In this episode, host Dr. Jacob Fleming and Dr. Jason Cox interview musculoskeletal radiologist Dr. Jon Jacobson about the current uses and future role of musculoskeletal (MSK) ultrasound. --- CHECK OUT OUR SPONSOR Stryker Interventional Spine https://www.strykerivs.com --- SHOW NOTES To begin, Dr. Jacobson provides insight into his professional journey, from training at Henry Ford Hospital to his current practice at Lenox Hill in New York City. He reflects on the formative period of his career spent under the mentorship of Dr. Marnix Van Holsbeeck, who played a key role in his training over MSK ultrasound. Dr. Jacobson also discusses his specific areas of interest, in particular, microvascular imaging, especially in the context of assessing blood flow following the treatment of rheumatoid arthritis. He underscores the need for further research and exploration on this topic. Next, Dr. Jacobson explores the significance of MSK ultrasound education across various specialties, emphasizing the mutual learning that takes place across different specialties. Dr. Jacobson has led an annual MSK ultrasound course in San Diego for nearly two decades. He believes that MSK ultrasound has the potential to be scaled up to a level comparable to MSK imaging with CT and MRI. He emphasizes the important role that a skilled technologist plays in this process. He advocates for investing in training efficient and experienced technologists. This includes designating a lead technologist with expertise who can train other newer technologists. Dr. Jacobson further addresses ultrasound-guided interventions such as joint, bursa, and tendon sheath injections. The conversation also delves into the different paradigms of interventional radiology, which can sometimes be viewed as the provision of a clinical service versus the performance of order-based procedures. Finally, Dr. Jacobson elaborates on his collaboration with a nonprofit organization called Imaging the World. Their primary mission centers around ultrasound in Uganda, with a particular emphasis on advancing shoulder ultrasound capabilities in remote and underserved areas, both in terms of diagnosis and treatment. Dr. Jacobson provides training to physical therapists and technologists in ultrasound procedures and ultrasound-guided injections. The next phase of these nonprofit's efforts is directed towards underserved rural communities in North America. --- RESOURCES Musculoskeletal Ultrasound Education https://www.jacobsonmskus.com/
In this episode, host Dr. Jason Cox interviews Dr. Marnix van Holsbeeck, who is considered to be one of the godfathers of musculoskeletal ultrasound. --- SHOW NOTES Dr. Holsbeeck begins by reflecting on his early life in Belgium. Raised in a family of medical professionals, he started his career as a general practitioner at the age of 24, all while pursuing a comprehensive radiology training. His introduction to musculoskeletal ultrasound occurred during his tenure at his father's practice, where he discovered not only his affinity for radiology but also his deep passion for patient interaction. He soon realized that conducting ultrasound was the perfect intersection of both of his passions for radiology and patient care. He then provides a history of the origins of musculoskeletal ultrasound, dating back to its inception in 1979-1980. He shares his experiences from his time in gastroenterology, where ultrasound was beginning to be utilized for diagnosing gallbladder stones. It was during this time that he became captivated by the ability to correlate a patient's medical history with the findings of the ultrasound. Dr. Holsbeeck also elaborates on his journey on musculoskeletal ultrasound, recounting the numerous single-contrast arthrograms he conducted in conjunction with ultrasound during his training. As a practicing general practitioner, he recollects performing therapeutic joint injections and he recognized the need for image-guided procedures, specifically ultrasound-guided therapeutic joint injections. This approach offered superior precision and visualization of the joint space, further enhancing patient care. He also shares the development of the Musculoskeletal Ultrasound program at Henry Ford in Detroit, Michigan, upon his arrival in 1989. He initially began using ultrasound in the Emergency Department with regards to abscess aspiration and drainage. This approach was subsequently integrated into the field of orthopedics. Dr. Holsbeeck highlights the pivotal role of ultrasound in the diagnosis of tendon tears, underscoring its capacity to promptly assess whether surgical intervention is warranted for the patient. --- RESOURCES Musculoskeletal Ultrasound Society: https://www.musoc.com/

