During the 2024 Oncology Nursing Society Congress, CancerNetwork® spoke with multiple registered nurses about research they presented on safely administering treatment options such as CAR T-cell therapy and bispecific T-cell engager (BiTE) therapy in patients with multiple myeloma and other malignancies. Ishmael Applewhite, BSN, RN-BC, OCN, a registered nurse at the University of Rochester Medical Center, highlighted the management of adverse effects including peripheral neuropathy in patients with multiple myeloma undergoing treatment with ciltacabtagene autoleucel (cilta-cel; Carvykti). He discussed these treatment strategies in the context of a presentation he gave on findings from the phase 3 CARDITUDE-4 trial (NCT04181827), in which investigators assessed treatment with cilta-cel in those who were refractory to lenalidomide (Revlimid).1 According to Applewhite, cilta-cel may offer “another path” aside from standard treatment options such as chemotherapy and give “more time” to patients with multiple myeloma. Additionally, Leslie Bennett, MSN, RN, a nurse coordinator at Stanford Healthcare, highlighted the importance of identifying and mitigating cranial nerve palsy (CNP) in patients with multiple myeloma who are treated with cilta-cel. At the conference, Bennett presented data on CNP outcomes across various studies, which included the phase 1/2 CARTITUDE-1 trial (NCT03548207), phase 2 CARTITUDE-2 trial (NCT04133636), and phase 3 CARTITUDE-4 trial (NCT04181827).2 According to findings from this presentation, patients had CNP onset at a median of approximately 3 weeks after beginning treatment with cilta-cel. Most cases of CNP tended to occur in male patients. Kathy Mooney, MSN, RN, ACNS-BC, BMTCN®, OCN®,clinical program director at Johns Hopkins Hospital and Johns Hopkins Health System, spoke about a study designed to evaluate the feasibility and safety of using BiTE therapy to treat those with cancer in an outpatient setting.3 Mooney emphasized multidisciplinary collaboration among nurses, pharmacy providers, and social workers as part of monitoring patients for toxicity as they undergo treatment with BiTE agents. Highlights From Today's Episode “The primary purpose of a certification is an assessment. . . as it's related to oncology nursing, certification evaluates mastery of knowledge and skills required to competently provide specialized oncology care. A certificate program is different. . . . These types of programs are usually short, non-degree–granting programs that provide instruction and training to aid participants in acquiring knowledge, skills, and competencies.” Timestamp (TS) 02:34 “I find that nurses who achieve certification often report an increased feeling of personal accomplishment and satisfaction, and I see right away that increased confidence they have after they've passed their certification. They believe that certification validates specialized knowledge, and it gives them a strong commitment to the profession.” TS 06:09 “The personal confidence and knowledge that the nurse attains from that certification, I do believe, leads to that improved communication and improved patient outcomes. Certification can also promote recognition from peers, and that recognition promotes professional autonomy, which in turn enables the oncology nurse to take on more leadership roles . . . and gives nurses the confidence to speak up and advocate for their patients.” TS 09:37 “Certification increases nurses' knowledge, it ensures that they are up to date on cancer care, and it helps them to be prepared to effectively manage symptoms associated with cancer and cancer treatments. I think teamwork and being able to use effective therapeutic communication skills while caring for people with cancer is something they will acquire, and not only communicating with people with cancer, but also their caregivers and other members of the interprofessional team.” TS 20:22 “Certification offers both personal and professional awards to nurses. It promotes professionalism and demonstrates a commitment to oncology nursing and accountability for our own professional development. While that's obtaining the initial certification, we can't forget maintaining certification. Endometriosis Awareness takes place across the globe during the month of March with a mission to raise awareness of a condition that affects an estimated 200 million people, about 1 in 10 women, worldwide. Endometriosis is an inflammatory condition where tissue, similar to the endometrial tissue that lines of the uterus, grows outside of the uterus.Addressing endometriosis early on is important for minimizing symptoms and potential complications, including fertility challenges. In some cases, endometriosis contributes to full-fledged infertility, or the inability to conceive within a year of trying (or six months, if the couple is over forty years of age).Endometriosis doesn't directly or automatically cause infertility, but it can make it harder for you to conceive. If endometriotic tissue forms on your ovaries or fallopian tubes, it can keep eggs from reaching the uterus, interfering with conception. Endometriosis can also affect fertility by triggering inflammation that may harm sperm or eggs, keeping them from moving freely. Close to an estimated 40% of women with infertility also have endometriosis.Keep in mind that fertility issues affect all sexes. About 1/3  of fertility issues stem from male factor conditions, 1/3 from female factor issues, and 1/3 from both. A doctor might recommend surgery if a person has severe pain that medication does not relieve. With surgery, a skilled surgeon, specializing in endometriosis, can not only find the endometriotic tissue, but also take out all or some of the affected tissue; however this is not a cure because, eventually, endometriosis and pain will return.Doctors typically recommend trying conservative treatment approaches first, opting for surgery only if initial treatments fail. In any case, there are some things to take into consideration, including whether one wants to get pregnant later. You might not be able to have children after certain surgical endometriosis interventions. Endometriosis Awareness takes place across the globe during the month of March with a mission to raise awareness of a condition that affects an estimated 200 million people, about 1 in 10 women, worldwide. Endometriosis is an inflammatory condition where tissue, similar to the endometrial tissue that lines of the uterus, grows outside of the uterus.On average, endometriosis can take