Bacterial vaginosis (BV) is known as a normal vaginal microbiota resulting in low lactobacilli; it affects one-quarter to one-third of reproductive-age women. The BV treatment landscape has not appreciably changed in decades: in the US, metronidazole and clindamycin are recommended as first-line treatments for symptomatic BV, and secnidazole and tinidazole are used as alternatives. Although these treatments are effective in the short term, up to 60% of women experience BV recurrence within 1 year of treatment. Some have more frequent recurrences. Suppressive vaginal metronidazole fails for 25% of patients and leads to secondary vulvovaginal candidiasis (VVC) in up to 40%, and many patients have BV recurrence after stopping suppressive therapy. But now a “new” therapeutic option has been in print and is attracting a lot of attention. DQC has been available in other parts of the world for decades, and recently published results from a new European clinical investigation (May 2024) adds more reassuring date. This has led many in the United States to call for trials in this country to begin FDA approval. Listen in for details.

At the age of sixteen, she was thrust into the Religious Technology Centre, and eventually became one of the top executives in Scientology, just beneath David Miscavige, Scientology's current leader.  We relate to the triggering language Deputy Attorney Ariel Anson asks Claire to define, "victim", "handling" and good standing" and relate to our own experiences being intimidated by them as children.  Christi shares her mother's obsessive self-policing, writing reports on herself, when struggling, after achieving the highest processing level of Scientology at that time- Operating Thetan 7- but not experiencing the promised life improvements, unable to support herself, and her children, instead spiraling into psychosis.  Victoria recalls her brother insulting her by calling her a "victim's advocate" when she believed a child victim who claimed Victoria's brother had had inappropriate contact with her.  Claire describes Scientology justice, and confirms members are disallowed to report crimes to outside authorities, or suffer Scientology penalties, including being ex-communicated, and declared a suppressive person.  We read from a Non Enturbulation Order (gag order) issued on a second generation member, raised in the Sea Organization, who spent many years on its prison program, the Rehabilitation Project Force, before he was ultimately declared Suppressive.  We share how honored, and excited we are that Claire Headley will be a guest on our next episode. Abstract: A protein kinase DYRK1A encoded in human chromosome-21 is the major contributor for multiple symptoms observed in Down syndrome patients. In addition, DYRK1A dysfunction has been associated with various neuronal disorders, including autism spectrum disorder and Alzheimer disease. Here we identified FAM53C as a novel suppressive binding partner of DYRK1A. FAM53C bound to the catalytic kinase domain of DYRK1A, whereas DCAF7/WDR68, the major known DYRK1A-binding protein, binds to its N-terminal domain. The binding of FAM53C inhibited the protein kinase activity of DYRK1A to itself and an exogenous substrate MAPT/Tau. FAM53C did not bind directly to DCAF7/WDR68, whereas DYRK1A tethered FAM53C and DCAF7/WDR68 by binding concurrently to both of them. DYRK1A possesses a nuclear localization signal and accumulates in the nucleus when overexpressed in cells. FAM53C induced cytoplasmic re-localization of DYRK1A and DCAF7/WDR68. FAM53C is thus a binding suppressor of DYRK1A, anchoring it in an inactive state in the cytoplasm. The results explain for the first time why endogenous DYRK1A is distributed in the cytoplasm in the normal brain tissues. FAM53C-dependent regulation of DYRK1A may play a significant role in gene expression modification caused by DYRK1A. Copy rights belong to original authors. Visit your primary care provider for additional medical advice.Definition & EpidemiologyGenital herpes is a common sexually transmitted infection caused by a virus called herpes simplex virus (HSV for short). There are two types of HSV. HSV type 1 commonly causes orolabial herpes (known as cold sores), and HSV type 2 typically causes genital herpes, which can present as painful blisters or ulcers in the genital regions. In recent years, an increasing number of genital herpes cases have been associated with HSV-1, especially in women. HSV infections are widespread among the global population and spread person to person through oral-to-oral contact or vaginal, anal, and oral