W tym już czasie hitleryzm opanował umysły i emocje bez mała wszystkich Niemców. Niewielu uniknęło tego skażenia. Jedną z jego najważniejszych składowych był antypolonizm. W minionym tygodniu Amnesty International opublikowało raport, którego celem jest analiza polityki stosowanej przez Izrael wobec Palestyńczyków. Dokument oparty został na danych gromadzonych w latach 2017-2021 i przekonuje, w oparciu o zapisy prawa międzynarodowego, że Izrael popełnia wobec Palestyńczyków zbrodnię apartheidu. Jest to kolejne tego rodzaju oskarżenie wystosowane wobec Izraela w ostatnich latach, już w przeszłości podobną argumentację zastosowały Human Rights Watch i izraelska organizacja B'Tselem. Izraelski rząd protestuje i nazywa działania Amnesty International antysemickimi, a sam raport uważa za przygotowany pobieżnie i bez właściwego przyjrzenia się rzeczywistym warunkom. Amnesty z kolei wzywa społeczność międzynarodową do podjęcia działań mających na celu wymuszenie zmiany polityki Izraela. Jak organizacja uargumentowała swoje stanowisko? Czy rzeczywiście możemy używać słowa „apartheid” opisując relacje izraelsko-palestyńskie? Czy tego typu raporty mogą wpływać na rzeczywistą politykę? Jeśli nie to na co właściwie są nam organizacje humanitarne? Did Anyone Really Win in Daton Fix vs Nick Suriano?This weekend was filled with exciting collegiate match ups. With streaming and broadcasting services the availability of watching these duals are at your fingertips. This wasn't a reality a short time ago when it was hard to find live college wrestling. One of the highly anticipated wrestling matches this weekend was between Oklahoma State's Daton Fix and Rutgers' Nick Suriano. These two have a deep history growing up in New Jerse. They also hold claim to one of the most talked about high school matches of all time. This weekends matchup followed theme with the match lasting 34 minutes. The match actually lasted even longer than that if you include all of the time spent reviewing by the officials. Ben Askren isn't surprised that every time these two meet it goes this way due to their similar high-pressure styles. The bout finally came to a controversial close with Fix serving Suriano his first loss of the season 3-2 due to a hands-to-the-face call. Askren is excited to see Fix and Suriano wrestle other top-ranked opponents but not each other due to the way their styles match up.Kaden Gfeller vs Anthony AshnaultAskren feels Kaden Gfeller vs Anthony Ashnault was the most exciting match of the Oklahoma State vs Rutgers dual. Gfeller fell to Ashnault 9-6. Askren thinks Gfeller is losing to these top guys because he struggles to generate enough of his own offense. After the win, Ashnault celebrated by doing the pistol fingers at Gfeller. Gfeller then pushes Ashnault at the handshake and both sides got a one-point deduction as a result.Patrick Glory vs Nick PiccininniGlory vs Piccininni was another exciting match this weekend. Glory lost to Piccininni with a final score of 9-4. Askren begs Glory to stop choosing bottom until he gets better. Glory has the potential to place 3rd at the NCAA Finals which would be a remarkable achievement considering the competition and Glory's age.Nick Lee vs Tristan MoranWisconsin's Tristan Moran went to Rec Hall and gave Penn State's Nick Lee his first loss of the season in second sudden victory 12-10. This was a huge upset not only in the rankings but also Moran has been struggling with his weight at the 141 lbs weight class.Jason Nolf vs Ryan DeakinNolf was able to stop all the chatter surrounding his bout with Deakin with a 17-7 major decision victory. Askren talks about how many people believed No. 3 ranked Deakin was the guy to finally beat Nolf. It will be exciting to watch the rest of Nolf's season.Austin Desanto vs Ethan LizakThe final score of 6-1 doesn't tell the story of how dominant No. 10 Desanto was against No. 8 Lizak. Desanto, at the end of the match, shoved Lizak’s head into the mat and then started taunting the crowd. Askren is always in favor of celebrations and showmanship and didn't really have a problem with what happened after the match.

Sure enough, the first full day with the new car, and I have to take my wife’s Pontiac G6 down to Maturi Pavilion. Why? I don’t quite yet know how to park our new monstrosity and her car fits in better places. Yup, it’s a Sunday in college wrestling and we’ve got some real fun stuff to discuss. I’m Jason Bryant and this is Short Time Shots, a look at the day’s happenings in college wrestling from around the world. I don’t have any funny anecdotes to share today, other than man, my fantasy football teams SUCK. Like bad. Horrible. Hashtag #NoOneCaresAboutYourFantasyTeam. I’ll start off at the dual I was at today as No. 4 Oklahoma State pulled away from No. 13 Minnesota 23-9 in front of over 3,000 fans at Maturi Pavilion. We saw two highly anticipated bouts and five overall that featured both competitors in the national rankings. After a rather uneventful Cowboy win at 125 pounds, Daton Fix of Oklahoma State knocked off Minnesota’s two-time All-American Ethan Lizak 6-1 at 133 pounds. Lizak is every bit of 133 and this new weight class should serve him well. Fix and Lizak got involved in some fun scrambles, but a pair of takedowns AND a reversal helped give the Cowboy redshirt freshman the W. I talk with Fix and a bunch of Gophers on The Guillotine Grapevine, which you can also find on the Mat Talk Podcast Network. Bonus wins by Chandler Rogers, Jacobe Smith and Dakota Geer allowed the Cowboys to pull ahead, even after a lost team point during the 133-pound bout. Minnesota did give their fans something to cheer about as Gable Steveson’s first home dual was a big W as he used a first period takedown and set of four nearfall points to top All-American Derek White 8-2 at 285 pounds. Again, hear from Gable, Eggum, Fix and Mitch McKee, who FINALLY beat Kaid Brock in a folkstyle match on the latest edition of The Guillotine Grapevine. No. 2 Ohio State had no Myles Martin and no Kollin Moore, but the Buckeyes had enough to beat Arizona State 22-17 in Tempe. Zahid Valencia of ASU beat TeShan Campbell at 174 pounds, while Micah Jordan registered a solid 18-8 major decision over Josh Maruca at 149 pounds. Lineups weren’t exactly at 100 percent for either team, but No. 5 Michigan topped No. 6 Lehigh 21-13 at Crisler. At 174, Myles Amine topped Jordan Kutler 6-4 at 174 … and really, that’s the only matchup that really had ranked wrestlers. We had an injury default with Malik Amine dropping a match to Jimmy Hoffman and Lehigh’s Jordan Wood majoring Luke Ready at heavyweight. Other than that, nothing really riveting, other than this match was supposed to be wrestled on Friday, but snow moved it to Sunday. Virginia Tech will limp home to Blacksburg after taking two losses on the road this weekend. No. 25 Northwestern used a major decision at heavyweight by Conan Jennings to knock off the 10th-ranked Hokies 20-16 in Evanston. The key match there came at 149 pounds where Shayne Oster upended Ryan Blees 8-7 in the tiebreaker. No. 12 Nebraska topped No. 18 Wyoming 22-12 in Laramie. A couple of interesting results from this dual on both sides. First, Zeke Moisey was upended by Wyoming’s Cole Verner at 125, then Tucker Sjomeling, who was just beaten the other night by Jens Lantz of Wisconsin, well Sjomeling beats All-American Montorie Bridges 13-11. Eye-Yeye-EYE. Branson Ashworth beat Nebraska’s Isaiah White 2-0. Nebraska picked up seven wins. No. 17 Wisconsin had redshirt freshman heavyweight Trent Hillger play hero on two occasions on Sunday. First against Princeton, Hillger earned a first period technical fall to push the Badgers past the Tigers 22-18. A few hours later, an overtime win over Tate Orndorff lifted Wisconsin past No. 21 Utah Valley 22-17. We’ll get to the tournaments in a sec, but we had some D3 action to get around to. John Carroll went 5-0 as it hosted the JCU Duals along with D2, D3 and NAIA teams. The Blue Streaks, and we don’t mean Martin Lawrence, beat Urbana, Waynesburg, Lourdes, Pitt-Bradford and Penn State … Behrend. Speaking of Urbana, the first-year Division II program earned its first win in school history by defeating another first-year program, Pitt-Bradford. Trevor Hankins’ technical fall at 184 pounds clinched the dual for the Blue Knights, who are coached by Seton Hill alum Brett Smith. Ok, tournament time and that’s where top-ranked Penn State rolled. One media professional, who shall remain nameless, told me earlier on Sunday that he appreciates how Penn State helps move a tournament along. Yeah, if Brock Lesnar welcomes people to suplex city, then Penn State wrestling .. wait, Pittsburgh is already calling itself Pinzburgh this year .. hey Penn State faithful, let’s come up with some type of name for Penn State that has some type of geographical marker like “city” or “town” or I dunno, maybe my friend Nate Schy, the Mayor of Bringtown, can come up with something. So Penn State won the tournament for a third straight year, scored 192 points and had EIGHT champions. The only weights they didn’t win were 125 and 149. Drexel was second with 109.5 points and five runners-up. Roman Bravo-Young, Nick Lee, Jason Nolf, Vincenzo Joseph, Mark Hall, Shakur Rasheed, Bo Nickal and Anthony Cassar. Two things MIGHT have gotten sorted out with the Penn State lineup. Brady Berge beat Jared Verkleeren at 149 pounds in a tight one before Berge would medically forfeit the finals. Cassar beat Nick Nevills convincingly in the semis at 285. I remember my first episode of Short Time, “depth is never a bad thing.” There wasn’t much in the way of ranked roadblocks in the finals, but Cassar had a solid major decision win over Drexel’s Joey Goodhart - Lancaster kid! - in the finals. Lock Haven won the Black Knight Invitational. THE BLACK KNIGHT ALWAYS TRIUMPHS! NC State didn’t see much in the way of super crazy battles at the Wolfpack open, but Tariq Wilson did beat Zach Sherman of North Carolina in the finals at 133. Best final of the New York State Intercollegiate championships was at heavyweight, where a pair of Division III All-Americans mixed it up. It was Oneonta State’s James Bethel who came away with the title, beating Ithaca’s Jake O’Brien 5-1. Bethel was a D3 runner-up a year ago. North Dakota State won five titles at the Roadrunner Open. Stanford freshman Shane Griffith, who’s still redshirting, was impressive in winning a title at 165 pounds. Menlo’s Terrill Sidner won at 149 pounds, beating high school phenom Jaden Abas and Junior World teamer Peyton Omania of Michigan State on the way to the title. The Missouri Valley Open took place in Marshall, Missouri for the women on Sunday. Marshall, as we know, is a nice community. There were plenty of notables. Sarah Allen of the University of the Cumberlands topped Wayland Baptist’s Junior world medalist Asia Ray with a second-period fall at 101. McKendree’s Felicity Taylor registered a solid title and a fall over Makayla Welch of King to win the gold at 116 pounds. Brenda Reyna of McKendree topped returning WCWA champion Andribeth Rivera of Campbellsville by fall to claim top honors at 130. Rivera had beaten Ali Howk in the semis, while Reyna topped Arian Carpio of Sunkist in her semi. Junior world champion Maya Nelson mixed it up with the college kids as she topped fellow Colorado native Ashlynn Ortega of King 11-1 in the finals. Ortega topped NYCRTC’s Bri Csontos in the semifinals there. Anna Naylor of the University of the Cumberlands won a 7-7 criteria final over McKendree’s Jasmine Bailey at 155, while veteran Brandy Lowe, who seems to have been around forever, gave McKendree another title with an 8-2 win over Alex Castillo of Campbellsville in the finals at 191. Fantasy football sucks the life out of me. It really does. This is Short Time Shots for Sunday, November 19, 2017 and let’s just say, I’m having more success starting guys like Case Keenum than I am “fantasy studs.” I’m also recording these in one take. If I flub, I flub. Who am I? Hall of Fame announcer Jason Bryant, that’s who! Very limited schedule across the country on this, the Sunday before Thanksgiving here in the United States, but the five Division I duals we saw all involved ranked teams. The question is, where do we start? We had two top 10 duals … ah, let’s start with the upset. And by upset, we mean No. 10 Lehigh pasting No. 4 Michigan at Grace Hall in Bethlehem, Pennsylvania 27-8. On paper, people believed this Michigan team could make a run at the NCAA title. I even mentioned to Richard Immel on Bonus Points on Friday that Michigan could take some dual losses and still come out as a threat. On Sunday, Lehigh wasn’t impressed by the credentials, the hype or the ranking as Pat Santoro’s 10th-ranked Mountain Hawks scored sizable wins from Ian Brown at 157, Jordan Kutler at 174 and Ryan “The Preisch is Right” at 184. At this rate, it’s doubtful anyone’s going to drop a weight, especially Preisch, who topped Domenic Abounader 3-1 at 184 and Kutler, who topped All-American Alec Pantaleo at 157. Scotty Parker using that RP Norley Pennridge strength registered another fall, although it was over Michigan backup Michael Volyanyuk. In Stillwater, we were expecting a tight dual with third-ranked Oklahoma State and sixth-ranked Minnesota. A bunch of matches were tight, but the dual score wasn’t. The Cowboys gave coach John Smith his 400th career dual victory with a 30-3 beatdown of the Golden Gophers. The Cowboys won five matches decided by two points or less. Minnesota’s lone bright spot came at 125 where returning NCAA finalist Ethan Lizak rallied to beat Nick Piccininni 10-9. Piccininni built an 8-1 lead at the end of the first period before Lizak chose top and scored six nearfall points in two different sequences. In the third, Piccininni chose neutral and gave up the go-ahead takedown. A riding time point would be the final difference maker. The rest of the dual -- uffda. Kaid Brock scored four takedowns to beat Mitch McKee 9-7 in a bout that wasn’t as close as the score indicated. Dean Heil needed a third-period takedown to do Dean Heil types of things and beat Tommy Thorn 3-1. No. 9 Virginia Tech looked strong to quite strong in a maroon battle against Central Michigan. The Hokies won eight out of 10 and too both head-to-head ranked matchups to earn the win in Cassell Coliseum. At 149, Solomon Chishko topped Justin Oliver 3-1 in a matchup of past All-Americans while Zack Zavatsky topped Jordan Ellingwood 7-4 at 184. For the second straight time, B.C. “The Headache Powder” LaPrade makes the show. He upset Collin Heffernan 3-1 in sudden victory. Being a Poquoson grad, I can’t give the New Kent guy TOO much pub. Arizona State took center stage, LITERALLY, as the Sun Devils hosted a theater-style dual against No. 25 Pittsburgh. The end result was more tragedy for the Panthers than it was comedy as the Sun Devils won eight of 10 bouts to cruise to a 32-6 win. Coach Zeke “My real name is Larry” Jones saw his team win both of the notable matches as Josh Shields beat Taleb Rahmani at 157 and Tanner Hall beat Ryan Solomon in one of those thrilling escape-riding time 2-0 wins. No. 13 Northern Iowa rebounded from Friday’s loss to win its first Big 12 dual with a 21-19 win over North Dakota State in Cedar Falls. At 141, Josh Alber scored the match-winning individual victory with a technical fall in the final match of the dual. Penn State crowned seven champs at the Keystone Classic at The Palestra hosted by Penn (singular). Zain Retherford won his 100th career bout during the course of the event, while Jason Nolf pinned his way through at 157 pounds. Bo Nickal scored three falls and a tech en route to the title at 184. Other Penn State champs were Jered Cortez at 141, Anthony Cassar at 197, Nick Nevills at heavyweight and Mark Hall at 174, who went fall, fall, tech tech, fall. Kinda like that whole duck-duck-grey duck thing they do out here. I still think it’s duck-duck-goose. Cornell won the New York State Intercollegiate championships, there wasn’t a whole lot of comp to challenge the Big Red this year. Lock Haven won four titles at the Black Knight Invitational at Army West Point -- yes, you kind of have to say the whole “Army West Point” thing now. Something about branding. The Bald Eagles got golds form Kyle Shoop-Shoop-ay-Doop, Chance Marsteller and a pair of Solanco Mules -- Ronnie Perry and Thomas Haines. Lancaster-Lebanon League representing the 717 there. NC State won five titles at the Wolfpack open. Yup. They hosted. Wisconsin won five titles out west at the Roadrunner Open hosted by CSU Bakersfield. Top win there was probably Ricky Robertson’s 3-1 overtime win over Emery Parker of Illinois. In Division III, John Carroll swept the John Carroll duals, earning wins over Case Western Reserve, Penn State Behrend, Lycoming, Alfred State and Lourdes. Gettysburg won three matches at the Green Terror Duals in Westminster, Maryland. Yes, the Green Terror is the nickname of McDaniel College, formerly known as Western Maryland. Coach Andy Vogel’s Bullets topped Division II Alderson-Broaddus 30-14, Penn State-Mont Alto 57-0 and Scranton 27-15. Gettysburg 133-pounder Louie Carusillo picked up the Dundie Award for fastest fall in the dual against Scranton. Cruz won the 125-pound title at the NCAA Division I Wrestling Championships held in St. Louis, Mo., March 16-18. Throuhgout his three-day journey in St. Louis, Cruz defeated Dylan Peters of Northern Iowa, Shakur Laney of Ohio, Sean Fausz of North Carolina State, Thomas Gilman of Iowa and Ethan Lizak of Minnesota to become the 22nd individual to win a NCAA title for Lehigh. Cruz downed the top seed Gilman 4-2 in the semifinals courtesy of a takedown in sudden victory. He secured a 6-3 win over Lizak in the championship match. At the conclusion of his junior campaign for the Mountain Hawks, Cruz owns two NCAA All-American honors and two EIWA conference championships. He finished with a 31-2 record this season. Transcript: [intro music] Host — Dan Keller Hello, and welcome to Episode Seventy-two of Multiple Sclerosis Discovery, the podcast of the MS Discovery Forum. I’m Dan Keller. Today's interview features Nathaniel Lizak, a young Australian investigator from the University of Melbourne who gave the first talk at the recent meeting in New Orleans of the Americas Committee for Treatment and Research in Multiple Sclerosis, or ACTRIMS. Mr. Lizak discusses new findings showing that moderately advanced and advanced multiple sclerosis are more unpredictable than anyone knew, but worsening disability may be slowed by highly effective therapies. But first, let’s look at new content on Msdiscovery.org. Our latest data visualization shows statistical snapshots of MS worldwide. Survey data from the MS International Federation show that, as of 2013, the estimated number of people in the world with MS increased to 2.3 million, or about 33 people with MS for every 100,000 people. But MS rates and access to care vary widely from country to country, as you can see from the data visualization. In a new job posting, the Stanford-affiliated Santa Clara Valley Medical Center Department of Neurology seeks a general neurologist. Subspecialty training in MS, movement disorders, or vascular neurology is preferred. You can post your job at MSDF at no charge to reach researchers and clinicians specializing in MS and related demyelinating disorders. [transition music] And now to our interview. Australian medical student, Nathaniel Lizak, and his academic mentors took a second look at how disease progresses in people with moderate and advanced MS and what can be done about it. Researchers are looking for better measures of disability, but the most common one is the Expanded Disability Status Scale or EDSS for short. Lizak and colleagues looked at worsening disease from several starting points, using an international registry known as MSBase that tracks medical record data on nearly 38,000 people with MS. They divided people in three epochs ranging from EDSS 3.0 (where people are moderately disabled but are fully ambulatory) to EDSS 6.0 and higher (where people need assistance to walk short distances). An older study suggested a steady worsening of disease after EDSS 3.0, which Lizak and co-workers questioned. Executive editor, Carol Cruzan Morton, spoke with him about their findings. Interviewer – Carol Cruzan Morton We are here at the ACTRIMS meeting in New Orleans, and you opened the conference with a really interesting paper. I wanted you to explain a little bit about what you were asking and why. What area of MS, what questions you are addressing? Interviewee – Nathaniel Lizak So we did this work under the MSBase group, which is an MS-based cohorts; it is an international really large study that has data from over 30,000 patients worldwide. We have access to all of their data, and we really thank our contributors throughout the world who have provided this. We decided – because we have so much power with so many numbers and so much data from patients – to look at the latest stages of multiple sclerosis which, so far, haven't really been that well explored. There have been three studies in the past which looked at disability and how it progresses in what they have called the moderately advanced stage of multiple sclerosis. So yeah, we looked at disability accumulation in the later stage of multiple sclerosis moderately advanced, which is defined before as between the EDSS steps of 3.0 or 4.0 and 6.0, and we wanted to look at what predicts how the disability accumulates, because a lot of the previous studies didn't really suggest anything really changes disability. There is this notion amongst doctors that once it hits these thresholds the trajectory is set, and there is nothing you can do to help patients. We didn't believe that. We were hoping there was something you can still do for patients, even once they have already accumulated substantial amounts of disability. We set this up to look at just how much variability there is in these later stages of disease and what we can do to take it from going really fast to going really slow, to preventing patients from getting even worse. We used our cohorts, ran lots of statistics, and we found some very interesting results. The first is that this late stage of disease is quite independent from what happens before. How many relapses people have early in the disease, how fast they got to the early disability landmarks, how fast they accumulated disability, if they were on therapy in the past—all of those things don't really seem to impact what happens later on in the disease. That is what we call the amnesic disease phenomenon. That is something that has already been explored in the past. We kind of confirmed that and saw that, that happens at lots of stages in multiple sclerosis. What is more interesting, though, is that we still found that patients have a lot of variability in what happens to them, even after they have accumulated substantial disability. So in technical terms, after EDSS 3.0, 4.0, and 6.0, there is still a lot of variability in what happens to patients after they have reached these steps, after they have already obtained disability. That suggestion that after the threshold the disease is set doesn't seem to be at all the case. That is all we observed in our patients. We had over 3,400 patients— we had 3,415 patients exactly. So it is quite I think, generalizable, our results. There is a lot of variation in what will happen at these later stages of disease. MSDF You can't predict what happens next. And it is different. Mr. Lizak It is different for everyone. It is independent of what happened before, and almost nothing predicts what is going to happen next. The only things that we found which did predict such as how does disability progress in these later stages, the first one was how many relapses they are having now. Not before, not early in the disease, but how many relapses are they now having per year at these later stages? We found that more relapses later in the disease still contribute to disability. That wasn't something that the other studies had actually shown, and I think that is to do with their methodology more than anything else. I think we are confirming that relapses are still important, inflammation is still important, we still need to treat it, no matter how far along the disability line the patients are. The relapse is still a problem. A more exciting thing that we found was that the immunomodulatory medications that the disease modifying therapies, the higher efficacies ones, the new medications, the longer patients are on those in the later stages of disease—so again, after those landmarks, after EDSS 3.0, 4.0, and 6.0—he longer patients are on those after they have gone into that disability the lower their likelihood of progressing even further to EDSS 6.0 and 6.5, which is mobility issues needing unilateral assistance or a walking stick, EDSS 6.0 or bilateral walking assistance EDSS 6.5. So those are pretty, obviously, important to patients in being able to move around without needing any aid. We found that we can prevent patients from getting to these later disability stages with longer time on disease therapies later on in the disease. So the conclusion we got from this is patients should continue being treated later in multiple sclerosis. Of course, it's always a risk/benefit calculation. You always need to take the side effects into consideration and look at the patient that you are seeing. It is not a blanket rule, but there are countries in the world where it's by policy you can't give therapies later on in disease after EDSS 4.0. New Zealand is one example. In other places in the world, it is just practice to stop giving treatments later in multiple sclerosis, and we are suggesting no there still is a benefit and you should be weighing that up when considering whether or not to continue patients on therapy, whether to start them on stronger therapies. There is evidence that we can still slow down how the disability will accumulate. That was our main message. We were a bit surprised to find out it was not what we were expecting, but we are very happy that we found such results. MSDF In your study, what drugs were categories as the high efficacy? Mr. Lizak I don't remember exactly every single drug, but we just put into two groups. The low efficacy being primarily the initial very first-line drugs, so interferon, glatiramer acetate, and teriflunomide; everything else categorizes as high-efficacy therapy, so natalizumab, fingolimod, alemtuzumab, dimethyl fumarate, cladribine, mitoxantrone, I might be missing a few. By no means are we saying that one therapy is better than another. We're just looking at the class effect of the really strong medications. We don't yet have the power to say this is the best medication after EDSS 6.0, this is the best medication after EDSS 4.0, or don't go on that one. We're just trying to say that the stronger perhaps second-line therapies often used second-line do have a better effect in this later period of disease, and doctors should be considering that when deciding what treatments their patients should go on, and patients should obviously be made aware of that as well. MSDF The idea that things that happen before don't affect the later stages seems on the surface to be at odds with the idea that progressive disease starts early, like treat early. Mr. Lizak …to try to treat disease as early as possible. I don't think it is at odds. There's been a lot of work, so far, to say that the earlier you treat your patients the better. We agree with all of that. We are not saying treating later is any better. Probably believe that treating earlier is better, but what we are saying is: a) continue treating, and don't stop treating. I completely agree that all of the patients that we found that after EDSS 3.0, 4.0, and 6.0 improved with more therapy after those landmarks, still probably did better earlier on in their phase of disease with therapy then. But I guess what we saw is the therapy they had earlier in disease won't make an impact now. You need to continue treating these patients for them to have an improvement. We still absolutely encourage the earlier treatment, the better. That evidence is beyond doubt in multiple sclerosis. We are definitely not challenging that. We completely agree with that. Our evidence just goes and takes it one step further of, the earlier the better, but it is not too late. MSDF Now you are doing this study in the context of a clinical practice. How has that changed – or has it changed – how people with MS are treated in the decision