A new study entitled, “Coenzyme Q10 + alpha lipoic acid for chronic COVID syndrome” has been published in the journal Clinical and Experimental Medicine which found that supplementation with Coenzyme Q10 (CoQ10) and alpha lipoic acid (ALA) may be helpful in Long COVID. COVID-19 can deplete CoQ10 levels and damage the mitochondria which are important for energy production and immune system function. CoQ10 and alpha lipoic acid can both be helpful in protecting and supporting mitochondrial function by reducing oxidative stress. CoQ10 deficiency can lead to decreased energy production resulting in fatigue and increased free radical production. Fatigue is by far the most common symptom reported in Long COVID so CoQ10 is at the top of the list of supplements to try with this condition. Alpha lipoic acid is a powerful antioxidant, and it is involved in mitochondrial energy production. ALA also has immunomodulatory properties and may actually be an anti-viral as well. The properties of both nutrients in theory make them a promising combination in the treatment of Long COVID. How was the study done? 174 patients (51% male and 49% female) aged 18-81 (mean of 51) who had COVID-19 previously and met the 2015 National Academy of Medicine diagnostic criteria of myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). 52% had comorbidities including chronic lung disease (16%, 28/174), diabetes mellitus (13%,23/174), psychiatric diseases (7.5%, 13/174), and rheumatic diseases (9.8%, 17/174). 17.8% (31) of patients had been previously hospitalized for severe respiratory SARS-CoV-2 pneumonia. 82.2% had mild/moderate symptoms during the acute phase. The mean duration of Long COVID symptoms was 5.9 months. The most common symptoms were fatigue (80%), impaired concentration (68%), sleep disorders (85%) disturbed smell and/or taste (60%), memory loss (45%), dyspnea (21%) and arthromyalgias (64%). Patients were divided into two groups. The first one (116 patients) received coenzyme Q10 (ubiquinone form) and alpha lipoic acid taken every day for two months at a dose of 100 mg of CoQ10 and 100 mg of alpha lipoic acid twice a day. The control group of 58 patients did not take either supplement. The characteristics of the patients in the two groups were similar at baseline. Patients in both groups also received a variety of medications including paracetamol, codeine, NSAIDS, antidepressants (duloxetine), anticonvulsants and analgesics (pregabalin and gabapentin). They also undertook psychological and psychiatric counseling, physio-kinesiotherapy, yoga, and Pilates. What were the results? “The primary end-point was to evaluate the effectiveness of the association of coenzyme Q10 and alpha lipoic acid in reducing fatigue, expressed as a reduction in Fatigue Severity Scale (FSS), at the second month (T1), of at least 50% (complete response) from the baseline (T0) or at least 20% (partial response) from the baseline (T0). A reduction in FSS < 20% from baseline at T1 was considered as a non-response. A complete FSS response was reached most frequently in the treatment group compared to the control group. An FSS complete response was reached in 62 (53.5%) patients in the treatment group and in two (3.5%) patients in the control group. A reduction in FSS score < 20% from baseline at T1 (non-response) was observed in 11 patients in the treatment group (9.5%) and in 15 patients in the control group (25.9%) (p < 0.0001).” Author's Conclusion “Despite the short follow-up period, we demonstrated a clinical benefit, suggesting the rapid effect of this therapy. On the other hand, because of the short follow-up duration, we do not know if this clinical benefit persists over time. See all the Healthcasts at https://www.biobalancehealth.com/healthcast-blog/ Mitochondria are discussed in antiaging advertising like they are understood by the common man.  Even TV discussions and podcasts discuss mitochondria like we are all biology majors, but to most people mitochondria is a word but most people don't know really what they are.  Most people think Mitochondria are “good”, but what are they? Some examples of ads that use the word Mitochondria, reads “Take these Anti-oxidants every day and you will heal your mitochondria, and live longer…” “Your mitochondria are sick and need to be treated with BLANK to bring you back to health, just buy….” In reality, most students of biology and medical doctors are the only people who understand what they are and how they work, what can make them “sick”, and how to make them operate normally within each cell of our body. Simply mitochondria are organelles that take BS and Oxygen and make energy.  You can think of it as “breathing for the cell” , by taking blood sugar and O2 and creating energy, CO2 (carbon dioxide) and water H2O.  Mitochondria are the tiny determiners of our ability to metabolize food and make the energy the cell needs to do its job effectively. I bet you never thought anything about the fact that when you breath oxygen it goes into your blood stream on your red blood cells, and delivered to your tissues by diffusion through capillaries and enters the cells within your tissues…but what does it do then to sustain us?  That is where Mitochondria come in…as they work as I described in the previous paragraph. Another important piece of information about the mitochondria is that they are passed from mother to child, which means your metabolism is secondary to the genes only your mother gave you!  We call this genetic communicator that tells cells how to function metabolically, mitochondrial DNA! To locate our mitochondria, we must take a microscope to the human body, and successively dissect her into smaller and smaller parts:  A whole human body, divided into organ systems eg. Skin, Gastrointestinal system, then divided into smaller parts called tissues such as muscles and neuro-tissue.  Smaller still are the specialized cells that make up the tissues, call cells.  Smaller still are the mitochondria that are located within each cell in the body, providing energy to the cell, to make the whole-body work! Mitochondria are small, oval, sub-cellular “ energy packs” located inside every cell in our bodies.  These small oranelles within our cells turn sugar (glucose) from our food, into ATP (energy) and Carbon Dioxide (CO2).  They act like miniature lungs, taking in sugar which is made of oxygen and carbon, and returning CO2 as a by-product.   Scientists say that mitochondria's activity is “respiration”!  The CO2 passes out of the cell and is excreted by the lungs. So why do we use Mitochondria as a catchword for a way to sway lay people to buy a product to keep them healthy?  It is a way to explain complicated physiology to people who aren't trained in a particular science like medicine or biology, and a way to sell products.  The word mitochondria is associated with sickness and how to get better and it becomes a catch word everyone knows and wants to “fix”. However, if it is a fad or an advertising method to sell something, knowledge of what mitochondria need to make energy, is a valid pursuit.  In short, your cells need plenty of oxygen, healthy foods, exercise and a clean environment to make energy effectively in your mitochondria. Because our food sources do not have adequate nutrition for us and our mitochondria, we must eat foods that are particularly packed with nutrients and or add supplements such as resveratrol, Alpha-Lipoic Acid, L-carnitine and omega 3 oils to heal our “sick mitochondria”. All the while we are trying to “help our mitochondria” with nutritious food and supplements, many of us are inactivating our mitochondria just when we need them most!  How you ask? Those of the many people who take statins are inactivating their mitochondria 24-7.  That is how a statin works!  Statins inactivate mitochondria to decrease the production of cholesterol in the cells.  So why are we killing our ability to make energy and to be healthy, just to decrease our chance of getting heart disease later? Statins are only worth the risk if you have already had a heart attack or are about to have one and have narrowed atherosclerotic arteries.  Statins don't treat anything, but your cholesterol lab values, and only prevent additional buildup of more plaque. Some of my patients think statins are cleaning up the arteries, but they do not do that!  The risk for a heart attack includes blocked arteries, inflammation, smoking and a high Homocysteine level and Family history.  To clean up blood vessels, Zetia is the atherosclerotic cleaner-upper without the risks of a statin!  Zetia leaves your mitochondria alone! To know if you actually have atherosclerotic plaque, you should have a cardiac calcium scan to see if you do or not!  I have high cholesterol, and no plaque (I had one test in 2002, and one in 2019. High cholesterol doesn't necessarily lead to heart disease, but it does definitely stop your mitochondria from working! Since the mitochondria are so important to health, we will talk about How mitochondria can get sick and cause us to be sick in many ways throughout our body.

Check out today's questions:    Ashley: Hello! My 7 year old son has very enlarged tonsils. He does have allergies and currently takes monolucast every night. I remember you talking about your daughter having enlarged tonsils and you did a protocol and it helped reduce them. He doesn't snore eveynight but sometimes he does. I don't want to have his removed and was hoping you could share what you did for your daughter and I could possibly try it on him. Thank you for your help and truly appreciate all your help.   Patti: Hi Dr. Cabral Wishing you well. I have listened to all of your podcast referencing breast cancer & many on cancer. I have had breast cancer twice in the same breast. I completed approximately 18 gene tests & none of them were found. I don't eat red meat, dairy or saturated fats & stopped eating eggs which I did not eat often. I do eat pasture raised organic chicken & turkey & wild caught sockeye salmon. Is there any research that they can contribute to an increase in cancer? Thank you as always!!   Jennifer: Hello Dr. Cabral! Thank you for taking the time to try to help me. I'm a huge fan and an avid listener! My naturopath ran bloodwork on my first visit with her and discovered my iron was high (169 with iron saturation at 56, TIBC was 304 and UIBC was 135). She did not check ferritin at the time, but everything else she tested was in the normal range. She had me take Alpha Lipoic Acid for a month and retested me 4 months later. I got "the virus" 2 months before the retest. At the retest, she tested ferritin (just regular ferritin, not saturation or any other measure). This time, my TIBC and UIBC were in the low range at 226 and 106, respectively. My iron went down to 120 and my iron saturation went down to 53. My ferritin was 247. My naturopath wanted me to take Alpha Lipoic Acid again to lower the ferritin, but I wonder about that, as my iron is fine now and my TIBC and UIBC are low. She wants to retest and if my ferritin is still high, start having me get my blood drawn to lower it. I personally wonder if the high ferritin is due to inflammation from the virus or if there's another missing piece here. I'm hoping you can steer me in the right direction of what to test for or any other ideas. FSH: 10.3 pg/ml PROGESTERONE: 34 FREE TESTOSTERONE: 116 DHEA, ETRADIOL, T3, FREE T4 , LH, TPO ANTIBODIES, TSH , CORTISOL: ( all in normal range). I started taking all of the supplements below and re-tested on 1/20/22 and my levels were: PROGESTERONE: 13 FREE TESTOSTERONE: 58 DHEA: 2.1 and everything else was in normal range. Since my first lab on 6/18/2020 I have been taking these daily: MYO-INOSITOL, NAC, CURCUMIN, QUERCETIN, OMEGA 3, G.I PROBIOTIC At what point can I stop taking these supplements? I am a 27 year old female I exercise almost daily height 5' 8'' weight 125 My periods have gone from 67 day cycles to 35 days cycles and lasts around 3 days. My skin has cleared up and I know longer experience frequent panic attacks! ( My periods are still extremely painful) I am hoping to not be dependent on these but definitely don't want to risk having my previous symptoms return.
