Asistanlık yıllarımdan bir nöbet. Kırklı yaşlarında bir kadın hasta 112 tarafından nöbet diye getirilmişti. Yakınlarından hastanın kasılma ve morarması olduğunu öğrendik, hasta bu kısmı hiç hatırlamıyordu ve özgeçmişinde bilinen başka bir hastalığı yoktu. Hastanın bilinci açık, GKS 15, nörolojik muayenesi doğaldı. Benim de aklıma ilk olarak nöbet geldi, hastanın tetkiklerini istedim, orderını verdim, bunlar yapılırken hasta tekrar nöbet geçirdi. Çok kısa sürmüştü ve postiktali olmamıştı. İkinci kez nöbeti olunca hastayı tomografiye gönderdik. Tabii öncesinde EKG çektik, çektiğimiz sırada hasta nöbet geçirmiyordu ve EKG o anki değerlendirmemle normaldi ve herhangi bir aritmi yok idi. Hasta BT’den döndüğünde ekip, hastanın çekim sırasında da nöbet geçirdiğini söyledi. BT’de akut patoloji yoktu. Mükerrer nöbetleri olunca hastayı monitorize ettik, nörolojiye haber verdik ve fenitoin yüklemeye başladık. Fenitoin devam ederken hasta VF'ye girdi ancak birkaç saniye sürmüştü ve defibrile etmeden hasta normal ritme geri döndü. Fenitoin infüzyonunu durdurduk, hastayı monitörize izlemeye devam ediyoruz, bu arada nöroloji de geldi. Hasta tekrar VF'ye girince tabiri caizse bizde jeton düştü. Hasta nöbet geçirmiyor VF’ye girip çıkıyordu. Zaten postiktal dönemi de yoktu. Dış merkeze koroner YBÜ’ye sevk edildi. Sonrasında aileye yapılan genetik taramada Na kanalopatisi saptandığını ve hasta ve çocuklarına ICD takıldığını öğrendik. Bu da bana ders olsundu. Aradan 8 yıl geçti, şimdi de belki bu yazı meslekte yeni arkadaşlarıma yol gösterici olur. Kalıtsal aritmi sendromları nadir görülüyor olmalarına rağmen, erken tanı ile ölüm riski ciddi oranda azaltılabileceğinden oldukça önemli bir başlıktır. Kırk yaş altında ani kardiyak ölüm insidansı yıllık 3/100.000’dir ve yapılan araştırmalar bu ölümlerin çoğuna, kalıtsal kalp hastalıklarının neden olduğunu göstermiştir. Bu vakaların %70’inde neden, primer aritmi sendromları, %30’unda ise aritmi riski oluşturan yapısal kalp hastalıklarıdır. En sık karşılaşılan primer aritmi sendromları​1​: Uzun QT sendromu (LQTS)Katekolaminerjik polimorfik ventriküler taşikardi (CPVT)Brugada sendromu (BrS)Daha nadirende,Kısa QT sendromu (SQTS)Erken repolarizasyon senromu (ERS)ve idiopatik ventriküler fibrilasyon (IVF)’ dur. Aritmi riski oluşturan yapısal kalp hastalıkları arasında ise​1​: Hipertrofik (Obstruktif) Kardiyomiyopati (HOCM, HCM)Dilate kardiyomiyopati (DCM)Ve Aritminojenik sağ ventrikül kardiyomiyopatisi/displazisi (ARVC/D) sayılabilir. Ani kardiyak ölüm dışında bu hastaların tipik başvuru şikayetleri çarpıntı, spontan geri dönen kısa bilinç kaybı (senkop) ve spesifik tetikleyicilerin uyardığı aritminojenik nöbetlerdir. Oxygen kills everything. More blood flow means more oxygen. You can take an oil that contains the precursor of what PGE1 needs to do its thing. A 12-inch pizza has 38 teaspoons of sugar. [44:15] The Importance of Oxygen for the Body    Decreasing oxygen in the cell can result in cancer. In a study, every heart that had been reduced oxygen got cancer. All the oils in a regular supermarket are processed, and the oxygen transferring is profoundly impaired. This is the biggest problem that we have today. When you have a lack of oxygen, you either kill the cell or grow yeast. The yeast is growing sugar. The cell is actually not using the oxygen because it can’t. The oxygen transfer is tied to the omega-6. Everybody in the world is missing the omega-6s. All the fish oil studies are finding that it fails.   [59:20] We Shouldn’t Have Self-Quarantined People are unemployed, and they are not going to have money for food. If you are in panic mode, your immune system will go down. When you are in panic mode, you are burning oxygen. It’s the worst thing we could have done is to panic people. The numbers are not in an epidemic that we are led to believe. The numbers are not growing. It doesn’t warrant an epidemic. The solution is worse than the problem. Today, I am lucky to have here with me Dr. V.A. Shiva Ayyadurai, the inventor of email and polymath, holds four degrees from MIT, is a world-renowned systems scientist, inventor and entrepreneur. His love of medicine and complex systems began in India when he became intrigued with medicine at the age of five as he observed his grandmother, a farmer and healer in the small village of Muhavur in South India, apply Siddha, India’s oldest system of traditional medicine, to heal and support local villagers. These early experiences inspired him to pursue the study of modern systems science, information technology and eastern and traditional systems of medicine to develop an integrative framework linking eastern and western systems of medicine. When the virus meets your cell membrane, it’s able to infect the cell easily. It can infect people much easier than any other viruses. It is possible that the virus was engineered. We are so focused