sexual contact. Transmission can occur during periods of subclinical viral shedding, as in even when individuals are asymptomatic. In 2020, the seroprevalence of HSV-2 in the United States was approximately 13 percent among patients aged 15 to 49, with more women affected than men. Fifty to 80 percent of American adults have oral herpes (HSV-1), which causes cold sores or fever blisters in or around the mouth. HSV is a lifelong infection characterized by periodic reactivations that can be triggered by fatigue, stress, or illness, among other factors. Antiviral therapy can shorten symptom duration in primary infection and can also treat and prevent recurrences. Types of InfectionGenital HSV infection can be classified into three types: primary, nonprimary, and recurrent. Primary – Primary infection refers to an infection in a patient without preexisting antibodies to either HSV-1 or HSV-2.Nonprimary infection, a patient has a first occurrence of a genital HSV lesion but already has pre-existing HSV antibodies that are different from the HSV type related to the genital lesion.Recurrent – Recurrent infection refers to the reactivation of genital HSV (so the patient already has pre-existing antibodies in the serum)Clinical Features The incubation period for developing genital herpes after exposure ranges anywhere from 2 to 12 days.Most patients with primary HSV infection are asymptomatic or mildly symptomatic. However, in more severe cases, individuals can present with painful genital ulcers, dysuria, fever, tender local inguinal lymphadenopathy, and headache. HSV infection also presents with characteristic 2-4mm wide skin lesions that are vesicular or ulcerated. The vesicles are often clustered and can be fluid-filled with underlying erythema. Sometimes vesicles might have a depression in the center (called “umbilicated” vesicles.” It's important to note, though, that the clinical presentation can vary based on the type of infection (primary, nonprimary, or recurrent). As a general rule of thumb, the initial presentation of a non-primary genital infection tends to be milder (as in, fewer systemic symptoms and lesions) than that of a primary infection. Recurrent infections also tend to be less severe than primary or nonprimary infections. Also, around 50% of patients with symptomatic recurrent infections might experience prodromal symptoms in recurrent infections, like burning, pain, or pruritus, before lesions become visible.Symptoms in patients with primary infections typically resolve after an average of 19 days, whereas symptoms in nonprimary or recurrent infections resolve within 10 days. Also, there are no clear differences in a clinical presentation based on whether the virus is caused by HSV-1 or HSV-2. However, infections due to HSV-2 are associated with a higher recurrence rate than infections due to HSV-1. Extragenital complications Genital HSV infection can cause extragenital manifestations that typically occur during the primary episode of HSV infection but can reappear with subsequent episodes. Complications include aseptic meningitis, urinary bladder retention, proctitis, and lumbosacral radiculitis. Other areas that can be affected outside of the genital area are fingers, eyes, and other skin areas.Diagnosis.A clinical diagnosis of genital herpes is usually initiated by the finding of vesicular or ulcerated genital lesions. The diagnosis can be confirmed with lab testing like viral culture, polymerase chain reaction (PCR), direct fluorescence antibody, and type-specific serologic testing. The most appropriate test for a patient depends on their clinical presentation. We might opt for PCR-based testing and cell culture if a patient has active lesions. Viral culture has typically been the gold standard method to isolate HSV, but HSV PCR assays are becoming increasingly popular as they have the best overall sensitivity and specificity. Cell cultures are most accurate in the early stages of the disease and have greater diagnostic yield with primary as opposed to recurrent genital herpes. Another method, type-specific serologic testing, tells us if a patient has type-specific antibodies to HSV, which develop in the first few weeks after infection and persist indefinitely. We might opt for this method if a patient has a history of genital lesions without a diagnostic workup or if the patient has an atypical presentation, in which case we may get type-specific serologic testing in addition to PCR. Management. All individuals experiencing a first episode of genital HSV should be treated with antiviral therapy, ideally as soon as