Welcome to another episode of the Words Matter Podcast.On this episode of the CauseHealth Series, I'm speaking with Dr Kai Brynjar Hagen about his Chapter 10 that he wrote for the CauseHealth Book, titled ‘Lessons on Causality from Clinical Encounters with Severely Obese Patients' (read Chapter 10 here).Kai Brynjar is a Senior Consultant in the Regional Centre for Morbid Obesity, in the North Norway. He is also General Practitioner, District Medical Officer for Communicable Diseases and as you can imagine has his handful advising on the current pandemic.He is Specialist in Community Medicine and is interested in ecological thinking in medicine, from the individual person to the policy level. And he is interested in primary causes of obesity development and factors that contribute to maintain obesity, such as trauma or other stressors (see his work here).So in this chapter we talk about: The different roles that he has within the Norwegian healthcare system and how these relate to his thinking around causation. We talk about his view of causation in relation to obesity and how the biomedical view and diagnosis of obesity is insufficient to fully understand the whole person and the causal processes at play. Kai Brynjar contrasts the biomedical diagnosis of obesity (which focuses on medical symptoms such as diabetes or psychcobehavioural aspects such as diet or motivation for physical activity. We talk about how he strives for what he calls a ‘genuine' causal diagnosis for severely obese patients. He tells us about the challenges of adopting a Whole Person Approach in larger structural and institutional settings which in he's worked. We talk about about the main focus of his clinical encounters with obese patients, which is understand their life story- including their life as a child, and the centrality that this dialogue plays in the creation of a story together with the person. So, this was another resonating conversation, with a clinician at the front line of helping people with complex causal stories. Here's the actual WHO definition that doesn't use the term morbid obesity but rather obesity class 1, 2 and 3.Obesity is frequently subdivided into categories:Class 1: BMI of 30 to < 35Class 2: BMI of 35 to < 40Class 3: BMI of 40 or higher. In this episode Fifi talks about her journey into obesity after the sudden and very unexpected death of her beloved father. She began to focus on caring for her mother, who was suffering from dementia and her three teenage children. Abstract: Background: There is growing recognition for a reciprocal, bidirectional link between anxiety disorders and obesity. Although the mechanisms linking obesity and anxiety remain speculative, this bidirectionality suggests shared pathophysiological processes. Neuroinflammation and oxidative damage are implicated in both pathological anxiety and obesity. This study investigates the relative contribution of comorbid diet-induced obesity and stress-induced anxiety to neuroinflammation and oxidative stress. Methods: Thirty-six (36) male Lewis rats were divided into four groups based on diet type and stress exposure: 1) control diet unexposed (CDU) and 2) exposed (CDE), 3) Western-like high-saturated fat diet unexposed (WDU) and 4) exposed (WDE). Neurobehavioral tests were performed to assess anxiety-like behaviors. The catalytic concentrations of glutathione peroxidase and reductase were measured from plasma samples, and neuroinflammatory/oxidative stress biomarkers were measured from brain samples using Western blot. Correlations between behavioral phenotypes and biomarkers were assessed with Pearson's correlation procedures. Results: We found that WDE rats exhibited markedly increased levels of glial fibrillary acidic protein (185%), catalase protein (215%), and glutathione reductase (GSR) enzymatic activity (418%) relative to CDU rats. Interestingly, the brain protein levels of glutathione peroxidase (GPx) and catalase were positively associated with body weight and behavioral indices of anxiety. Conclusions: Together, our results support a role for neuroinflammation and oxidative stress in heightened emotional reactivity to obesogenic environments and psychogenic stress. Uncovering adaptive responses to obesogenic environments characterized by high access to high-saturated fat/high-sugar diets and toxic stress has the potential to strongly impact how we treat psychiatric disorders in at-risk populations. There was this new advertisement that we saw through the TTC. It was trying promote Big Women, Huge Women wearing Lingerie. The issue with that is that Obesity is not healthy. Anorexia is not healthy. Really what I am getting at is that lets highlight more beautiful, natural size Women like Bianca Andreescu (Got her name wrong in the Rant) & Eugene Bouchard rather than Anorexic & seriously oversize Women who are internally undoubtedly unhealthy. Keep in shape, eat well & you know what? Enjoy a Cheeseburger. In my opinion though, its probably not good to order the whole Menu at the Mcdonald's.

Gastric bypass surgery is the most effective form of treatment for morbid obesity and a familiar term to most of us, but until recently, the underlying mechanism that makes it so successful has been largely misunderstood. Most people—including health professionals—attribute the weight loss that follows from this surgery to reduced stomach size and therefore reduced food intake and malabsorption, but the evidence is pointing elsewhere: to the changes in gut microbiota that occur after the surgery. Jessica is a stay at home mom with 3 girls aged 5, 4, and 2. She met her husband as a college bound kid in 2009 while at a healthy weight. They followed the societal script for families in small town Alberta, getting married and having kids in quick order. On paper she had everything that is supposed to make you happy, but in reality she was facing some very difficult adversity. She was up against undiagnosed mental illness, the loss of both of her husband’s twin brothers to suicide and ballooning into morbid obesity, to name a few. She hated herself so completely that it was impairing her ability to function in day to day life. Finally, in 2016 she had enough and made a change. Through seeking help and putting in the hard work to make major lifestyle changes, she has turned a lifelong struggle with weight and eating disorders into a platform to motivate and inspire others. She now lives an active lifestyle and has launched books and inspirational projects. Robanne works with bariatric patients to motivate them to be successful.

Stanley Nasraway, MD, FCCM, discusses his article published in the April issue of Critical Care Medicine, titled "Morbid Obesity Is an Independent Determinant of Death Among Surgical Critically Ill Patients." Dr. Nasraway is associate professor of surgery, medicine and anesthesia at Tufts University and chief of the surgical intensive care units at Tufts-New England Medical Center in Boston, Massachusetts. His article addresses the growing number of morbidly obese patients entering the intensive care unit and suggests that customized processes be developed to address this unique and challenging patient population.
