Today, we're speaking to Dr Sam Merriel, a GP, and NIHR Academic Clinical Lecturer in General Practice based at the University of Manchester.Title of paper: Factors affecting prostate cancer detection through asymptomatic PSA testing in primary care in England: Evidence from the 2018 National Cancer Diagnosis AuditAvailable at: https://doi.org/10.3399/BJGP.2024.0376Asymptomatic, informed choice prostate specific antigen (PSA) testing occurs in primary care in the UK in the absence of a national prostate cancer screening programme. This study shows that four fifths of prostate cancers are diagnosed following symptomatic presentation rather than from asymptomatic PSA testing. There is a 13-fold variation in asymptomatic PSA test detected prostate cancer between English GP practices, without clear explanatory practice-level factors. Patient factors amongst men diagnosed with prostate cancer, including ethnicity, age, deprivation, and multi-morbidity, have a significant impact on the likelihood of being diagnosed following asymptomatic PSA testing.

Astellas and The Prostate Cancer Foundation teamed up in September 2022 (Prostate Cancer Awareness Month) to provide free Prostate Specific Antigen (PSA) testing. Twelve private hospitals took part in this initiative and a total of 1 256 PSA tests were carried out. The PSA test is a blood test to assist in the detection of prostate abnormalities. The aim of this initiative was to raise awareness about the risks of prostate cancer and to encourage men to consider age and risk appropriate screening to ensure the early detection of prostate cancer. Laut Institut für Qualität und Wirtschaftlichkeit im Gesundheitswesen ist eine Standard-Früherkennung mit PSA-Test nicht empfehlenswert. Das zeigt jetzt die Auswertung von elf Studien mit mehr als 400.000 Teilnehmern.

Prostate cancer is the commonest cancer in men and is the third commonest cause of cancer death in the UK, causing more deaths than breast cancer. Monique Roobol, hoogleraar Besliskunde in de urologie aan het Erasmus MC te Rotterdam. “Er zijn te veel doorverwijzingen en er is te veel onnodig ingrijpen in de tweede lijn.”

An elevated PSA level in a patient with a history of cancer doesn't necessarily mean the cancer has recurred. There are no specific normal or abnormal PSA levels, but the higher the PSA level, the more likely cancer is present. When PSA levels continue to rise over time, other tests may be ordered.
