HSJ revealed last week at least £150m has been wasted planning for "new hospitals" that might never materialise. We explore where else trusts can turn, with major state-funded rebuilds now off the agenda for many areas. That could mean reviving private financing, a new drive on patch-up maintenance efforts, and a much greater reliance on community services and tech. Also this week, Samantha Jones' likely appointment as new permanent secretary of the DHSC, and another significant senior appointment to NHSE.Correction: £1bn - not £100m - is allocated to building safety works and reinforced aerated autoclaved concrete this year, although the point that more is needed still stands.

We discuss the new QOF indicators around Workforce and Wellbeing in General Practice. We learn what the indicators are and get hints and tips about how to achieve them. We come at this slightly sceptical of the workload this might put on practices but are glad of the opportunity to talk thought the underlying principles of trying to support practices, what that support might look like and how to approach this in a quality improvement way that might help all staff, including those charged with collecting this data. This leads us to talk about Compassionate leadership. It's International Women's Day and with this in mind, today's Business of Healthcare podcast guest really requires no introduction.   I am extremely proud to feature NHS England Director for Clinical Integration (and deputy lead responsible for the delivery of the Covid vaccination programme), GP, Mum of two and former NHSE first female Medical Director for Primary Care, Dr Nikki Kanani MBE.   To say I was super excited to do this interview would be an understatement and amongst the many things we managed to cover within our relatively short chat, I was particularly focused on finding out what it's really like being Dr Nikki Kanani;  How does Nikki manage her time, day to day, in her professional, and her home life?  When did Nikki's career evolve from GP to GP and national leader?  What does being a celebrity in her field really feel like and what are the practical impacts?  When is being better good enough versus delivering the very best, across Nikki's multi-hatted portfolio?  How does she feel about her work right now?  Bearing in mind that her role has often involved overseeing the release of (sometimes not well received) policy and, particularly where the vaccination programme is concerned, potentially divisive strategy, how does Nikki deal with conflict and ensure she has ample time for self-care and access to  personal sources of support?  And of course….I had to ask about the future for PCNs!  This is a fascinating listen from a truly inspirational female leader…. enjoy!    Work with me        I'm Tara Humphrey and I'm the founder of THC Primary Care, a leading healthcare consultancy. Key aspects or policy have been published in ways that are hard to access and even harder to interpret. Here, we gather an expert panel to discuss what proposed new NHSE systems for specialised commissioning mean in practice. In this episode, we explore: - What's new for Specialised Commissioning and what's set to change? - What are the potential implications of the proposed changes recently published? - Integrated Care Boards (ICBs) and their increasing role with Specialised Commissioning - What this means for the NHS and for industry in practiceThe episode begins with an overview and analysis of the changes from David Thorne (Principal Associate, Mtech Access and Business Development Director, Well Up North PCN). The Aussie share market started yesterday down 0.6%, however managed to close just 0.08% lower yesterday to 7,447. Only three sectors managed to post gains, while the tech and consumer discretionary sectors were hit the hardest. The tech sector is suffering from a spike in bond yields as markets anticipate a faster tightening cycle by central banks.Looking at the ASX200 stock performance, the top stock yesterday was battery materials and technology company, Novonix (ASX:NVX). The stock was up 10.8% after announcing its intentions to list on a second stock exchange, the Nasdaq in the US. AGL Energy (ASX:AGL) also performed well, rising 8.6%, following a bullish broker note from Credit Suisse, upgrading the stock to “outperform”. And some of the other best performing stocks included mining stocks Alumina (ASX:ALU), Champion Iron (ASX:CIA), Chalice Mining (ASX:CHN) and Iluka Resources (ASX:ILU). Reliance Worldwide (ASX:RWC) was the worst performer, down 3.4%, despite no news out from the company, and both Xero (ASX:XRO) and WiseTech Global (ASX:WTC) saw some selling as the tech sector came under pressure. The most traded stocks by Bell Direct clients yesterday was Wesfarmers (ASX:WES). The company is now the last one standing in the takeover of Priceline owner Australian Pharmaceuticals Industries (ASX:API), after Woolworths withdrew its proposal. Afterpay (ASX:APT), Telstra (ASX:TLS) and Fortescue Metals (ASX:FMG) were also highly traded yesterday. In the US, stocks recovered from earlier losses, staging an afternoon rally that put an end to the Nasdaq's four day losing streak. The tech-heavy Nasdaq closed 0.05% higher, while the Dow and S&P500 both fell, down 0.45% and 0.14% respectively. European stocks closed lower amid interest rate fears, ahead of key US inflation data out this week and more comments from US Federal Reserve Chairman Jerome Powell on interest rate hikes. For today, following the mixed session on Wall Street, the futures are suggesting the Aussie share market