In this episode of The Metabolic Classroom Dr. Ben Bikman focused on the effects of alcohol on insulin resistance, emphasizing how ethanol, the main form of alcohol, influences the brain and metabolism. He highlighted that alcohol is primarily metabolized by the liver and can cause insulin resistance through both direct and indirect mechanisms. Dr. Bikman detailed the molecular pathways by which ethanol inhibits insulin signaling, notably by disrupting the insulin receptor substrate (IRS1) and increasing oxidative stress, which impairs insulin's ability to regulate glucose.Ben provided evidence from studies demonstrating ethanol's impact on insulin resistance at the cellular and whole-body levels. Research showed that ethanol consumption leads to higher insulin responses during glucose tolerance tests, indicating a reduced sensitivity to insulin. This phenomenon was observed in healthy humans who experienced a significant increase in insulin levels after consuming alcohol, suggesting a profound metabolic shift due to ethanol's presence.The lecture also covered indirect effects of alcohol on insulin resistance. Many alcoholic beverages contain high amounts of sugar, exacerbating insulin and glucose responses. Alcohol disrupts sleep quality, leading to poor metabolic outcomes and increased cortisol levels, which further contribute to insulin resistance. Additionally, ethanol competes with other metabolic substrates, leading to fat accumulation in the liver and elevated glucose and fat levels in the body.Dr. Bikman concluded by discussing the inflammatory response triggered by alcohol, particularly through the concept of a "leaky gut," where ethanol causes gaps in intestinal cells, allowing harmful substances like lipopolysaccharides (LPS) to enter the bloodstream and induce inflammation. This inflammation promotes ceramide production, further contributing to insulin resistance. He clarifies that this is closely related to lipotoxicity, where the stored fat itself, rather than glycogen, is seen as the main culprit for this condition.Ben notes that while the notion of energy toxicity encompasses both glucose and fats, triglycerides, a type of fat stored in muscle and liver cells, play a significant role. However, studies, such as one on endurance athletes, have shown that muscle triglycerides alone do not cause insulin resistance, leading to the concept of the “athlete's paradox.”Dr. Bikman further explores the biochemical pathways involved in insulin resistance, emphasizing that specific lipid intermediates, particularly diacylglycerols (DAGs) and ceramides, are more relevant than triglycerides in causing insulin resistance. DAGs disrupt the insulin signaling pathway by activating protein kinase C, while ceramides inhibit insulin signaling and affect mitochondrial function, increasing reactive oxygen species and contributing to insulin resistance.Ben challenges the notion of energy toxicity as a primary cause of insulin resistance, advocating instead for a focus on lipotoxicity and its mediators. He concludes that chronically elevated insulin levels, rather than the stored energy itself, are the main drivers of insulin resistance, suggesting that the term “insulin toxicity” might be more appropriate. Abstract: Protein-bound ceramides, specialized ceramides covalently bound to corneocyte surface proteins, are essential for skin permeability barrier function. However, their exact structure and target amino acid residues are unknown. Here, we found that epoxy-enone (EE) ceramides, precursors of protein-bound ceramides, as well as their synthetic analog, formed stable conjugates only with Cys among nucleophilic amino acids. NMR spectroscopy revealed that the beta-carbon of the enone was attached by the thiol group of Cys via a Michael addition reaction. We confirmed the presence of Cys-bound EE ceramides in mouse epidermis by mass spectrometry analysis of protease-digested epidermis samples. EE-ceramides were reversibly released from protein-bound ceramides via sulfoxide elimination. We found that protein-bound ceramides with reversible release properties accounted for approximately 60% of total protein-bound ceramides, indicating that Cys-bound EE ceramides are the predominant protein-bound ceramides. Our findings provide clues to the molecular mechanism of skin barrier formation by protein-bound ceramides. Copy rights belong to original authors. Abstract: Chronic elevation of sphingolipids contributes to {beta}-cell failure. ORMDL3 has been identified as a key regulator of sphingolipid homeostasis, however, its function in pancreatic {beta}-cell pathophysiology