He reflects on the formative period of his career spent under the mentorship of Dr. Marnix Van Holsbeeck, who played a key role in his training over MSK ultrasound. Dr. Jacobson also discusses his specific areas of interest, in particular, microvascular imaging, especially in the context of assessing blood flow following the treatment of rheumatoid arthritis. He underscores the need for further research and exploration on this topic. Next, Dr. Jacobson explores the significance of MSK ultrasound education across various specialties, emphasizing the mutual learning that takes place across different specialties. Dr. Jacobson has led an annual MSK ultrasound course in San Diego for nearly two decades. He believes that MSK ultrasound has the potential to be scaled up to a level comparable to MSK imaging with CT and MRI. He emphasizes the important role that a skilled technologist plays in this process. He advocates for investing in training efficient and experienced technologists. This includes designating a lead technologist with expertise who can train other newer technologists. Dr. Jacobson further addresses ultrasound-guided interventions such as joint, bursa, and tendon sheath injections. The conversation also delves into the different paradigms of interventional radiology, which can sometimes be viewed as the provision of a clinical service versus the performance of order-based procedures. Finally, Dr. Jacobson elaborates on his collaboration with a nonprofit organization called Imaging the World. Their primary mission centers around ultrasound in Uganda, with a particular emphasis on advancing shoulder ultrasound capabilities in remote and underserved areas, both in terms of diagnosis and treatment. In this episode, host Dr. Jason Cox interviews Dr. Marnix van Holsbeeck, who is considered to be one of the godfathers of musculoskeletal ultrasound. --- SHOW NOTES Dr. Holsbeeck begins by reflecting on his early life in Belgium. Raised in a family of medical professionals, he started his career as a general practitioner at the age of 24, all while pursuing a comprehensive radiology training. His introduction to musculoskeletal ultrasound occurred during his tenure at his father's practice, where he discovered not only his affinity for radiology but also his deep passion for patient interaction. He soon realized that conducting ultrasound was the perfect intersection of both of his passions for radiology and patient care. He then provides a history of the origins of musculoskeletal ultrasound, dating back to its inception in 1979-1980. He shares his experiences from his time in gastroenterology, where ultrasound was beginning to be utilized for diagnosing gallbladder stones. It was during this time that he became captivated by the ability to correlate a patient's medical history with the findings of the ultrasound. Dr. Holsbeeck also elaborates on his journey on musculoskeletal ultrasound, recounting the numerous single-contrast arthrograms he conducted in conjunction with ultrasound during his training. As a practicing general practitioner, he recollects performing therapeutic joint injections and he recognized the need for image-guided procedures, specifically ultrasound-guided therapeutic joint injections. This approach offered superior precision and visualization of the joint space, further enhancing patient care. He also shares the development of the Musculoskeletal Ultrasound program at Henry Ford in Detroit, Michigan, upon his arrival in 1989. He initially began using ultrasound in the Emergency Department with regards to abscess aspiration and drainage. This approach was subsequently integrated into the field of orthopedics. Long before the train cars crashed on Feb. 3 and residents' lives were turned completely upside down, railroad workers had been warning anyone who would listen that the relentless cost-cutting and profit-maximizing practices implemented on the freight rail system by greedy rail executives and their Wall Street shareholders would endanger workers and the public, and they were right. In recent hearings held in East Palestine by the National Travel Safety Board (NTSB), testimonies by representatives from rail labor and even Norfolk Southern itself have revealed what workers already knew: a combination of reckless business practices by the rail carriers, including yearly staff cuts to rail maintenance workers and the outsourcing of vital safety functions to unregulated machines, and lax federal regulation of the railroads has created the conditions for catastrophes like East Palestine to happen with greater frequency. But it is not too late to reverse course and build a safer, greener, more efficient, more humane rail system. During his interventional training at University of Missouri, the musculoskeletal radiology program was rebuilt, and ultrasound was incorporated heavily. He used his ultrasound skills from vascular intervention in IR to learn musculoskeletal anatomy on ultrasound. He was drawn to MSK radiology due to the mechanical aspect of MSK work and the integration of