With the successful integration of Functional Neurology, Chiropractic, Naturopathy, Nutrition using the Asyra, Detoxification Programs, and LumoMed inner ear therapy. His Musculoskeletal Ultrasound training includes over 100 hours through the Gulf Coast Ultrasound Institute, 60 hours through AAOM, 90 hours through AOAPRM, and 60 through TOBI. He has been an assistant instructor for hands on practicum for diagnosis using musculoskeletal ultrasound for the 3rd Annual Platelet Rich Plasma & Regenerative Medicine Symposium in Los Angeles, California in 2015, and was a speaker at the Florida Chiropractic Physicians Association (FCPA) in Orlando, Florida in 2016 on Clinical Applications of Musculoskeletal Ultrasound. In this episode, Dr. Lieurance talks about going past the hormetic zone by putting stress on the body. We talk about the importance of brain health and why you should never wake up sluggish in the morning. If you need caffeine to get through the day, then something is going on with your adrenals. Dr. Lieurance reveals what tools he would use to start to repair the body and ditch your morning caffeine impulses. Tune in as Dr. Lieurance gives us the scoop on the importance of melatonin, ketones for the brain, and red light therapy. [01:20] Going Past The Hormetic Zone By Putting Good Stress On The Body We have a zone of familiarity, where it's like the status quo. We wake up and do the same thing, and the body becomes accustomed to familiar signaling. Whether it's your walk to work or the exercise you're doing at the gym, these things are going to signal certain genes in our body to buffer this stressor. When we have a good expression of those genes, we have the ability to buffer more stress. If you stress your body a little more than it is used to, your body will have to use extra strength. This becomes way more important for the brain. You can't strengthen the brain like you can muscles. [07:10] Waking Up Sluggish Is Not Normal – Dr. Lieurance Explains Why Cortisol is the polar opposite of melatonin. If you want to learn more about melatonin, get the book Melatonin: The Miracle Molecule: https://www.ultimatecellularreset.com/melatonin-miracle-molecule-book/ When you sleep, your body goes into this calm, parasympathetic, resting, digesting, and repair phase. Cortisol is supposed to be highest in the morning. However, people get up in the morning and are feeling sluggish. Lieurance says you shouldn't rely on caffeine to help you get going in the morning. If you're not tired when you wake up, then you have healthy adrenals, you got deep sleep, and your body is restored. [11:20] Are You Over Consuming Caffeine? Here are Some Recommendations From Dr. Lieurance The overconsumption of caffeine is going to affect your sleep. You need that deep restorative sleep to wake up and feel refreshed. Lieurance says DHEA can help people support their adrenals. B vitamins will also be critical to support your adrenals. Plus, Dr. Lieurance recommends trying MethylMax™. It is a revolutionary product containing the most advanced methylators combined in one suppository product. You can find MethylMax™ here: https://www.mitozen.com/ketokamp/?v=7516fd43adaa [19:30] A Melatonin Suppository Will Help Combat High-Stress Levels We naturally decline our production of melatonin as we get older. During sleep, our body is repairing. Depression, anxiety, schizophrenia, and other conditions have been shown to be improved in clinical research by supplementing melatonin. Also, there are countless studies about how melatonin will have a positive effect when it comes to battling viruses; it has a positive impact on survivability. Text me +1 (786) 364-5002 [01:37] The Way You Are Viewing The World Through Perspective And Energy The first time you ever see a propeller moving, you might think that it was a disc versus something that is actually moving. It has to do with the way that you're viewing the world and your perspective. If you could adjust the rate of vibration in which your eye perceives someone or yourself in the mirror, you'd see a running mass of electrons, protons, neutrons, and light; you wouldn't see a body. When you realize this is an energy system through which you create, you start to realize that you create according to the content of this energy system. And this energy system that we call, or at least one aspect of it that we call a body-mind unit, is literally a multi-generational database that holds everything that's going on in our whole bloodline. There are energies that support this life force and energies that will destroy us. [12:45] The Definition of Life: Love Is Flowing Through Our Cells Life is love flowing through a cell. Anything we do to restrict that, physically, mentally, emotionally, is going to create disease processes. In the ancient Aramaic, where there was a disease process going on, there was a solution to that disease process, and the solution was forgiveness. Now, you'll notice that the Greeks have taught us that forgiveness is this thing that is really nice for me to do because of course, it's all your fault. That is a total fraud. Letting someone “off the hook” will not resolve your pain and trauma. [20:36] How Still