making? Mr. Lizak The thing is, first of all, I am a medical student, so I don't make any of the decisions. Secondly, where we are based in Melbourne, Australia, there is already a tendency to treat patients later on in disease. Obviously we haven't published the results yet, so we haven't seen how much of an affect it will have worldwide. Perhaps now we will begin to start changing things. But in Australia where nothing was studied, no one was surprised to find that this was the case. All the doctors there already treat their patients later on in the disease. So it just confirmed that what they are doing is correct. No one has yet drastically changed how they are treating patients. We hope though that, say countries like New Zealand whereby policy after a certain EDSS score, after EDSS 4.0, after moderate disability has been accumulated, you can't put patients on disease-modifying therapy anymore—we hope that is where we will have the biggest impact. MDSF When you gave your talk, you talked about the earlier study. There wasn’t an appreciation for the variability. How did you come to ask that question in the first place? Mr. Lizak I have to give credit to my supervisor, as well as the whole MS based team that was behind this study, and obviously they conceptualized it a lot more than I: Dr. Thomas Kalincik and Helmut Butzkueven, in particular. But a lot of doctors, particularly our team, are not happy with that graph. Which it looks like everyone after EDSS 3.0 has the same trajectory. We looked at this, and we thought we wanted to do a study to prove this wrong. We didn't know exactly what we were going to find. We actually proved that what they first suggested of disease being independent to be quite correct. But they just missed the variance in the second half. It is independent, but it is still really variable. We looked at the graphs carefully, and we looked at the study carefully, and we made the note of they only have a mean value on that top half, they don't show how much variability there may or may not be in disease. We got confused. We said it is unlikely that patients have no variability at all after EDSS 3.0, and we decided not only are we going to look at what predicts the later disease, but we need to know just how variable is this disease this late, and we found that it is extremely variable. After EDSS 6.0, patients might go straight through to worse disability, and many will improve, and many will stay stable for many years. We were just unhappy with the message that the graph gave. Then we tried to scrutinize exactly where can we change this message, where can we improve this. MDSF That’s great. What questions are your colleagues asking you here about the study? Mr. Lizak I have had a lot of questions about this study, some more helpful than others. A lot of people have asked how will this change management? And I think we have just spoken a little bit about that. I am asked, as well, how do you tell patients that we can no longer predict their disease? We used to think that we could and now we just outright can't predict their disease and that is something that is going to be difficult to tell the patients. I think you need to frame it differently. It is not we can't predict how your disease is going to go, it is, we have hope for making it better. You might have been doing not so well up until now, but we still have hope to continue fighting. We haven't given up yet. And I think that is what we need to be framing it as. That is one of the questions I have gotten the most. A lot of people have asked about why we chose certain therapies, and there is very little evidence about which therapy is high and which therapy is low. We just used the available studies as well as the clinical experience and just compared how much they reduced relapse rates and so on. It was partly based on intuition and observation. It could be the case that some therapies should have been classified differently to what we did, but it is very hard to tell at this stage. Even then, even looking at the list, you should be mindful that a therapy that we classified as high efficacy might have actually been bringing that group down. And maybe should have been a low efficacy therapy, and maybe a low efficacy therapy was the only one working in that group, and it should have been in the high efficacy group. So obviously, be very careful when you look at that. At that strata, it is not meant to be telling anyone I should be on that drug or I shouldn't be on that drug. It is just meant to be saying that strong medications are better in this stage, but the decision of the medication should be a decision made entirely by the patient and their doctor, and it should only be used to influence and it shouldn't be taken any more than that.