visual spatial awareness and hand eye coordination involved in MSK ultrasound. He started out by learning steroid injections for sports injuries, commonly rotator cuff injuries. He now does around 20 diagnostic or interventional ultrasound procedures each day in his clinic. He opened his clinic with a partner, and did it slowly while still working at his prior job. He started working at his new clinic on his vacation days until he could build up the clientele to leave his prior job. One of the biggest challenges in opening his MSK radiology clinic was finding a sonographer able to do the complex MSK cases he was doing. The most common procedure Dr. Cox does at his clinic is ultrasound guided carpal tunnel release. He also does tendon barbotage for hydroxyapatite deposition disease for the rotator cuff tendons. His practice has grown largely due to the number of patients that are referred because they cannot get an MRI. During his interventional training at University of Missouri, the musculoskeletal radiology program was rebuilt, and ultrasound was incorporated heavily. He used his ultrasound skills from vascular intervention in IR to learn musculoskeletal anatomy on ultrasound. He was drawn to MSK radiology due to the mechanical aspect of MSK work and the integration of visual spatial awareness and hand eye coordination involved in MSK ultrasound. He started out by learning steroid injections for sports injuries, commonly rotator cuff injuries. He now does around 20 diagnostic or interventional ultrasound procedures each day in his clinic. He opened his clinic with a partner, and did it slowly while still working at his prior job. He started working at his new clinic on his vacation days until he could build up the clientele to leave his prior job. One of the biggest challenges in opening his MSK radiology clinic was finding a sonographer able to do the complex MSK cases he was doing. The most common procedure Dr. Cox does at his clinic is ultrasound guided carpal tunnel release. He also does tendon barbotage for hydroxyapatite deposition disease for the rotator cuff tendons. His practice has grown largely due to the number of patients that are referred because they cannot get an MRI. Today on The Download from Sounds Profitable; ad tech firms are under fire for data scraping, Amazon joins the social audio market, YouTube is paying podcasters to incorporate video, and more. Morning Brew’s Ryan Barwick reports that trade groups acting on behalf of publishers from the US, UK, and Canada are accusing ad-tech firms of unfairly scraping metadata from websites Once collected, this allegedly ill-gotten data is used to create contextual advertising segments for clients without the publisher’s consent, also undercutting the publishers’ attempts to directly sell contextual advertising deals. “Now that third-party cookies are dying and some ad dollars are shifting to contextual advertising—ads based on the content of the media, not on personal information—publishers want a (better) seat at the table and stronger terms as the industry adopts new technologies.” Richard Reeves, managing director of the Association of Online Publishers, summarized the issue of companies scraping data. “What we are now seeing is people almost brazenly walking through your home, and removing your furniture, and selling your assets elsewhere. And you don’t even know that they’re doing it, or you can’t receive any value for it. Just because you can doesn’t mean to say you should.” Data scraping isn’t new to podcasting, either. Transcription happens in podcasting, with and without a publisher's consent. It’s likely however this wider publisher issue plays out will have trickle-down effects to what companies can do with unlicensed podcast transcripts. Amazon has a new social audio app to make podcasters' dreams of being a DJ come true. Brad Hill of Rain News reports: “While early reports compare Amp to Clubhouse, Amazon’s promotional emphasis is on building interactive music shows, something like live, interactive radio.” Amp brings to mind an obscure podcasting tool Spotify launched for Anchor in late 2020. The feature, titled Shows with Music at launch, allows podcasters on Anchor to slot any song in the Spotify catalog between any pre-recorded podcast segments. Listeners with Spotify Premium would experience a seamless transition as if the music was baked into the podcast, while free listeners would hear a thirty-second preview of the song. Shows with Music still exists, technically, but has fallen by the wayside to become a feature hidden in the Anchor interface. Much like the forgotten podcasts from big-name creators covered last week. Amp, conversely, only requires listeners to sign up for a free Amp account to listen to creators. The Verge’s Jack Kastrenakes writes: “Amazon is positioning this as more of a radio-style service than a live chat service (there’s even a five-person cap on callers right now), which is probably for the best.” Shows with Music was a cool feature that enabled podcasters to live out their radio DJ dreams