Sciatica refers to the nerve pain resulting from an injury or irritation to your sciatic nerve. Sciatica is not a condition, but rather a symptom of another problem involving the sciatic nerve. The sciatic nerve is the longest and widest nerve in the human body. It runs from the lower back, through the buttocks, and down the legs, ending just below the knee. The sciatic nerve controls several muscles in the lower legs and supplies sensation to the skin of the foot and the majority of the lower leg. Causes of Sciatica Sciatic nerve pain is often a result of a herniated disk in the spine. Other causes include bone spurs on the lower back bones, tumors, or nerves affected by another health issue. Symptoms of Sciatica Sciatica symptoms include: Pain: Sciatica pain is a result of pressure exalted on the affected nerve. Most people who have experienced sciatica pain describe it as burning or like an electric shock. The pain often shoots or radiates down the leg on the affected side. Numbness and tingling: It occurs when you can't feel sensations on the skin in the affected areas of your back or leg. The numbness usually happens when you wake up or sit in one position for a long time. Muscle weakness: This is a more severe symptom. It means that muscle command signals are having trouble reaching their destinations in your back or legs. Urinary incontinence or fecal incontinence: This is a very severe symptom. It means that signals that control your bladder and bowels are not reaching their destinations. Risk Factors for Sciatica The common risk factors for sciatica include: History of back pain: A history of back pain or problems with the back may increase one's risk of sciatica. Having excess weight or obesity: The more weight you have, the more your back muscles have to work to support the body weight. That can lead to back strains, pain, and other issues. Your job: Jobs that require heavy lifting, a lot of bending, or prolonged sitting without proper back support may increase your risk of lower back problems. Physical inactivity: Sitting for long periods and a lack of physical activity can increase the risk of sciatica. Normal wear-and-tear: As we get older, wear-and-tear is normal, especially on the spine which can lead to pinched nerves, herniated disks, and other conditions that increase the risk of sciatica. How to Manage Sciatica Symptoms Effectively Depending on the cause, milder cases of sciatica usually get better with self-treatment. Some of the applicable self-treatment include: Stretching and physical activity help strengthen the core muscles and reduce back pain. A home remedy that targets the use of ice during the first 1-2 days when pain first begins. Another practical home remedy is applying a heat pack to the area if the pain persists longer than 2 days. Chronic sciatica pain can effectively be treated through chiropractic adjustments. Chiropractors use a special method called spinal manipulations where they gently move the vertebrae and joints back where they should be, using quick and soft thrusts. The adjustments and movements aim to fix the spine's alignment, reducing pressure on the sciatic nerve for pain relief and allowing the body to recover. Welcome to season 2, episode 2 of The Fuel Run Recover Podcast! In this week's episode, I sat down with fellow runner and physiotherapist Elliot Cooke to talk about all things running and injuries. We answer questions like, why do we keep hearing that running is bad for our joints? and, how can runners keep their joints healthy? How much does wearing the right (or wrong) shoes play a role in developing injuries? A little more about Elliot: Elliott graduated from the University of Manitoba with a Master of Physical Therapy and a Bachelor of Science (Chemistry and Biology). He practiced at Elite Physiotherapy for three years before deciding to open a new clinic with a modern approach to Physiotherapy. Elliott is a certified professional with The Running Clinic, SFMA levels 1 and 2, Acupuncture, Dry Needling, and Orthopaedic level 1. Elliott has also taken further education in Spinal Manipulation, TMJ pain, Long-COVID Rehabilitation, Hip OA, Pelvic Girdle Dysfunction, and Mulligan Mobilizations. Elliott competed for the University of Manitoba Bisons as a Captain of the Track and Field and Cross Country teams. He raced middle distance and distance events, specializing in the 3000m. He has run several half marathons, shorter local races, trail races, and one marathon. He was most recently the winner of the 2024 Frostbite River Run 5 Km, and the 2023 Manitoba Marathon 10 Km. Elliott spends his leisure time running, skiing, sailing, and playing squash. He is an armchair physicist and, despite being a lousy gardener, still loves to cook. Welcome to Wednesday Q&A, where you ask questions and we answer them! In this Wednesday Q&A, we answer your questions about neck crunching sounds, spinal manipulation, and pyramid pose. Your questions:* A couple of weeks ago I was practicing a class on LYT Daily. After doing a neck release with the block right afterwards getting out of the pose, I can hear around C1 and C2 there's a crunching sound, pretty loud. It's been a few weeks now. I'm not so worried about it as it is not hurting me or affecting me in any way except for the loud sound when I rotate my neck to the left/right as well when I do shoulder rolls. What do you think might have happened and do you think this sound will ever go away?* I've been wondering what you think about “specialists” who manipulate each other's spines. Is this safe and does it help in any way? What kind of method is it? I know enough about physiology and anatomy, and for me, it seems to be dangerous. There are a lot of videos like this on social media and it is disturbing. They do a show out of these cracks and popping sounds.* I have long legs and a short torso. Because of this, I find in some poses, like Pyramid, I can't keep my ribs off of my thighs without using blocks. But if I look back on videos of my practice, I do feel like my spine is neutral even without the blocks. However, I know that we receive cues to keep the ribs away from the legs in poses like that. Is it an issue that my ribs are on my thighs if I'm staying integrated in my core? Cervicogenic headache (CGH) refers to headaches caused by dysfunction in the neck, and experts estimate that 18% of chronic headache patients have cervicogenic headaches. Spinal manipulative therapy (SMT) is a form of treatment most commonly provided by doctors of chiropractic, and several studies have demonstrated that SMT is highly effective for patients suffering musculoskeletal disorders of the neck, including those with cervicogenic headaches. However, there remains little consensus on the appropriate number of SMT treatments to achieve maximum benefits for CGH. In a 2018 study, a team of researchers conducted a large-scale study involving 256 chronic CGH patients to determine how many treatments are needed to achieve optimum results using SMT for CGH. The investigators randomly assigned participants to one of four dose levels (0, 6, 12, or 18 visits) of SMT for six weeks. The type of SMT consisted of a manual high-velocity, low-amplitude (HVLA) thrust manipulation in the cervical and upper thoracic regions. The location of the spinal adjustment was determined by a brief, standard spinal palpatory examination from the occiput to T3 to assess for pain and restricted motion. For older patients and/or those in acute pain, the manual therapy was modified to a low-velocity, low-amplitude mobilization. To control for visit consistency and provider attention, patients continued to receive a light massage treatment once a patient's assigned number of visits was satisfied, until the six-week treatment period ended. After the conclusion of the treatment phase of the study, the participants used a headache diary to keep track of their headaches for the next year. The results showed that the patients who received the most SMT treatments had fewer headaches over the following twelve months. More specifically, the researchers calculated that six additional SMT visits resulted in about twelve fewer days with headaches over the next year.
