Send us Fan MailCould putting a few drops of breast milk in a preterm infant's nose actually improve cerebral oxygenation? In this episode of Journal Club, Daphna reviews a randomized controlled trial from the European Journal of Pediatrics investigating the physiologic effects of intranasal expressed breast milk (EBM) administration in preterm infants. The study found that infants receiving 0.2 mL of fresh breast milk intranasally three times daily showed significantly higher cerebral oxygenation levels, along with more favorable trends in heart rate and respiratory rate, compared to controls. While time to full oral feeding and length of hospital stay were unchanged, the safety data is reassuring. Ben and Daphna discuss what outcomes we should even be measuring, and whether the evidence is already good enough to just do it.----Effect of intranasal breast milk administration on cerebral oxygenation, vital signs, and transition time to full oral feeding in preterm infants: a randomized controlled study. Send us Fan MailDr. Bridget Young, researcher at the University of Rochester, shares early but promising work on using freeze-dried mother's own breast milk — including a skim milk fraction — as a fortifier for human milk feeds in the NICU. She explains why milk variability makes accuracy the central challenge in this approach, how her team validated a method using macronutrient analysis to consistently hit a target caloric density of 28 kcal per ounce, and why the skim milk fraction in particular can deliver a protein content that rivals commercial bovine fortifiers. She also introduces the concept of mother's own breast milk cream — a centrifuge-derived calorie booster that families in her unit have found deeply empowering — and lays out honestly what it would take to bring lactoengineering from a research setting into routine clinical practice.Support the showAs always, feel free to send us questions, comments, or suggestions to our email: nicupodcast@gmail.com. Send us Fan MailIn this Journal Club episode, Daphna presents the F-NeoBright trial — a pilot feasibility and safety study out of Hungary exploring intranasal fresh breast milk administration in neonates with moderate to severe HIE undergoing therapeutic hypothermia. With so few adjunct therapies available beyond cooling, the idea of harnessing breast milk's rich bioactive components — including neurotrophic growth factors, cytokines, and multipotent stem cells — to support the developing brain is both compelling and refreshingly low-risk. You just had a baby. You've got a million things running through your mind. But there's one thing you have to do right away: feed your baby. No matter how much you planned, it can feel stressful and overwhelming. That's where StrollerCoaster comes in, with a special three-part series devoted to feeding. In our first episode we start at the very beginning: Formula? Breastmilk? Both? While breastmilk is often recognized as the gold standard, we know every family's journey is different - and we're here to explore and support all feeding choices. We're joined by Dr. Christie del Castillo-Hegyi, an emergency physician, infant feeding expert, and co-founder of the “Fed Is Best” Foundation who shares evidence-based insights on newborn nutrition, the risks of insufficient feeding, and why flexibility and informed choice are critical for families. Then, in our Parenting Story of the Day, Nona Willis Aronowitz, author of a New York Times opinion piece titled “The Secret to Marriage Equality is Formula,” explains how feeding choices can impact not just your baby, but your relationship with your partner as well. Dr. Christie del Castillo-Hegyi / FB / IG / YT / In Dr. Christie del Castillo-Hegyi is a board-certified emergency physician, infant feeding researcher, and Co-Founder of The Fed is Best Foundation which advocates for safe, informed newborn nutrition. You've heard breastfeeding protects against allergies and asthma. But have you ever wondered what's actually happening inside your baby's body to make that possible? It's one of those things people say all the time, but rarely explain. And when you're in the thick of feeding, you deserve more than soundbites. In this episode, Dr Renee White sits down with Dr Toby Mansell, a researcher from the Murdoch Children's Research Institute here in Australia. His team studied nearly 900 babies from pregnancy through childhood, and what they found is genuinely fascinating.Together they talk about what happens at the molecular level when babies breastfeed, why certain fats in breast milk seem to reduce inflammation, and what this might mean for immune health down the track. It's science, yes. But it's also context. And hopefully, it helps things make a bit more sense. You'll hear about:How the Barwon Infant Study tracked 900 babies to understand what influences health from early lifePlasmalogens, lipids abundant in breast milk but absent in formula, and why they reduce inflammationWhat chronic inflammation in infancy means for allergies, asthma, and metabolic health laterHow breastfeeding changes infant metabolism at the cellular levelWhat this could mean for formula composition and supplements for all babiesWhether you're breastfeeding, formula feeding, or somewhere in between, this episode helps you understand what's happening biologically. It's research-based clarity without pressure or judgment.Resources & Links:

Welcome to our first monthly Q&A episode of Season 7!In today's episode, we discuss why C-sections are sometimes framed as “prevention,” how fear around tearing, fetal monitoring, or cord concerns impacts clinical decision-making, and why breaking the bag of waters is still routine practice despite clear risks and limited benefit. We explain what delayed cord clamping looks like during a cesarean and why vague language like “misplaced cord insertion” can unknowingly lead to unnecessary intervention. Additionally, we talk about nursing while pregnant, early breastfeeding challenges, frozen breastmilk, feeding to sleep, and other common postpartum concerns. Sit back, relax, grab a bottle of breastmilk, and dive into this episode…wait, BREAST MILK?? Is that everyone or just Danielle?! We've got a few debates going this week. One: is FroYo even good? Two: Do you try your friend's breast milk? Do you try your own?! Aside from the debates, Brooke shares the full needlepoint dictionary with us. After we show our WIP, we might need to head to our LNS and pick up a few things (only the real ones can crack that code). We jump from hot debates to air travel pet peeves to a nose job update (you heard it here first)!! Danielle visits the GYNO and nervously chats with her gynecologist during the appointment…if you know what we mean. Look, she's just an anxious gal when it comes to anything health-related. AND we can't forget the newest character in Danielle's house—Edwin. We'll leave it at that…It's clearly a late-night episode, and you know the gals get extra silly after dark. Please support the show by checking out our sponsors!BetterHelp: This show is sponsored by BetterHelp. Human milk oligosaccharides (HMOs) are complex sugars that pass undigested to your baby's colon, where they feed beneficial bacteria, strengthen defenses, and support healthy brain and immune development A systematic review and meta-analysis in Frontiers in Pediatrics found that breast milk continues supplying substantial amounts of HMOs beyond 12 months, supporting gut health and brain development into toddlerhood Certain HMOs, such as 3-fucosyllactose, even increase over time. This shows that the balance of individual sugars shifts across lactation and continues shaping your child's development after infancy Breast milk also contains myo-inositol, a sugar shown to boost synapse formation and neuronal connectivity, highlighting how breast milk delivers brain-supportive compounds beyond traditional nutrients To keep your breast milk as nourishing as possible for your baby, prioritize whole foods, minimize unnecessary medications, and reduce toxin exposure. If breastfeeding isn't possible, screened donor milk is the next best option
Breastmilk naturally changes throughout the day, guiding your baby's sleep, digestion, and alertness through subtle hormonal shifts These changes support your baby's developing body clock, especially in the first months of life Labeling pumped breastmilk for time-matching with your baby's feeding schedule supports better sleep, calmer moods, and smoother daily rhythms Time-matched feeding builds consistency and helps babies feel secure through growth and life transitions Breastmilk offers living nutrients and immune protection that infant formula can't match, supporting stronger immunity and well-being
