavagal has received several state and federal research grants to study endovascular stem cell therapies for ischemic stroke using small and large animal models of stroke in his research laboratory. He is considered a pioneering researcher the field of intra-arterial delivery of stem cells in stroke therapy. Wrapping paper is great when you have 2 functional hands or you use some sort of gift-wrapping service. If you have just one functional hand, it can be more challenging. You can find videos on YouTube demonstrating one-handed techniques, but I prefer the simpler way. Gift Bags! Seriously, make it easy on yourself and uses theses fancy mini shopping bags. Add some tissue paper to the bag, put the gift in, the lightly crumble some more tissue paper to put on top. Then you're done.