in a copyright-friendly manner. Another platform with a massive collection of licensed content at their disposal playing in this podcasting-adjacent space might just lead to more creativity and innovation in the social audio sphere. On Tuesday, podcasting ad tech company Gumball announced they had raised ten million in Series A funding. Brad Hill of Rain News reports, “Gumball, which was started by podcast comedy network Headgum, allows advertisers to programatically buy pre-recorded host-read ads. The system offers real-time inventory browsing, demographic audience targeting, and verification of placement and listening. The company lists a few brands which have used the system — Casper, CBS, Netflix, OkCupid, Squarespace, Warby Parker, and others.” Naturally, the Gumball system is deployed across the entire Headgum network, serving ads on flagship podcasts like The Doughboys, Punch up the Jam, and We Hate Movies. YouTube is taking podcasting seriously enough to put their money where their mouth is. Last Friday Bloomberg’s Ashely Carman reported both independent podcasters and podcast networks, all of which asked for anonymity, received offers ranging from $50,000 to $300,000 to create filmed versions of their episodes, as well as “other kinds of videos.” Video podcasts on YouTube historically have done well. As the platform matured from cat clips to encouraging content creators to make longer and longer content, the unedited video chat show took off. Productions like The H3H3 Podcast and The Joe Rogan Experience saw great success embracing the platform. That said, as Carman said, “However, the cost to build a studio, hire editors, and develop a fully functioning video publishing pipeline can deter networks and shows from adopting the platform.” In addition to these cash injections implying YouTube wants to seed more of a professionally-produced podcasting atmosphere, there are small infrastructure moves to suggest this is a long-term plan. Alex Castro at The Verge reminds readers that back in October YouTube began allowing Canadian users to listen to videos while the device was not focusing on the YouTube app or was locked. As YouTube power users in the US can attest, this feature is pushed very hard in YouTube Premium advertising as a good reason to sign up. Without putting on a podcast-branded tin-foil hat, it seems like making the ability to use YouTube like a podcasting app would be a huge step towards YouTube courting more podcasters and their audiences. On Tuesday The Hollywood Reporter’s J. Clara Chan published an exclusive announcement that UTA has launched Audio IQ, a data analytics service to facilitate podcast deals. The service will make use of social media, search results, and other open-source data to inform both clients and agents of a podcasts’ health to facilitate dealmaking. “While podcast analytics can often rely on historical data points like number of downloads or past ad revenue, UTA’s Audio IQ analyses also offer future projections — a tool that is particularly key in negotiations for shows that have not yet launched or for identifying emerging talent.” Audio IQ comes onto the field as massive podcast outfits buy up previously third-party analytics companies - see our February 18th episode for coverage of the Spotify acquisition of Chartable - and that atmosphere has UTA IQ lead Joe Kessler concerned. As Kessler is quoted by Chan’s article: “I’m hopeful that this announcement serves as a wake-up call for the podcasting industry to somehow coalesce around a common source of truth and data for the industry, because it’s sorely needed as it’s maturing.” Finally, a smaller bit of news that’s not technically a full news story yet, but we feel is worth keeping an eye on as things develop. There was something about Tuesday this week that lead to podcasting announcements. Ashley Carman tweeted the exclusive scoop that three senior leaders at Megaphone are leaving. CEO Brendan Monaghan, CRO Matt Turk, and COO Jason Cox, all in their positions prior to the Spotify buyout, have been confirmed to be leaving the company after their one-year contracts expired. Without putting on a podcast-branded tin-foil hat, it seems like making the ability to use YouTube like a podcasting app would be a huge step towards YouTube courting more podcasters and their audiences. On Tuesday The Hollywood Reporter's J. Clara Chan published an exclusive announcement that UTA has launched Audio IQ, a data analytics service to facilitate podcast deals. The service will make use of social media, search results, and other open-source data to inform both clients and agents of a podcasts' health to facilitate dealmaking. “While podcast analytics can often rely on historical data points like number of downloads or past ad revenue, UTA's Audio IQ analyses also offer future projections — a tool that is particularly key in negotiations for shows that have not yet launched or for identifying emerging talent.” Audio IQ comes onto the field as massive podcast outfits buy up previously third-party analytics companies - see our