My recent hormone bloodwork: LH 4.2 FSH 2.9 Testosterone 724 Prolactin 24.5 Need I be alarmed about my prolactin? From what I understand it can indicate a serious problem or just be due to endurance training. Over the past 2 years I've been running a lot. I trained for and completed a 50 mile Ultra Marathon and a Marathon. As of late I am lifting (squats 2x a week deads 1x a week). My running mileage is low comparatively to a few months ago. Some pull ups and push ups every day. Besides just being fatigued from training at times I feel and think I perform pretty well. All though I don't notice it work is a stressor and I'm certain that the shift work I do isn't helpful. I plan to follow up about my blood work. Should I be asking for a thyroid blood work? Is it dumb to just attribute the high prolactin to endurance training and low carb eating? Omega 3's Amy writes: To omega or not to omega? That is the question. For context my sleep is dialed in at 7-8 hrs per night, daily 10 min meditation, yoga and fitness, and eat a paleo (+rice) diet. Kavinace by Neuroscience is no longer available as a supplement. April 10th, 2019, the FDA issued a letter to several nutraceutical companies to discontinue supplements containing 4-amino-3phenylbutyric acid. The FDA has determined that 4-amino-3phenylbutyric acid, also commonly referred to as Phenibut is not a dietary supplement. We published a previous Kavinace article shortly after the FDA letters were sent out. As of the letter dated April 10th, the companies had 15 days to comply. This left people that had taken Kavinace safely for years in a bit of a quandary. Kavinace is, I mean, was a dietary supplement that many people took to help with staying asleep. It contained 950mg of a combination of taurine and 4-amino-3-phenyl butyric acid HCL per capsule. Most people did well on one to two capsules taken at night before bed. It wasn't a sleep medication, like Ambien or Unisom but did help to raise GABA to help with staying asleep at night. I used this myself and with patients to stay asleep at night. Commonly in perimenopause and menopause, women have trouble staying asleep. Often they fall asleep easily but will wake up in the middle of the night for hours. And by the time they are able to fall back to sleep, it is time to wake up for the day. Kavinace was very helpful because it contained 4-amino-3-phenyl butyric acid, which is a precursor to GABA. Meaning it is easily digested and can cross the blood-brain barrier to help convert to GABA and stimulate GABA receptors to stay asleep. One of the reasons that sleep is so important is terrible sleep can make you gain weight. People, especially women, will gain weight easily if they do not sleep well at night. Sleep is necessary for so many health reasons, it is also responsible in part for your metabolism. It really goes back to our adrenal glands. The adrenals secrete cortisol in a diurnal fashion. Meaning that cortisol is released highest in the morning and stays up through the day and drops at night so that you can sleep well. Commonly, (especially in hormone imbalance, perimenopause, and menopause) you will see cortisol rise in the middle of the night. That causes us to wake up in the middle of the night for no reason. Other than it is aggravating to be awake for hours in the middle of the night and tired during the day, the elevation of cortisol at night can negatively affect metabolism. Often we tell patients, forget waking up early to go to the gym. Just get that extra hour of sleep. One of the first things that we work on with patients is sleep. Sleeping properly is great for the waistline. Well you might be asking, Kavinace is no longer available, what do I do now? There are many options available to replace Kavinace and sleep well. Back when I first found Kavinace years ago, all we had available to us was GABA. GABA is a huge molecule that is not easily absorbed through digestion, so it was a waste to take. Now we have available, PharmaGABA, which can be absorbed through the digestive tract. PharmaGABA is a great alternative to Kavinace. You can take it as capsules at night or chewable tablets. The FDA has sent 11 companies (so far) for marketing dietary supplements that don’t meet its "guidelines". The agency issued warnings to three companies for marketing dietary supplements containing phenibut, which is sometimes sold as a sleep aid or to treat anxiety. It said that phenibut doesn’t meet the statutory definition of a dietary ingredient, which is generally a vitamin, herb or other natural substance used to supplement the diet. These products are considered adulterated because the FDA has determined that DMHA is either a “new dietary ingredient” for which the FDA has not received the required New Dietary Ingredient notification or that DMHA is an unsafe food additive. Today we're talking to Dr. Kelly Olson, an expert in the area of neuroscience, stress and sleep. We discuss chronic stress and adrenal fatigue, as well as some promising supplements and treatments to support healthy stress response and better sleep habits. Chronic Stress and Adrenal Fatigue We're all aware of the negative impacts of stress, how it has a huge impact on aging and can create a low-grade inflammatory response that contributes to a high percentage of illnesses. Of course, there are also positive aspects to some types of stress. Acute stress can provide a benefit, but you need to be able to control both the physiological and psychological response, particularly as it relates to coming back down afterward. Our bodies are meant to be able to manage this response by bringing down the heart rate and elevated chemical levels associated with stress, but it doesn't always work that way, which leads to chronic stress. Chronic or prolonged stress is associated with adrenal fatigue. But the problem with treating adrenal fatigue is that most practitioners go after the messenger instead of the source, which is akin to going after the symptom vs. the disease. Cortisol released from adrenal glands creates a negative feedback loop. But if the receptors aren't there, as happens in adrenal fatigue progression, the system doesn't work the way it should. Unfortunately, controlling adrenal function is often overlooked by doctors. Biofeedback for stress response has the ability to control the neural circuits, and cognitive behavioral therapy also helps people train themselves to be able to come back down from stress. Getting a Handle on Sleep The excess of chemicals in the brain like norepinephrine, epinephrine and cortisol can contribute to people's inability to calm brain activity for a good night's sleep. And sufficient and restful sleep is crucial for health and longevity – it's our built-in process to combat stress, formulate memories and get the immune system on track. In order to get a good night's sleep, all of the brain chemicals need to be properly balanced and work in concert together. Dr. Olson herself has suffered from cortisol inversion and low serotonin, which prevented her from getting the sleep she needed. Over time, she was able to correct this imbalance through a combination of methods targeting neurotransmitters, including adaptogens, 5-HTP and folate support. Eventually, she was able to get everything into a more normal range. Dr. Olson recommends these supplements for stress modulation: Ashwagandha, CortiCept, Phenibut and melatonin.