Point Relieves Mental And Emotional Stress When people are in disturbance or upset, the first thing they do is shut down their breath. When you shut down your breath, you literally lock down the mind energy that's trying to surface that you don't want to look at, you don't want to deal with. We lock those untoward energies into our unconscious someplace we don't have access to, but they're still there. Life is going to come along and resonate those things and move them and create the drama and the trauma and the pain that we experience. So, the veil has to open, the barrier between the subconscious and the unconscious has to open. How do you open it? You open it with the breath. In that still point process, we opened and moved into a space of bringing love into a present active state. The still point is a place where the breath as opposed to being held spontaneously stops. That's when the veil is wide open. Then you can process whatever your vitality has the strength to process; you can move out of your structure and be done with it. [29:37] The Importance of Forgiveness On Your Journey Forgiveness has nothing to do with me "letting you off the hook" because there's disintegrative energy because, in the Aramaic sense, there's sin in me; there's something that doesn't belong there. It loads you up and creates all kinds of disease processes. You can get all kinds of medical help, which will support and prolong the life of the cell, but if you don't deal with the original offending energy that's in the cell, real health will not be available to you. If you don't have the brain cells for the truth of what's happening in your life, you're going to create a perceptual structure, and you think you see what's going on. But the truth is, you're not seeing anything at all. People must work to build these brain cells to be able to work through what is happening. [35:21] One Basic Principle Around Fasting And Spiritual Practices One basic principle around fasting is that the part of the mind where the ego is seated, where the false self, the self that’s based in errant power, is fed directly from the stomach. When you fast, it gets no nutrition, and it starts to collapse. This is where the wider state of being can open and come into expression. [37:05] How To Get The Love To Flow With Our Breath All you have to do is breathe fully to start to open the gateway for that flood of love to come back. The still point process itself is one that when you engage in it and start moving things, it can be pretty intense, deeply profound. So it is something that Dr. Ryce only advises to be taught in person. You are meant to breathe fully, but if you've lived in dissociation, of shutting the breath down to dissociate from the content in your own mind, when you open the floodgates, everything in there starts to move. You go into what is called a healing crisis. This is where you've hit a new level of vitality; you get to start to process what needs to be processed. [39:53] Hate As A Disease And The Healing Process The opening words of The Book of John says, “In the beginning was the mind energy and the mind energy became flesh.” When we think a thought, it becomes a neuropeptide; the neuropeptide shows up in your structure, circulates around your structure until it finds a cell with a receptor site that matches, and it lands on that cell. It then inserts itself into the cell. When we realize that this is a process, then we see the neuropeptide of hatred as a disease process is in our cells. This means the healing process is going to look like having to deal with a lot of hate; it's going to be a reversal of the energy flow. As the hatred energy starts to come back out, the state is exactly the same. The symptoms for a disease process and a healing process are exactly the same. [51:31] How You Can Live By Dying If you go back to Yeshua, he says in order for you to live, you've got to die. There's you, the true being, the love that you are, that's designed to incarnate and live in this body. Then there's this false image of self that we fall into identification with; that's the self that has to die. It has to be taken apart. We've got generations of conversations about bodies and hate, fear and rage and all that, and it's all a lie. None of its true about who we are. When we wake up to the truth of who we are, then the work of dissolving who we are not begins. When people have a taste of that pure presence of love, it is so awesome. AND MUCH MORE! Today, I am blessed to have a THIRD conversation with Dr. John A. Lieurance. He is a Chiropractic Doctor, Registered Medical Assistant, Musculoskeletal Ultrasound Expert, and has practiced in Sarasota for 24 years. Dr. Lieurance has a gift for difficult cases where other practitioners have failed. With the successful integration of Functional Neurology, Chiropractic, Naturopathy, Nutrition using the Asyra, Detoxification Programs and LumoMed inner ear therapy. His Musculoskeletal