February 18th episode for coverage of the Spotify acquisition of Chartable - and that atmosphere has UTA IQ lead Joe Kessler concerned. As Kessler is quoted by Chan's article: “I'm hopeful that this announcement serves as a wake-up call for the podcasting industry to somehow coalesce around a common source of truth and data for the industry, because it's sorely needed as it's maturing.” Finally, a smaller bit of news that's not technically a full news story yet, but we feel is worth keeping an eye on as things develop. There was something about Tuesday this week that lead to podcasting announcements. Ashley Carman tweeted the exclusive scoop that three senior leaders at Megaphone are leaving. CEO Brendan Monaghan, CRO Matt Turk, and COO Jason Cox, all in their positions prior to the Spotify buyout, have been confirmed to be leaving the company after their one-year contracts expired. The three are now starting a blockchain company. The Download is a production of Sounds Profitable. In a syndemic, what are our options for building relationships and maintaining our support network? We talk about the successes and challenges of peer recovery support while in the midst of concurrent epidemics.  Jason Cox is the Lead Certified Peer Recovery Specialist with Alive RVA and SAARA of Virginia. He has worked alongside others in the community such as DEA, Discovery Education, SAFE Opioid Prevention Taskforce, and many others to assist in taking positive action in the community and reduce the stigma of SUD one step at a time. Jason is living in long-term recovery and plans to continue gaining knowledge to become the most effective Peer Recovery Specialist possible. Jason Cox is the GM for this series!!!! The pitch: You used to be in the supers game as a black hat, but you never hit the big leagues and it was a long time ago. Maybe the power faded, maybe you took a trick arrow to the knee, or maybe you just got wise. Now you just want to meet your PO and blend in, but since when did you ever get what you wanted? Last session wasn't recorded. Here's the summary: Rigel Tellers (Dead Star) and Jeong Chung (Six-Star) both grew up here, left, and find themselves back again. In his youth Rigel was a gang-leader who had dated Jeong for a time before she stole a bunch of drugs from him and disappeared. He got caught in an industrial accident at Wilson Chemicals (which had a different name at the time), granting him super strength and the ability to absorb and expel tremendous energy. He became a supervillain for a while before giving himself up for unknown reasons. He studied and got his degree to be a CPA while in prison, and is now trying to go straight. Jessica Finch (Songbird) is a retired hero who acts as his patrol officer, and encourages him to do community service as a part of his rehabilitation. Jeong is a trickier case. She woke up a few months ago from a car crash on the edge of town without much of a memory. She had a PI license in her possession, and set up shop in her old home town to try and piece together what happened. She knows she was a super-thief for the Whisper Syndicate, and the clues seem to suggest they have some kind of interest in Shore City. She has come to Rigel for help and, like a chump, he has provided it. Sometimes they meet at the Hawaiian BBQ place under Rigel's apartment, Big Luau Burger. Jeong doesn't actually like it, but it's convenient. In the last episode, Rigel's buddy Evan asked to meet with him. Evan used to be in the gang as a kind of negotiator, and is now a supervisor for Wilson. He is also dating Thunderstrike (Europa McNiel), whom Rigel dislikes intensely. The three of them met at The Library, a bar near the factory, and Evan asked Rigel to try and talk to the workers on his behalf. Apparently they see him as a sellout. Across town the head of the Union, Jimmy Soon, hired Jeong because he thinks Wilson is cooking the books. Jeong went looking to talk to some of the employees to look for leads, and ended up coming into the bar right when Thunderstrike was leaving. She stole Thunderstrike's phone, but was unable to hack it so she later dumped it in Evan's car. Evan saw her, and got excited about the idea of Rigel and Jeong going on a double date with Europa and himself. Rigel faked a big fight with Jeong so that people wouldn't realize the were heroing on the sly together, and Jeong left. Rigel drank more than he should have. Later Rigel goes to Jeong's and they compare notes. He heads home and she decides to shadow Thunderstrike. Six Star is acrobatic as hell so finds her way on to the Wilson grounds pretty easily, and hears Thunderstrike talking to someone on the phone about how nobody is going to find out anything. The other person doesn't agree, so she calls Hurricane Harry to join her for guard duty. Rigel is about to sleep but the police scanner alerts him to the fact that some police never reported in from the college area. Since he is still drunk, he gets a cab to take him out there. The cabbie is excited to recognize the ex-villain, and tries to get him to tell why he