a lesion appears. Most cases of genital HSV can be treated with oral drug therapy for 7 to 10 days, and as of 2021, the CDC recommends 3 different options for treatment: acyclovir, famciclovir, and valacyclovir. These drugs have been shown to decrease the duration of lesion healing time, duration of pain, and duration of viral shedding. For example, if the disease is disseminated or ocular, we may use IV acyclovir in complicated infections. It is important to note that treating the initial episode does not eliminate the latent virus. Patients are still at risk for recurrence and may require additional antiviral therapy. For recurrent infections, treatment options include episodic therapy and chronic suppressive therapy. Episodic therapy – involves patients starting therapy at the very first sign of prodromal symptoms (tingling, paresthesia, pruritus). Patients with infrequent recurrences or mild symptoms might opt for episodic therapy. Wendy: Chronic suppressive therapy – involves daily antiviral therapy to decrease the risk of reactivation and recurrences. Suppressive therapy is helpful in patients with frequent recurrences or severe symptoms or at high risk for severe infections, such as those with HIV.ScreeningAccording to the US Preventive Services Task Force (USPSTF), routine screening for HSV-1 or HSV-2 is not recommended in asymptomatic adolescents and adults. The lack of specific treatment interventions for asymptomatic individuals, and the significant limitations of serologic testing, including low specificity and high false-positive rate, outweigh the potential benefits of screening. Prevention. Measures to prevent genital HSV infections include the use of barrier protection, patient education, and chronic suppressive therapy. Barrier protection – The use of condoms is one of the best ways to prevent the spread of genital HSV infection and other sexually transmitted diseases. It reduces the risk of transmission while patients are asymptomatic but are in the viral shedding stage. Patients with active lesions or prodromal symptoms should abstain from sexual activity completely while having an active episode. Patient Education – Counseling patients along with their sex partner(s) about safe sexual practices can decrease the incidence of HSV in our community and prevent the further spread of the disease. Chronic suppressive therapy – can reduce recurrences and viral transmission.Conclusion: Genital herpes is a sexually transmitted, worldwide infection that can be asymptomatic but often presents with painful vesicles that progress to ulcers in the genital area. Even though the course can be shortened, and the symptoms can be improved with medications, it is frequently recurrent. So, prevention is key to avoiding complications.________________________________Now we conclude episode number 121, “Genital herpes.” You listened to Grace and Wendy discuss the basics of genital herpes. They explained that episodic treatment of genital herpes with antivirals helps reduce the severity and duration of symptoms, while suppressive therapy prevents recurrences and transmission. Dr. Arreaza reminded us that screening asymptomatic adults and adolescents is not recommended by the USPSTF. HSV serology has a low sensitivity and specificity. PCR and viral culture are better diagnostic tools in most cases. In this episode of the Regenerative Agriculture Podcast, John Kempf interviews Dr. Jill Clapperton, a plant physiologist with an intuitive understanding of the workings of the rhizosphere and an abundance of metaphors that clearly explain these mechanisms. Dr. Clapperton worked as the Rhizosphere Ecologist at AgriFood Canada and more recently started her own company, Rhizoterra, where she researches agronomic practices and develops technologies for scientific on-farm decision making. John and Dr. Clapperton delve into the science of the rhizosphere, discussing how the plant drives and controls the actions that transpire there. Each plant species exudes its own signature of carbon compounds, including sugars, amino acids, lipids, nucleic acids, and other compounds, which Dr. Clapperton refers to as ‘carbon skeletons’. The plant then works with the rhizosphere to add the ‘meat’ of mineral nutrients, oxygen, hydroxyl groups, and more to make the long or short carbon chain ‘skeletons’ functional. Dr. Clapperton describes the function of these carbon chains as having multiple “command lines” or calls to action, available in their service of the plant. These actions include things such as calling in nematodes to parasitize insect larvae that are feeding on the roots of the plant, or calling in nitrogen-fixing bacteria if the plant is a legume.  