Here we discuss how HIV is diagnosed and managed in the USA. About our Guest: Dr Ankita Kadakia, M.D. serves as the Deputy Public Health Officer for the County of San Diego. Prior to her current role she served as the Chief and Medical Director for the County's Tuberculosis Control and Refugee Health Branch and is the President of the California Tuberculosis Controller's Association. Dr. Kadakia is a board-certified Internal Medicine and Infectious Disease specialist. She has served in leading, coordinating, supervising, and managing operations for Covid-19 as well as coordinating the County Public Health Service's Tuberculosis Elimination Initiative. Throughout the COVID-19 pandemic, she has played a critical role as a medical subject matter expert and Tri-Chair of the County COVID-19 Vaccine Clinical Advisory Group.Prior to joining the County, Dr. Kadakia worked at the University of California, San Diego (UCSD) as an Assistant Professor of Medicine with a focus on HIV/AIDS and LGBTQ healthcare while practicing HIV medicine at UCSD Owen Clinic. While at UCSD, Dr. Kadakia created the first Transgender HealthCare Symposium and became a co- principal investigator for the San Diego Pacific AIDS Education and Training Center. Dr. Kadakia's passion, for serving underserved patients and preventing and treating Tuberculosis, STDs and HIV/AIDS, transcends borders. She has spent time in Uganda serving rural areas, partnering with Ugandans, to build and support schools for AIDS orphans and impoverished children. Kavi Devulapalli and Vishal Kadakia, reuniting at SIR 2023, to discuss the process of finding and negotiating locum tenens opportunities. Through conversations with people in the staffing industry, he has learned that their margins are around 30-40% of an IR's daily rate. Deciding whether or not to use a staffing agency is up to the physician and their networking needs, but there are an abundance of direct ways to connect with jobs through online communities, conferences, and device representatives. Having more personal contact with employers and practice owners can also provide more information about each site's work environment, case load, case variety, and expectations. Dr. Kadakia notes that some clients have signed exclusive locums contracts, so they are prohibited from working with physicians that do not go through the staffing agency. He also talks about national groups establishing locums departments to staff different branches in need of extra help, and how this could be an alternative to staffing agencies. Dr. Devalupalli mainly works with physician-owned practices and he emphasizes the value of speaking directly to practice owners. This is a good way to build trust and reduce bureaucratic inefficiencies. Dr. Kadakia shares a personal marketing tip: He creates a one-page introduction to his skills and reviews, which succinctly lets clients know what he can offer. Next, we discuss the changing job market as demand for IRs is increasing and trainee interest in locums is growing. IR training primarily occurs within a hospital-based setting, so it would be an easier transition for early career IRs to do inpatient locums, as opposed to starting in an OBL setting. Overtime, as one proves their skills, confidence, and efficiency, it will become easier to find outpatient opportunities. Finally, the doctors discuss the importance of transparency around contracts and fees. They give concrete examples of price floors and necessary terms to define in locums contracts. To remain competitive, we not only need generational diversity; we need to embrace, harness, engage modern talent. As a thought leader and trainer, Kadakia rebuilds a backdrop for millennials, reminding us that every generation had benefited from innovations. Now, by having their basic needs met, millennials are also able to self-actualize. She also debunks five commonly held myths about millennials: It's not lazy; it's productivity redefined. It's not entitled; it's entrepreneurial. It's not hand-holding; it's agility. It's not disloyal; it's seeking purpose. It's not authority issues; it's respect redefined. Approaching each misconception using her “One Coin, Two Sides” model, Kadakia treats the issues with equanimity. She gives us new language and positive attributes to reframe each of the stereotypes. According to Kadakia, millennials are primarily driven by two prevalent philosophies: “YOLO” and “FOMO.” The former, “You Only Live Once,” drives them to seize opportunities, even if it means moving switching jobs frequently. On the other hand, FOMO (“Fear of Missing Out”) causes them pain because they have so many choices. To balance the two, they collect experiences first and material goods later (or not at all). Contrast these motivations with those of older workers who went without not by choice but by circumstance and whose choices were severely limited, and you have a conflict of interest and misunderstanding. In the span of one decade, the “digitally enabled” generation moved from pagers to flip phones to smartphones. Remembering this lightning-fast shift can ease tensions in the workplace and can help us know what these workers expect. For example, they look for constant feedback and transparency; they expect flexibility and access; they count on growth opportunities and recognition. To create genuine workplace breakthroughs, Kadakia proposes we create environments characterized by “co-versity.” She explains, “Instead of a millennial network, it's having a generations network, where people of all generations can come together and converse, connect, and collaborate.”
