Strategies for rapidly and inexpensively identifying neurological/psychiatric weakness and distortion—combined with genomics and increasingly more sophisticated chemical analyses of blood and other body fluids—now provide us with simple, scalable strategies for delineating specific aspects of preclinical stages of neurological and psychiatric disorders. In parallel, we have an increasing understanding of how to engage the plastic brain in ways that reverse those weaknesses and distortions on a path to neurological normalcy. The word for broadly achieving such renormalization is prevention—or in already diagnosed patients, cure—two goals that up to now have rarely been achieved in brain-targeted medicine. This scientific explosion foretells a rapid transformation from a treatment-based to prevention-based brain medicine era. Dr. Michael Merzenich is an emeritus UCSF professor who is broadly recognized for his seminal research in the science of neuroplasticity, and for his team's efforts to translate this research into improvements in brain performance and organic brain health in normal and in variously struggling child and adult populations. He also led a UCSF team that developed the modern cochlear implant. Dr. Michael Merzenich is the professor emeritus neuroscientist at the University of California San Francisco. He is also a co-founder, chairman and chief scientific officer of Posit Science, the makers of BrainHQ, an online brain exercises and assessments program. Dr. Michael Merzenich studies neuroplasticity – the brain's powerful ability to change itself and adapt – and investigates ways plasticity might be used to heal injured brains and enhance the skills in healthy ones. Merzenich has studied how brains develop for more than three decades. His research has shown that the brain retains its ability to alter itself well into adulthood – suggesting that brains with injuries or disease might be able to recover function, even later in life. He has also explored the way the senses are mapped in regions of the brain and the way sensations teach the brain to recognize new patterns. Merzenich is trying to bring the powerful plasticity of the brain into practical use through technologies and methods that harness it to improve learning. He founded Scientific Learning Corporation, which markets and distributes educational software for children based on models of brain plasticity. He is co-founder and chief science officer of Posit Science, which creates "brain training" software also based on his research. That software, sold commercially under the names Brain Fitness Program and InSight, is supposed to strengthen memory, attention, language skills and visual-spatial abilities in aging adults. “This is the beginning of a revolution,” Merzrnich told The New York Times in June. Studies have shown improvement in those who use the software. Some critics, however, are not convinced that the gains translate into long-term benefits that can be generalized to daily challenges such as remembering where the car is parked. In the 1980s, Merzenich was on the team that invented the cochlear implant to restore hearing for the severely deaf. Merzenich is professor emeritus of neuroscience at the University of California, San Francisco. He has published more than 200 articles, received numerous awards and prizes and holds more than 50 patents. He was elected to the National Academy of Sciences in 1999 and is a member of the Institute of Medicine.

Erprobung eines Training der sequentiellen Analyse akustischer Reize bei Kindern mit einer Lese-Rechtschreibstörung

Die Lese-Rechtschreibschwäche (LRS) ist durch eine spezifische Störung des Erlernens der Schriftsprache bei normaler Intelligenz und adäquater Unterrichtung gekennzeichnet und betrifft mindestens 4 % der Bevölkerung. Eine familiäre Belastung wird häufig beobachtet, dennoch sind Ätiologie und Pathophysiologie dieser Störung unbekannt. Immer mehr Forschergruppen haben sich in den letzten Jahren mit den Ursachen von Lese-Rechtschreibschwierigkeiten auseinandergesetzt. In zahlreichen Studien an Patienten mit einer Aphasie nach linkshemisphärischen Verletzungen der Großhirnrinde (Efron 1963, Tallal & Newcombe 1978, v. Steinbüchel et al. 1999), Kindern mit Sprachentwicklungsstörungen (Tallal & Piercy 1973), sowie Kindern und Erwachsenen mit Dyslexie (Tallal 1980, Hari & Kiesilä 1996, Überblick bei Farmer & Klein 1995) konnten Defizite in der zeitlichen Verarbeitung schnell dargebotener Stimuli nachgewiesen werden. So benötigen Legastheniker deutlich längere Interstimulusintervalle um vor allem jene Konsonanten wahrzunehmen, deren Formanten einen raschen Wechsel durchmachen. Zur Beurteilung zeitlicher Verarbeitungsmechanismen wird dabei häufig die Ordnungsschwelle bestimmt. Sie ist definiert als die Dauer eines Interstimulusintervalls, die notwendig ist, um zwei zeitlich aufeinander folgende Reize zu unterscheiden und sie in ihre zeitliche Reihenfolge zu bringen. Basierend auf diesen empirischen Beobachtungen wird der kausale Zusammenhang zwischen Zeitverarbeitungsdefiziten und Störungen der Laut- und Schriftsprache diskutiert und veranlasste einige Forschergruppen zur Entwicklung von geeigneten Trainingsmethoden. Diese hatten nicht nur das Ziel, die zeitliche Diskriminationsfähigkeit rasch aufeinander folgender Stimuli