(37:16) • When you show up at flight school, they say “Look to your left and look to your right. One of you is gonna have a serious mishap or be dead choosing this job. Are you sure you wanna be here?” (37:49) • You think you're invincible, and you don't really think anything of it. (38:04) • Keegan tells the detailed story of that fateful day. They took my core body temperature. I was at 87 degrees. W tym odcinku kontynuuję omawianie doniesień z tegorocznego kongresu AHA.W czwartej części podsumowania AHA 2025 wracam do dwóch ostatnich kluczowych badań opublikowanych w NEJM – OCEAN oraz VESALIUS-CV – po wcześniejszych odcinkach poświęconych polypill, nowym terapiom hiperlipidemii, połączeniu DOAC z ASA oraz koncepcji SMuRFs-less. Badanie OCEAN dotyczy fundamentalnego pytania, czy po skutecznej ablacji migotania przedsionków można odstawić doustną antykoagulację u pacjentów z CHA₂DS₂-VASc ≥2, mimo że aktualne wytyczne ACC 2023 oraz ESC/PTK 2024 zalecają jej kontynuację niezależnie od sukcesu zabiegu, co dobrze podsumowuje dokument ekspercki EUROPACE: https://pmc.ncbi.nlm.nih.gov/articles/PMC11000153/. W OCEAN objęto obserwacją 1284 chorych po skutecznej ablacji, porównując ASA z DOAC przez 3 lata, a następnie wykonując rezonans magnetyczny mózgu w poszukiwaniu klinicznych i niemych udarów. Wyniki pokazały bardzo niskie ryzyko udaru w obu grupach – 0,66% rocznie w grupie ASA i 0,31% w grupie DOAC – przy istotnie większej liczbie drobnych krwawień u chorych leczonych DOAC. Dane te sugerują, że u wyselekcjonowanej, niewielkiej grupy pacjentów po bardzo skutecznej ablacji, młodszych, z niskim CHA₂DS₂-VASc i ścisłym nadzorem rytmu, możliwa może być przyszła modyfikacja zaleceń, choć nie dotyczy to większości chorych z migotaniem przedsionków. Drugim kluczowym badaniem jest VESALIUS-CV, pierwsze duże randomizowane badanie z ewolokumabem w czystej prewencji pierwotnej, u pacjentów wysokiego ryzyka naczyniowego, ale bez przebytych incydentów sercowo-naczyniowych. W badaniu tym, obejmującym ponad 12 tysięcy chorych leczonych statynami, ewolokumab obniżył stężenie LDL o 55% i zmniejszył częstość zawałów, udarów oraz zgonów sercowo-naczyniowych o 25% w 5-letniej obserwacji. Co szczególnie istotne, całkowita śmiertelność spadła o 20%, co jest rzadko obserwowanym efektem w badaniach prewencji pierwotnej. Jamie Dimon just announced, that for the first time in its history, they have dumped their SILVER shorts and have gone long on SILVER. JPM is the largest holder of physical SILVER in the world at 750 million ounces. That is KEY. That 750 million ounces of SILVER are subject to Trump's Executive Order signed back in December of 2017, that was renewed each year of Biden's presidency. That's not a coincidence. I believe that 750 million ounces of SILVER are going to be the new U.S. Strategic SILVER Reserve. But here's what's interesting and indicates that JPM is now a tool in Trump's hand, taking down the global banking cabal. The SILVER and Gold prices are controlled by two entities. The big bullion banks associated with the LBMA (London Bullion Market Association), which sets the “spot” price of “physical SILVER” in London and the COMEX on Wall Street, that sets the “paper SILVER” price for futures trading. It's a massive derivative market used to manipulate the price, where the same physical SILVER is traded at hundreds of times its worth because most transactions NEVER demand delivery. A truly “fractional” system rampant with fraud. But suddenly something changed on the COMEX. There was a massive increase in demand for physical delivery of SILVER, instead of taking “cash.” Someone was now beginning to hoard physical SILVER. That FORCED the bullion banks in London, to start emptying their vaults and shipping large amounts of SILVER to New York vaults at the COMEX. Guess who owns the largest SILVER vault on COMEX? None other than JPM. And now we know that they were the one demanding physical delivery of SILVER as they were unloading ALL their paper contracts and hoarding physical SILVER. We have watched for months, the flow of physical SILVER leave London and head to New York. The days of the bullion banks controlling the SILVER price are over and their vaults have been emptied, which FORCED them to buy SILVER and drive the price higher. JPM, who had been in cahoots with LBMA forever, just cut the legs out from under them and caused those bullion banks to take heavy losses from their SILVER shorts. JPM trapped LBMA by demanding huge leasing rates for their SILVER supply. That FORCED them to purchase SILVER in order to fulfill orders. That's what helped to end the manipulation of the SILVER price, as JPM went fully long for the very first time. Pacjenci ci mają wyższe ryzyko wczesnej śmiertelności, częściej doświadczają zatrzymania krążenia, wstrząsu kardiogennego i powikłań okołozabiegowych, a jednocześnie częściej brakuje u nich tradycyjnych czynników ryzyka, za to częściej występują te nietradycyjne, jak choroby zapalne, obciążenia rodzinne czy zaburzenia psychiczne. Leczenie tych chorych okazuje się mniej zgodne z zaleceniami – rzadziej otrzymują statyny, ACE-I, β-blokery czy leki przeciwpłytkowe – dlatego poprawa rokowania zaczyna się od poprawy terapii, w tym intensyfikacji statyn i zwiększenia stosowania pełnego GDMT. Peyo's comic album ‘Johan and Peewit' provided the platform for The Smurfs' debut on 23rd October, 1958 - a cameo that their Belgian creator considered an unremarkable side-hustle. But the tiny blue creatures (“Schtroumpfs”, in the original text) went on to become a global cultural phenomenon. Within a year, the Smurfs got their own stories, and by 1959 were starring in dedicated comics. Then came the merch: first as plastic figurines in cereal boxes, then as collectible cuddly toys.
