“I am not my illnesses,” says Mary Burgoon. Over the years, Mary has received six different mental illness diagnoses, including Bipolar Disorder, Harm OCD, and Borderline Personality Disorder, just to name a few. She has experienced suicidal thoughts, attempted suicide, and been hospitalized countless times. But still, Mary's inner strength and authentic self have always managed to shine through.Jim Burgoon, Mary's husband and life partner, has also undergone his fair share of challenges while coping with Mary's mental illnesses. April 25, 2015 would go down in history as the deadliest day on the world's tallest peak. Mt. Everest was slammed by a 7.8 magnitude earthquake. 18 people would die on the mountain; another 9,000 in the nation of Nepal itself. Jim Davidson, a veteran accomplished mountain climber, was on Everest that fateful day. Through his instincts and great fortune he managed to make it off the mountain alive. His account of that day, and the days that followed when he was struggling for survival, has resulted in one of the year's most riveting reads, THE NEXT EVEREST: Surviving the Mountain's Deadliest Day and Finding the Resilience to Climb Again(St. Martin's Publishing Group; on sale 4/20/21; $29.99). The largest earthquake in more than 80 years to hit Nepal altered a mountainside, a nation, and the global mountaineering community. As My Legal Einstein puts it on its website, the company " . . . is an online software technology company . . . that leverages the latest AI NLP technology to transform the legal contract collaboration and negotiation process." The episode begins with Jim describing his professional journey in founding My Legal Einstein. Jim, with an engineering degree from MIT in hand, started his career by working (outside legal tech) on the application of AI to video streaming. This work expanded into Jim's taking an interest in AI's applicability to data beyond the video streaming space. After exploring AI's application to still images and sound, Jim became intrigued by how AI can be applied commercially to written text. And, from there, it was a short step for Jim to understand how, with the law's everyday involvement with large language data sets, AI was a "natural" when it came to helping lawyers wrangle with contract management issues. Jim next takes listeners on a tour of My Legal Einstein's product offering. Here we learn that My Legal Einstein's mission overall is to make AI, and it application to contract management, readily accessible to lawyers. In fact, making "access to AI" easy and intuitive for lawyers is one of My Legal Einstein's core values. We also learn from Jim how My Legal Einstein assists lawyers' contract review work with contract clause extraction and collaboration tools that can be up and running within five minutes of a law firm's or legal department's signing into My Legal Einstein. As Jim, explains, shortening "time-to-value" for the My Legal Einstein customer is another core value for Jim's company, with the company achieving this time-to-value acceleration by - among other features - flattening the learning curve for putting My Legal Einstein to use. Jim tells tells us that one of My Legal Einstein's features is a built-in messaging service that sends extracted clauses to the client's business people with no need to "switch apps." As a web-based service, hosted by Google Cloud, Jim notes that My Legal Einstein necessarily takes privacy and security needs very seriously and even adds application-level security features to those that Google Cloud itself provides. Jim points out that My Legal Einstein offers a monthly subscription for its services (thereby avoiding long-term customers contract lock-in). And, as Jim notes further, his company also offers a free 30-day trial. The conversation next turns to how My Legal Einstein markets its product. Jim here talks about how the company makes itself known to potential customers by leveraging its AI know-how to provide educational content (in what Jim describes as an almost CLE-style) through various media (including presentations to bar association members) that familiarizes lawyers with AI's use in legal practice and dispels misunderstandings that foster distrust and even "fear" of AI among lawyers. Meet Jim Murray, a twisted individual crazy enough to actually enjoy property management. No, it’s true. His journey starts back in 2012 when he purchased his first House Hack, a four-unit property in Rhode Island. At the time, he was working full time for Fidelity Investments and was self-managing his property. Since then, he has jumped around to do some wholesaling, some flipping, to where he is today—a full time property manager and real estate investor. The unfulfilment with Corporate America had been building for some time since Jim made his first investment and one bad review too many, he did what most folks could only dream of and started his own company. So what makes Jim crazy enough to do the one thing most investors dread as it pertains to real estate? Simple, he says. He’s a systems-oriented guy who enjoys helping people succeed. Up to date, Jim manages over 600 rental units, owns and operates Lyon Property Management, and hosts The Cash Flow Kings Podcast. Going forward, he plans to continue to scale up his personal portfolio, transition more into the multi-family realm, and help more people grow their wealth. Our takeaways from our conversation with Jim:   1) Systems is the name of the game. Invest in technology that increases profitability. By having the right systems & processes in place, you attract and get to serve the right clients. And fortunately for us, business technology has never been easier to acquire and implement. So whether you self-manage or manage other people’s units, there’s technology out there that exists to help you out. (Find suggestion list down below.) However, understand this: Technology is meant to enhance, not replace. If you fail to develop & implement the proper processes first, investing in technology will do you or your business no good, and will probably only run you and your business dry of cash flow. 2) Fire bad clients. When starting out, you’ll be tempted to accept any and all business that comes your way. And that’s not really your fault. You won’t know what separates a good client from a bad one. But once you build your business to a respectable size, that’s when it’s time to visit our good friend, Pareto (80/20 Rule). Scale your respectable business into a sustainable one. Get rid of problem clients and double down your efforts on the good ones. Good clients (tenants, customers, etc.) are worth keeping around if you want to operate a sustainable business. But more importantly, bad clients are worth getting rid of in order to keep the good ones around. 