Today we are going over the basics of DEXA. This will help patients understand the test and what is being measured. It will also help clinical providers understand this test better. Let's dive in! The BasicsDEXA stands for Dual Energy X-Ray Absorptiometry-              Xray machine used for analyzing body composition-              Great at telling the difference between bone, fat, and lean tissue (mostly muscle)-              Uses 2 xray beams of different energies for this, hence Dual (via subtracting attenuation coefficients….or something like that)-              Most studied and widely used tool for looking at bone density-              USPSTF recommends all women over age 65 get one-              But 65 is way to late to get your first DEXA. The Cliff of old age is already luming at that point.-              I recommend getting one in your 20s or 30s to get a baseline and to help plan for the future. Vor dem Fettabbau steht die Fettmessung: hier die Folge zu Caliper-Zange, Bioimpedanz, DEXA-Scan und anderen Methoden zur Messung der Körperzusammensetzung. Zum Nachhören: die Folge, in der Genetik-Experte Dr. Stefan Wöhrer den pharmakogenetischen Test von Andreas ausgewertet hat – mit extrem überraschenden Ergebnissen.   Spezielle Rabatte für Gen-Tests gibt es für Hörerinnen und Hörer der Biohacking Praxis hier.   Eine sehr preisgünstige Caliperzange zum Herumspielen gibt es hier.   Eine aufwändigere Caliperzange (inklusive Körper-Maßband) gibt es hier.   Bioimpedanz-Waagen sind tatsächlich in der Regel – wie in der Folge erklärt wurde – zwar nicht absolut zuverlässig, aber relativ. Für zuhause sind sie eine vernünftige Möglichkeit, Entwicklungen zu verfolgen. Hier und hier gibt es zwei Modelle mit guten Bewertungen.   Es gibt in nahezu allen größeren Städten im DACH-Raum Einrichtungen, die DEXA-Scans anbieten, am besten googlen.   Weil er angesprochen wurde: Der „belgian blue“ oder „weißblaue Belgier“ ist der Bodybuilder unter den Rinderrassen. Die blauen Belgier verdanken ihren enormen Muskelanteil einer genetischen Mutation im Myostatin-Gen. Dieses Gen reguliert normalerweise das Muskelwachstum, und durch die Mutation wird seine Funktion gehemmt, was zu einem übermäßigen Muskelwachstum führt. Bilder gibt es im Internet zuhauf. Warum das für Menschen relevant ist? Bryan Johnson unterzog sich 2024 einer Follistatin-Gentherapie. Sie beeinflusst das Myostatin-System. Durch die Erhöhung von Follistatin wird die hemmende Wirkung von Myostatin auf das Muskelwachstum reduziert, was zu einer Zunahme der Muskelmasse führt. Diese Behandlung ist aber äußerst experimentell, Johnson ist mit den Ergebnissen sehr happy, wie er in seiner Netflix-Doku erklärt. (Übrigens wirklich eine tolle Doku.)

In this week's episode, I'm recapping my long weekend—saw Kings of Leon with friends, did some cleaning (and broke my vacuum), went shopping, and found my new favourite water-resistant runners. I'm also sharing my September picks and simple things that have made my life better, like how I cook my ground beef! Plus, I'm answering your questions!Q: Gift ideas for husband or boyfriend? Q: I'm thinking of getting my body fat tested by a DEXA scan, would you recommend it or is it a waste of money?Q: My best friend and her boyfriend have started going off the rails with their eating and exercise. They've lost a lot of weight in a short amount of time, and she didn't have much (if any) to lose, she talks about her cheat days, how she's fat and pinches her skin because it's skin and not fat!!, and she's even commented on what I'm eating a few times now. A protein bar or shake is high in calories, I'm like, wtf I never asked you! She's exercising 2-3 times a day, and she was never like before him. They live together and we live close to one another. I love her and used to enjoy hanging out with her but now it feels different, I also have a history or orthorexia so even though it's not affecting my behaviours, it is triggering. What would you suggest I do? I don't want to push her away because I think she needs help, but I also want to make sure this doesn't start to affect me more than it already is.Q: When reverse dieting someone to new maintenance after dieting how do you decide when o stop increasing calories? Obviously some scale fluctuation is normal but considering true gain would take a long time to accumulate and show up, how do you determine between normal fluctuations and overshooting?Q: What's one piece of advice you'd give to a beginner in the gym?Q: If you're working out at 7am and want to get out the door quickly, is it okay to have 15-20g of protein and 30g of carbs before and then a normal breakfast after? Our body composition matters when it comes to our health. Understanding how much of us is lean active tissue, knowing what our bone density is, and what proportion of our fat sits under the skin (subcutaneous) versus between our organs (the more harmful visceral) is valuable information to track over time.  