on the virus (the attacker) that we forget that we have a built-in immune system (the defender) available to us to protect us from harm. We need to think about how to take a systems approach to the body. [06:45] Sweden Didn’t Shut Down Sixty million people got H1N1, and 60,000 people died. Six hundred thousand people die of heart disease in this country. We should be calling a state of emergency on obesity. However, cancer and obesity fuel big pharma – so it’s no wonder people are turning a blind eye. Sweden didn’t shut down because they saw the numbers. Why are we forcing everyone to a quarantine model? A minimal set of people are dying. The people who are dying are old or have immune problems. We still have a lot of people who work with their hands and are smart. People know that something isn’t right. However, academics don’t say anything. We need to buff up the scientific establishment. Dr. Pedre found that there is a way to relax his body through meditation and yoga. Pedre wanted to hack his body through a combination of breathing and guided imagery. The body has an ability to conquer fears.  [09:30] The Consequences of Stress on the Body Stress increases gut permeability and affects the gut microbiome. If you are in a fight or flight, you can’t digest well. Stress won’t let you break down protein properly. If you aren’t breaking down protein, then you won’t be happy and enjoying life. Then, things can get through that aren’t meant to get through – like endotoxins. Why are we so interested in leaky gut? It leads to inflammation. Inflammation is the root of disease in the body. It can affect anyone from normal to someone with celiac disease. You might get hives, joint inflammation, and joint pain. Sometimes you won’t have any gut complaints whatsoever. There is probably a huge percentage of people with leaky gut. It is becoming a worldwide problem because of the exposure to pesticides and antibiotics. We are treating too many things with antibiotics. Instead of fighting a bug, we should be fortifying the terrain. [36:30] Foods That Cause Leaky Gut Gluten and dairy are part of the problem. Soy and corn are also problem foods. GMO foods are also giant issues for our guts. Some people are sensitive to artificial sweeteners. She didn’t know anything about fasting or detox. However, she was in an amazing place with her health. Shortly after, Dr. Mindy started having trouble with her sleep, depression, and anxiety. Mindy realized she needed to start doing something different. The wake-up moment for Dr. Mindy was when her family and friends told her it was menopause. “We are living in one of the most toxic times in human history.” -Dr. Mindy   [08:30] Dr. Mindy Started Fasting Mindy knew that she wanted to learn water fasting for three to five days. However, she had all these limiting beliefs about fasting. Mindy met a nine-year-old who was on his second day of a water fast. That’s when she realized anyone could do it. Then, Dr. Mindy started doing way too much fasting and tanked her sex hormones. Women need to be careful about how much fasting they do.   [10:40] Signs Women Are Fasting Too Much Hair loss is a significant sign. When you stimulate autophagy, it will start to clean the cells. It can also look at the cell and decide to kill it. When the cell dies, it doesn’t have a mechanism to handle the environmental toxins. If you are gaining weight, that is also a sign you are fasting too much. Spotting is a sign of low progesterone.   [13:10] How to Help the Detoxification of Toxins There are some simple things you can do: Dry brushing Loofa under armpits and on neck Walk during fasting Castor oil packs Coffee enemas Add a heavy metal detox.   [15:40] How to Fast for Your Cycle    If you have a regular cycle, start tracking it. Mindy uses the Clue app. On day 12 to day 14, there’s a short window where estrogen has to be made. You need glucose to produce estrogen. It will help you ovulate. If you are in menopause, that 12 to 14 day period means you need to step out of ketosis. Do a hormone building day. Eat beans, squashes, and potatoes. Once day 14 hits, you can go back to keto and fasting. On day 21, the same rules apply – step out of ketosis. The first month you try keto, you need to figure out ketosis for you.   [21:20] Will Carbs Slow Down the Progress for Women?   Instead of doing 15 hours of fasting, try 17 hours. Start doing two or three days of hormone building rather than seven days. Hopefully, you can balance the hormones but still give you the weight loss. Some women have been doing one meal a day and stopped losing weight. If that’s you, eating this way will actually make you lose weight. Step out and do something different.   [23:05] Problems With Getting Too Much Autophagy   You get stuck with your weight loss goals. Hair will fall out. Women will start getting hot flashes. Women will lose their periods. However, Dr. Breus works with people who want to sleep better – he wants to help the largest group of people.  [09:00] Destruction Means Growth Growth is a painful and destructive process. If you aren’t in pain, then you aren’t growing. If you want to grow, we need to destruct. Shift your mindset from IN the way to ON the way. [10:45] The Connection Between Hormones, Sleep, and Weight Loss Sleep deprivation is different for everybody. It’s a personalized situation. If you feel sleep-deprived, then you are. If you fall asleep within five minutes of hitting the pillow, then you have sleep deprivation. It should be a 10-15 minute process to get there. Hitting the snooze button more than once is sleep deprivation. Irregular bedtimes can cause sleep deprivation. What happens when your body is sleep-deprived? Cortisol increases. Appetite increases. Hormones will start to play a significant role. Ghrelin: increases hunger – this hormone will increase. Leptin: makes you feel full – it decreases by 15%. You will eat anything you can get your hands on. The most natural solution is not to be sleep-deprived. [17:15] The Importance of Melatonin When cortisol is high, melatonin is low. Melatonin is the key that starts the engine for sleep. You cannot sleep without melatonin being active and onboard. Be careful exercising too close to bedtime; it will take too long for your temperature to drop. A lack of melatonin can be an issue when experiencing jet lag. There are two situations where melatonin is something you should use: If you have a melatonin decline (age 50+). Autistic children. Do not take 10 mg of melatonin. Girls should not take melatonin under the age of 18 – it acts as a contraceptive. [24:45] Eating Before Bed    It depends on what you eat. Carbs make you feel sleepy. If you eat a big carbohydrate meal before bed, it will make you feel tired. Eat a 250 calorie snack before bed if you are hungry. Guava leaf tea shows that it keeps your blood sugar stable throughout the night. [27:55] Drinking Banana Tea Magnesium controls 300 functions in our body. Our bodies do not produce magnesium; we need to eat it. Even if you eat a bushel of kale, you still might not get the magnesium you are looking for.   [32:30] Are You a Night Owl or a Morning Person? Every person has a master biological clock ticking away inside of their brain, and dozens of smaller biological clocks throughout his or her body. Do a genetic test to find out your programming. Celebrity trainer, entrepreneur, and author, Thomas DeLauer. Thomas was hyper-focused on his business… a successful business at that, but he was afraid to step away or divert in any way towards anything else. As a result he was overweight. The only thing that getting healthy would do would shift time away from what mattered to him most (money and providing for his family). He had the hard-wiring to be excellent at both physical performance and business… They could complement each other. He became obsessed with homing in on exactly what made him an excellent businessperson. Thomas applied discipline and knowledge to his health and made some promises to himself. In a rather short amount of time, he went from being 280 lbs to 185 lbs and on the covers of some of the largest health and fitness magazines in the world. Now, he writes for some of the largest fitness publications in the world as an expert in diet, nutrition, and mindset. Thomas became a coach after he knew that he could provide what was needed to get the world’s best businesspeople the results that they need to be a better spouse, feel better, perform better, and increase their bottom line. The best part is so much of this can be done without dramatically changing your life, and seeing the optimization take place in his clients is sincerely the reason he continues to do this. His story is fantastic, so he was able to get on the cover of magazines. However, Thomas realized the fitness industry was all about looks. How you look should only be a reflection of how you feel on the inside. Thomas realized he needed to bridge the gap between health and fitness. The fitness industry dropped Thomas because he was no longer speaking about what they wanted to hear. When Facebook changed their algorithms, Thomas decided to work on YouTube to get his message out. If you put a YouTube video out, people will be able to find it for years. [09:25] How Inflammation Leads to Weight gain LDL and chemicals will pool up in your blood and cause damage. Our gut is a fragile ecosystem. When our lipopolysaccharides get into the bloodstream, they create a world of hurt. If you have heard of leaky gut, then you know what Thomas is talking about. When you are inflamed, your gut cells will become inflamed. Your gut will become weak and disrupted. They will block your ability to process glucose. Eventually, it leads to insulin