3) Set expectations and practice accountability. This is the culture that Jim cultivates within his own company that has allowed him to make a business and career in taking over distressed properties. From the very beginning, let tenants (or clients) know what you are all about and what you will do for them. You must make your tenants know it is a privilege to rent from you, and at the same time, you must treat it as such—a privilege. That means holding yourself, your tenants, and all other parties up to the standard that you set. And when the bar is not met, someone needs to be held accountable for their actions. In doing this, you will weed out bad tenants and keep the good ones happy. 4) Image influences perception. As Jim explains in his story, when talking to contractors while wearing scrappy jeans and work boots as opposed to a suit and tie, he was quoted for a lower price for the same work being done. And it’s understandable as this is a natural human bias. So why not use this bias to your advantage? Here’s the point: It’s not always best to look like you’re made of money. While this goes without saying to look appropriate, hygienic, and professional, you don’t always need to look super polished. Rather, fit the profession you’re playing. While there are times that call for formal attire, wearing so in casual settings tends to make others perceive you as willing to pay more for something or are just flat out bougie. If Jim could go back and talk to his 16 year old self, he’d tell him, “Buy more real estate in 2009.” In other words, take advantage of the real estate cycles and buy real estate sooner! An unexpected benefit of real estate investing, Jim said, was the opportunity to live with time, location, and financial freedom. A piece of advice Jim would tell his friends looking to get started in real estate would be to “Listen to other people.” Use the free content and information available to you online, whether it be other real estate & business podcasts, websites like BiggerPockets, or the thousands of educational real estate videos on YouTube. Jim recommends using zInspector to help you create and store tenant condition statements. This comes in handy during any tenant-related litigation, as well as have as an additional layer of legal protection for your business. Jim Heppelmann is the CEO of PTC, a technology software company with 6,500 employees in 30 countries. Jim was named one of “7 IoT leaders to Watch in 2017” by Hewlett Packard Enterprise, he was recognized as “IoT CEO of the Year” by PostScapes, “Technology CEO of the Year” by the Massachusetts Technology Leadership Council, and he received the CAD Society Leadership Award for his work with the Internet of Things.  Together with Harvard Professor, Michael Porter, Jim has co-authored three highly influential articles on the transformational impact of the Internet of Things on business.  Jim grew up on a dairy farm in Minnesota as one of eight children. It was during his upbringing on the farm that he really learned the value of hard work as well as the ability to take things apart and put them back together, which is in part why he was successful as a mechanical engineer.  It was actually one of his older sisters who inspired him to attend college and study to become an engineer in the first place. After she graduated from college and got her first job she was already making as much as their parents, and Jim was amazed by that. So engineering brought him to college, but once he got there he fell in love with computer science.  He studied at the intersection of where software meets engineering, which turned out to work great as he is now the CEO of a company that produces software for engineers. An inside look at the role of CEO A lot of people may think they have an idea of what CEOs do throughout the day, and of course it differs a bit for each CEO depending on the organization. But Jim shares what it is like for him day to day as a CEO.  He says, “I don't want this to sound derogatory in any way, but maybe I'm responsible for herding cats, because there's a lot of people that are part of a lot of initiatives, a lot of programs, a big organization. And I'm trying to keep all of that moving in the right direction generally. So I'm not doing the precise tweaks, but try to keep everybody understanding where we're going. And make sure they're moving in that direction, and they're moving at the right pace. So that, you know, it all comes together each quarter when we're delivering products or projects or trying to make the quarter quarterly results happen.” He also shares that a large part of his day is made up of meeting with his people to check in to see how things are going, what projects they are working on, how they are solving problems, etc...He doesn’t try to tell people what to do, but he makes suggestions and tries to guide people in the right direction.  One part of the job that gives him a lot of energy is interacting with customers. He really enjoys figuring out what it is they are trying to do, how PTC can help them, and getting feedback on the good and bad aspects of their products. All in all Jim sees himself as the vision guy. While he realizes the need for internal meetings and keeping the organization on track, he really loves working with the engineers on the next generation approach they are building and working with the customers on where the company is going and the vision for the future of PTC.  Why CEOs should have a short attention span Jim often wakes up in the middle of the night with new ideas or ways to solve a problem. And the joke at PTC is that about the time Jim gets bored with something that is when it really starts to work. That’s because as long as he is focused on solving something he is going to come in to work with those middle of the night ideas to keep tweaking and fixing certain things.  But Jim believes that CEOs should be a little impatient and have a short attention span. Why? Because Jim says the biggest problem with CEOs at his level of tenure is they have become entrenched in old thinking.  About some CEOs, he says, “They made a decision five years ago. Maybe even eight years ago, and for a couple years, they improved it a little bit. And then they've just spent the last five years tell everybody why it works. And saying we're not going to change it. And I'm a little bit of a different cut. I say, hey, we did this, we improved it, it's working. But we’ve got to think about what's next. And then let's not wait until it's not working to think about what's next. Let's some of us start thinking about what we're going to do next, even while this is still working pretty well.” Jim is always looking for the next round of changes that make PTC better, or that protect them from a new threat headed their way. This is a quality that he believes all CEOs should have. Always looking to the next thing, don’t just ride current success.  