Ultrasound training includes over 100 hours through the Gulf Coast Ultrasound Institute, 60 hours through AAOM, 90 hours through AOAPRM, and 60 through TOBI. He also has extensive training for sterile lab procedures and the processing of blood platelets, bone marrow aspirate, and adipose tissue through Oregen Biologics, Emcyte Corporation, Ron Gardener, M.D. (orthopedic surgeon), Regenestem and the Ageless Regenerative Institute. He has completed training with Dr Richie Shoemaker in 2019 for diagnosis & treatment of CIRS and with the “Shoemaker Protocol”. His techniques include Continuing Chiropractic Education: Chiropractic Neurology, – Applied Kinesiology “AK”, Cold Laser Therapy, Extremity Adjusting, Sports Injury Taping, Rehabilitation of Sports Injuries, Lumbar and Cervical Disc Decompression, Pettibon Scoliosis through the Carrick Institute of Functional Neurology as well as Vestibular Rehabilitation & Movement Disorders through the Carrick Institute in 2012,’16, ’17, ’18. Dr. Lieurance is the developer of Functional Cranial Release and teaches, as well as certifies, these methods to doctors around the world. In this episode, Dr. Lieurance opens the show by talking about my recent visit to his clinic. First, we dive into what we need to understand about back pain. When someone has back pain, the pain isn't the problem. Instead, it's what is causing the pain is the problem. If you're suffering from pain, then it's critical to get down to the root cause. That way, you can start getting proper treatment. We chat about TRT treatment, AVACEN, and PRP injections for back pain. Plus, we talk about the importance of fasting and why you want to consider taking a NAD supplement. https://www.mitozen.com/ketokamp/ / / E P I S O D E S P ON S O R S PureForm Omega Plant Based Oils (Best Alternative to Fish Oil): http://www.purelifescience.com Use ben4 for $4.00 off. [01:50] Understanding Back Pain Anybody that has back pain, their problem isn't the pain. Instead, the problem is what's causing the pain. Chiropractors are taught to look at structural aspects of the spine. When you make an adjustment on a patient, you're actually improving the mechanical movement of the spine. When people have back pain, they’re convinced something is wrong with a disc. However, it may not be a disc at all. That’s why Dr. Lieurance likes prolotherapy. It’s a procedure where a natural irritant is injected into the soft tissue of an injured joint. Temporarily, you'll have relief from your pain. If the pain goes away, then the person needs to do a series of injections to heal the core of the problem. [07:50] Healing The Body With TRT Treatment TRT (tissue regeneration technology) can help heal your body. The depth of penetration of this machine can treat hips and lower backs; it is really exceptional. TRT will stimulate stem cells and improve circulation. Dr. Lieurance will use that on a lot of patients with neuropathy. Any area where there's damage in the body, we can use TRT treatment to enhance significant healing in the body. [10:10] How The AVACEN Works The AVACEN works on the sympathetic, parasympathetic nervous system. There's an incredible amount of representation of the hand in the brain. So, giving stimulus to the hand can have very powerful effects on the autonomics. Your autonomics are really the area of your nervous system that are taking the stress when you are in sympathetic. Basically, the AVACEN will switch people more into the resting and digesting mode. [13:10] Getting PRP Injections The PRP injections are done around the ligaments. The PRP goes in as a liquid, and there's a little bit of an anesthetic in there. Very shortly after, it turns into a solid, like gelatin. Basically, it's grafting where the damage is in your back. The process is quick, but it is not enjoyable. PRP shots will help remove some of that fatty tissue in the back. These fatty tissues will develop in the back when those muscles aren't getting activated enough. [20:20] The Feast-Famine Cycle Calorie intake is what dictates this feast-famine signaling. You're signaling your body to grow and repair, or you're signaling your body to clean and recycle. We have access to unlimited food, all season, all day, all night, and we take advantage of that. When you're feeding, you're inhibiting your body to actually clean and repair itself. So, we want to balance that out. That is why intermittent-fasting and fasting have become so popular. People are finding that fasting is mitigating a lot of diseases; they're literally able to reverse diabetes. Plus, fasting can improve cancer, autoimmune diseases, degenerative neurologic diseases, and all sorts of things. If you look at a lot of diseases, it’s happening because there is too much growth. Overall, balance is the answer. [34:15] About NAD (Nicotinamide Adenine Dinucleotide) NAD is a substance that the mitochondria use to make energy, and it becomes depleted very quickly. When you provide