This EP features Wayne Diesel.  Wayne is a Physiotherapist born in South Africa. In the late 1990s, he worked in private physiotherapy at the Sports Science Institute of South Africa, where he worked with several National Teams, including Women's Gymnastics, Men's Hockey, Football, and Rugby, as well as the African, Olympic, and Commonwealth Games. In 2002 he moved to the UK to work for Gloucester Rugby Football Club. Carl Reader has a BSc (Med)(Hon) Biokinetics (Exercise Therapy) Degree from the Sports Science Institute of South Africa.  Based in Cape Town, South Africa he has over 20 years of experience and expertise in helping golfers of all levels optimize their functional movement patterns to move and play better. Carl makes a second appearance on our podcast to discuss his theory on "Right Side Posture Bias" and how that can adversely affect a golf-swing. As he elaborates on the concept he illustrates the issues it creates: Improper posture, aim and alignment An inability to get to the lead or front side in the down-and-through swing, and An inability to properly and powerfully clear the hips through impact As he addresses those maladies he talks about the mindset of golfers who suffer from Right Side Posture biases.  He also illustrates how this syndrome is oftentimes the cause of lower back pain. Episode 64: Professor. Tim Noakes - Fighting For Fat Professor Tim Noakes is the most prolific scientist and medical doctor in the world on the topics of exercise science and sports nutrition. In this podcast, Prof. Noakes joins me to talk about his research in to the ketogenic diet, the legal challenges he has faced questioning the currently 'accepted' nutrition advice, how he reversed his own type 2 diabetes, the beneficial health effects of eating a high fat diet and why processed and refined carbohydrates are so harmful to health. We also talk about why he decided to become a signatory on the "Settling the Virus Debate" challenge headed up by Dr. Sam and Dr. Mark Bailey, Dr. Tom Cowan, Dr. Andrew Kaufman and others. We discuss some of the issues surrounding germ theory and how he came to realise that there are some serious fundamental flaws in the science relating to virology.  Prof Noakes studied at the University of Cape Town (UCT), obtaining a MBChB degree and an MD and DSc (Med) in Exercise Science. He is now an Adjunct Professor at the Cape Peninsula University of Technology (CPUT) , following his retirement from the Research Unit of Exercise Science and Sports Medicine. In 1995 he was a co-founder of the now prestigious Sports Science Institute of South Africa (SSISA). For the final 15 years of his academic career he was rated an A1 scientist - the highest possible rating indicating a world-leading scientist - by the South African National Research Foundation. In 2008 he received the Order of Mapungubwe, Silver, from the President of South Africa for his “excellent contribution in the field of sports and the science of physical exercise”. Prof Noakes has published more than 750 scientific books and articles. He has been cited more than 21 000 times in scientific literature and has an H-index of 77. He has won numerous awards over the years and made himself available on many editorial boards. Episode Highlights [03:09] Professor Noakes' Career Professor Noakes went to medical school before shifting to research and science.  He started writing the Lore of Running for the public, which is currently ongoing with its fifth revision.  Originally a supporter of the high-carbohydrate diet, he realised he was giving the wrong dietary advice after struggling with Type II Diabetes. [07:50] Against the Food Industry Professor Noakes lost funding from a company in the food industry after he stopped promoting their grain-based diet.  He was sued for his book, Waterlogged, because it went against the sports drink industry.  [12:30] The Pressure from the Academe  Academic mobbing is the pressure to quit your academic work. Professionals such as deans and professors went against Prof Noakes.  Professor Noakes chose research and science because of the toxic environment in the field of medicine. The academic and medical profession is brutal, with long work hours and debt. [15:39] Diet Guidelines Professor Noakes recommends eating based on what humans have evolved to eat. Historically, humans have hunted big animals and turned to agriculture for food. While reasonably healthy, vegetarian diets can be expensive and require highly refined food. He advises eating non-processed and animal-based food. [19:17] The Human's Digestive Evolution The human diet used to be mainly animal produce. Carbohydrates were only eaten during shortages. It isn't easy to adapt from an animal-based, high-fat diet to a high-grain diet in a short period.  