But I don't think that's where mythic storytelling begins. In this post/podcast, I explore seven practices and mindsets that help writers reconnect with what the lost art of mythmaking--not just borrowing of old symbolic forms, but accessing the deeper creative process from which meaningful stories emerge in the first place. TEE can change the surgical plan in real time.Findings such as underestimated aortic regurgitation, mitral pathology, or a PFO may directly alter cannulation and cardioplegia strategy. Cross-clamp time = myocardial ischemic time; bypass time = systemic stress.Both are key predictors of postoperative complications including renal injury, bleeding, and ventricular dysfunction. Redo sternotomy risk is driven by anatomy, not just number.Aorta adherent to the sternum, conduit position, and chamber pressurization define risk more than the number of prior surgeries. Think longitudinally—ACHD surgery is lifetime planning.Surgical materials and strategies must account for future interventions, especially in younger patients. Notes: Notes drafted by Dr. Alysha Joseph, aided by generative artificial intelligence. What are the key steps in congenital cardiac surgery from incision to closure? Preoperative planning is multidisciplinary, involving surgeon, anesthesia, cardiology, and ICU teams; high-risk inductions (e.g., critical AS, Williams syndrome) are identified early TEE is performed immediately after induction to reassess anatomy and may reveal new findings (e.g., underestimated AI, mitral disease, PFO) Median sternotomy is performed, followed by creation of a pericardial well to optimize exposure Heparin is administered prior to cannulation; arterial and venous cannulas are placed for initiation of CPB Cross-clamp is applied and cardioplegia delivered to arrest the heart, allowing a still and protected operative field Surgical repair (e.g., Ross procedure) is performed, followed by de-airing, cross-clamp removal, and reperfusion Patient is weaned from bypass with TEE reassessment, hemostasis achieved, and chest closed What is cardioplegia and how is it delivered? Cardioplegia is a potassium-rich solution that arrests myocardial activity and reduces metabolic demand Most commonly used solution in the U.S. is Del Nido cardioplegia, originally developed for pediatric myocardium Delivery strategies include: Antegrade (via aortic root) – standard approach Ostial (direct coronary delivery) – used when aortic root cannot be relied upon Retrograde (via coronary sinus) – useful in severe AI or coronary disease NOTE: Severe aortic regurgitation can impair antegrade delivery and requires alternative strategies and LV venting What do cross-clamp time and bypass time represent clinically? Cross-clamp time = duration of myocardial ischemia while the heart is arrested Bypass time = total duration on CPB, reflecting systemic exposure to non-physiologic circulation Prolonged cross-clamp time (>2–3 hours) increases risk of myocardial dysfunction, especially with poor baseline function Longer bypass time is associated with increased risk of renal injury, coagulopathy, and bleeding These metrics often reflect both case complexity and intraoperative challenges What is hypothermic circulatory arrest (HCA) and when is it used? HCA involves complete cessation of blood flow to allow a bloodless surgical field Typically used in complex aortic arch repairs Patients are cooled to ~18°C to reduce metabolic demand and protect organs Duration is ideally limited to

The one-size-fits-all approach doesn't address those with hypermobility syndromes, mast cell activation syndrome, dysautonomia, or complex chronic illness. In perimenopause, conditions you've dealt with for years can change significantly. Symptoms that were manageable for years suddenly become more intense, pain patterns shift, sleep becomes less reliable, and many women find themselves wondering whether this is "just perimenopause" or something else entirely.What makes this phase especially challenging is that hormones don't operate in isolation. Estrogen, progesterone, connective tissue health, immune function, mast cells, autonomic regulation, sleep, and stress physiology interact simultaneously. Myopically looking at menopause through the lens of hot flashes and hormone replacement therapy makes us miss the much more complex reality facing women who are already navigating chronic health conditions. We're celebrating our 10th anniversary all year by digging in the vaults to re-present classic episodes with fresh commentary. Today, we're revisiting our 2023 conversation with Robert De Leo of Stone Temple Pilots. ABOUT ROBERT DE LEORobert De Leo is best known for his work in Stone Temple Pilots. Collaborating with lyricist and vocalist Scott Weiland, Robert wrote or co-wrote the music for many of the group's classic songs, including “Plush,” “Creep,” “Wicked Garden,” “Vaseline,” “Interstate Love Song,” “Big Bang Baby,” “Down,” “Hollywood Bitch,” and “Between the Lines.” Outside of their music in Stone Temple Pilots, Robert and his brother Dean have also written and recorded songs for their band Talk Show, as well as Army of Anyone, which is a collaboration with Filter frontman Richard Patrick. Since the death of Weiland and subsequent lead singer Chester Bennington, the De Leo