After a positive induction with her eldest, she didn't have any particularly strong feelings about what she hoped for from this birth, so when concerns were raised about her baby's size, she felt comfortable accepting another induction. If you're good with that, let's crack on...  One of the ongoing themes of this show – and, by extension, my life – is growth, constantly evolving oneself into a slightly better version of who we are and who we want to be. That's not an unusual aspiration – surely most of us who are doing it for ourselves want to get better, don't we? To be perpetually in motion in pursuit of a worthy goal is often considered to be the hallmark – or one of the hallmarks - of a good life. Capital G, capital L. If you – like me – are wired up this way you'll know the feeling of intrinsic excitement that comes from immersing yourself in The Thing that You Love To Do: you maybe feel - as I do – that you're somehow vibrating at a higher speed than almost everyone else.... The truth is that being in this state – let's call it Flow - Mihail Czikszentmihaili's term for a peak state of absorption in a task or work... So we tend to feel at our best when up to our necks in The Work. To me this is a Good Thing. But there's a catch to changing yourself and evolving yourself... You have to kill yourself. Not completely and forever, anyway. No – what you have to do is kill the person you were in order to allow the person you are becoming to emerge from the sloughed skin of who you used to be... The person you have been up until now is not the person you need to be to take you to the next level – and there's no room for passengers on the journey. If you can somehow manage to drag your own sorry carcase over the line, all power to you, my friend. But I guarantee you this: you won't be able to drag anyone else over the line. The margin for error is so small, the drain on your strength, energy and commitment is so appalling, that you can – if you're obsessed and fortunate enough – do it for yourself, but you can't do it for anyone else. And the person your family and friends have known for years? Well – they are the first casualty of your endeavours... Old you – even up until today – may be little more than a snivelling, quivering heap of fears, doubts and excuses, justifications and rationalisations. Kill Old You. Every day, tear out from yourself one small part of the limitations and beliefs that no longer serve you. Tear your old self apart in order to make room for your new self, building yourself to be better than before. Change is hard, yet it is the one constant of life. Change is inevitable: growth or decay... nothing stands still... everything is either getting better or fading away. Ultimately, over time, you'll hardly recognise the person you have become... the slight changes often happen undetected – normalised each day but compounding up over years – endlessly ratcheting-up what “Normal” is for you, so the imperceptible death and casting off of the Old Self happens without you really noticing it. But at some point you pause, and it hits you... in an unguarded moment you see yourself as you have become... Look cold and hard - regard yourself with an unflinching eye and recognise the transformation you have wrought. The weak forged into strength, the miser turned philanthropist, the fearfulness crafted into courage, the loafer turned worker, the old you has perished... killed by your own hand and your own will in an act of premeditated force. This week, I talk with Andre Hardy about his fresh take on noir in his debut novel Tear the City Down! Listen to hear about: How Andre Hardy's debut novel, Tear the City Down, grew from his lifelong love of noir—particularly Walter Mosley's Devil in a Blue Dress, which showed him a fully realized Black protagonist in a genre he already loved. The ways Hardy uses San Diego's stark dualities—tourist paradise versus underserved neighborhoods, enormous wealth versus systemic oppression—as the foundation for Coltrane's story and the novel's larger questions about social mobility, morality, and what happens when you actually reach the top. Why Hardy shied away from a traditional stoic noir hero, and created Coltrane to be masculine, dangerous, and deeply vulnerable. How the mystery expands from one missing girl into something far larger, illustrating Hardy's belief that seemingly isolated injustices often lead back to systems of power and exploitation. How Hardy drew on his own NFL experience, love of jazz, reading, and upbringing in San Diego to give the novel its texture. A traumatic shoulder injury can leave you wondering: Did I tear something, and do I need an MRI or surgery? In this episode of the Rehab Science Podcast, I discuss the case of a friend who developed persistent posterior shoulder pain after a fall while mountain biking. Although his shoulder had improved significantly over several months, he continued to experience pain with throwing and reaching into the backseat of his car. We discuss the potential causes of posterior shoulder pain after trauma, including rotator cuff and labral injuries, and why symptoms alone don't always tell us which structure is responsible. More importantly, we explore how clinical findings such as strength, range of motion, instability, apprehension, pain at rest, and changes in function over time can help guide treatment decisions. In this episode, we discuss: The potential causes of posterior shoulder pain after a traumatic injury The anatomy and function of the rotator cuff, particularly the infraspinatus and teres minor How traumatic injuries can affect the labrum Why a suspected structural injury doesn't automatically mean surgery is necessary When imaging may be helpful after a shoulder injury Why improving strength and function are important clinical indicators How progressive shoulder mobility, strengthening, and stability exercises can support recovery The importance of eventually returning to the specific activities that provoke symptoms Why we should treat the person and their functional limitations rather than simply treating an MRI finding Key takeaway A significant injury can occur without requiring surgery. When someone is demonstrating progressive improvement in mobility, strength, and function, conservative rehabilitation is often an appropriate place to start. The goal isn't always to identify every structure that may have been injured. The goal is to determine what the person needs to restore function and build the capacity to return to the activities they enjoy. [00:00] Introduction — what's new in rotator cuff research [00:39] Breaking down the new JAMA Internal Medicine paper: a Finnish population-based study of 600+ patients (ages 40–75) who received bilateral shoulder MRIs [01:00] Key finding: imaging findings were nearly identical between asymptomatic (96%) and symptomatic (98%) patients; full-thickness tears were the only finding more common in symptomatic patients (14% vs. 6%), and even that difference lost statistical significance after adjustment [01:32] What it means: most rotator cuff MRI findings are normal, age-related changes — and MRI may be overused when the clinical diagnosis is already clear [02:22] The "hair loss" analogy: a tear is often a normal part of aging, not automatically something that needs fixing [02:55] The "50-to-60-year-old house inspection" analogy for interpreting an MRI report [03:41] Can you just watch a rotator cuff tear? Reviewing natural history data out of Washington University (Yamaguchi, Keener) [04:02] Partial-thickness tears: roughly one-third progress over 5–7 years. Full-thickness tears: roughly half progress over 5 years [04:50] Bottom line: if a patient is asymptomatic and fully functional, there is no indication to intervene — annual reassessment is reasonable [05:27] Physical therapy as first-line treatment — does it actually work, and how? [05:47] The mechanism: rotator cuff strengthening counterbalances the deltoid, maintaining subacromial space during overhead motion [07:21] PT success rates by diagnosis: ~70% for full-thickness tears, ~95% for impingement/tendinopathy and partial-thickness tears [07:55] When is surgery the right answer? Distinguishing acute traumatic tears from degenerative tears [08:10] Acute traumatic tears (e.g., a fall) are generally treated surgically [08:26] Degenerative tears that fail 6–12 weeks of non-operative management are reasonable surgical candidates [08:53] Tear location matters — tears involving the "rotator cuff cable" carry more biomechanical load and may be more likely to fail non-operative treatment [09:07] Timing is rarely urgent — most degenerative tears can be addressed within 6 months to a year without changing outcomes [10:22] Acute traumatic tears in younger, active patients: counsel surgery within 2–3 months; short delays (including during COVID) didn't appear to change outcomes [10:59] Injections: is a steroid shot still a good first option? [11:15] How corticosteroid injections work — and why they're different from narcotics (reducing inflammation vs. masking the pain signal) [13:05] General guidance: one or two injections is reasonable; by the third without improvement, it's time to discuss surgery [14:00] The data linking 5+ injections to higher retear rates — and why confounding by sicker, more chronic patient populations makes this hard to interpret cleanly [15:15] PRP (platelet-rich plasma): does it actually work for the rotator cuff?