Gorillas have certain digestive mechanisms that can break down carbohydrates into fat. Humanity's evolution as hunters resulted in a shorter bowel that is ideal for eating animal produce. [23:47] On Long-Distance Running Running is not a sustainable activity to bring into older age. Orthopedic problems can develop, especially after decades. It is not an ideal long-term activity. [26:22] Weight Control Removing carbohydrates helped Professor Noakes control his weight. He also found amazing benefits from his biweekly CrossFit sessions complemented with running. Now, he has more muscle mass and better flexibility than before.  [30:52] Professor Noakes' Writing Professor Noakes is rewriting the Lore of Running to debunk myths and misconceptions. He is also working on an article about eating before or after exercise. He recently discovered the reason why carbohydrates are necessary for exercise. Regardless of your diet, avoiding hypoglycemia during exercise is the main goal. [37:05] Preventing Cancer Through Diet The low-carb, high-fat diet is ideal for preventing cancer. Insulin resistance, which can develop from a long-term high-carb diet, underlies most cancers. [40:30] Busting Myths  The central government theory is true. Reduction of exercise performance to reduce lactate and glycogen is a myth. Measuring oxygen consumption and glycogen muscle storage is unimportant. His friend once subscribed to a high-carb diet before switching to low carbs. His running greatly improved. Dave Scott, a well-known athlete, has also seen improvements after changing his diet. [45:39] What to Drink When Running Homeostatic regulation allows you to determine if you need to drink more to keep normal sodium levels. Drinking too much water can cause hyponatremia, which is when your sodium levels drop, causing brain swelling and reduced brain blood flow. A genetic defect causing the oversecretion of hormones can cause you to drink too much. It is best for marathon runners to drink only when they feel thirsty. Dehydration is not a medical condition. And this is not just a blanket statement. Prof Noakes goes through why this is the case.  I was super nervous before chatting to Prof Noakes. Afterall, how often do you get the chance to talk to and thank your hero? But instantly his kindness put me at ease, and we spent almost an hour together talking about this mans legacy. He shared where he is now after the horrendous 4 years of being bullied, tormented, then put on trial. You will be appalled at the way he was treated, and if it wasn't for his wife.. well you listen for yourself. Thankfully he was exonerated, and so was the low carb diet. If there is one of my podcasts that is a must listen, this is the one. Prof Noakes studied at the University of Cape Town (UCT), obtaining a MBChB degree and an MD and DSc (Med) in Exercise Science. He is now an Emeritus Professor at UCT, following his retirement from the Research Unit of Exercise Science and Sports Medicine. In 1995 he was a co-founder of the now prestigious Sports Science Institute of South Africa (SSISA). He has been rated an A1 scientist by the National Research Foundation of SA (NRF) for a third 5-year term. In 2008 he received the Order of Mapungubwe, Silver, from the President of South Africa for his “excellent contribution in the field of sports and the science of physical exercise”. Prof Noakes has published more than 750 scientific books and articles. He has been cited more than 19 000 times in scientific literature and has an H-index of 71. He has won numerous awards over the years and made himself available on many editorial boards. He has authored many books. In 2003 he received the UCT Book Award for Lore of Running (4th Edition), considered to be the “bible” for runners. Since 2011 he has written his autobiography, Challenging Beliefs: Memoirs of a Career, published Waterlogged: The serious problem of overhydration in endurance sports (in 2012), published The Real Meal Revolution (in 2013), co-authored with Jonno Proudfoot, David Grier and Sally-Ann Creed, and following that the child-focused version of this book, Raising Superheroes (in 2015). The Institute was built to provide a facility that would primarily fund research in sports performance. The application of this research would provide sports personnel of all disciplines with the means to improve. Noakes and du Plessis also wanted to use it as a platform to build public interest in the country's top sports people and build state pride.Prof Noakes has published more than 750 scientific books and articles. Following this, he studied at the University of Cape Town (UCT) and obtained an MBChB degree in 1974, an MD in 1981 and a DSc (Med) in Exercise Science in 2002. In the early 90s, Noakes teamed up with Morné du Plessis to drive the founding of the Sports Science Institute of South Africa (SSISA). The Institute was built to provide a facility that would primarily fund research in sports performance. The application of this research would provide sports personnel of all disciplines with the means to improve. Noakes and du Plessis also wanted to use it as a platform to build public interest in the country’s top sports people and build state pride. Prof Noakes has published more than 750 scientific books and articles. He has been cited more than 19 000 times in scientific literature, has