Have you ever found yourself on a life or career path that took you completely by surprise? When a plan you'd mapped out so clearly in your head didn't unfold the way you'd imagined — or hoped — it would?Well, my guest in this podcast episode, Lana Manikowski, is no stranger to that kind of turning point. Many women around the world are undecidedly childfree. The reasons can be very diverse and for many women and partners the cause never found. It is a very challenging situation on a personal, relationship based, family, societal, and even evolutionary level. It's against the norms and conventions of society and our evolutionary instincts, dreams, expectations, desires, that are unwanted denied to us. It's a very different scenario to the one of an individual or couple willing not having children. The pain and the mourning of the child that's never been born is beyond imagination. Time can heal such a wound in some women, yet others will struggle for a lifetime. Factors contributing to the way individuals deal with a loss vary and can positively or negatively affect the mourning process, as well as the ability to cope and overcome the hardship. A topic that every woman and man can potentially be confronted with and need to address when starting to consider whether or not to have a child. It is however a taboo topic and one that many avoid to consider, especially when postponing their reproductive health plans and family plans to a later stage in life. One way of securing a more flexible family planning, which is however depending on the country a woman lives, more or less controversial or even forbidden, is egg freezing. All dreams encounter limitations. We desire some limitations to define the scope of our dream. Undesired limitations typically fall into two categories. Real limitations respond to plans to overcome them. For example, real limitations may include finances, time, skills, or more. Windmills, however, are undesired limitations that appear real, but really aren't. Generally, windmills do not respond to logical plans. This episode explores the concept of windmills deeper than previous episodes. I list several typical windmills and describe some illogical and irrational methods of dealing with them. To see things through my animals' eyes means not putting my own experience, feelings, and beliefs onto them. - Nicole Birkholzer :: Projection often means that we're doing something to the animal while seeing the world through our animal's eyes allows us to do something with our animal. :: When our animals speak to us, be aware, that type of behavior is their way of expressing themselves, we have a choice. We can either dismiss them as quirky or annoying or get frustrated with them, or we can choose to stop and listen. Because these furry and feathery guides of ours as simply reflecting back on what's going on in our life and in their life. :: When we run on autopilot. When we're not tuned in. When we are not present, they see it often as their job to trip us up, or lie on our keyboard, or become a no-show at the fence. They make us aware that we're physically there but not available. Therefore, quirky or unexpected behaviors are really your animal's way of asking you to come back into balance. Their quirky behavior is their a way to wake you up. :: Living with animals enriches our lives in many ways. But when the behavior is suddenly unexpected even dangerous their actions become our concern. If your dog has separation anxiety and whines and cries every time you pick up your keys that's an undesired behavior. Or, if your new cat is fighting with your older cat and one of them is not using the litter box, well that's a problem. Or, if you're fostering three bunny sisters and one of the bunnies charges you keeping you away from bonding with the other two, that's not fun. Because you expected a snuggly bunny situation. Or, why does your horse suddenly not step up to the mountain block when usually she stands like a statue? What is going on? :: In these situations, you often reach out to your veterinarian or your trainer. Wondering if there is something physically, wrong. If there is a training issue? You might bring the bunnies back because the one that charges you is just not manageable. Often the vet or trainer can't find a solution because it is not necessarily a training or health issue that's going on most often it's a behavior issue that can't be fixed with treats or medicine. So what is really going on in those situations? :: In most cases, the quirky or undesired behavior relates to some type of unease. Be it unease within you, because you're not breathing, or too occupied. Or, something brings them unease in their environment. I discovered that there are five areas that can influence our life either with ease or unease. :: The five areas are, the people we're surrounding ourselves with can bring ease or unease to us. Our purpose, fulfilling our purpose or feeling we have not yet fulfilled our purpose is something that can bring ease or unease into our life. Our environment, if we love where we live, if we love how we surround ourselves, can bring a lot of