Achieving work-life balance as a CEO  As the CEO of 6,500+ employees, Jim seems very relaxed and happy. He doesn’t seem stressed out at all. One thing that has helped him with this a lot is having a work-life balance. He not only leads the organization, but he also makes time to spend time with his family, take care of the animals on his farm, and cook. But that wasn’t always the case. Jim says when he first became a CEO he burned the candle at both ends, he tried to do everything on his own. And overtime he learned that was not sustainable. When sharing what he learned he says, “what I should do is focus my energy where I really bring a lot of value to the table, again, which tends to be around product strategies, marketing strategies, marketing messages, competitive strategies, and so forth. And let somebody else manage the financial plan, let somebody else manage the professional services margins, and things like that because I don't need to do that and I don't bring a tremendous quality to it, you know, nothing super unique or special. And at the end of the day, you’ve got to pick your battles, there's just not enough time for a CEO to be in charge of everything. So I got a lot happier after I realized I should roll with the punches and just really add value where I think I have the most value to add.” This is such great advice for all CEOs, don’t try to have your hand in all the cookie jars. Don’t take everything upon yourself and don’t think you have to have all the answers. You will get burnt out that way. Surround yourself with people who are good at what you are not good at and rely on them.  Three ways to battle entrenched thinking  No matter what industry you work in, you may find that people around you are entrenched in old ways of thinking, especially if they have been in a certain role for a long time. People don’t like change, it’s just a fact of life. So how do you change that? Jim has three ways that he battled entrenched thinking inside of PTC when he first joined.  Make change part of your company branding--Create a company culture that likes change. Part of what Jim did to change the culture was he adopted some slogans like Take a Fresh Look. Everything about the company should embrace change and discourage getting complacent.  Lead by example--Live out the values you want to see inside your organization. If you want employees to embrace change, you must first be the one to embrace change. Make sure people know it is not about making one change and then staying there, it is about constant change. The point is to try to be that company who changes all the time, you can't pin them down, because they're too busy changing.  Celebrate change--Recognize individuals who step out and do something different, even if what they tried didn’t work.  The impact of augmented reality on the future of work  One of the major trends Jim is paying attention to at the moment is augmented reality and the impact it is having on the future of work. There are so many applications for this technology. And one way it could help has been magnified by Covid. One quarter of the world works behind a computer screen and digitalization has done amazing things for knowledge workers who are in these positions especially in these times when they can easily work remotely from their homes. But for the other three quarters of the world the front line workers don’t have any other option but to go to work physically at a specific location or they can be laid off or fired.  Augmented reality (AR) could really solve this problem for frontline workers. It can also help people make fewer mistakes, do their jobs more efficiently, and it can even help with the current skills gap problem. In the US the demographics have changed, and we no longer have new generations of workers going into factories and industrial work, for a variety of reasons. People felt like this work was moving overseas, this type of work is no longer seen as desirable, etc...So what we now have is a majority of people in this industry reaching retirement age with no one younger coming in to take their places. This is the skills gap problem.  How could AR help solve this problem? It could record the knowledge of the current workers to be used for training in the future and it could also be used to show potential workers what the work is really like to convince them to go into that field of work.  As Jim shares, “AR can play a huge role. Because for example, we can capture the expertise of the retiring worker, that kind of AR YouTube idea. We can capture that expertise, store it, and when a new worker shows up through a Hololens, or phone or tablet actually redeploy that coaching that's been digitized into the physical environment for the benefit of the new worker. So the new worker can be coached in the actual environment by somebody who retired two years ago. It's just a very powerful idea.”

How to Find a Lawyer When I was 20 years old, I saw the movie Scarface, with Al Pacino. The idea of trafficking in cocaine looked exciting. I called acquaintances I had in Miami. Before long, I orchestrated sales in multiple-kilogram quantities. I coordinated people to transport cocaine across the country. Foolishly, I convinced me that since I didn’t handle the cocaine myself, I wasn’t really breaking the law. As a precaution, long before I got caught, I hired a criminal defense lawyer. But I didn’t have any idea on how what questions I ask to hire a lawyer. As a result, I gave hundreds of thousands of dollars to a lawyer who told me what I wanted to hear, rather than what I needed to hear. In my case, the result of hiring the wrong lawyer translated into a 45-year prison term. Don’t make the same mistake. Learn everything possible about the steps you should take. Learn how to find the right lawyer. My partner, Justin Paperny, describes some similar challenges. He had been working as a stockbroker and he’d never been in trouble before. In Lessons from Prison, Justin reveals some of his bad decisions. By not understanding how to hire a lawyer, he flushed tens of thousands down the drain. When he wasn’t honest with his lawyer, he put himself in a worse position, exposing him to significantly harsher punishment. Our team interacts with thousands of people that face challenges with both civil and criminal law-enforcement agencies. Perhaps a story about Darren, one of our clients, may illustrate the challenge that people face when they don’t quite know how to find a lawyer.  Hiring a Lawyer:  Darren had been in business for more than a decade and he worked with corporate lawyers extensively. He contacted our team when he realized that some of his decisions on the job were about to expose him to problems with regulatory agencies, and possibly to challenges with the Department of Justice. Darren’s problem reminded me of my own challenges. As I wrote in Lessons from Prison, I was a stockbroker at UBS when I learned that some of my decisions brought me to the attention of officials at the Securities and Exchange Commission. I knew that I needed a lawyer, but I didn’t know how to find a lawyer. When authorities like the SEC, the FCC, the FTC, or the FDA start asking questions about how to hire a lawyer.  Use Contacts:  Check with people you know who have had experience in working with criminal defense lawyers. Learn from their experiences. If you know someone who has been to prison, ask them what they heard about their experiences of working with criminal defense lawyers.  Learn about the costs before making a hiring decision.  