On today's show, host Dana Pellebon is in conversation with Dr. Mary Fariba Afsari about her new memoir, Labor: One Woman’s Work. Dr. Afsari is a child of Iranian immigrants, a working mother, and the founder of a mobile health clinic, bringing reproductive healthcare to rural patients in the Pacific Northwest. In 2015, Dr. Afsari literally mobilized her practice to meet women where they are. Her clinic on wheels–FemForward Health–travels throughout rural Oregon because too many of these communities lack full-scale OB-GYN services. Dr. Afsari says that people have started driving to find her because they've been recommended by word of mouth. It's a sign of how broken the healthcare system is that a mobile RV clinic is providing better care than industrial medicine to women of color and rural women. Post Dobbs, Dr. Afsari says the fight for reproductive justice is more important than ever.  Dr. Afsari's memoir chronicles her career serving women like her grandmother who died of a pregnancy related complication. She says she wants readers to get a sense of the range of experiences she has from obstetric emergencies to joyous births. They also discuss Dr. Afsari's philosophy of meeting patients with curiosity, how race plays a central role in whether a woman will survive a pregnancy, the lack of gender-affirming care, and the criminalization of OB-GYNs post Dobbs.  Residents of Dane County may be aware of a similar service providing  mobile forensic nurse exams. Mary Fariba Afsari, DO, is a board-certified OB-GYN and the founder of FemForward Health, a mobile women’s health clinic in Portland, Oregon. She completed her medical school at Touro University college of Osteopathic Medicine, her Obstetrics and Gynecology residency at the University of Connecticut School of Medicine and holds an MS in Health Communication from the Tufts University/Emerson College joint program. Dr. Afsari is a passionate advocate for healthcare equity and reproductive justice. She speaks widely on the intersections of medicine, identity, and systemic healthcare reform. The Government has announced a new €10 million scrappage scheme designed to encourage motorists to switch from older petrol and diesel vehicles to electric cars.    Under the proposal, drivers who scrap a vehicle more than 13 years old could receive up to €8,500 in support towards the purchase of an electric vehicle, with 65% of the funding reserved for people living outside Ireland's major cities.   Supporters say the scheme could help accelerate the move to cleaner transport, while others have raised questions about affordability, charging infrastructure and its suitability for rural Ireland.   To discuss the opportunities and challenges presented by the initiative, Alan Morrissey was joined by John Casey of the Clare EV Owners Club and Eddie Punch of Independent Ireland. Rural America has moved toward strong Republican voting even though Republican governance helps explain worse health outcomes in rural areas, from hospital closures to opioid policies. Michael Shepherd finds that rural areas disproportionately suffer from Republican policies but Republicans successfully blame Democrats for worse health outcomes, especially when Democratic policies can be framed as helping immigrants or racial minorities. There's no obvious way out for Democrats, who own health policy and its outcomes.

Last week, the Health Minister confirmed the purchase of an almost 44-acre site for the development of a new hospital for the Midwest. Minister of State Dooley says while his preference was for the new facility to be based in Clare, increased investment in ambulance services, which will soon be announced, will make a difference.

France is opening its arms to foreign scientists, particularly from the US, as the Trump administration pulls back from climate research. French GPs and trainee doctors are up in arms over proposals to address 'medical deserts', which they say would make the problem worse. And as Europe marks the 80th anniversary of Europe Day, Algeria commemorates the 8 May, 1945 massacre of civilians by French colonial forces.  Ever since US President Donald Trump started defunding and dismantling US scientific institutions, France has made a push to get scientists to move. In March the French minister in charge of research asked universities to fund programmes to attract American scientists. In 2017, after Trump first pulled the US out of the Paris Climate Accords, Macron launched a recruitment drive aimed at climate scientists working in the US. Two of those grantees, Ben Sanderson and Philip Shulz, talk about the experience of leaving the US for France, and what the current environment is like for climate scientists today. (Listen @1'10)With 87 percent of France considered a "medical desert", lawmakers and the government are looking to tackle doctor shortages. But the proposals – to regulate when specialists can open their private practices and require health professionals to work two days a month in areas with chronic shortages – have met with strong opposition from GPs, trainee doctors and students. Yassine Bahr, vice-president of the French junior doctors union (ISNI), and Anna Boctor, president of France's Jeunes Medecins (young doctors) union, talk about why the proposals won't solve the problem and the sense of injustice at being held responsible for a situation that is not of their making. (Listen @20'20)On 8 May 1945, during a celebration of the end of WWII in Europe in the Algerian city of Setif, French colonial authorities shot at Algerians holding pro-independence signs. The ensuing riots then spread to neighbouring cities where the authorities  unleashed a campaign of reprisals to crush the unrest – indiscriminately killing tens of thousands of Algerian men, women and children.
