Her second began with prodromal labour and ended in an emergency caesarean.During her third pregnancy, Shelby realised she didn't fully trust birth — and she was no longer able to return to the birth centre due to her previous experiences. Bow Ties and Blink Reflexes: Periocular Perils on SkinFlint In this month's episode, Sue & John invite a well known vet with his eye on pet eyes - David Williams. (00:00) John opens SkinFlint. Today's focus is the periocular region — small real estate, big consequences — from itchy eyelids and rogue hairs to lumps and lesions that can threaten vision. Chapter 1: The Eyelid Is the Windshield Wiper (03:54) David Williams joins: associate lecturer in veterinary ophthalmology at Cambridge, teaching students and travelling between practices to manage eye cases. He's been operating that morning on a severe corneal ulcer — a reminder that “eyelid problems” often matter because of what they do to the cornea. (06:10) Mucocutaneous junction basics. David frames the eyelids as protective, moving structures that maintain the ocular surface. The lid margin is more than skin: it includes meibomian gland openings that support tear film stability. (07:21) Tear film (briefly, promise). David describes modern tear film understanding as a mixed mucin and aqueous layer with a lipid surface. The eyelid spreads it over the cornea; tears behave as a non Newtonian fluid, becoming “runnier” as the lid moves, which helps smooth distribution during blinking. (09:08) Rubbing: skin itch vs eye pain. Sue highlights the GP dilemma: is the dog rubbing because periocular skin is pruritic (allergy) or because the eye is painful? David describes the vicious cycle (irritation → rubbing → more inflammation) and stresses looking at both eyelids and the globe. Stopping self trauma (sometimes a collar) can be part of breaking the loop. (11:01) Red eye triage and eyelid eversion. David's practical tip: clinicians don't evert eyelids enough. If both palpebral conjunctiva (inside the lid) and bulbar conjunctiva (on the globe) are inflamed, conjunctivitis is more likely. If the globe looks red but the inner lid does not, widen your differential (including uveitis and glaucoma). Don't guess from the outside. (11:45) The lash trio explained. Trichiasis: normal hairs rub the cornea due to lid conformation. Distichiasis: extra lashes from the meibomian gland orifices at the lid margin. Ectopic cilia: a lash emerges through conjunctiva and points at the cornea — often very irritating. David suggests checking for allergy clues elsewhere (paws, general pattern) if you suspect pruritus is the driver. (13:33) Distichiasis: common, not always guilty. Many dogs have distichiae in both eyes without ulcers. Lashes may delay healing rather than cause the initial lesion. Plucking, freezing or electrolysis can lead to regrowth (sometimes shorter and more abrasive). If treatment is truly needed, David prefers approaches that remove the lash follicle within the eyelid to reduce recurrence. (16:45) Older Cockers and “saggy lash syndrome”. David describes age related lid changes in Cocker Spaniels where long lashes start rubbing persistently. He flags the importance of a Schirmer tear test, as dry eye commonly co exists and must be addressed alongside lid conformation. (18:15) Two quick diagnostics. A topical local anaesthetic drop can be used diagnostically (briefly) to see if discomfort reduces, but repeated use is unsafe for the corneal epithelium. David also reminds listeners to check the third eyelid: lymphoid follicles on its inner surface can keep an eye irritated and are only found by everting it. Chapter 2: Lumps, Bumps and the Cat Exception Clause (22:56) Eyelid masses: chalazion or tumour? David frames the common dilemma as meibomian gland inflammation versus neoplasia, though many cases end up managed surgically either way. In dogs, most eyelid tumours are benign (often meibomian adenomas/epitheliomas) and are usually suitable for wedge resection. His bias: remove earlier rather than later (smaller surgery, lower anaesthetic risk than waiting). (24:38) When heat helps. If you suspect a meibomian gland abscess or granulomatous lesion, David suggests warm compresses applied regularly (as hot as comfortably tolerated) to encourage drainage and reduce the lump. (26:20) Cats: higher suspicion. David contrasts this with cats, where eyelid tumours are more likely malignant (including squamous cell carcinoma) and may look ulcerated or invasive. He mentions photodynamic therapy as a tissue sparing option in delicate areas like eyelids. (27:40) A feline oddity to remember. A dark, round medial canthus mass in a Persian cat may be an apocrine hidrocystoma (benign), and similar lesions can appear at other mucocutaneous sites. (29:03) Melanomas. Behaviour varies by species and site. David is generally more concerned in cats than dogs and flags mucocutaneous junction