Any defendant searching to find a lawyer may want to research the lawyer’s area of expertise. If a lawyer has extensive experience in a given area, that lawyer may be more familiar with case law, statutes, arguments, and decisions that could prove beneficial. Consider questions to ask before you speak with the lawyer. About each lawyer’s rate for billable hours, Important to know so a defendant can get an idea of how much to budget. In some cases, a lawyer may be able to resolve a plea negotiation and guilty plea in less than 100 billable hours, preparing for trial in federal court would likely require several hundred billable hours.   About the lawyer’s position on accepting a flat fee for the entire case, A flat fee may or may not serve the interest of the defendant. Every case is different. On the plus side, a defendant that has a flat fee knows exactly how much to budget. On the negative side, a lawyer that receives a flat fee may be disinclined to devote much time to the case after he is paid. About the types of cases, he or she practiced, A lawyer that specializes in defending tax cases may not be the best fit for a defendant facing criminal charges related to mail fraud that could result in the loss of liberty.   About the lawyer’s expertise in sentencing matters, More than 75 percent of the people that face charges in federal court eventually have a sentencing hearing. What level of expertise does the prospective lawyer have with sentencing in the given jurisdiction?   About the size of the lawyer’s team, A lawyer with a large team may be able to outsource work to associates or paralegals that bill at a lower hourly rate. The lawyer will have discretion, but it’s helpful to know that the lawyer is cost-conscious of a defendant’s resources.   About the lawyer’s philosophy on communication with clients, Some lawyers make themselves readily available to defendants, even providing cell-phone numbers for communication. Other lawyers prefer to work independently, leaving defendants out of the communication loop. About whether the lawyer would be receptive to accepting a 5-hour retainer to get started, just to see if a good fit exists for a productive working relationship. Some lawyers will not get started on a case without a minimum retainer. In federal cases, that initial retainer often exceeds $50,000. As Jim experienced, it’s difficult to make an assessment on an initial free consultation. If you’re searching to find a lawyer, create a plan. To the extent that it’s possible, create a series of questions that will help you gauge whether the attorney is the right fit for you. It’s important to be honest about your current situation and what you expect. If you go into a meeting with a lawyer and minimize your conduct, you may set yourself up for a bad outcome. Brian, for example, was a director with a large financial services company. His employer had an insurance policy for “officers and directors.” The policy would pay legal fees for people in specific positions. When federal authorities indicted Brian, he hired an experienced white-collar criminal defense attorney that would send all bills to the insurance company. The law firm launched a scorched-earth defense strategy, and quickly burned through the $500,000 cap on legal fees. When that insurance bill expired, the lawyers told Brian that he should plead guilty and that he would be responsible for the remainder of his bill—which amounted to an additional $75,000. “I always knew that at the end of the day, I was going to plead guilty,” Brian told me. “When I went to find a lawyer, I went along with what they said. They wanted to contest every document. But every time they read a document, they charged exorbitant fees. There bills showed that I was burning through more than $20,000 on legal fees some days.” If it’s true that Brian knew he would eventually plead guilty, when he looked to find a lawyer, he may have been better suited to find an attorney with a strong track record of making great plea deals. At the end of the day, any defendant that needs to learn how to find a lawyer should follow the path that Darren pursued. To the extent possible, use contacts to ascertain the expertise, honesty, and competence of lawyers with regard to specific types of cases, in a specific jurisdiction. Approach those lawyers with a list of questions that will help you determine if you think that you work well together. To the extent possible, ask the lawyer to accept a limited engagement to test the waters. Most importantly, make certain that you have the financial resources available to support the decision you make.

It is amazingly effective. And I want people to think of social media a little bit differently when you're working from home. That 10-minute break. You could go on social media because that might be the only way you have contact with another person. Yeah, I've been doing this work-at-home thing now for well over 20 years, and my wife and I have learned a lot of lessons. I've learned a lot of lessons about productivity and tools. And, of course, you mentioned the security side of it, which is a big deal. And so what I started doing this week is I started on Sunday, I held a kind of free for all webinar where I invited people to, and we spent a couple of hours together. In fact, in this webinar, we talked about people's situations. What the issues were. I'm going to be doing it again today. I think what I'm going to try and do is something at about three o'clock this afternoon, and then probably at about seven o'clock this evening, and I'll do it two or three more times this week, just going over what tools can help them. One of the tricks I've learned over the years? What do you need to worry about from a security standpoint? You know, when should you be using that laptop? When should you remote to a desktop at work? Why is the remote desktop one of the most prominent security exploits known to man? And how can you work around that? So obviously, Jim, as usual, this is just all for free. I'm not trying to, you know, beat you down and sell you something. If you want my services, that's fine. Jim We're talking with Craig Peterson, our tech talk guru. Now, Craig, what's probably the most important thing people should do while working from home. Now I know our company has set up, what are these VPNs and all this stuff? What's the most important thing people should do? Craig Well, also, as part of the webinar, I went into some details about why VPNs' can make it worse for you and your security for your business. So don't think that a VPN is a panacea. All those paid VPN Services that you hear advertised on the radio are not going to help you get into the office. They're designed to provide a little bit of security. But, in a more direct answer to your question, what's the most important thing? I think personally, with over 20 years of experience doing this, that it has to do with separation. So President Trump is right. You have to separate yourself from your usual living area if you can. All separating has to mean is something that's going to give you that psychological hint. That's going to help with your subconscious too. Okay, I'm sitting in this chair facing this direction in this part of the