NAD you're giving a lot of energy to the mitochondria. Sadly, every twenty years, you get a 50% decline in your energy levels. If you're not sleeping, you're drinking alcohol, or you have chronic stress, then you are greatly depleting your NAD. Lieurance created NAD+Max. Each suppository contains 500mg of NAD+, 250mg of NR (Nicotinamide Riboside), and 250mg of NMN (Nicotinamide Mononucleotide). You can find it here: https://www.mitozen.com/product/nadmax/?v=7516fd43adaa. [38:45] How The Microbiome Will Benefit From Fasting Fasting is a stressor to the microbiome. You have all these critters in your gut. When you take away food, then you are taking away its nutrients. So, it will be doing the same thing for them that it is doing for you – clearing the weaker bacteria. Once you start to refeed, that's when the microbiome can actually come back stronger. You will have a more robust gut, and you will have stronger bacteria. [43:35] What To Do After The Fast After the fast, you want to signal the body to go and do repairs. For the most part, we want to signal mTOR after a fast, and the best way to do that is with protein. Lieurance is a big fan of essential amino acids as a supplement. If you really want to support your microbiome in its repair and regeneration, then take high-dose melatonin. AND MUCH MORE! Lieurance opens the show describing the advantages of a functional cranial release. He then explains how our body likes to make energy and the benefits of melatonin on the process. He dives into the amount of melatonin we should be taking, how we should take it, and how we can naturally produce more melatonin. The force is expansive, and it takes the pressure off of the bone called the sphenoid bone, which is in the center of your head. Information around cranial work needs to get out there. It’s a little known technique that people need to hear about. People with chronic headaches or balance disorders can significantly benefit from a functional cranial release. [05:15] Understanding Melatonin People can take higher doses of melatonin for inflammation. Melatonin also works on the mitochondria; it’s an essential structure for every cell in our body. You don’t want to burn your cell out. Things are going in and out of the cell membrane. You want it to be as clean as possible. The mitochondria make its own melatonin. It does it for a significant reason – when things get too hot, melatonin will cool it done. You get oxidation from the burning of energy; melatonin will quench the oxidation. Oral absorption of melatonin is low – only about two percent is absorbed into the body. The suppository is a direct route into the bloodstream. Check out Dr. Lieurance’s product SandMan Sol™. It contains 250mg of Glutathione, 200mg of Melatonin, Terpene Blend (Limonene, boswellia), and their proprietary AP0-808™ probiotic strain. The body’s two most potent antioxidants, Glutathione, and Melatonin together! [12:30] How Cancer Works Cancer happens when the cell no longer can make energy efficiently, and it goes to a primitive way of creating energy. Instead of making 36 ATPS, you only make 4. Because of inflammation, you make less energy. The Cytokine Storm is what kills people. Melatonin can penetrate the cell and quench the cycle so you can start making energy effectively. [19:50] You Won’t Shut Down Your Ability To Make Melatonin There is an idea that too much melatonin is somehow a danger. However, there is no negative feedback. When you take testosterone, you’ll make less of it. With melatonin, this doesn’t happen. You can take high doses, quit, and the very next night, you’ll produce the same amount of melatonin. [21:10] Other Cool Things About Melatonin Melatonin is critical for disease prevention and slowing down aging. When don’t have a lot of energy, it’s common for people not to clean their house. Whenever everything is clean, everything is more effective. The same thing happens in your body. When you don’t have energy, your body will start doing what is most important and go into survival mode. [25:25] How To Increase Melatonin In The Body Getting sunlight first thing in the morning will help produce melatonin in your body. People wear sunglasses too much. Be smart about it. Don’t go out on the paddleboard all day long and burn your eyes. However, it would help if you had sunlight throughout the day. Before 11 am, and after 4 pm, you should be able to navigate without sunglasses. The green and blue lights will suppress melatonin. People are watching TV, on their cell phone and their iPad. It’s keeping you up at night. Buy red rope lights to put around your house. The lights on the wall switches should not be used at night. Buy blue-blocking glasses and turn on the night shift for your phone. AND MUCH MORE! Dr. Lieurance is the developer of Functional Cranial Release and teaches, as well as certifies, these methods to doctors around the world. In this episode, Dr. Lieurance opens the show revealing his