melanomas as potentially more aggressive — excise when feasible. Chapter 3: VKH — The One You Don't Sit On (30:45) Sue brings up VKH (Vogt Koyanagi Harada) syndrome, a true derm ophthalm crossover where delay can cost vision. David describes it as autoimmune disease against melanocyte associated antigens, classically seen in Akitas (but not exclusively). (31:49) Clues and consequences. Skin signs may include periocular poliosis and vitiligo (white hairs/depigmentation) around the eyes and lips. The urgent issue is ocular: chorioretinal inflammation that can progress to retinal detachment and blindness. (33:38) Treat early, treat hard. David cautions against slow escalation. Steroids alone for weeks may waste time, especially because azathioprine takes time to reach effect. If the posterior segment looks inflamed, he favours starting azathioprine early alongside steroids. (35:06) Sue agrees: start decisively, get ophthalmology input, and monitor closely with baseline and follow up bloods due to bone marrow suppression risk (especially noted in Akitas). (35:51) They land on the headline: periocular cases reward teamwork — dermatology and ophthalmology together can prevent wrong turns and speed up patient comfort. Announce a sacred fast; proclaim a solemn assembly. 16 Gather the people; sanctify the congregation; assemble the aged; gather the infants, even babies nursing at the breast. Let the groom leave his bedroom, and the bride her honeymoon chamber. 17 Let the priests, the Lord's ministers, weep between the portico and the altar. Let them say, "Have pity on your people, Lord, and do not make your inheritance a disgrace, an object of scorn among the nations. Why should it be said among the peoples, 'Where is their God?'" 18 Then the Lord became jealous for his land and spared his people. 19 The Lord answered his people: Look, I am about to send you grain, new wine, and fresh oil. People are sneering...fists raised...yes, they are saying they can do what they want...they are defying God. Hear them? Listen to what is really being said: “Ha! We can do what we like. Watch and see! And we will do it again! So where...where… is your God when I do it to His face? Where is He who calls all that we like to do...all what I want to do...evil…? Where is He to punish what He calls sin?” God says, "From dust you were taken and to dust you will return..." No one escapes death. No one. God will not be mocked. He says that, "The wages of sin is death."That is what God told Adam because of his and his wife's sin. Yes, and to dust the world will be reduced to also... …soon now...maybe here in America sooner than Africa. I don't know. But remember, Nineveh repented. God spared them for several generations—over 200 years I think it was. Pray America... pray Illinois… and Lombard… repents also… repents—before it is too late. Yes, as we know what the consequences of sin are. Death. Yes, from dust we too were taken and will return... But with that mark...made with water… and Word…we won't stay there. 15 Blow the trumpet in Zion; consecrate a fast; call a solemn assembly; 16 gather the people. Consecrate the congregation; assemble the elders; gather the children, even nursing infants. Let the bridegroom leave his room, and the bride her chamber. We have...we are...it is called Lent. It is called the application of ashes...consecrating you again for burial...with Christ...in that living and ever flowing Water...the very waters of life... So that we will be spared eternal destruction...flames and ash... To be instead raised to eternal life. Take the ash now...death now...so that life eternal is ours later. Yes, Lent is preparation for our death now, but only for Easter later—our very own Easter. Easter...our Easter, is only possible with the Cross...with Jesus' death...and our death in Him...in Water...in Word...in this life...in this world...with its ways.... Lent is a time to fast and pray. To die to self. To die to sin. To switch our focus. To remove things in our lives that distract and blind us from the truth...for that which brings death and destruction.... Yes, to focus on dying...to let our sin nature die again...to drown it...that those things of sin to be removed...cleared away...cleaned off...the things that entrap us again in this world...in sin...that bring God's wrath and doom...fire and ash.... ...let them go...walk away...let them die...to be replaced with life...that new life to come...that He even gives a start of this here and now...a down payment from His Easter for the Easter to come... Yes, because then it does... Don't you know, Easter already has come? Then for sure, it is to be ours next! So...yes, lets get ready... 