    I almost finished my July 4th call week. Good news! Portland did not blow its face off this year. No open globes, no eyelid lacerations, no thermal injuries, no sclopetaria. Bad news: a Friday holiday means every unanswered call from every patient at every one of our offices lands on the on-call doctor, and I earned every one of them. I walk through three cases. First, a cataract surgery my partner had to abort mid-procedure because the patient had six clock hours of zonule laxity. I explain what zonules actually do, why pseudoexfoliation ruins your day, and how a viscoelastic-driven pressure spike at 1 AM sent me to clinic to "burp the wound" instead of shipping the patient to the ED. Second, an inpatient consult for suspected optic disc swelling, and why nobody outside ophthalmology reliably examines an optic nerve. Third, the classic infectious disease vs. ophthalmology debate: does every asymptomatic patient with candidemia need a dilated eye exam? IDSA 2016 says yes. The American Academy of Ophthalmology says no. I'll tell you where I actually land and why I still show up anyway. Also: my full case for licensing fireworks rather than banning them, and a little pushback on the once-a-year concern for veterans and dogs. Takeaways: Zonule laxity, loose or broken suspensory fibers that hold the natural lens in place, can force a cataract surgeon to abort mid-procedure and refer to a retina specialist for a safer staged approach; pseudoexfoliation is one of the most common causes The viscoelastic gels used during cataract surgery can plug the eye's drainage system and cause an IOP spike that lasts up to 72 hours; oral Diamox is standard, but if the patient can't keep it down, options are IV Diamox in the ED or manually burping the wound in clinic Ophthalmologists are effectively the only clinicians reliably trained and equipped to examine the optic nerve, which is why hospitalist consults for suspected disc swelling should be treated as a normal part of the job, not an imposition The IDSA (2016) recommends dilated exams for all non-neutropenic candidemia patients within one week of diagnosis; the American Academy of Ophthalmology recommends against routine screening in asymptomatic patients. (Adam@TheWorldview.com) By Adam McManus Finnish MP, convicted for quoting Bible, denied transit through London airport Finnish Member of Parliament Päivi Räsänen, who was recently convicted by Finland's Supreme Court for quoting the Bible, has been told that she cannot go through London's Heathrow Airport due to her alleged “hate crime” conviction, reports LifeSiteNews.com. Writer Rod Dreher broke the news on his Substack on Wednesday. The incident has not yet been detailed by any other outlets, but Dreher's report was reposted on X by the senior counsel of Alliance Defending Freedom, which has defended Räsänen in court over the years. War Secretary Pete Hegseth publicly prayed to King Jesus on July 4th On Independence Day, War Secretary Pete Hegseth delivered this extraordinary prayer at the White House. Listen. HEGSETH: “King Jesus, we come humbly before Your throne, praising You for all the providence You've bestowed upon this nation. For over 250 years, from George Washington to President Trump, we have dedicated this nation to one nation under God, and certainly, in God we trust. “And as we think 250 years ago to George Washington on bended knee in Valley Forge, appealing to Heaven on behalf of his troops in this young nation. We've appealed to You from the beginning, and may we continue to appeal to You. And tonight, we pray for our President, President Trump. Give him safety, give him wisdom. “Lord, we pray for our Vice President, our First Lady, our Second Lady, the cabinet members here, all the governors assembled here as well, our civil magistrates, Lord God. In Psalm 35:18, King David offers this prayer to God: “I will give You thanks in the great assembly; among the throngs I will praise You.” Students for Life gives Congress an ‘F' after Planned Parenthood's funding resumes Pro-lifers across America are reeling over the Republican-controlled Congress's failure to keep Planned Parenthood defunded. Now, Students for Life has taken the dramatic step of notifying every member of Congress that they have received a failing pro-life grade as a result, reports LifeSiteNews.com. Last July, President Donald Trump signed into law his “One Big Beautiful Bill”, a wide-ranging policy package that included a one-year ban on federal tax dollars going through Medicaid to entities that commit abortions for reasons other than rape, incest, or supposed threats to the mother's life.  While it denied millions to Planned Parenthood, pro-lifers warned from the start that it was only a temporary solution and urged Congress to enact a more permanent measure ahead of the provision's July 4, 2026 expiration date. Maine's Democratic Senatorial candidate suspends campaign Democratic Maine Senate candidate Graham Platner ended his campaign on July 8th after facing a detailed allegation of sexual assault, an astounding and rapid fall for the leftist oysterman whose dramatic rise and grassroots support helped him overcome establishment opposition to secure his party's nomination, reports Politico.com. But a persistent accumulation of controversies weighed on Platner's campaign for months. Monday marked a turning point: Democratic leadership, many of Platner's staunchest leftist backers, and key groups who had been helping fund his bid withdrew their support and pressured Platner to drop out after POLITICO reported a new allegation of sexual assault. Since his withdrawal from the race, several Democrats have expressed interest in running against Republican incumbent Senator Susan Collins. Appearing on Fox News, Democratic Senator John Fetterman of Pennsylvania did not pull any punches and spoke directly to Graham Platner on national television. FETTERMAN: “A woman credibly accused you of raping her. He will be remembered as an accused rapist. He's always been a dead man walking.” FOX NEWS HOST: “He choked up a couple of times during that announcement. Do you think he thinks he's the victim here?” FETTERMAN: “Oh, yeah, definitely. Bro, you will only be remembered as the accused rapist that got pushed out of your election. That's your legacy, bro. You are a guy that cheats on your wife. You're the guy that roughs up your ex-girlfriends. That's your legacy, bro.” Trial of Charlie Kirk's alleged shooter reveals testimony from trans roommate In the trial involving Charlie Kirk's September 10, 2025 assassination, suspected shooter, Tyler Robinson, faces several charges including aggravated murder, a capital offense, reports the New York Post. On July 9th, Robinson's preliminary hearing entered Day 4 in Provo, Utah, as prosecutors played redacted video testimony from his ex-lover, Lance Twiggs, a man who pretends to be a woman named Luna. Twiggs was Robinson's former roommate and romantic partner, who was granted "use immunity" for his statements to investigators. Twiggs revealed Robinson's alleged tearful confession after returning home from killing Charlie Kirk — hours before turning himself in. In never-before-seen footage, Twiggs told prosecutors on July 9th that Robinson was walking around their apartment before breaking down and declaring he “wishes he hadn't done it.” The video showed prosecutor Ryan McBride interviewing Robinson's partner, who described Robinson's alleged regret when they spoke in person on September 11 — one day after Kirk was killed. McBRIDE: “Did he talk about what he had done?” TWIGGS: “Didn't go into detail. I just asked him, in person, if what he said was true the night before, and he said it was. He started crying a little bit, and said he wishes he hadn't done it.” Proverbs 12:17 says, “An honest witness tells the truth, but a false witness tells lies.” As related texts were read aloud, Kirk's widow, Erika Kirk, was heard crying in court, according to the Daily Mail. The video and accompanying texts — shown for the first time in court during a preliminary hearing — offered the strongest picture yet of how the accused lone gunman allegedly confessed to the assassination. 31-year-old Charlie Kirk was fatally shot in the neck during a Turning Point USA event at Utah Valley University.
