Today's disagreement is about transgender health care for young people and the future of the trans rights movement.Gender Affirming Care is a method of treating gender dysphoria by affirming a child's gender identity and tailoring medical and social interventions around this identity. This might begin with a social transition and often leads to a medical protocol that involves puberty blockers and then cross sex hormones.Almost a year ago, we had a conversation with leading national clinicians, Dr. Jack Drescher and Dr. Erica Anderson about the state of gender affirming care for young people in the United States. If you haven't listened to that episode, I highly recommend it as a helpful primer for this conversation. Since then, a lot has changed in the conversation about trans care for young people.In April 2024, the Cass report came out in the UK. It is one of the most thorough reviews of the evidence base for gender-affirming care. It also called into question the limits of what we know and don't know about care for minors. Right now, 27 states have enacted laws to ban or restrict the practice and the supreme court will be ruling on it later this year. And the United States has a new presidential administration who recently issued the executive order: “Protecting Children from Chemical and Surgical Mutilation," which attempts to put forth a national ban on gender affirming care.We ask some important questions around gender affirming care and the trans rights movement. Have trans advocates and medical practitioners overstated the quality of the evidence base for gender-affirming interventions for minors?Brianna Wu is a political commentator and trans rights activist. She is executive director of the progressive Rebellion PAC.Dr. Marci Bowers is a pelvic and gynecological surgeon with more than three decades of experience in healthcare. Dr. Bowers was most recently the president of the World Professional Association for Transgender Health (WPATH). She was the organization's first transgender president.Thank you to Brianna and Marci for coming together to have this critical conversation. It's a difficult topic and as you'll hear, a difficult conversation. But we need more of this. We need more people willing to come together and have hard conversations out in the open, and we really appreciate you both.This conversation is pretty “in the weeds” when it comes to youth gender medicine and the state of the research. If you have the context, that's great – you can skip ahead. If you don't, here is a highly efficient glossary:WPATH is the leading trans health organization in the world. Its “Standards of Care” or SOC - have served as the national guidelines for youth gender medicine in most gender clinics.The Standards of Care have evolved a great deal over time: SOC 6, published in 2001, required multiple psychological assessments and a period of Real Life Experience (RLE), living as the preferred gender, before any medical interventions. The most recent SOC, SOC 8, was published in 2022. It removed many of these requirements which activists have described as “gatekeeping.”The Dutch Protocol, also known as “watchful waiting”, is a more conservative alternative to gender-affirming care. It focuses on providing large amounts of psychological support and monitoring during childhood. If the dysphoria continues to persist, then putting young people on puberty blockers and cross sex hormones in late adolescence.Language around gender identity is constantly evolving. Brianna uses the term FTM (or Female to Male) to describe females who transition. For the same demographic, Marci uses “transmasculine.”We also talk about Hormone Replacement Therapy (HRT) and Facial Feminization Surgery (FFS).Okay, that will hopefully create a solid foundation. Take a deep breath. Open your mind as far as it's willing to go. “There are very specific fears that people have that are specifically related to their own childhood, and I'd like to give an example. A mom with twins had a kidnapping fear. She was afraid every time she saw a car drive by her house that her twins would be kidnapped. Now this mother was herself adopted when she was a newborn, but her adoption did not become final until she was one year old. Her twins were approaching one year. I was struck by the anniversary of her fear of kidnapping, and when I asked her who she thought was driving the car that drove by her house, she blurted out, 'my biological mother - adoption was never an issue for me, I have the best parents,' she said, but her fears about her babies being kidnapped were rooted in her own guilty feelings. She said, 'I get to keep my biological babies and my biological mother did not. I can have biological babies and my adoptive mother could not.' Carrie's fantasy that her biological mother was threatening to kidnap her babies represented both her fears of retaliation for her aggressive victories over both her biological mother and her adoptive mother, and the repair of her disavowed feeling of loss by a reunion with her biological mother. This meaning of the memory, this understanding of the memory, resolved her kidnapping fear. It dissolved.” Episode Description: We begin with an overview of the importance of mothers' childhood memories in their experience of their own children. These memories are of the conscious sort and also the not-so conscious. They are of the loving as well as the misattuned versions. "The challenge for mothers is to understand the complexity of their own childhood memories and to help their babies and toddlers adapt to the everyday ups and downs of life, as well as to the exceptional ones." We discuss typical fears, sleep problems, 'mutually-regulated patterns', naming body parts, nakedness, weaning and screen time. Ilene ran mother-baby-toddler groups for 35 years and shares with us her relentless curiosity for what we all bring to the parenting experience. Our Guest: In 1982, Ilene Lefcourt established the Sackler Lefcourt Center for Child Development - programs for parents and their children from birth to three years. She was the Director, led the Mother-Baby-Toddler Groups, and provided Developmental Consultation to parents for over 35 years. She saw over 1,000 families and taught Child Psychiatry Residents and Parent-Infant Psychotherapy Trainees about her work. She has been a faculty member at the Columbia Psychoanalytic Center since 1995. Ms. Lefcourt is currently in private practice in New York City. ...What we finna do come tomorrow? ...especially if the overseer of your choice doesn't win ...will you fear power or contest with it? A hallmark feature of any presidential election in a racist country is the continued psychological warfare against the colonized, especially Black people, who, along with Arab Muslims, will be inevitably blamed, scapegoated or strategically pitted against one another in service of the powers that be, for no other reason than if we were to coalesce our efforts toward our own interests and liberation, that could signal the end of empire and all the symbolism, pomp and circumstance that they hope will keep us distracted and placated. We are living in a lie, a constant paradox: Black people are no better off from the open theft and profiteering of our culture by white people but appropriation masked as "multiculturalism" in the media is at an all time high and judging by the decreasing numbers of Black folks in physical public space in major metropolitan cities, is showing no signs of slowing down (if you can just "be" Black, why would you need Black people?). Record numbers of white people are using tiktok and other technologies to fine tune their mimicry of what they think Blackness is while at the same time decrying we won't be "replaced"; Vanity fair is publishing articles about the "radicalization of america's white young men" and giving a well known white supremacist a whole spread about how wealthy "conservative extremists" are calling themselves "anti-imperialists" in a fake populist rebrand while a privately owned newspaper is upset at its billionaire owner for blocking their presidential endorsement and also regularly publishing Israel centric propaganda around freeing the hostages when there are twice as many or more Palestinians held captive before oct 7? This is sometimes peppered with actual unbiased coverage of the genocide here and there (to some individual writers' credit). The US says it is committed to protecting Ukrainian sovereignty after Russia's attempts to annex their land but not that of the Palestinian people...who also had their land stolen? In 2021, Kamala Harris was quoted saying wars are fought over oil but in a few years it'll be over water Elon Musk is building what will be the world's largest supercomputer in South Memphis, TN in a predominately Black community and authorities there greenlit the project with no community input or oversight in a part of the city that is currently plagued by a water crisis, replete with lead and other pollutants. This is 3 years after the community started a successful grassroots resistance movement that stopped two companies who were planning to build a major oil pipeline right in the middle of a historic Black neighborhood. They did the bulk of their organizing during and despite of a Trump presidency. European investors are buying farmlands in water rich areas, mostly in Africa and even in parts of the US to sell to areas that are water poor and dividing up last remaining farmland on earth to take advantage of what they call... “Empire crops” This has been happening under democractic and republican administrations. So again, what's going to be our answer? Will it b a departure from what colonialism wants, thinking about something other than yourself? What's it going to be after you slip that paper in that box?
