“I think poor discharge planning is that top contributor [to readmission]. And by that, I mean discharge planning that doesn't assess a patient's educational level, their support at home, what resources they have, like transportation and finances, and then to go further, evaluating if the patient even understand the reason they were admitted and then how to manage their care once they leave. Highlights From This Episode “Of course readmissions are inevitable, but ultimately, high rates may indicate that there's a problem. Something is wrong. The quality of our care is not up to par. So looking at the rate of unplanned readmissions encourages hospitals to look inward, to see what's going on, and find the gaps.” TS 2:31 “The number one thing we can do is review the patient's social determinants of health. We're seeing this assessment tool used more and more in the hospital system, and it can truly help identify high-risk patients. … But it really takes into consideration a patient's environment, and it includes five components—access and quality of education, economic stability, healthcare access and quality, home environment, and then the patient's community.” TS 5:17 “Recently, we had a patient that was seen in our ED [emergency department] for nausea and vomiting. And then due to that follow-up call the nurse made, she was able to get another set of labs drawn on the patient, found that they had an electrolyte imbalance, and then got the patient set up for fluids in an outpatient setting. So I think that really prevented that patient from going back to the ED, probably for the same reason they were there in the first place.” TS 18:00 “When we reviewed the data, we saw our readmission rates had dropped by 51% at the six-month mark, and same with our ED visit rates. And then our referrals to the continuous care team jumped 155%. … But we were able to discover some other opportunities through the process. So for example, through the chart audits completed, we were able to identify an increased need for our pain management services. There was a large number of patients that the reason for visit was pain, so we ended up expanding our templates for our pain management providers to meet that need and ultimately reduced the admissions for pain.” TS 22:38

In this conversation, we will examine how trauma is both managed and reported on throughout Scotland. The Scottish Ambulance Service (SAS) receives over 1.5 million calls a year and responds to over 500,000 emergency incidents per year. In the episode, we will discuss The Scottish Trauma Audit Group (STAG) as a national audit group within the Scottish National Audit Programme of Public Health Scotland. STAG measures and is tasked to improving the quality of care, overall experience, and long-term outcome of patients with significant injuries throughout Scotland. We will also examine the Advanced Practitioner in Critical Care scheme in Scotland and how it has developed, how it is managed and some of the recent adaptations according to need throughout Scotland. We will also look at the need for key interventions when critical care is not available. To do this I am speaking with Martin Esposito. Martin is a Consultant Paramedic for Major Trauma within SAS. In this role, he is the lead for all the major trauma work within SAS and also line manages the Advanced Practitioners in Critical Care. As well as trauma, is has a special interest in Pain Management and is the SAS lead for pain management as well as an Honorary Tutor at Cardiff University. Simon belongs to a family of dairy farmers. He runs a veterinary business with his wife, Camilla. [04:27] Simon's First Proudest Moment Simon considers his wedding day as his first proudest moment. It's a constant challenge to juggle the roles of being a husband, a father, and an entrepreneur. His wife advises him to look after his health and family relationships because he gets engrossed with his business. [07:12] Running the Business with His Wife Camilla was a successful Biology teacher before joining Simon's veterinary practice. Running a business means experiencing many frustrating moments. But the couple learned to grow along the way. Currently, they are thinking of changing their business' managing structure. [08:25] Simon, The Veterinarian Since he was young, Simon wanted to be a veterinarian. His love for animals stemmed from growing up on a dairy farm surrounded by animals. It took a while for Simon to realise that he wanted to set up his own business. [09:35] Simon, The Entrepreneur Practising a profession and running a business requires different sets of skills. In 2021, his business grew by 20% despite the pandemic. There were winners and losers in the veterinary profession when COVID-19 struck. Hiring staff members and facility renovations were positive challenges for Simon and Camilla during the pandemic. [10:43] Separating Business and Family Simon's family often had their spouses as business partners. His family is still trying to compartmentalise both business and family matters. There's no magic formula to balancing your role as a spouse and business partner. Couples can learn along the way. [12:42] Simon's Second Proudest Moment His second proudest moment is buying his own Land Rover Defender. In retrospect, Simon believes asking his sister to include the Defender in his business' logo manifested his dream of buying the car. Ever since he became environmentally conscious, Simon felt bad about buying the car. However, he couldn't let it go. [17:20] On Growing the Business There are limiting factors in expanding a business. Simon wants to maintain a profitable practice while keeping his staff happy. There should be an endpoint to how many clients a company can take. Assessing your core values and business goals is essential to understanding your limits as a growing entrepreneur.