room. That helps a lot to train your body as to what you should be doing. Then you also need to get a whip and, and use that lightly on any family members that decide to walk-in because now look at mom and dad are home now so I can just walk in anytime and ask any question, right? In the honey-do list, I'm going both ways. So make sure they understand, hey, I'm working to leave me alone. Here's another thing that I found to be extremely helpful when working from home. And that is to use a technique that came from Italy. It's called the Pomodoro Technique. You might want to look that up that's Italian, very poorly pronounced on my part. Jim Italian for Tomato, and tomatoes are the best. Craig Yeah, exactly. Think about you and I, were younger, our moms had these kitchen timers shaped like tomatoes, or maybe you had an egg one. You twist the top of it, and it was a 30-minute timer. The Pomodoro technique is simple enough, and you set yourself a 15-minute or 20-minute goal. 20 -minute seems to be optimal, at least for me. Okay, I'm going to do this particular piece of research to get this part done, right. It's something you can do in 20 minutes. You set that timer and let it run for 20 minutes. When it goes off, the task over whether or not it is done. You finished that work for this time period. You can extend a little bit if you'd like to, but no more than about 10 minutes, get up and walk around. It is time to go and get your glass of water, to go and chat with the spouse or kids, whatever it might be. You take a 10-minute break, and then you set down again, set yourself another 20-minute goal, and get it done. It is amazingly effective. I want people to think of social media a little bit differently when you're working from home. That 10- minute break. You could go on social media because that might be the only way you have contact with another person if you're at home working alone. It's not necessarily an evil thing. Just make sure you don't get sucked into it. They design social media to be sticky and to keep you around. So those are the big ones. What you can do, what you shouldn't do, to make working at home, work for you without creating problems not just for you, but security problems to the business's network. It’s Not What It IS… It’s What It DOES! When it comes to sales copywriting, one of the biggest mistakes people make is getting hung up on WHAT they sell. Not what your products or service actually DO for the people that buy them. Hook and sell on emotion, easing of a problem with anything we sell. We all need to remember that people buy what you're selling based on the desires it satisfies or the problems it solves. As Jim references in the podcast, we don’t just sell tea, a book, or a drill, we sell products that help people ease their pain, solve their problems, or help them reach their goals and dreams. Never forget that. TWO TIPS: First, before you ever start a copywriting, advertising, or email marketing session, begin by answering these three powerful questions. 1) What problem do you solve or desire do you satisfy? 2) Who do you solve the problem or satisfy the desire for? 3) How do you solve that problem or satisfy the desire? By doing this, you’ll put the emphasis where it belongs: on the end result you offer. Most people focus on their product and the fact they want more sales (and FAIL miserably). I suggest you get clear on the result you provide, who you provide it for, and how your product makes it possible for them to get what they want. Second, sit down and make a list. List off at least 5 desires and 5 problems your product or service satisfies and solves for your target audience. Then, next to each of those items write out the EMOTION or emotionally charged outcome tied to solving that problem or satisfying that desire. Finally, next to the positive emotion, write out the emotion or emotionally charged outcome tied to NOT solving that problem or satisfying that desire. How will life feel if they don’t solve this or get this done? Tying the emotion to both the positive and negative gives you incredible power in both your understanding of what you’re really selling AND in your ability to communicate in your sales copy. Bottom line: STOP selling what your product or service IS and START selling what it DOES!

As Jim takes a drink it is now his turn to get some background. He asks Sheriff Bart how a dazzingly urbanite like him, wound up in a rustic setting like Rock Ridge. As Bart begins to tell his story, the music swells and we get the Blazing Saddles theme done as full orchestra with a heavy western motif. A long line of wagons is heading west along the prairie. Bart explains that in '56, he and his family were part of that wagon train. He then corrects himself. He explains they were not so much a part of it. He says, "You might say we was bringing up the rear." Suddenly, the entire Sioux nation was riding down to attack them. Today's guests are Sean German and Dave Pallas. Dr Carolyn Lam:                Welcome to Circulation on the Run, your weekly podcast summary and backstage pass to the journal and its editors. We're your co-hosts, I'm Dr Carolyn Lam, associate editor from the National Heart Center and Duke National University of Singapore. Dr Greg Hundley:             I'm Greg Hundley, associated editor from the Pauley Heart Center at VCU Health Sciences in Richmond, Virginia. Dr Carolyn Lam:                A big number of acute ischemic stroke patients receiving endovascular therapy in the United States are receiving this therapy only after inter-hospital transfer. What are the temporal transient outcomes following this inter-hospital transfer? Very important discussion coming right up with our featured paper. But for now, sit back, relax with us. We're going to discuss a couple of papers that we found were interesting in this week's journal. Dr Greg Hundley:             Very good, so thanks Carolyn. I'll start off, and I'm going to talk a little bit about stress induced cardiomyopathy, and we also know it as takotsubo cardiomyopathy, looking at a paper from Dana Dawson from the University of Aberdeen in the United Kingdom. Takotsubo cardiomyopathy can result in a heart failure phenotype with a prognosis comparable to myocardial infarction.                                                 In this study, the investigators hypothesize that inflammation is central to the pathophysiology in natural history of takotsubo cardiomyopathy. They prospectively recruited 55 patients with takotsubo cardiomyopathy, and 51 age, sex, and comorbidity match control subjects.                                                 During the index event, and at five months of follow-up, the patients with takotsubo cardiomyopathy underwent a cardiac MRI study in which they looked at ultra-small, super paramagnetic particles of iron oxide, or USPIOs, enhancement for detection of inflammatory macrophages in the myocardium. What would the studies show? Patients with acute takotsubo cardiomyopathy had macrophage-mediated myocardial inflammation.                                                 