remains unclear. Here, we generated a mouse model lacking Ormdl3 within pancreatic {beta}-cells (Ormdl3{beta}-/-). We show that loss of {beta}-cell Ormdl3 does not alter glucose tolerance, insulin sensitivity, insulin secretion, islet morphology, or cellular ceramide levels on standard chow diet. When challenged with a high fat diet, while Ormdl3{beta}-/- mice did not exhibit any alteration in metabolic parameters or islet architecture, lipidomics analysis revealed significantly higher levels of very long chain ceramides in their islets. Taken together, our results reveal that loss of Ormdl3 alone is not sufficient to impinge upon {beta}-cell function or whole-body glucose and insulin homeostasis, but loss of Ormdl3 does alter specific sphingolipid levels. Ceramides as a New Biomarker for Cardiovascular Risk Prediction Guest: Vlad C. Vasile, M.D., Ph.D. (@VladCVasile) Host: Stephen L. Kopecky, M.D. (@DrSteveKopecky) Ceramides are lipids that are ubiquitously expressed. They are present in all cell membranes. Some ceramides have been associated with risk of atherosclerotic disease in patients with known coronary artery disease. Researchers at Mayo Clinic and others have demonstrated that ceramides can also predict negative events in patients without known coronary artery disease. Joining us today to discuss ceramides is Vlad C. Vasile, M.D., Ph.D., a preventive cardiologist and co-director of Cardiovascular Laboratory Medicine at Mayo Clinic in Rochester, Minnesota. The high temperature, humid environment caused by face masks activates sebum secretion, which causes sebum, sweat, and waste to become entangled and block pores. Skin also becomes sensitized due to continuous friction, resulting in "mask trouble." According to an analysis of Naver's search volume over the past eight months: the keyword "skin trouble" has increased in search rankings by 18% Naver's search volume for "mask trouble," a corona-related keyword, surged 161% Naver's shopping results in the cosmetics-beauty sector increased by 3,233%. The term "maskne," a combination of mask and acne, has also started appearing, showing the popularity of these searches. THE TELL TALE SIGNS OF MATURE SKIN Caring for mature skin involves a little more TLC than skin in its 20s or 30s. As we age, the amount and activity of the fibroblasts in the skin is reduced, as is the synthesis of collagen. Skin becomes noticeably drier thanks to a decrease in natural oils. On the bright side, this means that mature skin is prone to fewer breakouts. On the other hand, it means that skin begins to lack firmness and can develop a more pallid colour. HOW BEST TO CARE FOR MATURE SKIN Remember the old saying, “Prevention is better than a cure”? The ideal time to start an anti-ageing routine is while the skin is still in its prime. This doesn't mean that it's too late to take care of your skin if you've left it a little longer though. The Korean Beauty routine aims to provide hydration to dry, tight skin, working to protect and strengthen it. The ingredients in anti-ageing K-Beauty products also target dullness, age spots and pigmentation, and work to smooth existing wrinkles. The goal of these products is to stimulate the process of cell renewal and recover some of the skin's lost firmness. MY FAVOURITE KOREAN BEAUTY INGREDIENTS FOR MATURE SKIN Our favourite ingredients to look out for in Korean Beauty products for mature skin include: Galactomyces – a type of yeast extract made through a process of fermentation. It is perfect for ageing skin as it offers nourishment, hydration, renewal and toning effects. Niacinamide – a Vitamin B3 component. It improves skin's elasticity, enhances the barrier function and revives skin tone and texture. Adenosine – a yeast-derived ingredient that functions as a soothing and skin-restoring agent. rh-Oligopeptide – a small polypeptide of 53 amino acids that stimulate epidermal cell growth. This ingredient has been shown to speed the healing of wounds and reduce scarring. Ceramides – lipid molecules found within cell membranes that hold skin cells together. They form a protective layer that plumps the skin and helps it to retain moisture. In skincare products, ceramides are used to replace the skin's natural lipids. Sodium Hyaluronate – an ingredient found naturally in the human body that is important for sustaining skin as it ages. Sodium hyaluronate (and it's cousin, hyaluronic acid) hold up to 1,000 times their own weight in water, making them excellent moisturisers TOP K-BEAUTY PICKS FOR MATURE SKIN Many Korean Beauty products specifically target ageing skin and anti-ageing concerns.
