In this episode, Tom Varghese, MD, FACS, is joined by Timothy Pawlik, MD, FACS, from The Ohio State University. They discuss Dr Pawlik's recent article, “Association of Discharge Against Medical Advice with Surgical Outcomes and Healthcare Cost,” in which the authors found that discharge against medical advice (DAMA) among surgical patients is associated with increased 30-day readmission, complication, fragmented care, and higher healthcare cost. DAMA patients were younger, socioeconomically vulnerable, and often had substance use or psychiatric disorders. Disclosure Information: Drs Varghese and Pawlik have nothing to disclose.

Mirko Zilahy è nato a Roma.
More on the red heifer... and bestiality. Also, the purchase of precious stones Dama ben Netinah, an apparent idolater. Which seems to have been acceptable when it shouldn't have been. And then he is honored by having a red heifer to sell too. Also, the question of viability of animals bought from non-Jews for the sake of offerings. Plus, the question of the role of biblical figures, including Yitro, in providing offerings to the Children of Israel, after Egypt, and also later.
In today's pages of Talmud, Avodah Zarah 23 and 24, the sages explore the biblical obligation to honor one's parents. The story of Dama ben Natina is shared, to illustrate the mitzvah of filial piety. What else can we learn from this story? Listen and find out.
Several challenges are raised against Shila’s interpretation of Rabbi Eliezer, who prohibits purchasing a red heifer from pagans based on a drasha on the word "vayikchu." The well-known story of Dama ben Netina—a non-Jew highlighted as a model for honoring one’s parents, illustrating the extent to which one must go to fulfill this mitzva—is cited in objection. This story raises questions both about Shila’s interpretation and about the alternative explanation that Rabbi Eliezer prohibits the purchase due to concerns about bestiality. Either way, if purchasing from pagans is not permitted, how did the rabbis intend to acquire the red heifer from Dama? Additional difficulties are posed against Rabbi Eliezer’s ruling that animals may not be purchased from pagans for sacrificial purposes.
Dama ben Nesina was approached by the Jews to provide precious stones for the apron of the High Priest. Dama declined since the key to the safe was beneath his fathers pillow and his father was resting. The gemara discusses the signficance of this tale. The gemara then proceeds to wrap up the discussion around Rabbi Eliezer's opinion prohibiting the purchase of a red heifer from pagans (or possibly non-Jews).Sefaria: https://www.sefaria.org/Avodah_Zarah.24a?lang=heEmail: sruli@babbleontalmud.com