ease. But, it can also bring unease if we're not happy in our environment. Then there are the peers, the connection we have to others outside our world. That, of course, can bring a lot of ease or unease into our lives. And the fifth category or area is our health. Because being healthy usually means are at ease, but if we have aches or pains we feel unease. :: If the animal is consistently feeling unease from the first four categories, people, purpose, environment, pack, or herd, the unease can eventually lead to dis-ease. And with that, we came full circle. Unease leads to dis-ease which leads to health issues. :: With this episode, I invite you to start paying attention to your projections. When do you project your own feelings onto the animal and when you're truly feeling your animal is informing you? :: And, start looking into some of the animal behaviors you notice. Are they quirky? Is your animal simply trying to give you a chance to tune in and come from your head back into your body? :: Or, is there something creating unease in your animal's world and for that, you look at the five areas, the people, the purpose, the environment, the herd, or pack, and your animal's health. Hey There, Mama-Friend!! In today's episode, we are talking about how to rephrase your responses when your child is melting down or acting aggressively, so that they are better equipped with how to handle their feelings as they grow into self-regulated humans. I know I'm not the only one who's ever snapped at my kiddo and busted out the one liner, "Use your words!" when my toddler has his whiney voice on, or is screaming because something didn't go his way. And, hey, I'm also guilty of telling my toddler, "Share your toys!" when he's being super 'selfish' and just wanting to hog all his toys to himself. Are you guilty of saying those things, too?! You're not alone! However, I'll let you in on a little secret. Saying these quick, one-liner phrases to our kids may address the behavior, but it doesn't actually teach our kids how to fix the behavior, leaving them with not knowing how to handle their emotions or the situation after that! When we don't teach our kids how to communicate, their behaviors may actually get worse because they know they are in the wrong, but they don't know how to fix it; in turn, this could lead to a bigger meltdown or a more aggressive behavior...and we don't want that, do we?! So, grab a pencil and paper, because we are going to learn how to rephrase our responses to help our kiddos learn how to communicate their emotions and desires in a kinder way, leaving you yelling a bit less and filled with a little more peace throughout your day! When it comes to Social media the algorithms run the show. Every social platform want to engage their users and provide an experience that is enjoyable and fun. In effort to create this environment, the algorithms that have been created are designed to show you more content like content that you have engaged with in the past. This algorithm acts like a filter that allows for certain types of post to be shown to you and keeps other posts away from you. You're brain has a system that acts and responds very similarly to these social media algorithms. Every second your senses gather up to 11 million bits of information around you, but the conscious mind can typically only process 50 bits of information per second. Therefore, it needs a filter to manage this information to prevent overwhelm. What makes it though the filters is determined by the part of the brain called the reticular activating system. The reticular activating system is responsible for a number of functions, including sleep and behavior modification. It also acts as the gatekeeper of information. This allows the brain to ignore meaningless and repetitive information and remain sensitive to other inputs. In today's episode we discuss why your brain's reticular activating system will keep you stuck in a negative loop and how to get out of that cycle and achieve your desired results. It's extremely disempowered when you are stuck in this cycle. At times it may feel too heavy for a change to be possible - which leads to feeling hopeless. Yet, God did not create you to operate in this manner. He created you to be the person that is described in Psalm 1 - the person who is like a tree planted by water, which yields it's fruit in season and whose leaf does not wither. Whatever they do prospers. Sounds good right? Let's discover how you can use your reticular activating system to your advantage instead of disadvantage. Reasons for this, and whether there are reasons common to substance and non-substance reward, remain elusive. We present a computational model of common potential mechanisms underlying the difficulty in resisting habitual behavior to obtain reward. Consider that a person has long been regularly taking a series of actions leading to a purchase of alcohol, cigarette, or betting ticket without any hesitation. Referring to the recently suggested representation of states by their successors in human reinforcement learning as well as the dimension reduction in state representations