They also demonstrated modulation of peripheral monocyte subsets and increased systemic pro-inflammatory cytokines. This systemic inflammation persisted for five months, and then at that five-month time point, the cardiac MRI evidence of the macrophage presence was diminished. Dr Carolyn Lam:                Wow, Greg. So this is right up your wheelhouse, isn't it? Can you explain? What are the clinical implications of these MRI findings? Dr Greg Hundley:             It was really interesting. For the first time, they've linked an ongoing inflammatory process using the USPIO contrast agent with MRI actually going on or operative in the heart, and they associate that with systemic markers in the circulation.                                                 They help us elucidate the mechanisms and the pathogenesis of takotsubo cardiomyopathy, and systemic and myocardial inflammation really may start to now serve as a therapeutic target for patients with acute takotsubo cardiomyopathy. Dr Carolyn Lam:                Very interesting. From stress-induced cardiomyopathy to early onset myocardial infarction. The first paper I chose really answers the question, "What is the relative prevalence and clinical importance of monogenic mutations, that is, a single mutation that significantly increases risk, versus a polygenic score, which really measures the cumulative impact of many common variants, in early onset myocardial infarction?"                                                 The co-corresponding authors were Doctor Amit Khera and Sekar Kathiresan and both from Massachusetts General Hospital, and they performed deep coverage, whole genome sequencing of more than 2,000 patients from four racial subgroups hospitalized in the United States with early onset myocardial infarction defined as myocardial infarction before the age of 55 years, and compared this to 3,761 population base controls.                                                 What they found was that a monogenic mutation related to familial hypercholesterolemia was identified in 1.7% of the patients, and associated with a 3.8-fold increased odd of myocardial infarction. In comparison, the high polygenic score, which was composed of 6.6 million common DNA variants and defined as the top 5% of the control population distribution, now, that was identified in 10 times as many patients, so 17% of patients, and associated with a similar 3.7-fold increased odds of myocardial infarction. Dr Greg Hundley:             Interesting. How do we apply this clinically, Carolyn? Dr Carolyn Lam:                These findings really lay the scientific foundation for the systematic identification of individuals born with a substantially increased risk of myocardial infarction. The important point is both familial hypercholesterol mutations and a high polygenic score are associated with more than three-fold increased odds of an early onset myocardial infarction.                                                 However, the high polygenic score cannot be reliably identified on the basis of elevated LDL cholesterol, and yet has a 10-fold higher prevalence among patients presenting with early onset myocardial infarction. So very intriguing that both groups matter. Dr Greg Hundley:             Very good. My next paper is from Adrian Hobbs at the London School of Medicine, and is looking at the role of endothelial C type natriuretic peptide as a critical regulator of angiogenesis and vascular remodeling. We know that a central pathway coordinating both neovascularization and ischemic extremities in PAD is driven by vascular endothelial growth factor or VEGF-A4.                                                 But preclinical studies and other large scale clinical trials have been disappointing because administering or using VEGF-A to promote angiogenesis or arteriogenesis in PAD really hasn't occurred. This group focused on endothelial-derived CMP. Why? Because it plays a fundamental role in regulating vascular homeostasis. It controls local blood flow and the resistance vasculature, and systemic blood pressure, and reduces the reactivity of leukocytes and platelets.                                                 So, what were the results? Clinical vascular ischemia was associated with reduced levels of CMP and it's cognate NPR-C. Moreover, genetic and pharmacological inhibition of CNP and NPR-C reduced the angiogenic potential of the pulmonary microvascular endothelial cells and the human umbilical vein endothelial, and it isolated vessels ex vivo.                                                 So, the study really defines a central pathophysiological role for endothelium-derived C type natriuretic peptide via activation of cognate natriuretic peptide receptor C in angiogenesis and in vascular remodeling. Moreover, the work demonstrates the therapeutic utility of pharmacologically targeting NPR-C to restore deficits in these processes following ischemia and injury. Dr Carolyn Lam:                Interesting, from new mechanisms and targets to good, old, major risk factors for coronary heart disease. Back to the basics but in a really, I think, nicely done paper from Dr Pencina and colleagues from Duke Clinical Research Institute.                                                 Now, their objective in this next paper was to compare the associations of key, modifiable coronary heart disease risk factors with incident coronary heart disease events based on their prognostic performance, the attributable risk fractions and treatment benefits overall and by age.                                                 And so really aiming at quantifying the importance of these major, modifiable risk factors for coronary heart disease. What they did is they used pool participant level data from four observational cohort studies sponsored by the NHLBI, and they created a cohort of more than 22,600 individuals ages 45 to 84 years old who are initially free of cardiovascular disease.                                                 And these individuals were followed for 10 years from baseline evaluation and followed for incident coronary heart disease. They estimated that age, sex and race captured up to 80% of the prognostic performance of cardiovascular risk models. When we add either systolic blood pressure or non-HDL cholesterol, diabetes or smoking to model with the other risk factors, the prognostic performance, as measured by the C index, increased by only 0.004 to 0.013.                                                 However, if you look at it from the attributable risk and absolute risk reduction standpoint, lowering the systolic blood pressure of all individuals to less than 130, or lowering LDL cholesterol by 30% would be expected to lower a baseline, 10-year coronary heart disease risk of 10% to 7% and 8% respectively. Dr Greg Hundley:             That's a lot of data, Carolyn. Help me synthesize all that. Dr Carolyn Lam:                This is a take-home message. Although the individual modifiable risk factors contribute only modestly to the overall model prognostic performance, when we eliminate or control these risk factors, they would actually lead to a substantial reduction in total population coronary heart disease.                                                 