will open 0.56% lower. What to watch today: Keep an eye on Ramsay Health Care (ASX:RHC). The company reached agreement regarding a new volume-based agreement with the NHSE, the National Health Service in England, which makes its services available to the NHSE and its patients to meet the ongoing demands resulting from the COVID-19 pandemic. If the agreement is well received by investors, its shares could lift.Economic news out today includes Australia's balance of trade data for November. Australia's trade surplus decreased to A$11.22 billion in October from a downwardly revised A$11.82 billion in the previous month. Consensus expects trade surplus to decrease again to A$10.6 billion in November. Separately, retail sales for November will also be released today.Oil prices fell on demand concerns given the rapid global rise in omicron infections, while the gold price edged higher despite US 10-year Treasury yields hitting two-year highs. And the seaborne iron ore price traded slightly higher to US$126 a tonne.   Trading Ideas: Bell Potter have upgraded their rating on coal mining company, Whitehaven Coal (ASX:WHC) from a HOLD to a BUY, and have also increased their price target from $3.50 to $3.60. Bell Potter believe the coal price strength from mid-2021 should now be flowing through to realised prices, which should materially lift free cash flow. Operational risks at Narrabri remain, however, should abate over 2022. Therefore, Bell Potter believe WHC is cheap on most valuation metrics, and that supports their upgrade to a BUY. Now, WHC closed 5% higher yesterday to $2.89, which implies about 25% share price growth in a year.Trading Central has a bullish signal on EML Payments (ASX:EML). This signal indicates that the stock price may rise from the close of $3.22 to the range of $4.30 - $4.50 in

After the accompaniment of “that” letter from NHS England alongside the most recent Standard Operating Procedure, general practice has reacted. Anger, frustration, sadness, shock, and depression are all words permeating through the primary care world at the moment. With workload and demand at an all-time high and staffing numbers stagnating or in decline across many areas, and a (so far, highly successful) vaccination programme to deliver, the general feeling is that this is a slap in the face from central government and the mainstream media alike. Against this depressing backdrop, our joint Practice Index, Practice Manager panel return and are joined by GP and Clinical Director Dr Hussain Gandhi, to reflect on the detail of the letter, the implications of it, the impact on digital evolution in primary care, workload, capacity, patient education and most importantly, how we move forward from here. Rani talks to Dr Andrew Tresidder in this episode. Andrew is a medical doctor and GP by background. He is a strong advocate of patient empowerment and of holistic and complementary approaches in healthcare. Andrew qualified from Guy's Hospital in 1983 and trained as a GP mainly in the South West of England. Andrew's current interests are in education (Health of Health Professionals), clinical work (South West Clinical Lead for NHS Practitioner Health), investigations and quality work for Somerset Clinical Commissioning Group, GP Appraisals for NHS England, and in human factors for wellbeing and performance. He has taught widely on Health and Self Care for Health Professionals for organizations including NHSE, BMA, GP Trainers Groups, and others.  Andrew's book is a free download from healthandself.care. He is currently Clinical Lead of Somerset CCG Pastoral Care for Front Line Staff Cell. Early in 2020, he was an early adopter and enthusiast for Thinking Pitstops as a cognitive tool to achieve deep and lasting affective change, helping people through a precise intervention to reach personal destinations in a seemingly effortless and deceptively simple process.  He has a particular love of nature and the outdoors, and a great interest in the effect of nature upon our health and psychology.  Highlights of this episode:What got Andrew interested in self-care and holistic approaches to wellbeing including complementary medicine and flower remedies. The word doctor coming from Latin means teacher, many solutions to many problems in life are educational, about understanding. We think we learn from our education. Life has a curriculum. It wants us to learn as well. Life is a co-creative process. We're all participating. And we're all actors in a fascinating big drama, and we're all learning all the time. And the day we stopped learning is the day that it might feel it's not worth waking up. As health care professionals, it's like we have been taught how to treat car crashes but not how to prevent these from happening in the first place. That it is easier to pop a pill than make some sustainable lifestyle changes. It takes an illness, sometimes a serious illness or tragedy in our personal life, before we realize that we need to wake up from a dream state and realise what's important in life, to balance the head and heart. Logic can only inform us. It's intuition that must guide us. Logic unguided will take us most elegantly up one of the many blind alleys. But once you use intuition to guide you, it's like using your internal satnav. That flower essences are information from nature captured in water preserved in brandy, the benefits and how to use them.Andrew's website: https://www.drandrew.co.uk/Visit our website: drsranisuraj.comFollow on Instagram: @drs.raniandsurajLeave a review on Apple Podcasts or email us with any questions.