turned himself in. When they get to the college though, the numerous cops laying inert on the ground convince him to go. Dead Star investigates and finds they have been hit by sleeping darts. He heads towards the superhero studies wing, and disturbs people in Dr. Faulkner's office. He hears a voice in his ear "No one spies on the Whisper Syndicate!" He fills the hallway with light and a horde of demonic ninja hiss and disappear in a cloud of smoke. He extinguishes the fire he finds in Dr. Faulkner's office by pulling the water pipes down, and pulls the good doctor himself out of the building Six Star is at Wilson Chemicals when Hurricane Harry arrives, and follows the capes to a secure back room. Well.... secure to most people. She employs her superhuman acumen and training to contort and worm her way through the ducts, and finds herself in a lab. Four figures float in tanks, one of whom looks disturbingly like Rigel. And that is where we left off.

Tim McMahon and Jason Cox are co-owners of Brightstone Transitions, a Young Adult Transitional Program. They met over a decade ago in Georgia when Tim was directing a state-funded wilderness therapy program, and Jason showed up late for a job interview dressed in a suit and tie. Eventually, they connected as they both believed in the value of the connection between field guides and students. They share why state-funded wilderness therapy camps closed in the South and how they went onto start Brightstone. Bios from Brightstone Transitions Website: Tim McMahon is the Founder and Owner of Brightstone Transitions. Tim has been working in the field of therapeutic programming for over 20 years. He has helped design and implement residential, therapeutic, and academic programs across the country. Tim is married with two children. When not at work, he enjoys spending time outdoors with his two daughters. While managing 10 intensive residential treatment programs located in 6 different states across the country, Tim began to realize that there was need for resources to help young adults who, despite therapeutic intervention, needed support to move forward. These young adults appeared stuck, and were often unable to make plans that allowed them to advance. These young adults needed guidance, but often did not respond to the traditional clinical interventions. It was out of Tim’s passion for helping these untraditional young adults that Brightstone Transitions was born. Tim has a hands-on approach to running Brightstone. Tim believes that the key to reaching these young adults is through personal relationships. With that in mind, Tim designed Brightstone to be a very small and very individualized program. Tim brings a wealth of experience and personal knowledge to Brightstone which he uses to help his clients and families build a foundation for success. He helps clients to define clear goals and develop plans to work toward those goals. Tim believes in an action oriented approach that moves clients beyond the classroom and into real life experience. Jason Cox is the Director of Business Development for Brightstone Transitions. He brings a wide variety of experiences and passions to the Brightstone team. Jason comes to Brightstone with years of management experience in both therapeutic programming and summer camp programs. Jason has a great deal of experience in designing and implementing programming to help young people. Jason started his career working with inner city youth in the Atlanta area in a wilderness based programming for over seven years. Helping to develop programing for boarding schools on the East Coast became his main focus before moving on to other programing areas. Through the years, Jason has had the opportunity to work with several camps and schools across the country. Jason accepted a job as the Director of Conferences, Retreats and Summer Programming for the Greater Seattle YMCA. Jason then moved to San Francisco to work with the Boys & Girls Club of San Francisco where he served as Assistant Camp Director of Programs and helped to connect Bay Area inner city youth with healthier lifestyles through the outdoors. After several years of work with the Boys & Girls Club of San Francisco, Jason took an opportunity to run a program near his family in North Carolina. During that work, Jason developed a passion for helping young adults that led him to Brightstone.  Jason takes an active approach to working with clients and their families, but his work does not stop at the admission process. He continues to be involved with clients and their families throughout their time at Brightstone. A major focus of his work with young adults is helping them find and pursue their individual passions. Jason understands that this often leads to young adults finding newways to be successful. Jason received a bachelor’s degree in Political Science with a concentration in Southern Urban Minority Politics from East Carolina University. Jason enjoys the outdoors and spending time with his family. Jason and his wife’s enjoy taking their son on adventures across the country.