Host Amelia Phillips and exercise physiologist Brendan Barry discuss the technology of  DEXA scans and how they can be a helpful tool in tracking, and motivating us to stay healthy. Lacy writes: Hi Robb and Nicki! I 1st want to thank you for all that you do, you are truly changing the world! I am a mom of 3 kids, ages 4, 3 and 5 months and I work full time. As you can imagine, I have a lot on my plate but am trying my hardest to be as healthy as possible: I do the best that I can with sleep (as best one can do with young children), usually infrared light, attempt to eat healthy/get sufficient protein, workout, etc. but feel like I am spinning my wheels getting nowhere. Each pregnancy has added pounds and with each additional child I find less and less time to get quality workouts in. I guess my question is, as parents who have "been there" what would you recommend prioritizing? I am currently at my heaviest weight but am breastfeeding (VERY successfully partly due to LMNT) and find that my milk supply suffers everytime I try to lose weight and I'm sure my hormones are absolutely jacked up due to being pregnant and/or breastfeeding for the past 5 years (&counting). I started training bjj over 10 years ago (when I still had braces) and it wasn't until our gym started following you and the Paleo lifestyle that all the wounds in my mouth started magically healing and I could finally sleep through the night. I am amazed by how undereducated our society is about the importance of the food (and poison) we put into our bodies. I really appreciate the effort you two put forth to make better humans. I recently started getting into podcasts so I apologize if you've already covered this: How reliable are DEXA scans? I just got one done and the results were shocking to say the least. I am a 34y/o female, 5'2, never had a child, I weighed in at 112 lbs, wear size 0 jeans and this thing marked me at 32% body fat. According to their chart, that puts me in the category of obese. Is this exactly what they mean by skinny fat? Like I'm just fat and bones, no muscle? *Mind blown emoji* I will admit, in the past year I haven't been training. I am active duty military, got promoted just about 12 months ago, and I feel like I have no time for myself in my life anymore. Do I cook, sleep, do laundry or finish my homework in my limited spare time? I would say, if anything, that I am definitely calorie restricted unintentionally, which worries me about training if I barely have time to eat as it is. I guess the other part of my question is how do I start over? Eating sufficiently? Training? Destressing? It's good to take down some notes on where you are now before you start any kind of self-improvement journey. This year I want to maximize my health so I decided it would be a good idea to get some kind of a record in place to set a baseline of my current health and fitness. This way I can check back in over the years and find out if I've managed to maintain my bone density, where my fat distribution moves over the years, and lets me know where I am doing well and where I have room for adjustment.I chose to get a DEXA scan to record this information, and it was an interesting experience. It didn't take very long, only about ten minutes. Some of that was just strapping my legs together and making sure my arms were next to my body while I laid on the scanning bed.You can schedule a DEXA scan without a doctor's recommendation now, but there aren't a lot of places that provide this service outside of an insurance system. I had to drive an hour away, but I consider the knowledge worth the effort to acquire.Remember that making progress checks with yourself is important because our mind tends not to notice our gains and only focuses on the next goal post – and we often move our goal posts over time as our gains become a new normal.By checking in and noticing all the positive changes, you can keep your negative self-talk in check and celebrate how far you've come. Check out today's questions:    Rob:  Hi Doc, me again. Sorry forgot to ask about my blood sugar. I've been wearing a CGM for almost 2 months now and I was shocked how I was basically pre-diabetic. Fasting glucose was about 120 and it could spike all the way up to 180 after eating my prep meal which could involve something like lentils, sweet potato and peas + olive oil. I have gone to a completely starch free diet where I'm eating plenty of cruciferous veg, pulses. 20g+ protein in every meal and healthy fats. No fruits, grains or starches such as sweet potato. I do have a cheat meal per week and anytime I try to add a starch, my blood glucose spikes again. Am I supposed to be on a starch free diet forever? Why is my fasting glucose still around over 100 when I'm not eating any starches? This is so confusing. Bearing in mind that I already get my 10k steps and I'm generally moving after my meals (washing dishes). Please help, I'm so lost.   Anonymous: Hello, I'm 38 years old and my Dexa Scan came back with upsetting results. I have -2.0 T score in my spine which is significantly decreased from 2 years ago. I am wondering if you know anything about CalZbone (Cissus quadrangularis) supplement or IMD supplements (which have an Oseto product containing this). I am worried about putting things into my body if I don't know about any potential side effects, especially long term. At the same time i'm working on my nutririon, i'm working with an endocrinologist for on my hormones and thyroid (which have been a mess for years), i'm also working on increasing the amount of calcium in my diet and making sure I have adequate Vitamin D and K.
