Some guests build buildings. This one built an appetite for a whole city.Lee Brian Schrager sat down with us at Buró over Pimm's Cups and took us all the way back, to Massapequa, Long Island, where he was bagging Chinese takeout on weekend nights by the age of ten. His father had left the garment business, retired young, moved to Florida, and opened restaurants with his brother that, in Lee's own words, did everything wrong and lost a lot of money. Lee has spent a career doing the opposite.We got the whole arc: the Culinary Institute of America, opening Torpedo as one of the first bars in South Beach in the late 80s, and seventeen years at InterContinental Hotels, room service manager to VP. Then Southern Glazer's handed him a sleepy one-day FIU wine tasting in 2000, and he decided it could be something enormous. By 2002 it was the South Beach Wine & Food Festival, today a five-day, sixty-thousand-person event that has raised close to forty million dollars for FIU's Chaplin School. The USS Indianapolis sinking remains one of the most devastating naval disasters of World War II. On July 30, 1945, Japanese submarine I-58 torpedoed the heavy cruiser in the Philippine Sea after it completed a secret mission delivering components for the Little Boy atomic bomb to Tinian.Indianapolis sank in about twelve minutes. Nearly nine hundred sailors and Marines reached the water alive, but only 316 of the nearly twelve hundred people aboard survived. For more than three days, the survivors drifted across miles of open ocean with little food, almost no fresh water and failing life jackets. They endured burns, injuries, dehydration, hallucinations, exposure and real shark attacks whose exact death toll can never be known. No rescue force had been sent to find them. The Navy's movement-reporting system failed to recognize that an entire warship was missing, and patrol pilot Wilbur Gwinn discovered the survivors only after a faulty antenna drew him to a hatch above an enormous oil slick.In this episode of Terrifying & True, we follow the secret atomic mission, the attack by Mochitsura Hashimoto's submarine, the twelve-minute sinking, the desperate work of men such as Lewis Haynes, Thomas Conway and Edward Parke, and the dangerous open-ocean landing by PBY pilot Robert Adrian Marks. The Jerusalem District Court has placed a temporary injunction preventing the transfer of crocodiles to the Ketziot Prison as part of a campaign spearheaded by National Security Minister Itamar Ben Gvir to place the large reptiles in moats surrounding the detention facility. A final decision on the matter will be made when the state responds to the petition by Let the Animals Live, an Israeli animal rights group that protested the move. Fender may have just committed brand suicide because history is not on their side."

I believe the most frightening vehicle to travel from point A to point B in would be a submarine – and I am including cable cars, and rocket sleds, and tightropes, and wingsuits, and early diving bells, and high-altitude balloon gondolas, and luge sleds, and the kind of fixed-wing plane where you have to walk on the wing, and human cannonballing, and ziplining over a volcano in that analysis. Let's see if today's story changes my mind.On today's episode: you will learn why the history of victory in human warfare favours how much you resemble your own anus; you will to understand how your brain actually craps itself worse than your bowels when confronted with something enormous approaches unseen from underwater; and if you ever had a particularly bad day at work and felt really underserved by your employers desire or ability to do anything about it, this episode is doing that thing where it points back-and-forth between you and itself and nodding strongly. And if you were listening on Patreon… you would learn why the first recorded military submarine mission sounded like bad porno; you would see why life in a confined space can get you full-body duct taped to a chair with a gag in your mouth; and you would hear cautionary and sobering observations about the aromatic and claustrophobic life of a fellow listener who spent years in the silent service.I'm going to have you imagining everything from what a Roman solider would have thought watching a tank shell remove your co-workers head, to what it feels like to be slowly pulled into the spinning blades of a submarine, to the experience of free-balling it from the sea floor to the surface in a pressure suit which is said to be one of the most frightening emergency procedures in the world. I spend no small amount of time figuring out new and terrible ways for you to experience fear in a vehicle. There's just something about going through a horror when you're trapped in a moving conveyance that doesn't obey or respond to your panic. I think a submarine is a perfect example, considering if you pull over and try to get out, everyone on board dies terribly. Key Learning Points Why these guidelines matter: This is the first joint AHA/ACC clinical practice guideline specifically on acute PE, bringing together a truly multidisciplinary writing committee (cardiology, pulmonology, hematology, emergency medicine, interventional radiology, surgery, and others). Prior guidelines existed from individual societies, but nothing this comprehensive had been updated in roughly five to six years. New PE clinical categories (A through E): One of the most impactful changes is replacing the old “massive/submassive” and “low/intermediate/high risk” labels with five categories that form a severity continuum. Category A is subclinical (incidental PE found on imaging in asymptomatic patients). Category B covers symptomatic but low-severity patients. Category C is where much of the clinical complexity lives — symptomatic, hemodynamically stable patients subdivided into C1, C2, and C3 based on RV function and biomarkers. Category D represents incipient cardiopulmonary failure (transient hypotension, normotensive shock with end-organ dysfunction). Category E is frank cardiopulmonary failure, with E2 being the sickest — refractory or recurrent cardiac arrest. Respiratory modifiers (hypoxia requiring supplemental oxygen) layer onto C, D, and E. Diagnostic approach: Clinical evaluation comes first — history, exam, and validated decision tools (Wells score, revised Geneva, PERC). If clinical probability is low and D-dimer is normal, imaging can be safely avoided. If either is concerning, imaging is warranted. CTPA remains the preferred imaging modality due to superior sensitivity, specificity, wide availability, and ability to assess clot burden and alternative diagnoses. VQ scanning is still appropriate when CTPA is contraindicated, and VQ SPECT offers better reproducibility and specificity than traditional planar VQ if available. Echocardiography is not a diagnostic test for PE but is important for risk stratification — RV size, TAPSE, and tissue Doppler measures all contribute prognostic information. Anticoagulation updates: Anticoagulation remains the cornerstone of treatment. For patients potentially needing advanced therapies (C3, D, E), parenteral anticoagulation is started first. A notable recommendation: low molecular weight heparin is generally preferred over unfractionated heparin, based on evidence showing more effective VTE risk reduction, more predictable pharmacokinetics, no need for routine monitoring, lower rates of heparin-induced thrombocytopenia, and no increase in major bleeding. The committee acknowledged this may create discomfort for clinicians accustomed to unfractionated heparin’s easy reversibility, but the difficulty of achieving and maintaining therapeutic levels with UFH was a significant concern. Advanced therapies: Catheter-based thrombolysis, mechanical thrombectomy, systemic thrombolysis, and surgical embolectomy all received mostly class 2B recommendations (“can consider”) for C3 and D categories, reflecting that current evidence shows improvement in short-term surrogate measures (RV/LV ratio, hemodynamics) but lacks definitive hard outcome data on mortality. For category E1 patients, recommendations are stronger (class 2A). Multiple trials are expected soon — HI-PEITHO, PEERLESS-2, PE-TRACT, PERSEVERE, TORPEDO, and PROG — that should substantially inform future updates. PERT teams: Pulmonary embolism response teams are encouraged, particularly for C3, D, and E patients. They’ve been shown to reduce length of stay. For institutions without PERT capability, establishing consultation networks with larger centers is recommended. Post-PE follow-up: Patients shouldn’t be “left in the wilderness” after discharge. The guidelines recommend communication within the first week to ensure understanding of diagnosis and treatment, an in-person visit at or before three months to assess for persistent symptoms and discuss anticoagulation duration, ongoing surveillance for chronic thromboembolic pulmonary disease, and periodic reassessment for those on extended anticoagulation.