past health problems with mold and Lyme disease. After healing himself, Dr. Lieurance was able to reopen his practice and help his clients heal and detox. Dr. Lieurance reveals his biohacking tricks like TRT, intravenous laser therapy, magnesium, glutathione IV pushes, and CVAC. Plus, Dr. Lieurance explains why he is big on suppositories and nasal sprays. Be sure to check out all of his products and use the code “ketokamp” https://advancedbiomecorp.com/. // E P I S O D E S P ON S O R S PureForm Omega Plant Based Oils (Best Alternative to Fish Oil): http://www.purelifescience.com Use ben4 for $4.00 off. http://www.Kettleandfire.com/ketokamp and use KETOKAMP at checkout to save 15% on your entire order. [03:10] About Dr. Lieurance Lieurance had a lot of health problems growing up. Eventually, Dr. Lieurance was diagnosed with Lyme disease. At one point, he had to shut down his practice and lost it all. After healing himself, Dr. Lieurance was able to reopen his practice. Now, Dr. Lieurance is able to tell his patients that he knows exactly how they feel when they come to him with Lyme disease. [08:00] How Dr. Lieurance Helps His Clients Biohack TRT is true shockwave therapy. This therapy is becoming more and more well known. The TRT device penetrates eight inches deep into the body and produces a true shockwave. The healing benefits are like nothing you have ever seen before. Lieurance will also set up a high-dose magnesium IV. Magnesium is very alkaline. By alkalizing the blood, you will get better absorption with an oxygen product. High magnesium has been researched for traumatic brain injuries. The magnesium will help light our brains up. Another biohack Dr. Lieurance uses is intravenous laser therapy. This way, your blood will get showered with the laser; it is hugely favorable. Laser therapy is harnessing the beneficial aspects of sunlight. The lasers boot our immune system. 70% of the laser power is absorbed in the first millimeter of skin. [17:10] About The Lymphatic System The lymphatic system is the gutter system. The lymphatics go everywhere! Nutrients will pass into tissues. Those tissues create energy, and the metabolic waste gets shuffled through the lymphatic system. Eventually, the waste goes through urination or defecation. [18:15] Utilizing a Glutathione IV Push Glutathione is the master antioxidant. It’s great for catching toxins. Glutathione is an amazing molecule; it’s something that our liver produces in a healthy situation. Glutathione is the primary support for our viral immunity. Plus, it’s essential for good quality sleep. Lieurance says he prefers getting people on glutathione suppositories. [15:30] Why Dr. Lieurance Is Big On Suppositories It’s a simple process to enter a suppository. Once the suppository enters the rectum, it dissolves. You don’t even know it is there. The nutrient will slowly release in your body. When you take things orally, you will have a breakdown of enzymes in the liver. [33:30] About The CVAC System CVAC stands for cyclic variations in adaptive conditioning. There are only 65 of these machines in the world. Lieurance says it’s something people can easily get in and out of. The science says a CVAC machine is more powerful than hyperbaric treatments. There is something in the brain called the glymphatic system. The CVAC will activate the glymphatic system. Basically, it helps your brain take out the trash. CVAC will also stimulate stem cells. [36:15] Using ZEN Nasal Spray™ To Calm The Mind ZEN Nasal Spray™ is made from different medicinal plants from the Amazon for the purposes – to calm and quiet the mind and to reduce anxiety and stress. The blend of herbs is magical. The ZEN has a burn to it; most people tolerate it really well. People who aren’t meditators can use it prior to meditating because it is so calming. [42:35] Why Nasal Sprays? The absorption orally can be deterred through stomach acid. When you do things through the nasal, you can easily address upper respiratory issues. Whereas, if you took it orally, it would go into your bloodstream. When you’re ready to start traveling, you’ll need to check out the Travel Hacker Kit. ABC has discovered a method for travelers to fly without being insulted at the cellular level on the plane by EMF and poor air quality with both a special nasal spray as well as NeuroDiol™ 125mg & 300mg. [47:25] The Hidden Benefits of Melatonin Melatonin suppresses cancer growth. Plus, melatonin will have a profound effect on protecting mitochondria. Also, it helps fight against stress in general. Melatonin has the ability to protect during infections. AND MUCH MORE! Dr. Cook’s early career as an anesthesiologist and medical director of an outpatient surgery center that specializes in sports medicine and orthopedic procedures provided invaluable training in the skills that are needed to become a leader in the emerging fields of Musculoskeletal Ultrasound imaging, nerve hydrodissection, and stem cell medicine. During