"I did not have a period. It's not unrelated that then I had a series of five stress fractures." Nancy Boyd, former D1 collegiate runner and now DPT with a practice specializing in providing pelvic floor physical therapy, joins the Lane 9 Project for this episode. Nancy shares her experience with late puberty onset, i.e. primary amenorrhea, at the age of 20 competing in D1 athletics. She developed an eating disorder, and experienced a series of five stress fractures. She had surgeries, a lot of time off, and some not-greaet experiences within the healthcare system. She went on to get her doctorate in physical therapy, to help athletes and humans at all levels and abilities get back to movement in whatever ways are accessible to them. In this episode, I dispel a few common myths around estrogen and menopause. I delve into the dangers of low estrogen levels and debunk the notion that perimenopause is solely characterized by estrogen dominance. I also touch on how taking the 'natural' approach to this midlife transition may do more harm than good! The term estrogen dominance has been widely used in the past, but it may not accurately describe the hormonal imbalance experienced during perimenopause. In fact, estrogen tends to be erratic - up, down and all over the place. Instead, we need to recognize the potential dangers of low estrogen levels, and consider hormone replacement as a key strategy at this time in our life. While low estrogen levels in peri-menopause can lead to various symptoms, including hot flashes, mood swings, vaginal dryness, and decreased bone density, it's far more concerning of what diseases it can lead to later in life. I know that the concept of going through menopause naturally seems like a lovely idea but not bringing hormone replacement on board is unhealthy and could potentially be harmful in a lot of ways. Estrogen is vitally important for more than 400 different functions in the body and with low levels we set ourselves up for a much higher chance of dementia, cardiovascular disease, osteoporosis, and the list goes on. In the U.S., two-thirds of diagnosed cases of dementia and Alzheimer's disease are women. A number of studies have shown that lower estrogen levels "before, during, and after menopause" is a risk for late-onset Alzheimer's disease. However, the risk factors associated with declining estrogen levels and late-onset Alzheimer's disease in women as they age is not widely known. Estrogen is a regulator of glucose metabolism in women and brain estrogen deficiency sets in motion a brain energy metabolism crisis in women—particularly after menopause. A decline in estrogen in menopause is commensurate with a decline in cerebral glucose metabolism. Thus, the inability to efficiently metabolize glucose in the aging brain (glucose hypometabolism) predicates the need for an alternate fuel to sustain the brain's high demands for energy . The demand for an alternate mitochondrial fuel in low estrogen states results in a compensatory shift to ketone metabolism that is enabled by the metabolism of fats derived from myelin-a nerve sheath substance that wraps around the axon extension of nerves. That catabolism of myelin (grey matter demyelination) is a signature hallmark of grey matter atrophy and the loss of brain volume, which in turn is a pathological feature in the progression of Alzheimer's disease in the aging brain. Plus, female ApoE4 carriers may be at higher risk for glucose hypometabolism and an earlier onset of myelin breakdown. Additionally, ApoE4-related mitochondrial dysfunction is also linked to the development of late-onset Alzheimer's disease. Since "mitochondrial DNA is maternally inherited and mitochondrial defects that contribute to the risk for brain metabolic deficits that include glucose hypometabolism and oxidative stress is seen in adult offspring of mothers with a history of Alzheimer's.", optimizing mitochondrial function in the aging brain is of critical importance—particularly in ApoE4 carriers. On a recent episode of Ask The Egg Whisperer, Ana Lucía shared a great question. Here's what she wrote, “Thank you Dr Aimee for your show! I just had a miscarriage. I am 42, my progesterone and estradiol levels are very low (8 and 147). I've been taking estradiol 6mg and Crinone daily. Why are they low and how can I improve them? Thank you very much for your guidance.” In answering this question, it's important to understand how you're taking progesterone (Crinone) and estradiol. Let's break this down a little: for progesterone, the level in your blood is based on the mode of administration. When I have a patient on Crinone (vaginal form), and their progesterone level is 8 - that's because you're placing it vaginally. It has to go through the walls of the vagina, get absorbed, and then go into your bloodstream. You could add more progesterone to your protocol to raise your progesterone, but I certainly don't want you to worry that your progesterone levels were too low for a healthy implantation to occur if this is a level checked after a frozen embryo transfer. I like the level to be around 10 in a medicated FET cycle if using vaginal progesterone. These numbers sometimes make us worry unnecessarily (especially when we go to Google for answers). You might see things that make you really upset, like information saying that your progesterone should be over 20. And, that's sound advice if you're using an injectable form of progesterone. But, with Crione, which is a vaginal form, you're just fine. Talk to your doctor and share your concerns for additional reassurance. Lorena: Hi Dr Cabral! I'd love to get your view on the following: For young women that don't have their periods whether it is due to primary or secondary amenorrhea, are chronic levels of low estrogen an issue? Estrogen levels of 211 pmol/L for example. I've read that: 1. Not having a period is strongly correlated with below normal bone density. 