The Allergist
Crossing the threshold of more precise allergy care

The Allergist

Play Episode Listen Later Feb 18, 2025 27:49 Transcription Available

"We use thresholds every day without even thinking about it. When we do a food challenge in our office, whether we know it or not, we're relying on the fact that there is a threshold for every patient. Some are going to be on the first dose, some are going to be at the last. And that's just part of what a threshold is." – Dr. Doug MackFood allergies aren't one-size-fits-all, but we don't talk enough about thresholds—the amount of an allergen it actually takes to trigger a reaction. Should every patient be following the same strict avoidance rules? Or should we be aiming for something more precise? To help us make sense of thresholds, we welcome back Dr. Douglas Mack, pediatric allergist, clinical immunologist, and Vice President of CSACI. Dr. Mack is known for his work on food allergy treatment, including oral immunotherapy, epinephrine use, and—you guessed it—thresholds. On this episode, Dr. Mack and Dr. Mariam Hanna dig into:What a threshold actually is and why it matters in clinical practice – How thresholds shape food challenges, oral immunotherapy, and allergen risk assessments, even when we don't explicitly discuss them. How much allergen is too much? – Why two patients with the same allergy can have vastly different tolerance levels, and why strict avoidance isn't always the best approach.Eliciting dose, cumulative dose, and the numbers that matter most – How clinical trials define reaction thresholds, and how allergists can use these data points to guide patient care.Real-world factors that affect thresholds – How co-factors like exercise, illness, sleep deprivation, and even hot showers can shift a patient's threshold and increase reaction risk.Precautionary labels, airplane bans, and the science behind food allergy risk in everyday life – What research tells us about trace exposures, why precautionary labeling varies worldwide, and how allergists can help patients navigate the gray areas of food safety.Thresholds aren't just theoretical—they impact how we advise patients, assess risk, and tailor treatments. Dr. Mack walks us through the science behind these numbers and how allergists can apply them in daily practice.Visit the Canadian Society of Allergy and Clinical ImmunologyFind an allergist using our helpful toolFind Dr. Hanna on X, previously Twitter, @PedsAllergyDoc or CSACI @CSACI_caThe Allergist is produced for CSACI by PodCraft Productions

E27: [Bonus] Marc Andreessen and Anton Troynikov on Why AI Will Save the World

Today, we have a special bonus episode: a discussion between Marc Andreessen, the co-founder of Andreessen Horowitz, and Anton Troynikov, founder of AI database company Chroma. We're proudly sponsored by Vanta. Get $1000 off Vanta with https://www.vanta.com/zen. This conversation originally took place on Twitter Spaces, where Anton and Marc talked about Marc's recent, provocative essay, Why AI Will Save The World. Marc explains why AI will not destroy the world but instead may save it. Towards the end of the discussion, Marc takes questions from a select audience of entrepreneurs and builders in AI. Given how much we've talked about AI and the various doomer debates on Moment of Zen, we felt that this conversation was too good to be missed. We're so thrilled that Marc, Anton, and a16z allowed us to republish it here. Both Marc and Anton have been on Moment of Zen before (see LINKS) Moment of Zen is part of the Turpentine podcast network. This week we're looking at a new way of thinking about the role of the state in our society: the idea of the ‘precautionary state'. What is it? What are its implications? And is it a good thing? At a time of breakdown in our public health service, unaffordable childcare bills, and a cost of living crisis, questions over how our society should be governed, and what the state should provide, are pressing. Meanwhile, the response to the Covid-19 pandemic and the vulnerabilities in the energy and food supply chains exposed by the war in Ukraine reveal, some think, state failure to plan ahead and make provision, just in case. One person who has thought long and hard about what functions the state should exercise, and how it ought to perform them, is Albert Weale, Emeritus Professor of Political Theory and Public Policy here in the UCL Department of Political Science. Longstanding listeners to UCL Uncovering Politics may remember an episode we did with him a couple of years ago on his major book Modern Social Contract Theory, which explored the principles that should guide decisions on the role of the state. Albert is now building on that foundation to develop a new approach to thinking about the role of the state, which he calls the ‘precautionary state' – one that moves from ‘just in time' systems, to a ‘just in case' approach, with ample provision of public goods. The Lakers went to Oklahoma City on the second of a back-to-back knowing they wouldn't have LeBron James and D'Angelo Russell but also found out for "precautionary" reasons the team would also be without Anthony Davis. But behind a great second half from Dennis Schröder (who rolled an ankle) and solid play from Austin Reaves and Troy Brown Jr., the Lakers leave OKC with a 123-117 victory over the Thunder. Tune in as Stone Hansen from the Upside Swings NBA Draft podcast, Joe Soro from SynBlades.com/lakersball.com, "MagicMan" Sean Grice, and Gerald Glassford from the Lakers Fast Break share thoughts on a big victory for the team, where it leaves them in the standings, why the Lakers sat down AD for the Thunder vs. the Grizzlies, and how the new additions stack up for the long haul. Plus Stone answers your questions on the NBA Draft as he talks about possible Laker pickups and who is generating some early NBA draft buzz. In celebration of 75 years of Independence, Prime Minister Narendra Modi announced free booster doses at government vaccination centres for 75 days from July 15 to September 30, 2022. A few days after this announcement, the news website The Wire published a shocking story on how the Central Drug Standards Control Organisation - the CDSCO - the country's drug regulatory agency- never approved of precautionary doses. In this episode, we speak to Banjot Kaur, the reporter who broke this important story this July. Early this year, a Telangana court asked The Wire to take down several articles related to vaccine policy based on a defamation suit by Bharat Biotech, which manufactures Covaxin. One of these stories pulled down was Banjot's story. She has been writing on science and public health for 12 years, and has worked at Down to Earth Magazine and Times of India.