anywhere from 4–11 years before diagnosis occurs. Compared with white women, Black women are less likely to be diagnosed with endometriosis. Between misdiagnoses, assumptions that we're  hypochondriacs/drama queens/drug seekers, and additional erroneous assumptions that can go as far as suggesting that symptoms must be from an untreated sexually transmitted infection (or a 'botched abortion gone wrong')— often times, medical professionals don't demonstrate real interest in determining what was causes the symptoms of someone living with an endometriosis diagnosis.Some providers STILL  hold on to antiquated views about endometriosis mainly affecting people of European descent and racist tropes that Black people don't experience pain the same way white people do. We MUST check our providers' racial, gender, and cultural biases to ensure health access and equity for ALL PEOPLE. Equally crucial? Follow through. Following on from a workshop titled “JEDI Journey: This is the Way,” our diverse panel discusses the importance of processes such as integrating the social determinants of health (SDOH) into information systems via Z codes to advance Justice, Equity, Diversity & Inclusion (JEDI) & anti-racism. Until you incentivize it, it does not become a priority. We need to look at policy with a JEDI lens, so Justice, Equity, Diversity, and Inclusion, but I also add in anti-racism, to become anti-racist we have to center and discuss race. We are looking at our policies using an equity impact tool, and we are looking as possible harm as well as alternative approaches.” ~ Daytheon Sturges PA-C “One of the quotes I heard recently is “Nothing about us without us,” it takes bringing those stakeholders to the table, working alongside them and with them, to figure out how we get to more equity in this space.”  ~ Holly Pilson, MD “It's important that the minority people who are leading these efforts are doing it alongside and with the majority members of our departments and institutions, because it takes both together. “It's important to equip the champions and provide education. I have my lived experience as a gender and racial ethnic minority, but I'm not a (DEI) expert.” ~ Holly Pilson, MD “Medical students have consistently said that orthopedics as a specialty is less welcoming. I don't know if it's the surgical culture, some the other specialties mentioned as being less welcoming were also surgical.” ~ Holly Pilson, MD “I like to offer a DEI toolkit that the Physician Assistant Education Association (Diversity and Inclusion Advancement Commission) has developed.  It's 6 steps of a quality improvement loop.” ~ Daytheon Sturges PA-C “Target the leadership structure: what is the racial composition? What voices are there? Do you have buy-in? These are the people who are yielding and wielding power. We need to look at admissions and ask how can we kick the door open and look at our applicants holistically, because this is where the gatekeeping is. Living with an endometriosis diagnosis can create challenges for couples that often lead to disastrous misunderstandings.And lastly, work. How do you navigate work relationships and responsibilities during a flare? Do you take vacation? Short-term disability? Should you tell your co-workers and supervisor that you have endometriosis and explain to them what that potentially means each month? Cities can answer their problems if you bring their leaders, their champions, and the voices of all the cities communities together.”  “Emergency medicine is the only part of the US health system that is user-triggered, and as Prof. McClellan pointed out, the only truly equitable part of the US health system is EMTALA (The Emergency Medical Treatment and Labor Act). What becomes grueling is to try and help in situations where you need to move people beyond the emergency room.”

This we are shaking things up and breaking the model with guest Brandon Mullen, BSN, RN-BC. Brandon has reinvented the Practice Manager as we know it today. He's not sitting at a desk in the Practice 40+ hours a week. He's fractional, spending half of his time onsite at Aiyana Atelier in Virginia and the other half at home in Pennsylvania! How does this work, one might ask? Brandon has been in Nursing since 2009, had a career in healthcare technology, and then answered the call when Ashley Carmen, FNP-C quickly grew her solo owner/injector Practice in Old Town, VA and needed help! Brandon is obsessive when it comes to systems, processes, project management, and details! That's why he's been able to manage both on and off-site as Ashley's grown Aiyana Atelier from now one location to two! From routing phones to shared docs, photo albums, and some help from Asana, he can run a tight ship from 4 hours away! Brandon strategically allocates his time to take vendor meetings, work on client experience, and manage the hands-on things in the Practice during his week long visits. When he's at home, it's all about the administrative work, website updates, planning the second location buildout & launch, and things that we could all use a little peace and quiet to accomplish on the day to day. As Brandon and Ashley scale Aiyana to a second location in Middleburg, he's flexing into different parts of his fractional role. Now, he is going between his home and two locations, and he's responsible for maintaining the level of service, ambience and experience Aiyana's clientele expects at both locations. As in all things that double, it's double the inventory, double the marketing, double the client responsibilities, but he's created such a defined process, that Aiyana's second location is set up for success! Brandon has also been actively consulting with other Practices as they continue to grow. He is fresh off a lecture at AmSpa where this idea & his experiences as a fractional Practice Manager was well received! It's not always feasible to find the best talent locally, and it's hard to find the right fit even when they can be onsite 100% of the time, so this New Frontier is certainly catching traction. One of Brandon's passions (and ours!) is helping Practice owners and new managers to connect with other industry professionals in the same role through an association or network. With a dedicated space just for practice owners and managers, the hope is to take the collective knowledge of the group and share it just like we do with Injectors. For now, he's busy launching a second location, and there's little doubt that more will soon follow, so perhaps when that's done, he can tackle this new challenge!