2. Heart function - low estrogen is associated with a increased risk of heart disease as estrogen helps with the formation of hormones like nitric oxide and so keeps levels of vasoconstrictors low. 3. Increased risk neurodegenerative diseases as estrogen is highly protective of brain cells because the cells called microglia are turned off by estrogen and progesterone thus reducing inflammation in the brain. Do you agree on the above? Tessi: I am a 36 year old, and have addison's disease and hypothyroidism. I recently suffered a Missed Miscarriage (silent miscarriage) after being 8 weeks pregnant. Is this common for people who have autoimmune issues? I read that it this type of miscarriage only happens to 3-5% of females...I really want to try to get pregnant again but scared this may happen again.... Currently I'm waiting to get my big 5 lab results back so I can start my healing journey. Lorena: Hi, it's me again :) If the risk for osteoporisis, heart disease and brain diseases are increased on a woman with amenorrhea what would you advice be other than getting the period back? At this point, I give up on getting my period back. It's been years without it since I came off birth control and never got it. I've gone to the best endocronologists and gyneocologists in Germany and they can't figure out why I can't get my period back. All of my hormones are normal according to them. Maybe a little on the lower side when it comes to E2, LH and testosterone but that's about it. I even ran the saliva test with your team and they said that my estrogen, DHEA and progesterone were a little low but nothing crazy out of range for me not to have my period. I did all of your protocols (CBO, CBO finisher, heavy metal detox & more). Foundational protocol level 3 with extra vitamin D & magnesium. One endocrinologist suspects hypothalamic amenorrhea but tbh I doubt it. I'm a 25 year old vata (bmi 20.1. & 14% body fat.,10k steps per day & 30 mins of strength training 4 times a week. Eat 90% plant-based with plenty of protein, carbs and fat approx 2000 calories). Maybe body fat is a little low? I'm doing everything right and I just want my period back. My wife, for years wakes up most mornings with a locked jaw. You can actually hear her jaw pop, like when you crack your knuckles, when she tries to open it most mornings. Sometimes she says it's stuck for a little bit before she can open it. She has been to a chiropractor and seen other doctors and dentists, but most doctors/dentists recommends she gets a shot of botox to help address it, which she doesn't want to do. Is there anything you could recommend to address this? She has PCOS, but otherwise is a very healthy woman. Thank you. Check out today's questions: Rosemary: Asking for help for all who have (what supplements etc.,) for Dupuytren's Contracture (this from the internet) is a common problem that develops most frequently after age 40. It is more common in men than women, though both can be affected. It is usually hereditary occurring in people of European and Western Asian ancestry. It is unusual in Oriental or African-Americans. Dupuytren's usually starts as a painless nodule in the palm. It can progress to fibrous cords extending into the fingers, most commonly ring and little. These bands can cause contracture of the fingers. It can progress slowly or rapidly. The flexor tendons are not affected. Kerry: Hi DR Cabral, What you you advise to support a 8 yr old how has had rheumatic fever and now has been prescribed daily antibiotics till their 21st birthday. Your advice is so appreciated Jason: I’m wondering what you recommend as a sun screen that’s not toxic to the body. Even with a good diet of mainly plant based foods with some fish and good exercise of Pilates and yoga, I still will wake up sometimes very bloated/puffy. I have been into using jade rollers, dry brushing and foam rolling to help out my lymphatic system and it does seem to help a bit. I was wondering if there was a way to prevent the puffiness and what your thoughts are on these “trendy” tools. Thanks! Katie: Hi Dr. Cabral! I’ve heard all your podcasts focused on estrogen dominance and your recommendations, but I was wondering if you could talk about low estrogen in women (who are too young to be going through menopause). Do you see this a lot in your practice? I’ve had extensive hormone testing done (both convention endocrinologist and integrative practioners) and my estrogen levels are consistently low, even when considering the e:progesterone ratio. I know you can’t make specific recommendations for me, but I was hoping you could spend some time discussing why this happens to some young women and what your suggestions are. What are the negative symptoms of low estrogen? How to balance these hormones? I think this would be really great for a lot of us struggling to find answers about our hormones when there is so much information out there is about pacifying too high of estrogen... but sometimes its the opposite issue! If you search up seed cycling for examples, so many articles and testimonials are about how it was aimed to cure their estrogen dominance and regulate their cycle. I know you mentioned that seed cycling could also help menopausal women with low estrogen, and that most women can benefit from it, but if you have any more to say on the topic (low estrogen in young women) I think there’s a lot of us who would love to hear your insights. side note: all other markers (cortisol, pituitary mri, FHS, thyroid) came back totally well and good. Haven’t had a period in 2 years, not on BC, on ultra sounds a lot of follicles present so I have been left with the “mild PCOS” diagnosis and very little options other than ‘not to worry’ because i will be able to have kids some day and that ‘there’s always the pill’ if I’m that concerned about mensturating regularly (which is definitely not an option for me ha). I have been really struggling with my health the past two years, so much so I had to take time off from college (academics are so important to me so this was a really big blow) and has had a huge impact on my mental and emotional help. I have been treated for parasites, eat a super healthy diet, and am currently following you CBO protocol. Without getting into EVERYthing I have tried and done (much like your journey, its been a wild ride), I guess I’m wondering if this hormonal imbalance really could all be related to stress and my emotional state (despite stress hormones being in normal range) that have resulted from all my gut and digestive issues. When we talk about estrogen we are almost always referring to estradiol (the most powerful estrogen). This hormone helps regulate important functions in the body such as your weight, how much fat you store (and where), heart disease, bone density, mood and so on. Low levels of estrogen can cause serious and significant symptoms which need to be identified and treated as soon as possible. Testing for low estrogen is easy (compared to high estrogen) and should always be accompanied by progesterone and sex hormone binding globulin.