Choosing where to have your baby is something that probably isn't discussed in as much detail as it should be during your antenatal appointment unfortunately. Liz and Elidh, top London midwives, talk us through what your options are from home birth, midwife led and obstetric units. We also cover pain management options, elective caesareans and going private. We talk about the stats supporting each choice and what you should consider when making the decision and things you should try not to worry about. We also talk a little about how the process of labour is supported by a calm and consistent environment. Finally, we end with the birth stories of both midwives and Idia which ranges across all of the options from home birth to emergency c-section and here you realise just how varied the spectrum of births can be, no matter your back ground or training. Improving the Primary Care Network DES service specification

Improving the Primary Care Network DES service specification How to improve the PCN DES Welcome, please comment and I will try an answer SUBSCRIBE AND COMMENT Check out my original review where I went through the DES: here--https://youtu.be/K1qwyxt63f0 Declaration - I am not involved in the negotiations. I am Nottingham City East PCN Clinical director but these are my own thoughts. For some backstory in 2015 I wrote a further Blueprint for Primary care : https://egplearning.co.uk/a-further-blueprint-for-primary-care/ with a colleague as Jeremy Hunt launched the five year forward view. Some of these aims have been achieved (indemnity) but much can still be done.  How to improve the DES- I will talk firstly about general changes and the headline points. Then each specification in turn with my more innovative (controversial) ideas at the end.Ethos The release time of the specification has caused a lot of frustration. Even more so in the first webinar where little engagement was offered, just reading the spec. This led in part to one CD resigning live. NHSE doing an open consultation on the draft is a good thing. Feigning listening is not.  At points, the specification makes reference to evidence. In particular vanguard projects. However, it neglects to mention that vanguard projects have had significant higher levels of funding for each aspect and therefore converting this to specifications for networks to follow is a fallacy.  You cant use a recipe that feeds four people that costs £10 and say provide the same meal for £2 all the time - even with using foodbanks.  The original outline of the DES was to stabilise primary care. This draft does not do this but further destabilises it by utilising the additional roles above the capacity of work the can create and negates the fact networks will be part funding them. This was a poor way to show that NHSE/I was listening to the needs of general practice. Many comment the draft is intentionally too far to the right to allow any softening to be accepted as better than the original. This is a sceptics dream.  For this process to work- show General Practice that NHSE is truly listening. Much of this has been done amazingly by Dr Nikki Kanani and I do not envy her position. I am mindful of do not shoot the messenger being very apt here.  Lets move on. The following is a combination of my views and those from the various forums I engage in.  Trust  General practice. If I had control I would keep existing structures that could help foster better collaborative working and simply offer the resources to networks to use for their local area.  General Practice has proved to be the most cost-effective method of delivering healthcare currently doing 90% of the NHS workload for less than 9% of the funding. Trust the networks to deliver primary care with the simple metric of improving the health of the network population.  Ideally to the global sum but is not then to the networks directly to use as they seem fit to provide roles or services for better patient care along them 5 categories as best for their population.  However, we do not live in a world of trust despite GPs consistently being the most trusted role in the country.  So if working towards the confines of adjusting the existing DES and its specification let us have a look.  Worth looking at the Berkshire, Buckinghamshire and Oxfordshire LMC document link:
