Treatable but not curable, endometriosis affects one in 10 women and girls of reproductive age. But it's not all that well understood and recognized, and can go undetected, leading to years of avoidable suffering and causing time away from school, work, and other activities. UConn Health is part of a collaboration with The Jackson Laboratory for Genomic Medicine and the state of Connecticut called EndoRISE, which seeks to raise awareness and advance research of endometriosis. In this episode, Dr Prina Ruparelia, joint clinical director for respiratory medicine at St Bartholomew's and Royal London Hospitals, talks to Dawn Liz Powell about identifying and managing chronic cough in primary care.Dr Ruparelia explains that after ‘worrying conditions', such as lung cancer, have been excluded in patients with chronic cough, a thorough history should be taken to identify any potentially ‘treatable traits'. Treatable traits, she says, are factors (for example, smoking) that might cause a cough. Once identified, these traits can be addressed to help manage the cough.One trait, in particular, discussed in this episode is cough hypersensitivity. Dr Ruparelia explains that this is ‘under-recognised' and probably relates to a dysregulation of the neuronal pathways that control cough — resulting in a cough response to relatively minor irritants,such as perfume or laughing. The idea of developing dementia is probably one of our greatest health fears. We tend to think of it as an irreversible disease that gradually robs us of our faculties. But podcast guest, psychiatrist Dr Kat Toups is one of a group of doctors and researchers who argues that certainly in its early stages, the disease is actually reversible. The mistake - she says - is to think of dementia as an illness with a sole cause and instead we need to see it as multi-system disease, which requires a coordinated approach to work out what is injuring the brain and then provide effective treatment. Toups calls her approach the three Rs: remove, replace and regenerate. So that means taking away anything causing problems; providing the nutrients and hormones to help the brain function optimally; and then using a number of neuroplasticity techniques to enable the brain to form new connections and regenerate. Patients in her latest study who have undergone her protocol have seen a remarkable improvement. Their brain scans are now showing less signs of aging than those who don't have dementia. All of her subjects have now raised their cognitive scores into the normal range. In one case a fine artist who had had to stop painting has been able to start painting again and has even opened a new exhibition, and another patient has just started a new business. New research has found that an enlarged prostate gland is causing serious problems for more than 2.8 million Australian men and people assigned male at birth over the age of 50. The research by YouGov for Boston Scientific found most men fail to recognise the seriousness of what is a common health condition. The problems include increased frequency to urinate, and the risk of obstruction, bladder blockage and life-threatening symptoms. One man who spoke to SBS News explained how his prostate grew to the size of a softball forcing him to have surgery.

The spine has some natural curvatures that are very important to its normal function and alignment. These curvatures play a vital role in maintaining spinal strength, flexibility, and distributing mechanical stress during movement. There are two essential curvatures: lordosis, which bends forward in the cervical and lumbar spine, and kyphosis, which bends backward in the thoracic spine. These curvatures should exist within a specific range for normal spine function. Understanding Kyphosis Kyphosis often includes rounding of the shoulders and upper back, which can lead to discomfort, radicular pain, and stiffness in the thoracic spine. This condition is commonly referred to as a "rounded back" or "hunchback." Primary Causes of Kyphosis: Postural Kyphosis: In today's tech-driven world, poor posture, especially with prolonged device use, can contribute to postural kyphosis. This type is treatable, especially when addressed early. Sherman's Kyphosis: This structural kyphosis results from a misshaped vertebra, forming a wedge-like structure. Early intervention is crucial to prevent worsening over time. Treatment Approaches Treatment strategies vary depending on the cause of Kyphosis: Postural Kyphosis: With its relative flexibility, the thoracic spine responds well to therapies, rehabilitation, and postural exercises, helping to improve posture and alleviate symptoms. Structural Kyphosis: When kyphosis becomes resistant to correction through posture alone, more intensive approaches are necessary. These may include specific chiropractic care, physical therapy, extension-focused exercises, muscle strengthening, and corrective bracing. In treating kyphosis, timing plays a critical role. The sooner it's addressed, the better the chances of successful outcomes, both visually and symptomatically. Proactive treatment during childhood or early stages of adult development can help reshape the spine, prevent further misalignment, and is the key to a healthier, pain-free spine. Waking in a cold sweat, waking yourself up as you cry out in fear to find yourself in your own bed but still filled with feelings of horror or dread from a disturbing dream – we've all had nightmares, and for people who experience them frequently, they can be disruptive and distressing. Enter Prof Michael Schredl of the Central Institute of Mental Health in Mannheim, Germany, who has been spent 30 years in the sleep lab and working with patients and who has researched the efficacy of psychological treatments like Image Rehearsal Therapy (IRT). Michael talks to Dave and Laura about: the difference between nightmares and night terrors causes and factors of nightmare frequency the relationship between waking attitudes and nightmare content how particular images trigger different emotions for individuals but how the basic pattern of a dream gives hints as to what experience is being dramatized in the dream. the difference between idiopathic (not related to real-life experience) and post-traumatic nightmares Image Rehearsal Therapy (IRT), its applications and limitations the important role of self-empowerment in dealing with nightmares By the end, Michael can advise us all to “Have nice dreams – and even if it's a nightmare, you know what to do.” Appropriately, the Dream of the Week is a nightmare of a plane flying into a wall of water and being crushed in ice. Lillian Kreppel had to advocate for herself and seek a second opinion before receiving a delayed diagnosis: HPV-related stage II anal cancer. During treatment, she found stark limitations in HPV information, including lack of public awareness and patient education, inadequate guidelines for cancer screening, and a lot of HPV-related social stigma. Now a survivor, Lillian leads the HPV Cancers Alliance, an advocacy organization she co-founded to help improve HPV education for patients, providers, and the public.