this Part 2 discussion, you'll discover: -Dr. Cook's Opinion on the use of beta blockers in preparing for performance...9:30 Gave a talk to over 2,000 doctors early in his career Was given a beta blocker by an associate; Heart rate went down to 25 bpm. Never taken another beta blocker since. Sweet spot for everything. Biohacking too high or too low is problematic Has used beta blockers with anesthesia to calm the heart rate. -Plasmapheresis, what Dr. Cook's considers to be one of the "next big things" in the medical industry...11:58 Plasmapheresis A method of removing blood plasma from the body by withdrawing blood, separating it into plasma and cells, and transfusing the cells back into the bloodstream. It is performed especially to remove antibodies in treating autoimmune conditions. It has also been studied as a treatment for neurocognitive and Neuro degenerative problems like Alzheimer’s and Parkinson’s disease. It’s kind of like an oil change in your vehicle. If there is systemic inflammation, often that inflammation is in the plasma of the blood. Different from parabiosis Same procedure as Dr. Mark Urdahl and I discussed on the podcast a few months ago. (Listen to the episode .) Most people who seek plasmapheresis have a serious conditions like Lyme, dementia, Parkinsons, etc. Effect on a healthy person: No negative effect. Nothing wrong with a thorough examination Length of the procedure: 2-3 hours, 1x/week for 6 weeks. -Dr. Cook's thoughts on the use of cannabinoids in his practice...18:50 We have cannabinoid receptors in our brains. Part of our nervous system is called the endocannabinoid system. Expect a surge of use in the next 10 years among health optimizers and chronically ill people. "Entourage effect" Unicorn: a company that is purifying cannabinoids and get all the solvents out Delta 8 THC vs. Delta 9. Rick Simpson Oil: highly potent What are terpines? is one of the smartest, most cutting-edge physicians I've ever met. In one single dinner in San Francisco with him, I learned... ...how he's using the nootropic methylene blue for solving a difficult medical case... ...how he's upgrading stem cells to make them far more potent... ...how he's treating depression effectively in as little as 20 minutes with a special brew that includes ketamine, NAD and vitamin IV's... ...how he's reversing musculoskeletal disorders using something called "hydrodissection"... ...and much, much more... Dr. Cook founded BioReset Medical Corporation and as acting President, operates a Regenerative Medicine and Pain Medicine practice that offers leading edge non-surgical solutions in orthopedic medicine, sports medicine, regenerative pain medicine, and stem cell medicine. He is a board-certified anesthesiologist with over 20 years of experience in medical practice. Currently Dr. Cook is President of California Anesthesia, Anesthesiologist, Interventional Pain management, and Regenerative Medicine and Medical Director of the National Surgery Center, Los Gatos, CA. In addition, he sits on the scientific advisory board of several high profile medical companies including BM DOC, FREMEDICA & VASPER SYSTEMS. During our discussion, you'll discover: -What Dr. Cook did with methylene blue, and how he activated it with light...8:55 Met an expert who used it to treat interstitial cystitis, a condition that causes very painful bladder that is difficult to treat. Administered via IV, then uses blue LED light over it. There's a micro biome in the bladder. Blue light causes methylene to absorb into the bladder. Dislocates infections from the wall of the bladder. -Hydrodissection: What it is, and how it differs from prolotherapy...14:35 Sticking a needle under direct vision via ultrasound into a tissue plain in-between two muscles. Operating in a fascial plain: arteries, veins, tendons. Previous procedures involving dissection would cause scarring, sometimes leaving patients in worse shape than when they started. Pressure of fluid separating the tissue plain reduces scarring. How is it different from prolotherapy: Prolotherapy treats ligamentous laxity (joints) Hydrodissection treats the nerves and arteries that go into the joints. Treatments are done in tandem. Combination of ultrasound and x-ray is called fusion. Dr. Cook used this therapy with the Washington Nationals baseball team. -How to think of ground force going through the body...23:45 You must have structural integrity in the nerves. As force (from punching a punching bag for example) moves through the body without structural integrity, you get torque leaking out. Puts stress, wear and tear on the joint. This "leakage" compounded over time causes permanent damage to joints, ligaments, etc. Focus of treatment is to relieve this stress, or leakage, and create high quality movement to create structural integrity and prevent recurrence of the problem. - Dr. Cook's approach to stem cells, and why...28:10 Assess the patient, then tailor an appropriate treatment