Dr. Clapperton also details how the nutrient density of a crop varies based on the cover crop species grown prior to the crop. The root exudates of different plant species develop entirely different colonies of soil fauna to bind the nutrition they need and deliver it to the plant. Because of this, a multi-species cover crop will provide a greater number of different nutrients available in the soil to the following crop. Dr. Clapperton outlines a detailed list of the crops with a compatible rhizobium; for example corn and canola rhizobia are incompatible. Dr. Clapperton also describes the various forms that photosynthates can take within the plant structure, underscoring the need for a robust photosynthesis cycle. As a metaphor, Dr. Clapperton suggests it’s like starting with the train engine, which is the photosynthate, and then continuing to add cars onto the train by building these very different molecules from photosynthesis. In the complicated plant/mycorrhizae relationship, the mycorrhizae may need slightly more acidic root exudates in order to grow its hyphae, so it gathers minerals and transports them into the plant, trading them for more amino acids and carboxylic acids and organic acids. Scenarios occur where secondary plant compounds used for fighting disease are made, and if the plant doesn’t need them, they will be broken down and redeveloped into other compounds.  In addition, Dr. Clapperton describes how microbes seem to “hoard” amino acids and mineral elements and that if the soil fauna contained only microbes they would out-compete the plant for nutrients. Thus, predator/prey relationships come into play in which the plant calls for other organisms such as bacteria and fungi to eat and digest the microbes, making the nutrients being held by the microbes available for the plant. She describes the methods plants use to continually assess their needs and send chemical signals to the rhizosphere. For example, if there’s an aphid on the leaves, the plant sends a signal to ramp up secondary metabolite production to lignify the leaves. Dr. Clapperton’s latest research focuses on maintaining a preventative environment in plants, finding and replicating the factors that lead to plant resistance to disease and pest attack. For example, nematodes trying to puncture the roots of an onion crop can be parasitized by fungi, and insects trying to eat the roots can be preyed upon by nematodes. Dr. Clapperton is researching these interactions and learning how we can develop the conditions for them to occur. Learn more about her research on the Rhizoterra website. A set of T cell clones (TCC) isolated from HLA-DR-, Dw-, DQ-matched allogeneic MLCs was found to proliferate autonomously when stimulated with cells carrying a wide range of class I or II specificities. This apparently unrestricted proliferation was relatively weak, and only low levels of IL-2 were present in the supernatants of stimulated cells. Autologous as well as allogeneic PBMC and B lymphoblastoid cell lines (B-LCL) were capable of stimulating such clones, which were also restimulated by suppressive, but not by helper, TCC. Moreover, such clones displayed the unusual property of autostimulation. mAb inhibition experiments suggested that class II- or class II-restricted antigens were involved in stimulation. Thus, certain "broad" mAbs (TU39, SG520) reacting with multiple locus products inhibited activation of these reagents, but none of those reacting more specifically with DR (TU34, TU37, L243, Q2/70, SG157), DQ (TU22, SPV- L3, Leu 10), or DP (B7/21), or mixtures of these mAbs, were able to do so. Evidence from sequential immunoprecipitation experiments suggested that mAb TU39 bound class II-like molecules other than DR, DQ, and DP on TCC and B-LCL, and it is therefore proposed that such putative novel class II-like molecules may carry the stimulating determinants for these autoreactive clones. DY-reactive clones lacked helper activity for B cells but mediated potent suppressive activity on T cell proliferative responses that was not restricted by the HLA type of the responding cells. Suppressive activity was induced in normal PBMC by such clones, as well as by independent suppressive clones, which was also inhibited only by mAb TU39. These findings lead to the proposal that DY-reactive autostimulatory cells may constitute a self- maintaining suppressive circuit, the level of activity of which would be regulated primarily by the availability of IL-2 in the microenvironment