an H-index of 71 and has been rated an A1 scientist by the National Research Foundation of South Africa for a second 5-year term. He has won numerous awards over the years and made himself available on many editorial boards. He has a passion for running and is still active, running half marathons when he can. He is a devoted husband, father and grandfather and now, in his retirement, is enjoying spending more time with his family. The latter two are now also published overseas. In 2014 he co-wrote Always Believe in Magic, with Kevin Musikanth and Jonathan Kaplan, which is the story of the UCT Ikey Tigers journey to winning the 2014 Varsity Cup. The Banting Pocket Guide was published in 2017, co-authored with Bernadine Douglas and Bridgette Allan and most recently he has co-authored Lore of Nutrition with Marika Sboros. This details his journey from prosecution to innocence. Following the publication of the best-selling book, The Real Meal Revolution, he founded The Noakes Foundation, the focus of which is to raise funding to support high quality research of the eating plan described in the book. "It is completely unethical for any medical doctor or any dietitian to tell a patient to eat less saturated fat."  There is a big message in this for you if you are a medical doctor, Dietitian or anyone advising people on what to eat. And this is not just a blanket statement. Prof Noakes goes through why this is the case.  I was super nervous before chatting to Prof Noakes. Afterall, how often do you get the chance to talk to and thank your hero? But instantly his kindness put me at ease, and we spent almost an hour together talking about this mans legacy. After his research convinced him that a high fat, low carb diet is the healthiest option for many, Prof Noakes founded The Noakes Foundation in 2012. Prof Noakes retired as Professor of Exercise and Sports Science at the University of Cape Town (UCT) in 2014 and is now an Emeritus Professor at UCT. While his career has seen him involved in some controversial circumstances, love him or hate him, you cannot deny his contributions to scientific research. Martin is a clinical performance nutritionist and the founder of Mac-Nutrition, a thriving nutrition consultancy boasting sought after long-term internship and weekend mentorship programs. This week welcome to the show Tim Noakes.  Prof Noakes was born in Harare, Zimbabwe in 1949. As a youngster, he had a keen interest in sport and attended Diocesan College in Cape Town. Following this, he studied at the University of Cape Town (UCT) and obtained an MBChB degree in 1974, an MD in 1981 and a DSc (Med) in Exercise Science in 2002. In the early 90s, Noakes teamed up with Morné du Plessis to drive the founding of the Sports Science Institute of South Africa (SSISA). The Institute was built to provide a facility that would primarily fund research in sports performance. The application of this research would provide sports personnel of all disciplines with the means to improve. Noakes and du Plessis also wanted to use it as a platform to build public interest in the country’s top sports people and build state pride. Prof Noakes has published more than 750 scientific books and articles. He has been cited more than 16 000 times in scientific literature, has an H-index of 71 and has been rated an A1 scientist by the National Research Foundation of South Africa for a second 5-year term. We get into that today, and over the five years since, I think, this has happened what he's been up to and his fights against type II diabetes as well in South Africa. I can't believe it's been over three years since you last come on, and I have no doubt a lot has happened since then that we can get into today. Tim Yeah. Guy Tim, there's a question that we ask everyone on the show at the moment and that is, if a complete stranger stopped you on the street and asked you what you did for a living, what would you say? Tim At present I'm retired, so I do what I want to do. Essentially, I'm finishing up a five-year trial against me on the basis that I gave information on Twitter, which was considered unconventional. What we did was, we put the low-fat diet on trial. We said, "Okay, if you're going to try me for this nonsense, let's see whether there's any evidence that the low-fat diet is healthy." Malcolm Collins is professor of human biology at the University of Cape Town and a chief specialist scientist at the South African Medical Research Council. He is one of the leading researchers at the Exercise Science and Sports Medicine Research Unit, which is located at the Sports Science Institute of South Africa, a multidisciplinary institution committed to promoting sporting excellence and the health of Africans. This podcast complements BJSM's recent chat with Professor Tim Caulfield [http://bit.ly/10xfp8w], but focuses more on sports injuries and talent identification in sport. Professor Collins, who discovered the first gene variants associated with chronic tendinopathies, explains the implications for clinical care and screening.
