Educational Pearls: Physiologic stimulation of ventilation occurs through changes in levels of: Arterial carbon dioxide (PaCO2) Arterial oxygen (PaO2) Hypercapnia is an elevated level of CO2 in the blood - this primarily drives ventilation Hypoxia is a decreased level of O2 in the body's tissues - the backup drive for ventilation Patients at risk of hypercapnia should maintain an O2 saturation between 88-92% Normal O2 saturation is 95-100% In patients who chronically retain CO2, their main drive for ventilation becomes hypoxia An audit was performed of SpO2 observations of all patients with a target range of 88–92% at a single hospital over a four-year period This found that excessive oxygen administration was more common than insufficient oxygen and is associated with an increased risk of harm Individuals at risk of hypercapnia include but are not limited to patients with COPD, hypoventilation syndrome, or altered mental status References Homayoun Kazemi, Douglas C. Thoughts on the episode? Let us know.Divya Ramachandran helps startup founders do the hard work needed to bring new ideas into the world. And while she advises on strategy and investment and go-to-market and minimum viable product, her real focus lies in sharpening the mind and body of her clients. That is, she guides them to clarity, which helps them prioritize and make better decisions. And she helps them explore their energy expenditure. Energy is partly about working long hours, but it's also related to how much the founder's identity is bound up in the eventual success or failure of the business.Through emotional clarity and self-awareness, people who are doing hard things to make the world a better place can find peace and joy even within the most intense moments of the struggle.Enjoy!LinksDivya's WebsiteThe Guest House, by Rumi (translation Coleman Barks)Support the Show.

While all eyes and brains are on what SCOTUS thinks about making Trump emperor-king, a lesser known case will be heard Monday that could have a huge impact on how social media can (or cannot) keep election workers safe this year. Murthy v. Missouri arrives at the high court as the result a lawsuit filed by the attorneys general of Missouri and Louisiana, along with a group of social media users—including some doctors and right-wing commentators—who argued that officials in the Biden administration censored their online speech about COVID-19, the 2020 election, among other issues The plaintiffs don't claim that the administration directly silenced their speech. Instead, they argue that, by working with social media companies to limit the spread of misinformation, the government unlawfully chilled the free expression of their ideas. Gowri Ramachandran serves as deputy director in the Brennan Center's Democracy program. The amicus brief filed by her team from the Brennan Center in Murthy draws the Justices attention to another aspect of election disinformation . Ramachandran explains to host Dahlia Lithwick that combating election disinformation has always been important, but it is especially critical now, as election workers struggle to keep on top of voting issues. Later in the show for Slate plus subscribers, Mark Joseph Stern joins to talk about the 5th U.S. Circuit Court of Appeals taking a swing at teens' access to contraception, and a new effort to combat the scourge of judge-shopping. What is the experience of improvisation and what are its neurobiological correlates? One way to flesh out an answer, or at least to get the lay of the land, so to speak, is to look at how improvisation compares with other kinds of brain states and states of experience that have been explored with science, such as dreaming and psychedelics. That's what we're embarking upon here. In this video, you are going to discover that there are some quite interesting relationships between what's happening in the brain during improvisation and what happens to the brain under the influence of dreaming and classical psychedelics. So let's talk about levering the power of public health... How important are civic engagement and politics in elevating the power and practice of public health? It's still sad how it took us a pandemic for people to recognize that we really should be thinking about the importance of public health. That COVID-19 pandemic—that is what happens when we don't have a great structure, especially on a national level. We should be taking action hand-in-hand, for as today's guest said, public health is ingrained in all that we do in life. Unfortunately, here we are, forgetting its importance as soon as things got “well” with us. But we don't want that to go on, that's why I invited Leela Ramachandran. She is a Research Associate for the Public Health Institute of Western Massachusetts and an Elected School Committee Member at Acton Boxborough Regional School District. Through experiences in research and projects, she realized her passion for dismantling systems of oppression, centering historically marginalized populations, and decolonizing the field of public health. Join us as we learn how we can take part in this initiative and hopefully create a better version of both the world and ourselves. Why you need to check this episode: Recognize how important the role of civic engagement and politics are in relation to public health; Understand the power of your network/s in helping activate people's eagerness to take action for a better world; and Gain insights from Leela's experiences and tips on how you can take part in putting the power of public health & advocacy into mainstream “It's important to find the ways that you can contribute. There are plenty of ways to do things; you just have to want to do it and find the thing that works for you.”– Leela Ramachandran Notable Quotes: “I don't think I'd be the physician that I am today without my public health degree.” – Dr. Berry Pierre “All this is public health, and that's why I love public health so much. It's so ingrained in every profession; everything that we do in our lives is somehow related to public health… I just think public health is one of the most important things that you could do. And it's not just like a job; for me, it's perspective.” – Leela Ramachandran “We're important when we matter when it's a national crisis—international, actually—and then all of a sudden, it's kind of forgotten about even though the public health duties do not end.” – Leela Ramachandran “In this beautiful, capitalist country that we live in, money talks and money does things, and money allows you and affords you the opportunity to implement really good public health programs; lack thereof can be detrimental. And so, oftentimes, public health depends on policy, funding, our work.” – Leela Ramachandran “I am not the world's best expert at international policies and politics, but other people are… I think it's just doing what you can in circles of influence. I can't get the whole town to care, but if I can get five of my friends to care like each one teaches one, then they can go get their friends to come on. It's really about who's in your network, who can you pull into this, and who they're affecting.” – Leela Ramachandran “In a world that really overwhelms you, you have to pick something and just go for it. And if not's working, that's fine; it doesn't have to be perfect on the first run.” – Leela Ramachandran Sign up at www.listentodrberry.com to join the mailing list.