zu steigern, sondern auch gleichzeitig die laut- und schriftsprachlichen Leistungen zu verbessern (Merzenich et al. 1996, v. Steinbüchel 1995, Warnke 1993). Allerdings wird die Effektivität dieser Trainingsverfahren von einigen Autoren in Frage gestellt, zumal viele der bisher veröffentlichten Studien methodische Mängel aufweisen und eine Interpretation der Ergebnisse erschweren. In dieser Arbeit sollte nun geprüft werden, ob die zeitliche Diskriminationsfähigkeit durch ein Zeitverarbeitungstraining tatsächlich verbessert werden kann und ob dann auch Transferleistungen auf das Lesen und Rechtschreiben zu verzeichnen sind. In der vorliegenden Studie wurden leserechtschreibschwache und sprachentwicklungsgestörte Kinder der 5. Jahrgangsstufe untersucht. Es erfolgte eine zufällige Aufteilung der Stichprobe in Trainings- und Kontrollgruppe. Die zeitliche Diskriminationsfähigkeit wurde mittels Bestimmung der auditiven und visuellen Ordnungsschwelle sowie des Richtungshörens untersucht. Hierzu diente das Trainingsgerät Brain-Boy-Universal der Firma MediTECH sowohl zur Erhebung diagnostischer Daten als auch dem Training der Zeitverarbeitungsparameter. Die Datenerhebungen zur Eingangsdiagnostik, ersten und zweiten Retest wurden in drei Untersuchungsblöcke unterteilt und betrafen beide Gruppen. Zwischen Eingangstest und ersten Retest fand nur für die Trainingsgruppe die achtwöchige zusätzliche Trainingsphase der Parameter Ordnungsschwelle und Richtungshören statt. Die Erhebung des zweiten Retests erfolgte nach sechs trainingsfreien Monaten. Beide Gruppen nahmen während der gesamten Untersuchungszeit am üblichen Schul- und Förderunterricht teil. Ein Gruppenvergleich vor Trainingsbeginn ergab für die Ordnungsschwelle und das Richtungshören, sowie in den Lesetests keine signifikanten Gruppenunterschiede. Lediglich die Kontrollgruppe zeigte im Rechtschreiben signifikant bessere Testleistungen (p £ .019) im Vergleich zur Trainingsgruppe. Beim Vergleich der Verläufe wurde diese Ausgangswertdiskrepanz unter Einbezug der Kovariaten des Ausgangswertes berücksichtigt. Nach dem Training kamen wir zu folgendem Ergebnis. Durch das Zeitverarbeitungstraining wird vor allem bei Kindern mit hohen Ordnungsschwellenwerten eine deutliche Verringerung der auditiven und visuellen Ordnungsschwelle bewirkt. Dieser Trainingserfolg korreliert offenbar mit dem Training selbst und entbehrt sich einer Dauerhaftigkeit unter Trainingsabstinenz. Dagegen wird mit dem Training des Richtungshörvermögens kein Erfolg erzielt. Hinsichtlich der Rechtschreibleistungen zeigte der Gruppenvergleich vom Ausgangstest zum ersten Retest einen hoch signifikanten Unterschied. Dieser kam deshalb zustande, weil sich die Kontrollgruppe unmittelbar nach dem Training hoch signifikant verschlechterte, während gleichzeitig die Trainingsgruppe keine signifikante Veränderung zeigte. Bei der Analyse der langfristigen Veränderungen zwischen beiden Gruppen präsentierten allerdings sowohl die Kontroll- als auch die Trainingsgruppe eine Leistungsverbesserung, welche wahrscheinlich durch den fortgeführten Schul- und Förderunterricht herbeigeführt wurde. Deshalb ist eine alleinige Rückführung auf ein Zeitverarbeitungstraining nicht möglich. Bezüglich des Lesens kann davon ausgegangen werden, dass ein Zeitverarbeitungstraining zu keiner deutlichen Steigerung der Lesegeschwindigkeit und Verbesserung der Lesesicherheit geführt hatte. Der grenzwertige Signifikanzfall betreffend des kurzfristigen Trainingseffektes auf die Lesesicherheit in einem einzigen Fehlertest (PLT-Fehler) ist unter Berücksichtigung, dass mehrere Signifikanzberechnungen erfolgten (Alpha-Fehler), nicht mehr signifikant. Schließlich lassen die Befunde der Erhebungen vom ersten zum zweiten Retest keinen Hinweis für einen anhaltenden Trainingseffekt zu, denn die Trainingsgruppe zeigte hierbei sogar eine Verschlechterung in der Lesesicherheit. Da im Gesamtverlauf vom Ausgangstest zum zweiten Retest beide Gruppen in Lesegeschwindigkeit und Lesesicherheit besser geworden sind, wird letztlich eine Leistungsverbesserung basierend auf einem Zeitverarbeitungstraining widerlegt und auf den fortgeführten Schul- und Förderunterricht zurückgeführt. Die schriftlichen Befragungen von Eltern, Lehrern und Kindern zum Thema Zeitverarbeitungstraining lieferten kein einheitliches Bild. Während die Eltern mehrheitlich den Trainingseinfluss trotz fehlender objektivierbarer Trainingseffekte vorwiegend positiv beurteilten und zudem die Fortführung eines Ordnungsschwellentrainings befürworteten, standen Lehrer und Kinder dem Training eher ablehnend gegenüber. Unser Nachweis über die Trainierbarkeit der Ordnungsschwelle erbrachte weitgehend mit der Literatur übereinstimmende Ergebnisse. Eine Stabilität der unmittelbar trainierten Zeitverarbeitungsparameter Ordnungsschwelle und Richtungshören ließ sich über einen längeren Zeitraum nicht bestätigen. Transferleistungen auf Lesen und Rechtschreiben konnten nicht beobachtet werden. Somit ist nicht davon auszugehen, dass die aufgrund von Zeitverarbeitungstherapien berichteten Verbesserungen spezifische Effekte eines Zeitverarbeitungstrainings sind. Da Therapieversuche kausaler Art weiterhin umstritten bleiben und die Lese-Rechtschreibstörung auf unspezifischen Einflussfaktoren beruht, wäre eine symptomorientierte Therapie am sinnvollsten.