17 Between the vestibule and the altar let the priests, the ministers of the Lord, weep and say, "Spare your people, O Lord, and make not your heritage a reproach, a byword among the nations. Why should they say among the peoples, 'Where is their God?'" We can answer this...we can answer them—answer and say that it is not yet too late...even for them: Tell them that: He is here...in His Word...which endures forever...in the Living Water that marked us forever to be His...and in His Supper where He meets with us...feeds us...helps us to live in this world that is going to be in flames soon...a heap of ash all too soon... And then Easter. Easter is so near. Ours...and yes the Earth's too. It too is waiting...it too will also be reborn out of those ashes...just as we will be, it will be free of sin's decay...no longer filled with death... All new and green...alive...really alive...as it was meant to be...forever. Yes, He has called us. We have come. He has marked us. We have cried out and we will do so until the end—call to Him. We will look to Him. We have turned to Him for mercy. We mourn and cry out: "Spare your people, O Lord...!" And what does He do? How does He answer? He has already...He has acted...has answered. Jesus came and died...Jesus rose again...Jesus came to us—to you in water and word...and does in bread and wine. He has marked us for death so that we can live. Yes, He responds...He answers because.... 18 Then the Lord became jealous for his land and had pity on his people. 19 The Lord answered and said to his people, “Behold, I am sending to you grain, wine, and oil, and you will be satisfied... All of this has started already to come to us...in Word and Sacrament....because of Easter... We are being filled up...with Him....today even...because of Easter... …and then we will see—the world and the devil will see. Easter is almost here. God says, "...I will no more make you a reproach among the nations." This… All because Easter has already come. Ours is next. Morning is about to dawn. He is coming...in the fire of His Glory... He will rise upon the earth—death dying in that intense… on His light. Then it is up and out...for all those marked by sign...in His name...as all is burned into ash.... …and then glory...and then the Easter morning...the forever Son forever...our Emmauel with us… aways, world without end. Amen.

Matthew 9:9-13 NLTAs Jesus was walking along, he saw a man named Matthew sitting at his tax collector's booth. “Follow me and be my disciple,” Jesus said to him. So Matthew got up and followed him. Later, Matthew invited Jesus and his disciples to his home as dinner guests, along with many tax collectors and other disreputable sinners. But when the Pharisees saw this, they asked his disciples, “Why does your teacher eat with such scum?” When Jesus heard this, he said, “Healthy people don't need a doctor—sick people do.” Then he added, “Now go and learn the meaning of this Scripture: ‘I want you to show mercy, not offer sacrifices.' For I have come to call not those who think they are righteous, but those who know they are sinners.” The Pharisees called the people Jesus was hanging out with “scum.” Jesus removed the barrier the pharisees had and dismantled the prejudice and in doing that actually established a new boundary. The new boundary was towards prejudice, judgment and even privilege.What drove Jesus' actions and words? Freedom, righteousness, justice, mercy, and love. When we follow Him, we have the best opportunity to do the same. Tear down barriers made by man and install boundaries built by God.Is there anywhere in your life that you have created an unhealthy barrier or outlook between the those you consider righteous and those you consider sinners?Let's pray together: “Heavenly Father, I have to admit that sometimes I act like You, but sometimes I act like a Pharisee. Hospital transfers and tears are the 2 biggest fears amongst pregnant mamas and especially you who listen to this podcast. So what better way of facing those fears than by listening to a mama who had, according to herself, the most empowering birth even though she had both one of the bigger tears that only around 5% of women get and a hospital transfer from her home birth.People usually assume I think a natural birth through and through is the be all and end all because I am a huge advocate about educating about natural physiological birth and the benefits of it. But if you've listened to me for any length of time you know this is not true, and if you're a student or mentee of mine you know it even deeper.I'm all for education in pregnancy and optimizing body, mind and soul for your desired birth outcome which I believe mostly have to do with inner work, facing fears and deeply getting to know your body, baby and having a relationship with your intuition.Once the first contraction hits you are now to surrender to the great mystery of birth. And a positive birth experience is not necessarily a natural one. Not holding on to a certain outcome for your happiness is so important and trusting that there is wisdom in the way you birth even if it's not the way you wanted it.