in the brain, we assumed that the person has acquired a rigid representation of states along the series of habitual actions by the discounted future occupancy of the final successor state, namely, the rewarded goal state, under the established non-resistant policy. Then, we show that if the person takes a different policy to resist temptation of habitual behavior, negative reward prediction error (RPE) is generated when s/he makes "No-Go" decisions whereas no RPE occurs upon "Go" decisions, and a large positive RPE is generated upon eventually reaching the goal, given that the state representation acquired under the non-resistant policy is so rigid that it does not easily change. In the cases where the states are instead represented in the punctate manner or by the discounted future occupancies of all the states (i.e., by the genuine successor representation), negative and positive RPEs are generated upon "No-Go" and "Go" decisions, respectively, whereas no or little RPE occurs at the goal. We suggest that these RPEs, especially the large positive RPE generated upon goal reaching in the case with the goal-based reduced successor representation, might underlie the difficulty in cessation of undesired habitual or addictive behavior to obtain substance and non-substance reward. Dr. Allen Ho is a fellowship-trained head and neck surgeon who focuses on head and neck tumors, including HPV(+) throat cancers and thyroid malignancies. As director of the Head and Neck Cancer Program and co-director of the Thyroid Cancer Program, he leads the multidisciplinary Cedars-Sinai Head and Neck Tumor Board, which provides consensus management options for complex, advanced cases. Ho’s research interests are highly integrated into his clinical practice. His current efforts lie in cancer proteomics, HPV(+) oropharyngeal cancer pathogenesis, and thyroid cancer molecular assays. He has presented his research at AACR, ASCO, AHNS, and ATA, and has published extensively as lead author in journals that include Nature Genetics, Journal of Clinical Oncology, Cancer, and Thyroid. Ho serves on national committees within the ATA and AHNS, and is principal investigator of a national trial on micropapillary thyroid cancer active surveillance (ClinicalTrials.gov ID: NCT02609685). He maintains expertise in transoral robotic surgery (TORS), minimally invasive thyroidectomy approaches, and nerve preservation techniques. Ho’s overarching mission is to partner with patients to optimize treatment and provide compassionate, exceptional care. Weighing treatment options for thyroid cancer, with deep consideration for the patient’s lifestyle, could become the new norm in assessing whether surgery is the best path. Dr. Allen Ho states, “if a patient is a ballerina or an opera singer, or any other profession that could be jeopardized due to undesired consequences of thyroid cancer surgery, then the best treatment path maybe active surveillance.” Undesired consequences of thyroid cancer surgery could be vocal cord paralysis, damage to the parathyroid glands resulting in calcium deficiencies, excessive bleeding or formation of a major blood clot in the neck, shoulder nerve damage, numbness, wound infection, and mental impairment due to hypothyroid-like symptoms. Or in the case of a ballerina, undesired scarring could jeopardize a career. The above risks occur in approximately 10% of thyroid cancer surgeries. Although, some thyroid cancer treatment centers have a much more reduced incidence of undesired consequences, while others much higher. In order to address the above and remove the risk of thyroid cancer surgery, Cedars-Sinai has become the first west coast hospital to launch an active surveillance study as optional treatment for thyroid cancer. The study includes 200 patients from across the country who have chosen the wait and see approach rather than hurry into a surgery that could result in undesired, major life changes. The story of Jacob is one of the most fascinating and interesting of all the characters in the bible. Jacob was the younger of two brothers. He tricked his brother into selling his birthright. Then later he and his mother tricked his father into passing the blessing to him in place of his brother. WHen all this was found out, Jacob had to flee for his life. It is at this point that our scripture takes place. Jacob meets Rachel the well and he was smote with her. Rachel, was beautiful, and Jacob worked for Laban for seven years to earn Rachel’s hand in marriage. Jacob fulfils his end of the bargain, but Laban tricks him and sends his older daughter Leah to be married. Leah was not pretty. Given Jacob’s history of cheating and trickery, we might say that he got what he deserved. Rachel represented everything Jacob wanted. He saw in her all the desires of his heart fulfilled. Leah represents the undesired things in life. We all plan for Rachel, but there are times that we wake up with Leah. No matter who we are, we will all wake up to a Leah at times. Often in life we get what we did not choose. What do we do when we wake up and find Leah? We can get mad.. We can fight it. We can run from