That's because if we look at the attributable fraction and the absolute risk reductions, we see that they actually really matter. The take-home message too from Dr Pencina was that metrics used to judge the importance of these risk factors should therefore be tailored to the question being asked. Dr Greg Hundley:             Very good. That was a very nice summary, Carolyn. Dr Carolyn Lam:                Thanks. Let's move on now to our feature discussion, shall we? Dr Greg Hundley:             Very good. Dr Carolyn Lam:                Trials have established that endovascular thrombectomy dramatically reduces disability after acute ischemic stroke due to intracranial large vessel occlusion. In fact, guidelines almost immediately adopted endovascular thrombectomy as a standard of care. However, that has created some problems.                                                 The main one being that hospitals equipped to carry out this procedure are largely limited to tertiary centers in urban areas. This is, of course, important because that means that patients may need to be transferred from another center to receive such treatment.                                                 Today's feature paper discusses this very issue, a terribly important one, and I'm so pleased to have the author with us, Dr Shreyansh Shah from Duke University Medical Center. We have our editorialist, Dr James Grotta who's director of the Mobile Stroke Unit project at Memorial Herman Hospital.                                                 And we have an associate editor, Dr Graeme Hankey from University of Western Australia. So, such an important topic. I think Shrey, could you just jump right in and tell us what your study showed. Dr Shreyansh Shah:         I'm very excited to present findings of our study, and as a Carolyn mentioned, this study is going to have a very important implication in our country here in US on the creation of systems of stroke. I think the findings are already applicable to other countries also where we are seeing endovascular care getting more and more used.                                                 As Carolyn was talking, endovascular treatment is very important and lifesaving measure. But unfortunately, it is not available at every hospital. Patients are often transferred across different hospital or institution before they can receive this endovascular care.                                                 What we did in our project was we looked at the data from the hospital that's participating in Get With The Guidelines®® Stroke, which is a quality improvement program here in US. It looked at the endovascular thrombectomy used especially in relation to inter-hospital transfer.                                                 What we found was big proportion of patients receiving endovascular care, up to about 43% to 45% of patients, were getting the care after transferring across different hospital. The outcomes in this patient were worse compared to the patient who were receiving endovascular care if they had come directly to the hospital.                                                 While there was no difference in mortality between these two groups, the endovascular care, after inter-hospital transfer, resulted in a higher rate of symptomatic ICH, patients are less likely to be discharged to home, which is the preferred outcome. And patient was also less likely to be able to ambulate independently prior to the hospital discharge.                                                 There was also delay in endovascular care initiation for patient who received this after inter-hospital transfer. I think this particular study highlights the magnitude of this problem, and that's why it's going to be important for people who are studying systems of care. The fact that about 45% of patient had to get inter-hospital transfer before endovascular care tells us that we still need to take significant steps in increasing access to this lifesaving therapy. Dr Carolyn Lam:                Thank you and indeed James, I really love the editorial you wrote that accompanied this. I mean you highlighted its importance, and you also noted that what was unusual about the paper was that even after controlling for the delay in initiating endovascular thrombectomy, there was still worse outcomes in the patients who were transferred. Could you share some thoughts? Dr James Grotta:              It is a very timely issue. Now that we have a very effective treatment, the big challenge we have is getting it to the patients as fast as possible. Right now, our system, as is pointed out, means shuffling patients from one hospital to another.                                                 I think that clearly with stroke treatment, any sort of stroke treatment, the faster we deliver it, the better. Other studies have shown that transferring patients is associated with a delay of treatment, and this study showed the same thing.                                                 There was a substantial delay in getting the patients treated if they required a transfer. And as you pointed out, however, this did not explain the entire or was not at least the entire explanation for the worst outcome. So, it is a little bit of a mystery.                                                 I do know from personal experience that transferring patients from hospital to hospital, it's not exactly a black hole, but you lose control of the patient when they're being transferred. These are patients who have large artery occlusions. That means they have their middle cerebral artery is blocked.                                                 And so, the area of brain that's affected is in a very tenuous shape. So, any drop-in oxygen concentration from breathing problems or of any drop-in blood pressure might further worsen the stroke. So, this could happen in transit. So, it's possible that in the process of transfer, these sorts of things happen.                                                 I do think that we do have to be a little bit careful in that by remembering that this was not a randomized comparison, so patients that were treated directly and those that were transferred were not randomized. And so, although they appear to be balanced in a lot of the important variables like their stroke severity, there may be other things that we can't account for that could explain some of the worst outcomes.                                                 I'd like to ask Dr Shah whether he identified any things in ... well, he and his co-authors think might have contributed to some of the worst outcomes.  Dr Shreyansh Shah:        To answer Dr Grotta's question about what other factors may have played a role in the worst outcome that we saw in patients who were getting inter-hospital transfer, I think as we correctly pointed out, transferring this very sick patient is very tricky. As we know, the hemodynamic instability or variability plays an important role in outcomes of stroke patient.                                                 