brothers and drummer Eric Kretz continue to make music as Stone Temple Pilots with current vocalist Jeff Gutt. At the time of our conversation, Robert had released his debut solo album, Lessons Learned, a deeply personal collection of original songs. Featuring a handful of guest singers, it's a different sound than listeners might expect from De Leo, exploring the vibes of Laurel Canyon, Americana, roots music, and a healthy dose of one of Robert's musical heroes, Glen Campbell. In this episode you'll hear a lot of great new music and gain a deeper appreciation for some of the amazing Stone Temple Pilots catalog as Robert grabs his guitar and breaks down some of those classics in mind-blowing depth. Dogs are Smarter Than People/Write Better Now Last week, we talked about pinch points both on the podcast and on the blog, and honestly? Nobody seemed super into it, but we're finishing up this week. This post is going to be a bit more about the first part of act two of a three-act story, focusing on the time from the first pinch point to the midpoint. Pause for a plea: Look, I know plot structure isn't sexy the way character development or drama and obstacles and conflict are, but it's super important. It makes a difference in your book wooing readers and in it wooing agents. K.M. Weiland has a really lovely graphic that we've included in the podcast notes about where to put those pinch points. Weiland is a bit of a goddess about structure and what she says about this first pinch point is this: It comes about 37% of the way into the story. It tells us that the bad guy has some power. It can be a whole big scene or just the tiniest of moments It sets up “the next 1/8th of the story, in which the character will slowly begin to grow into a new awareness of his story's many truths–and specifically the truth about the nature of the conflict in which he is engaged.” Right after this big and important pinch point, the hero of your story aka your protagonist moves into the section of the book that comes before the book's halfway point or midpoint. Weiland calls this space from 37% to 50% a realization place and scenes for your character growth. The protagonist understand what's going on a bit more. She starts to react with that knowledge informing her reactions and then her actions. Cool, right? She writes, “In itself, the First Pinch Point does not reveal the true nature of the conflict to the protagonist. Rather, it foreshadows it by providing a peek at facts the protagonist has barely grasped as yet.” She uses the movie ALIEN a lot to explain this. At the first pinch point, the crew realizes that the alien creature isn't what they were thinking it was. Their choices start to be informed by that until the midpoint, which Weiland calls the MOMENT OF TRUTH. At the midpoint in ALIEN that alien smashes its way out of one of the crew's chest. The truth of what they are dealing has exploded in the ship and on the screen (and on your novel's page). “It's instructive when watching movies to observe the protagonist's facial expressions prior to the Moment of Truth and then afterward. Before the Midpoint, he'll often look baffled as he struggles to keep up with the conflict. Then the light dawns in his eyes at the Midpoint, and from that moment on, there's a look of knowing determination on his face,” she writes. Larry Brooks defines pinch points as “An example, or reminder, of the nature and implications of the antagonist force, that is not filtered by the hero's experience.” DOG TIP FOR LIFE Sometimes in life, your defining moments don't come at the midpoint. - Mr. Murphy So, what he's saying is don't think that there are certain points and ages in your life where you have to get things done. Life is not a book and it doesn't need to be a three-act structure. PLACE TO SUBMIT These are via Authors' Publish. Islands has a new record out called "What Occurs" and Islands lead singer Nick Thorburn couldn't be more stoked to release it Friday June 21st. Lead singer Nick Thorburn leans against the jet black wall of LA's "Club Zebulon" and languidly sips his White Claw spritzer. "I love the new record. It's called "What Occurs"" he offers, blue eyes piercing through the hazy dimness of the hipster mecca. "But the thing is, I spent dozens of hours on this album, I put my friends on it, we collabed on it, it was a beautiful experience putting it together, but here's the thing - A.I. might do a better job than bands." Nick pours himself into a booth at the back of the bar and slides all the way back, like a greased up newborn, to the booth jukebox, where he shuffles through potential selections with his nimble fingers. "The Doors, Bon Jovi, John Cale, Paula Abdul. All the new music sucks" he sputters as he snaps his fingers for a waiter. "Garcon! I'm about to spill!" The server makes their way over as Nick, like a petulant little puck, pours his White Claw spritzer all over the table. "Want a tip, lady? Don't buy no Brooklyn Bridges!" Nick stands and starts "flossing". "My s$$%t blows all that s%^&t out of the water!" All of Club Zebulon is now looking at Nick. "But me and Jon went on a roadtrip and man... AI might just be better than bandssssss!" Nick gyrates like Weiland at a sock-hop. "... I don't care what Jon says, and how good AI writes songs about