it. We can ignore it and pretend it didn't happen. Or we can allow Leah to do what Leah is supposed to do. Leah produced more things in Jacob’s life than Rachel ever did. We generally do not look for God in the Leah times. Seldom do we look at Leah and thank God for her. What do we learn from Jacob? Jacob asks Laban why have you deceived me? The word used in the Hebrew is the same one used to described what He and his mother had done to Isaac. Most bible scholars believe that this was a revelation to Jacob. In addition to the revelation, through Leah came the most fruitful blessings to God’s people. The truth is that we most often find God not in our pleasure, but in our pain. The lineage of Jesus is traced back to Leah. Rachel only gave Jacob two children. Neither of them amounted to any more in the history of time. By the end of his life, he did not request to be buried with Rachel, he ordered that he be buried with Leah because he had come to understand the blessing that she was in his life. It is through the Leahs in life that God brings about the blessings and growth in our life.

Weighing treatment options for thyroid cancer, with deep consideration for the patient’s lifestyle, could become the new norm in assessing whether surgery is the best path. Dr. Allen Ho states, “if a patient is a ballerina or an opera singer, or any other profession that could be jeopardized due to undesired consequences of thyroid cancer surgery, then the best treatment path maybe active surveillance.” Undesired consequences of thyroid cancer surgery could be vocal cord paralysis, damage to the parathyroid glands resulting in calcium deficiencies, excessive bleeding or formation of a major blood clot in the neck, shoulder nerve damage, numbness, wound infection, and mental impairment due to hypothyroid-like symptoms. Or in the case of a ballerina, undesired scarring could jeopardize a career. The above risks occur in approximately 10% of thyroid cancer surgeries. Although, some thyroid cancer treatment centers have a much more reduced incidence of undesired consequences, while others much higher. In order to address the above and remove the risk of thyroid cancer surgery, Cedars-Sinai has become the first west coast hospital to launch an active surveillance study as optional treatment for thyroid cancer. The study includes 200 patients from across the country who have chosen the wait and see approach rather than hurry into a surgery that could result in undesired, major life changes. About Dr. Allen Ho Allen Ho, MD, is a fellowship-trained head and neck surgeon who focuses on head and neck tumors, including HPV(+) throat cancers and thyroid malignancies. As director of the Head and Neck Cancer Program and co-director of the Thyroid Cancer Program, he leads the multidisciplinary Cedars-Sinai Head and Neck Tumor Board, which provides consensus management options for complex, advanced cases. Ho's research interests are highly integrated into his clinical practice. His current efforts lie in cancer proteomics, HPV(+) oropharyngeal cancer pathogenesis, and thyroid cancer molecular assays. He has presented his research at AACR, ASCO, AHNS, and ATA, and has published extensively as lead author in journals that include Nature Genetics, Journal of Clinical Oncology, Cancer, and Thyroid. As it relates to first responders operating in emergency situations, their behaviors can be classified into one of two categories: Desired and undesired. Desired behaviors are those rooted in safety and best practices. Hopefully these are the behaviors taught during basic training and reinforced during on-going training and promoted during emergency responses. Undesired behaviors are those rooted in unsafe behaviors and practices. These can be taught in training or can develop over time as a result of drifting away from best practices. Behaviors are encouraged or discouraged with reinforcement. Reinforcement can be positive (i.e., providing something desirable or withholding something undesired) or negative (i.e., withholding something desired or providing something undesired). Behaviors (both positive and negative) are influenced by reinforcement (both positive and negative). Let me provide you with a few examples of how these concepts marry up. Positive reinforcement of desired behavior: A first responder performs tasks safely and consistent with best practices and is publicly acknowledged and recognized by a supervisor for the behavior. Negative reinforcement of undesired behavior: A first responder performs tasks that are unsafe and inconsistent with best practices and is privately counseled with a warning that continuing the undesired behavior will result in a punitive consequence. Negative reinforcement of desired behavior: A first responder performs tasks safely and consistent with best practices and is admonished or ridiculed by a supervisor for performing the task with a primary consideration for personal safety. Positive reinforcement of undesired behavior: A first responder performs a task that is unsafe and inconsistent with best practices and a supervisor who is aware of the behavior fails to address it.
