Ellena describes the emotional and physical hurdles she faced as her baby spent time in NICU and she herself was in ICU, while also navigating the complexities of breastfeeding a premature baby.For her second birth, Ellena sought out a private midwife and chose to have a homebirth, giving birth to her daughter at 42+1 weeks. Today, I am really honored to interview an incredible woman, Barb Higgins. I've known Barb since I was in 4th grade and ran track and field with the local Parks and Rec community. In high school, she was my cross-country coach, and she led us to win our local Division, then State, and then The New England Championship. In this episode Alex shares his pregnancy and birth journey as a trans man, including his choice to embark on solo parenthood. He sought guidance from an IVF clinic and embarked on a number of tests to ensure his egg quality and reserve were favourable before deciding on a sperm donor. Once pregnant, he opted for MGP care through his local public hospital and was induced at 38 weeks due to intrauterine growth restriction (IUGR). Maternal human milk is the absolute best food for newborn nutrition that supports all of their accelerated growth in the first year. And although we have a basic understanding of the composition of human milk, every woman's breast milk has a different complexity when it comes to the nutritional content. But, you may be wondering, what exactly is breast milk composed of? For our 48th podcast episode, I review the general composition of human breast milk and what the variations are between preterm, term, and donor breast milk. I also discuss some of the anti-infective protective properties in maternal breast milk and how a preterm mother's milk has unique and complex properties that are lacking in donor breast milk. I discuss human milk fortification, the different formulations available, and why it is so important in our preterm population. I share why donor breast milk is used in preterm infants over formula and explain its protective mechanisms against necrotizing enterocolitis (NEC). Finally, I discuss how the pasteurization process, although important, impacts the composition of donor breast milk. I share why it is so crucial that mothers feel supported and are educated. Building trusting partnerships with NICU mothers will help to promote their milk expression, establish breastfeeding, and nurture the feelings of motherhood. A supportive environment coupled with education, has the potential to positively impact the mother's milk production in the NICU and beyond. Since it's the season of giving I wanted to talk about one of the greatest gifts of all - that of donated breast milk. My special guest on this episode of the boobingit podcast is Dr Natalie Shenker, Cofounder of The Human Milk Foundation. Natalie is an Oxford-trained doctor and researcher, and she co-founded the Hearts Milk Bank in 2016. In 2019 she was awarded a UKRI Future Leaders Fellowship at Imperial College London. In this episode Natalie talks to us about how donated breast milk is used by hospitals around the world to help give sick and premature babies the best possible start in life when mother's milk is not available. Natalie tells us why donor milk is needed more than ever, with babies being born to mothers with Covid 19 and too ill to feed them. The last three months in particular have seen hospitals and families in urgent need of donor milk for babies. We also look at why it's so important for pregnant people to get vaccinated and protect themselves against Covid 19.
Over 15 million infants around the world are born prematurely each year and can experience health complications and lengthy hospitalizations as a result of underdevelopment and immature immune systems. One of the best resources for these babies can come from their mother's breast milk, which has unique growth factors and nutritional qualities that can help combat a number of serious conditions that can impact premature infants. But for babies whose mothers may be unable to produce milk due to their baby's premature birth, or have health conditions or medications that preclude breastfeeding, and a physician recommends breast milk as the preferred choice for those infants' needs, what are the options? The answer comes from a resource that we might not all be familiar with. You've heard of blood banks where blood donated by the public is having a life-saving impact? Did you know the same resource exists for breast milk, with donor banks across the country providing this valuable resource to these tiny patients?! Joining me to discuss work being done to help improve access to these donor milk resources and ensure their beneficial impact can reach even more infants in need is Lisa Maillart with the University of Pittsburgh. She and a team of researchers partnered with the largest donor milk bank in the U.S. to help improve their existing systems and processes and realize significant outcomes. Their work will be featured in an upcoming edition of the INFORMS journal Service Science.

In this episode Michelle shares the story of her pregnancy, birth and breastfeeding journey with her son Ollie. Michelle has polycystic ovarian syndrome, or PCOS and having been told that she may find it difficult to conceive, she was quite surprised when she fell pregnant really quickly. Michelle's pregnancy was very straight forward, and she tells a beautiful story about how she celebrated and honoured her pregnancy at each stage to help her connect with her baby and honour her transition to motherhood. Following Ollie's fast and complication free homebirth, Michelle went on to have a long and difficult breastfeeding journey, where she ended up relying predominantly on donor milk. We talk about the practice of breastmilk sharing and the community that has sprung up around it. We discuss how giving and receiving donor breastmilk through an online platform is a perfect example of bringing traditional practices of ‘Village Parenting' into the current time, another step in our shared passion for breaking down the unsustainable ideal of nuclear parenting and shifting towards interdependence.

Today’s episode is one of the more thought-provoking ones and intentionally so. I speak with Christchurch mother-of-two, Kendall Green about her complex but beautiful feeding journeys with her daughters. Kendall was one of the first listeners to message me about Motherness and told me from the outset about her passion for advocating for milk sharing. I immediately knew she was someone I had to have on the podcast. Kendall’s experience covers a lot – a homebirth birth turned hospital birth, supply issues, lactation consultants, donor milk top-ups, feeding via an SNS tube, and weaning off top-ups to exclusive breastfeeding. She then went on to donate milk herself, set up a milk sharing service, and do extended breastfeeding. I know, it’s a lot. But feeding is a lot. And as Kendall says, it’s a fraught topic. In my experience, there is nothing more controversial, questioned, or debated more in motherhood than the way that you choose to feed your baby. I want to be clear that we’re not at all anti-formula, but what we are calling into question is mothers knowing that donor milk is an option. My hope is that this episode brings to light the amazing benefits of it, and allows you to consider the sacredness of human milk from Kendall’s perspective.