for the specific problem. Joints, spine, IV treatments Terms to know re: stem cells: Totipotent: capable of giving rise to any cell type or (of a blastomere) a complete embryo. Pluripotent: capable of giving rise to several different cell types. Multipotent: develop into more than one cell type, but are more limited than pluripotent cells. Autologous: obtained from the same individual Mesenchymal: An adult "professional" stem cell from a variety of ways. . Whether adult stem cells can retain their healing properties without repairing tissue. No concrete answer yet. Differing approaches, regulatory apparatus is affecting the ability to answer V Cells (very small embryonic stem cells) Autologous Using umbilical cord blood and Wharton's jelly. You can donate placenta and umbilical cord blood. Wharton's jelly is the tissue that comes off the wall of the umbilical cord. Wharton's jelly, UC blood and amniotic fluid is blended together to make "matrix" of the placenta. Anti-infectious, anti-inflammatory "Nature gives us a lot of clues..." Not that many stem cells in the UC blood. PRP (platelet rich plasma) PRP lysate -Why exosomes may be the most important treatment in the future...55:25 Take stem cells and take to a lab, grow them in tissue culture; immortal cell line. Put stem cells on a ketogenic diet. Why do people still need to go overseas for treatment? "More than Minimum Manipulation": a term created by the regulatory apparatus on stem cell therapy. Must be done in Mexico; can't make there and bring across the border. Culture expanded Stem Cell Culture expanded Natural Killer Cell Therapy - Dr. Cook's unique approach to treating depression and managing pain...1:05:15 Ketamine therapy Turns off a pathway in the brain; turns on receptors (think turning off and on your computer) Makes people feel emotionally safe; gives hope. Your mind leaves "bread crumbs" of this feeling of hope; you remember it the next day, and the next... Creates a parasympathetic state -And MUCH more... SHOW NOTES After having such a great response to Mike’s last interview (Session 27) I thought we should have one specifically on Ultrasound diagnosis and needle guidance for the knee. Mike goes into what knee conditions can be diagnosed beyond reasonable doubt by using MSUS. He also goes into how certain he can be your injection will be in the right location by using Ultrasound Guidance. As I spoke about on the episode here’s my article on Musculoskeletal Ultrasound https://www.p2sportscare.com/musculoskeletal-ultrasound/ Here’s a link to the video Mike and I put together showing how Ultrasound can assist in diagnosis of your knee condition. SHOW NOTES If you have spoken with me personally about clinical topics over the last 4 years you would know I love Musculoskeletal Ultrasound (MSK or MSUS). In fact, I bought one for my clinic and unlike many doctors who would buy it to have someone else take the images, I actually learned how to do it. Why? Because it was so cool! I could actually see exactly what I was treating or I would see what cases I needed to refer out. I was and still am blown away by how detailed MSK is. Mike was the guy who taught me. He recommended I start with the Jon Jacobson Book on Ultrasound. You can find that here http://amzn.to/2moiQEq MSK is a very underused but very accurate method of imaging injuries. Do you have a shoulder, knee, ankle, hip or groin injury? Can’t get access to an MRI? Desperate to see what’s wrong? Ultrasound can see a rotator cuff tear, meniscal tear, or a tendonitis within a minute or 2. Learn all about it here. Here’s is the extensive article I wrote on Musculoskeletal Ultrasound through this link https://www.p2sportscare.com/musculoskeletal-ultrasound/ Contact Mike Jablon at http://orthopedicultrasound.net/index.html Visit our site for references on this session http://www.P2SportsCare.com.
Therapy Insiders Podcast -->>Physical therapy, business and leaders
What is musculoskeletal ultrasound? How can it help physical therapists better diagnose and treat patients? Find out from Dr. Andrew Ball on this episode of Therapy Insiders podcast.
Ultrasound is rapidly revolutionising point-of-care medicine in many specialties. The last few years have seen technological advances make musculoskeletal ultrasound more practical, opening up opportunities for diagnostic and therapeutic interventions. In this podcast, BJSM's editor Karim Khan investigates the value of musculoskeletal ultrasound and how to integrate it into practice, with Kim Harmon, director of the Sports Medicine Fellowship at the University of Washington, US, and Sean Martin, Clinical Faculty at Eglin Airforce Base, US. They discuss their experiences of the technique in clinical practice (0.44), which procedures it's useful for (4.22), how to get started (12.03), using musculoskeletal ultrasound in the training room (13.33), the value of collaborating with radiologists and orthopaedics (15.58) and credentialing (17.46).