resistance, which will lead to weight gain. You can change a lot with your diet, but you can change more with your lifestyle. [13:00] What Can Cause Leaky Gut It depends on the person. Dairy and grains can be issues. Anytime you consume something so much, your body starts to develop an immune response to it. Anything can be inflammatory. High fructose corn syrup is a big problem. Almonds can cause inflammation in one person and cause no problems in another person. [15:30] How Keto Can Be Anti-inflammatory and Pro-inflammatory Keto can be anti-inflammatory because ketones are a signaling device. Ketones are a fuel, like a macronutrient. However, they also transmit a signal that helps reset the body. Ketones have the ability to communicate to reduce inflammation. The purpose of keto is to try and control inflammation. When you start eating too many omega 6 fats and low-quality oils, you will develop a problem. Omega 6s are from soybean oil, sunflower oil, and canola oil. They are already rancid. Some omega 6s are good. When they are unstable, and in abundance, it will cause inflammation.   [27:00] The Three Biggest Mistakes with Keto    Consuming too many Omega 6s. People freak out too much about protein. Eating processed foods. If you are consuming a lot more calories because it’s processed, then it’s going to be easier to overeat. [34:00] Staying In Ketosis Too Long If you go for too long, you can make yourself glucose intolerant. Your body can reject the glucose and send your blood sugar too high. This will happen until your body can adapt to glucose again. You should be cycling in and out of ketosis. What happens when you reduce your fat intake and increase carbs? The body starts pulling fat from your body. We are always changing [37:40] The Biggest Fasting Mistakes Breaking a fast improperly is a mistake. You are so sensitive to what you consume. When you break a fast, keep it lean and clean. Do not go sloppy when you break your fast. Keto coffee and bulletproof coffee will break your fast. 500-600 calories of fat will break your fast. Over ten calories or so will break a fast. People get addicted to fasting. There is a binge and purge type of mentality. Fasting should be healthy, not something to deprive ourselves. It should help us challenge ourselves and make us stronger. Do not fast when you are sick. [42:30] Do What Works For You Some people have various strategies for different things. For instance, some people don’t listen to Thomas because they may not agree with his keto content, but they agree with his fasting content. We don’t all have to agree on everything. Today we welcome a specialist in Weight Management and Bariatric Surgery for adults and adolescents, Dr. Robert Cywes. He has been doing bariatric surgery for 18 years, performing over 8,000 surgeries. Dr. Cywes’ medical training began in Cape Town, South Africa, where he received his medical degree from The University of Cape Town. In 1989, Dr. Cywes moved to North America and completed a year-long residency in Pediatric Surgery at Ohio State University’s Columbus Children’s Hospital. After completing his pediatric surgery fellowship at the University of Michigan’s C.S. Mott Children’s Hospital, Dr. Cywes was appointed as an Assistant Professor of Pediatric and Fetal Surgery at Vanderbilt University in Nashville, Tennessee where he did hepatic stem cell research. During this time, Dr. Cywes became increasingly interested in adolescent obesity and the impact of the liver and metabolic syndrome on young patients. Dr. Cywes’ research led to a comprehensive understanding of the toxicity of chronic excessive carbohydrate consumption as the primary cause of obesity and so-called obesity-related co-morbidities, and he became interested in developing a clinical program to treat obese patients using this knowledge. Dr. Cywes relocated to Jacksonville, Florida where he joined the Department of Pediatric Surgery at the Nemours Children’s Clinic and Wolfson Children’s Hospital. This led to a national meeting in Jacksonville where guidelines for adolescent obesity surgery were established.   Dr. Cywes established JSAPA to continue his work in both adolescent and adult obesity treatment and surgery, and in 2013 opened a practice in Palm Beach County, Florida. He now works with a highly experienced team of professionals from a variety of medical sub-specialties to better care for obese patients. He has developed the practice into an internationally recognized Center of Excellence for obesity surgery. The practice uses a cognitive behavioral therapy approach that addresses carbohydrate addiction, along with bariatric surgery, to help patients manage their obesity long term. In this episode, Dr. Cywes opens the show discussing the correlation between carbohydrates and toxic drugs. Our bodies do not need to run on sugar; our bodies prefer a keto lifestyle.