And it is very likely that during the transfer process, there is not adequate control of their blood pressure variability, their oxygen saturation, and this ends up affecting their brain leading to worst outcome. The other possibilities also, as Dr Grotta was explaining, this is not a randomized control trial.                                                 And although we balance for number of important factors that can affect stroke outcome, there might be a selection bias in transferring patient who are more sicker and also patients who received thrombolysis with TPA but did not improve, while the patient who were directly arriving to the hospitals and getting endovascular care, they received the TPA.                                                 It is possible that they started to improve and still received a thrombectomy at the same time. So that group may have been more favorable in that respect, which could have also played a role in better outcomes with patient who are directly arriving. Dr Carolyn Lam:                Interesting. And, you know, with the mention of TPA, I really have to bring James back. I loved your mention about potential solution using mobile stroke units. And since you direct one of them, could you tell us what you meant there? Dr James Grotta:              Yes, of course, I have to state at the outset that I have a little bit of a bias about mobile strokes, and so I do it every day. What a mobile stroke unit is, for those who don't know, it's basically taking the emergency department to the patient.                                                 It's an ambulance with a CT scanner on board and the ability to treat with TPA in the field. But in addition, it's also the CT scanner. We can do CT angio and identify large vessel occlusions on the mobile stroke unit, not to mention the fact that you have a vascular neurologist either in-person or by telemedicine examining the patient.                                                 So clinically, you can make the determination also much more accurately than any sort of pre-hospital stroke scale, whether the patient has a large artery occlusion. That way, you don't have to take the patient to the nearest hospital. You can bypass the nearest hospital, take them right to the thrombectomy center, therefore, avoiding the transfer process.                                                 We've been implementing this in Houston, and there are now about 30 mobile stroke units around the world. The innovation actually started in Germany by Dr Fassbender about a decade ago in Hamburg, Germany. We are conducting a randomized trial, comparing mobile stroke unit care to standard management to see how much better outcomes occur as a result of this faster treatment.                                                 We obviously can treat patients with TPA faster. For example, a similar study from the Get With The Guidelines® a few years ago showed that only 1% of patients treated with TPA in emergency departments get treated within the first hour after symptom onset simply because it takes an hour in the emergency room itself to do the evaluation of the patient and get them treated.                                                 Whereas on our mobile stroke unit, at least a third and probably 40% of the patients we're treating with TPA, we can get treated within that first hour where there may be an exponential better benefit. But we don't yet know really how much that translates to better benefit, and also, of course, mobile stroke units are more intensive in terms of the amount of facilities on board and costs.                                                 So, we need to look at the cost-effectiveness. If it produces only a marginal reduction in disability but costs a fortune, then it's not worth it. But in fact, in our experience, it's pretty practical. We can cover almost the entire City of Houston, which is the fourth largest city in the country, with one mobile stroke unit. When it's well-integrated, it requires careful integration with the fire department and other hospitals in the city.  Dr Shreyansh Shah:        At those two conferences, I came across a very interesting talk from Dr Grotta's group about rendezvous with the EMS which allows extending their coverage area significantly. I think we definitely need more and more innovative solutions like this where we can identify patients by their origin, whether they have large vessel occlusion or not, and then triage them appropriately at the centers that can perform endovascular therapy. So as a result, we can provide them earlier therapy and hopefully, it will lead to better outcome. Dr Carolyn Lam:                Thank you Shrey and James for these incredible insights. Now, Graeme, I want you to have the last word and reflections from down under. Dr Graeme Hankey:        Firstly, just to congratulate Dr Shrey and colleagues on this terrific study that reports a contemporary United States experience, a very broad one across the country, really highlighting how since 2012, until a year ago, there's been a six-fold increase in the number of patients being transferred for endovascular therapy.                                                 And we're all experiencing that around the world. And moreover, since the DAWN trial and the DEFUSE trial were published just over a year ago, which is when this study stopped, there's been an expansion of the window from six hours out to 24 hours.                                                 So, in the last year, which this study doesn't cover, we've seen an exponential increase in the number of people being transferred from rural and remote areas who have had a stroke up to 24 hours ago being considered for endovascular therapy if their CT angiogram at the base hospital shows a large vessel occlusion.                                                 This is likely to be not only internally valid, but externally valid to all of us around the world. It reflects our experience of this avalanche of cases coming. And it's provided a lot of challenges for those who are trying to deliver the service at the tertiary referral center.                                                 And it highlights that nearly half of the cases who are having endovascular therapy are coming from external sites. As Jim has really highlighted in his editorial, it challenges us to reassess the current practice of inter-hospital transfer. Today’s guest helps us understand how to break through procrastination and finally get that book finished. His name is Jim Woods, and he’s an author, editor, writing coach, and storyteller. Jim understands productivity and time management at a deep level. He also has a gift for seeing the weak spots in a story and knowing how to improve them. On this episode, you’ll learn lots of practical tips for getting your book finished. You’ll also learn why you should beware of story craft books, how to be more productive, and why it’s important to be a creator and not just a consumer. Key Takeaways 1.