the 2 Jons Podcast in many genres, it may be better, but can it do THISSSSSS???? Nick smacks the jukebox with his light blue loafer. "Johnny Be Good" comes to life and Nick starts Twisting! And then all of Zeb starts Twisting! "This is just like Back To The Future! With Doc Brown, Marty and the Mom? Weeeeeeeeeeee!" Stone Temple Pilots, often abbreviated as STP, burst onto the rock scene in the early 1990's, becoming one of the most commercially successful bands of that era. Their debut album, Core, was released in late September 1992, and we're highlighting it in January 1993 when many of their hits were on the charts.The DeLeo brothers form the heart of the band: Dean DeLeo's guitar work provides the sonic foundation, and Robert DeLeo's bass playing adds a groove to the band's sound. Eric Kretz anchors the rhythm section on percussion. And front man Scott Weiland completes the lineup with his charismatic presence and distinctive vocal style. Weiland's emotion and raw energy helped make "Core" a pivotal release in the grunge movement."Core" hit he airwaves with a sonic explosion, with an energy that captured the angst and spirit of the early 90's grunge movement. Several tracks from this album would become anthems of the 90's generation, led by tracks like "Plush" and "Sex Type Thing." Despite its commercial success (reaching number 3 on the Billboard 200), "Core" was initially met with poor critical reviews, with many critics considering it too derivative of other grunge artists like Nirvana and Pearl Jam. Over time, however, "Core" would receive more positive reviews as it took its place as a seminal release of the grunge movement.Lynch takes us on this journey back to 1993, when the Stone Temple Pilots were introducing themselves to the world. CreepThis song is a slower, more introspective track that highlights the band's grunge influences. The lyrics deal with feelings of isolation and alienation, with Weiland delivering a haunting vocal performance. The acoustic arrangement is a nice contrast to the heavier tracks of the album.Sex Type ThingThe opening track to the album features an aggressive guitar riff and a powerful vocal performance from Weiland. It was controversial due to its lyrical content, which is written from the perspective of a rapist. Despite the controversy (or perhaps because of it) “Sex Type Thing” became one of STP's most recognized songs.PlushThis track is arguably the most famous song from the album. “Plush” showcases STP's softer side with its acoustic guitar intro and catchy chorus. Weiland's lyrics were inspired by a news story about a woman who was found dead in San Diego, adding a layer of mystery and intrigue to the song. The video for "Plush" won the band an MTV Video Music Award for Best New Artist and features the band playing in various desert scenes.Wicked GardenA hard-hitting rock track known for its gritty guitar riff and energetic chorus, this may be considered a deeper cut. The song deals with the loss of innocence and the darker side of human nature, themes that recur throughout the album. Over time “Wicked Garden” has become a fan favorite and a staple of STP's live shows. ENTERTAINMENT TRACK:Main theme to Star Trek: Deep Space NineThe third entry into the Star Trek franchise takes place on a fixed space station near the border of the Federation. It had its debut in January of 1993. STAFF PICKS:Love Is On the Way by Saigon KickBruce leads off the staff picks with a one hit wonder. Miami-based Saigon Kick released this single from their second album, "The Lizard," and it went to number 12 on the Billboard Hot 100. Jason Bieler provides guitar, keyboards, and backing vocals for the band, and wrote this song. Lead vocals are provided by Matt Kramer. Additional band members were Tom DeFile on bass and Phil Varone on percussion. Regret by New OrderRob brings us the lead song from New Order's sixth studio album, "Republic." The single went to number 28 on the Billboard Hot 100. New Order formed from the remnants of the band Joy Division. The album was made in order to save the Hacienda - a Manchester club part owned by the band. Songwriter and front man Kurt Cobain describes the song as a tribute to his wife and daughter, but it took on significance as a farewell note of sorts after Cobain's death. Nick employs the help of Poldi Weiland, a traditionalist foodie, homesteader, and host of the Year of Plenty Podcast. Poldi gives some points in why adding organ meat into our everyday diet is worthwhile. In addition, the guys open up about bone stock and its uses more than just soups and stews. Born German, Poldi has been back and forth a few times and has experience with hunting cultures both in the Old world and new. After a brief run through of German hunting culture, NIck notices some similarities of traditions held here in the states. The guys then dig a bit deeper as Poldi explains how organ meat isn't a fad, but honest to goodness real food, for an everyday diet. In Poldi's case its more important to get it down than to fluff it up and fancify it. Mixing it into ground, or even just taking small portions at face value is how he does it. Bone broth has also been an interest of Nick's and Poldi explains the benefits of adding broth or stock to an afternoon drink instead of another coffee.
